Mysterious Encephalitis Deaths in Muzaffarpur

Mysterious encephalitis has resulted in the death of more than 54 children in Muzaffarpur and the toll is rising gradually.

Under the present epidemiological conditions, where the tests so far have shown negative results for Japenese encephalitis, the earstwhile most likely diagnosis, auptosy examination is becoming a must.

For any unnatural disease deaths the autopsy of different organs can reveal the histpo-pathological discription of the tissues so that it will become easier to come nearer to the likely diagnosis.

State govt. can declare the present encephalitis epidemic officially as Mahamari under the act. At the same time all the disease transmission preventable measures, taking into account the disease to be transmitted by all likely possible routes in this case as by infected mosquito bite (most likely), by flies, by fecal oral route or fomites, by cough or sneezing should be taken urgently.

With 41 children falling prey to the 'mystery disease' ravaging North
Bihar, the Muzaffarpur district administration on Sunday [26 Jun 2011]
recommended that the State government invoke the Epidemic Diseases Act
throughout the afflicted area. While 41 is the official death toll,
unconfirmed reports from different pockets of the Tirhut division put
the figure at 48.

On Sunday [26 Jun 2011], the district magistrate and the civil
surgeon sent a letter to the State Health Department, recommending
that Muzaffarpur be declared an epidemic-zone.

None of the 18 samples sent to the National Institute of Virology,
Pune has tested positive for Japanese encephalitis (JE) [virus], the
Nipahvirus (named after Nipah in Malaysia), or the Chandipura virus
(named after the village in Maharashtra's Nagpur division).

A 5-member expert committee has been constituted to formulate the
kind of treatment to be administered in the district's afflicted
blocks.

With the death toll in the mysterious disease rising to 47 on Sunday
[26 Jun 2011], the Bihar government is likely to declare it an
epidemic. Under the epidemic Act, the Health Ministry will be
empowered to take samples of brain tissues of the mystery disease
victims.

During the last 48 hours, 6 deaths have been reported, including 5 at
KD Kejriwal Maternity Hospital. The hospital, which has admitted 82
patients since [11 Jun 2011], reported 24 deaths till Sunday evening
including one on Sunday [26 Jun 2011]. 17 children are being treated
at the hospital. Muzaffarpur's government hospital, Sri Krishna
Medical College and Hospital, reported one death during the last 48
hours. It has been treating 8 patients. Patna Medical College and
Hospital has 7 patients.

At the National Institute of Virology, Pune, and Rajendra Memorial
Research Institute, Patna, 24 blood and cerebral spinal fluid samples
have tested negative for Japanese encephalitis (JE) [virus or
antibody? - Mod.TY]. These institutes now want brain tissue samples to
ascertain the disease that experts describe as a kind of viral
encephalitis. _Culex_ mosquitoes are suspected to carry virus from
animals to humans [if it is JE virus, which apparently it is not, as
indicated in the report above. - Mod.TY]

Muzaffarpur district magistrate recommended the state government late
on Saturday night [25 Jun 2011] to declare Gandak catchment and
suburban areas of Bochaha, Minapur, Mushahari, Kanti, Ahiyapur,
Sahebganj, and Paru as epidemic zone. The district administration has
set up a 5-member doctors' committee to advise line of treatment and
help prepare a contingency plan. Dr Arun Shah, one of the members of
the committee, said Muzaffarpur cases needed the same treatment as
Saharanpur encephalitis that led to a large number of deaths every
year till 2000. "A study by the Indian Council of Medical Research
concluded that a toxin from kasaundhi trees, being transmitted to
humans through insects, caused the disease. The trees were cut and the
disease subsided. Litchi trees and mosquitoes present in the region
should be studied for this disease," suggested Shah.

Health minister Ashwini Kumar Choubey said, "We have received
Muzaffarpur administration's advice to declare the disease as
epidemic. We have started the process. We urgently need brain tissues
to decode the mystery. The government will take a final call on Monday
[27 Jun 2011] after assessing the situation. Experts have given us
enough hints of it being some kind of a viral disease affecting brain
and causing sudden death. Declaration of epidemic would help us get
the exact diagnosis," he said.

