Unknown Diseases Epidemic in Muzaffarpur Bihar

By- Dr Navneet Ranjan(Public Health Specialist) : 138 cases of unknown disease epidemic (Japanese encephalitis?) have been reported from Muzaffarpur districts of Bihar in till 4th June 2011. About thirty six percent of these (n=50) have died. Few cases from the adjoining districts of Muzaffarpur have also been admitted to the hospitals at Motihari.

The majority of the affected villages have reported only single cases. The suspected encephalitis cases have not stopped for admission at Muzaffarpur?s Kejriwal Maternity Hospital (KDKMH) and Sri Krishna medical college & Hospital (SKMCH) KDKMH is admitting the maximum number of cases for treatment and some other cases are being referred to Patna medical college hospital. Blood samples and spinal fluid samples were collected by a team of doctors came from New Delhi and NIV, Pune. Doctors are treating the children for fever and convulsions so far with anti-viral, anti-bacterial and anti-convulsion therapy.
It can be suggested to do Entomological surveys (apart from lab investigation) in the affected villages that may reveal the high density of Culex tritaeniorhynchus and Culex vishnui group ? the vectors of Japanese Encephalitis (JE) to confirm the diagnosis.
Public Health Interventions to combat this unknown disease
The interventions should start with since a vector borne outbreak is suspected, vector breeding site. Whilst the target would be the following blocks of Muzaffarpur districts Bochahan , Kanti, Kudhani, Minapur ,Mehshi, Kunhari,Sakara,Mushari,Paru and Marwan

Following steps are suggested along with Govt, PRI, and AWW:

1. Organize meetings in the affected villages, followed by adjoining villages. Mothers should be a major intervention group. Tell parents about benefits of using of free bed nets and their use throughout the year. About keeping children clothed, if they can afford to, about keeping cattle sheds etc clean and other routine vector control measures.

2. Warn parents of the danger signs; ask all fever cases to be immediately taken to the PHC.

3. All vector control measures should be activated, as are referral networks for sick children from PHCs to the govt hospitals and those necessary drugs are in stock.

4. Take this as opportunity to tell community about the importance of immunization, sanitation, etc.

5. Contact the local Doctors association and inform all private practitioners.

By ? Dr Navneet Ranjan(Public Health Specialist) : It was observed that Children are admitted mainly to KDKM and SKMCH, Muzaffarpur who suffered from unknown disease. It?s also worth to note the closure of children?s ward at Sadar Hospital,Muzaffarpur at this crucial juncture. So far 140 cases were admitted out of them 50 deaths occurred,77 patient were discharged,7 reffered whereas 4 are under treatment. There are no case admitted in the last two days(as on 7/7/2011).There are many teams of doctors, scientist came to Muzaffarpur to understand this unknown disease.

On the basis of discussion with Child specialists of Muzaffarpur it is evident that the disease is not at all bacterial, but it is viral. Every symptom (High fever with convulsion) of the disease is matching with those of encephalitis. However, technically speaking, the conclusive finding would be clear only after the lab investigation report.

We are still waiting for the final diagnosis of this unknown disease. outbreak communication play great role ,if implemented effectively, will result in greater public resilience and guide appropriate public participation to support the rapid containment of an outbreak, thus limiting morbidity and mortality. In addition, effective outbreak communication will minimize the damage to a nation?s interna-tional standing, its economy and its public health infrastructure. .

Scientists baffled over mysterious deaths in Bihar

New and re-emerging viruses are a major challenge for scientists at the National Institute of Virus (NIV) here.

"The NIV is at par with international technology and the best human resources and can deal with any emergency situation, but new viruses do pose a challenge," according to Vidya A. Arankalle of the Department of Hepatitis, NIV.

Citing the examples of Severe Acute Respiratory Syndrome (SARS) and Avian influenza H1N1 which primarily were imported viruses, Dr. Arankalle said that while the institute had played a major role in dealing with the outbreaks, there are some mysterious viruses like the one that resulted in the death of over two scores of children in Bihar that continue to baffle the scientists here.

"Detection of virus becomes very important. One can proceed for diagnosis and treatment only after the detecting the nature of the virus," she said, admitting that the scientists were still in the dark about the "unknown" disease that killed so many children in Muzaffarpur district of Bihar last month. ?We are working on it but something more needs to be done."

The samples picked up from the patients and from those killed in the Bihar outbreak have been tested at the state-of-the-art Electron Microscopy Laboratory of the NIV but with no success. "It is baffling," says Atanu Basu of the Electron Microscopy department. The high resolution electron microscopic studies of H1N1 virus here had shown how the virus replicates in the lower lungs of the patients which is claimed to be a major breakthrough. The latest equipment, named 'Tanya', available with the department can magnify the image up to 2.5 million times.

Two central teams have already visited Muzaffarpur to investigate the outbreak but were unable to arrive at any conclusion. The teams consist of experts from the best institutions from across the country.

The scientists at the Institute which is under the Indian Council of Medical Research (ICMR) take pride in the identification of the fatal Crimean Congo Hemorrhagic Fever (CCHF) that broke out near Ahmedabad earlier this year resulting in some deaths including that of a doctor and nurse at the hospital where the patient was admitted. The virus was detected within 24 hours and instructions for isolation of patients were issued instantly which saved many lives.

The Virus Research Center (VRC), Pune came into existence in 1952 under the joint auspices of the ICMR and the Rockefeller Foundation, as a part of the global programme of investigations on the arthropod-borne group of viruses. In view of its expanded scope and activities, the VRC was re-designated as the National Institute of Virology (NIV) in 1978. The NIV is identified today as the WHO Collaborating Center for arbo-viruses reference and hemorrhagic fever reference and research. NIV is also the National Monitoring Center for Influenza, Japanese encephalitis, Rota, Measles and Hepatitis.

New Delhi, Aug 16 (IANS) As many as 55 people, mostly children, have died in Bihar in the last one-and-a-half months due to encephalitis, the Rajya Sabha was informed Tuesday.

Health and Family Welfare Minister Ghulam Nabi Azad said clinical and epidemiological data suggests that it was an outbreak of Acute Encephalitis Syndrome resulting in 150 cases and 55 deaths, mostly among children.


In a written reply, the minister said these cases were reported early June to middle July.