Chief minister Nitish Kumar said that irrespective of blood sample
tests ruling out Japanese encephalitis, JE vaccine should be given to
all children below 15 years in affected areas.

There is not enough information in this report from the Times of
India to even guess what is responsible for the deaths of these
children - possibly poisoning of some kind, but given the outbreak in
Muzaffarpur around 30 miles away, I thought that this may be of
interest

[Unfortunately, there still is not a diagnosis for these cases, with
JE, Nipah, and Chandipura viruses having been ruled out. Although JE
virus has been discarded as the etiological agent in this outbreak,
vaccination against JE in this endemic area is a wise decision.
ProMED-mail awaits with interest further information as tests and
epidemiological studies progress.

"In many districts of western UP, Uttarakhand and Haryana, outbreaks
of an acute ?&#128;&#156;encephalopathy?&#128;? syndrome dubbed as ?&#128;&#156;mysterious
disease?&#128;? have been an annual feature for many years. The disease
affects rural young children during winter months of September to
December, and carries case fatality rates of around 75-80%. Most
investigators assume it to be viral ?&#128;&#156;encephalitis?&#128;?. According to a
rough estimate, at least 500-700 young previously healthy children are
losing lives every year for last many years in this region. Many
national investigating agencies failed to diagnose the entity despite
investigating for many consecutive years. Year after year the doctors

A team of investigators published their findings in form of three
papers in the reputed peer reviewed journals of the country. At the

_Cassia occidentalis_ was renamed to _Senna occidentalis_ by the
taxonomist some 10 years ago.
In animals we see this plant causing a
syndrome referred to as alset downers. the animals are down, not
depressed and will eat and drink if feed is provided to them, but they
are unable to rise to their feet. They generally have diarrhea, some
degree of weakness, dark urine (being down, and unable to rise, an
indication of muscle damage and likely myoglobin being dumped into the
urine). The toxic agent is poorly characterized but appears to be
throughout the plant.

Therefore the problem was easily solved by cutting down the senna
plants. But the idea that insects transmitted to toxin from the plant
to the humans is pure fabrication, and litchi [lichi] trees are not
toxic

Bernama is reporting 17 fatalities from SKMC, and "about 38" from KDKM hospitals.

Encephalitis continues The incidents of the suspected encephalitis or brain fever is continuing here with four more new cases of the disease admitted to two local hospitals on Wednesday.

Incidence of brain fever recedes in state

MUZAFFARPUR: The incidence of suspected encephalitis or brain fever has just slowed down with an improvement in weather conditions, but it has not yet come to a halt in Muzaffarpur district. Fresh cases of children suffering from high fever, convulsion and even unconsciousness are still pouring into two local hospitals treating the disease.

Four more patients of brain fever were admitted here till Wednesday afternoon including three at Kejriwal Hospital and one at Shri Krishna Medical College and Hospital (SKMCH). Even on Tuesday, two children died and as many new patients had been admitted. According to SKMCH paediatrics department head Dr Brajmohan, the total number of children dying with the symptoms of suspected encephalitis in Muzaffarpur district had gone upto 39 till Wednesday afternoon. Meanwhile, a team of doctors from National Institute of Virology (NIV), Pune, headed by Dr Tandle visiting this district for the last one week, extracted brain tissue through the nose of a dead child at Kejriwal Hospital with the permission of the child's parents on Tuesday and sent the sample to Pune for its analysis to detect the exact virus attacking the children.

The identification of virus may lead to prevention and treatment of the disease in an effective manner, doctors hope. Now, the proposal of Muzaffarpur DM S K Mall to declare this disease an epidemic may be kept pending.

23-25 fatalities at Shri Krishna Medical College and Hospital (SKMCH)
17-24 fatalities at Kejriwal Hospital (KMH)
5 fatalities at various villages
2 fatalities in Patna
1 fatality in Motihari

Total between 48 and 57.

New Delhi, Patna: An ?uncommon? or a ?new virus? may have hit Bihar?s Muzaffarpur, where a mysterious disease has claimed 50 children ? all aged below 10 ? till now, experts have said.


The National Institute of Virology (NIV), Pune, has ruled out Japanese Encephalitis (JE), Nipah and Chandipura virus after testing the samples. ?It could be an unknown or a new virus. We have ruled out JE, Nipah, Chandipura, but are yet to come to a conclusion,? NIV Director Dr A C Misra said.


He said it would take some days to identify the virus as they had not got the ?right samples? yet. ?We have asked for brain biopsies which we think would help identify the virus. We are likely to get one sample tomorrow,? he added.


Muzaffarpur Magistrate S K Mall confirmed the NIV team collected one brain tissue sample from a child, who died on Tuesday.


A team from the National Center for Disease Control, which had also visited the area, is yet to come out with its results. The team had got samples of cerebrospinal fluids but those would not be enough, according to experts.


A senior official in the Union Health Ministry said nothing could be done unless the virus was found. ?As of now, we don?t even know whether it is a water-borne, mosquito-borne or air-borne virus. The interventions will depend on the nature of the virus,? he said.


With three more deaths in the last 48 hours, the toll reached 50 on Wednesday. Over 20 patients are still admitted in various hospitals.


Officials said there was no need to declare an epidemic, the main objective of which would have been to empower doctors to take brain tissues despite objections of the victims? families because of the procedure of opening the skull.


The NIV team, however, said there was no need to open the skull. It can be collected with a needle through the nostril. ?Taking brain tissues after death is not a complicated process,? said Dr Arun Shah, a member of the team.


Bihar Health Minister Ashwini Kumar Choubey said as the main objective to get brain tissues was served, there was no immediate need to segregate a particular region as epidemic -hit.

Study to tame mystery disease

Muzaffarpur, June 29: National Institute of Virology (NIV), Pune, has started studying the brain tissues of dead children in the laboratory raising hopes of a possible end to the mystery disease that has claimed 44 lives so far.

The NIV team expressed hope that the findings of the laboratory experiment would help cure the children who have been falling prey to the mystery disease, suspected to be encephalitis.

Officially, 44 children have died since the outbreak of the disease, the first of which was reported on June 10 at Kejriwal Maternity Clinic where three children lost their lives. The figures include children, mostly aged between three and nine, who died during treatment at Sri Krishna Medical College and Hospital (SKMCH), and Kejriwal Maternity Clinic in Muzaffarpur.

District magistrate (DM) Santosh Kumar Mall told The Telegraph: ?The head of NIV, Dr B.B. Tandle, accompanied by an expert team of technicians, had visited Muzaffarpur recently and stayed here for a couple of days to study the susceptibility of children to suspected encephalitis virus. The institute was in the dark to come out with a final conclusion when it began investigating the case at a laboratory in Pune. However, Tandle had taken samples of blood and serum of the ailing children for necessary laboratory examination.?

Meanwhile, Tandle had approached the DM and senior health officials to obtain the brain tissues of a dead child for further examination. Following persistent demands, the district health authorities have provided brain tissues of two dead children from SKMCH and Kejriwal Maternity Clinic. A team of doctors procured brain tissues of dead children after getting the necessary consent of the parents of the deceased child.

?The availability of the brain tissues will help ascertain the kind of virus affecting the children. This will also help in further investigations and prescribing a medical treatment,? Mall quoted Tandle as saying.

Mall added that it would take at least a fortnight to come to a conclusion about the virus after an in-depth laboratory tests.

Mall, who formed a committee of five doctors of paediatrics discipline, held a detailed discussion at the SKMCH today to adopt same line of treatment under the standard protocol.

Two doctors ? neurologist Dr Ashok Kumar and Dr Krishna Pandey of Rajendra Memorial Research Institute ? advised the doctors at both SKMCH and Kejriwal Maternity Clinic to maintain one line of treatment on the ailing children.

Today, four new admissions ? three in Kejriwal Maternity Clinic and one at SKMCH ? with suspected cases of encephalitis were reported here today. Deputy superintendent of SKMCH Dr Sunil Kumar Shahi told The Telegraph that the committee has prepared a guideline to be followed by other hospitals including public health centres to treat the children falling prey to the suspected encephalitis.
