The above comments on the state of health care health care in Argentina in general and Buenos Aires in particular due to the exploding number of pandemic H1N1 infections are widely reported. The number of confirmed fatal cases has risen to 29, in the past several days, including two reported today in Rosario, and reports suggest at least 15 additional fatalities are suspect cases. The state numbers identify almost 1400 confirmed cases, including 25 fatalities in the metropolitan area. Similarly, in the adjacent Santa Fe province the number of confirmed is 34, although private labs have confirmed 40 additional cases. However, these are not included because they have not yet been officially confirmed. Moreover, 200 suspect samples have been collected from patients in Rosario (see <a rel="nofollow" href="http://flutracker.rhiza.com">Maps</a>]) and sent for testing and media reports suggest there are over 800 cases in Santa Fe province. Similarly, Brazil announced its first confirmed fatality today, who was a truck driver who developed symptoms while in Argentina.

Thus, the state numbers are depressed by a sample backlog at the state for testing suspect cases or confirming positives identified at private labs. Moreover export of cases signal significant under-reporting.

The collected samples are also likely to represent a small percentage of the total. This undercount is common worldwide. In the United States that surveillance system for season flu only tests about 0.1% of samples and it is likely that pandemic flu is similarly undercounted, most cases are relatively mild and most labs are now limiting testing to severe cases or clusters, but even clusters such as outbreaks at schools or summer camps only test a small representative sample from the outbreak.

In Agentina, like some regions in the US, testing has halted and a pandemic H1N1 diagnosis is made based on clinical presentation.

Consequently the number of infections is orders of magnitude higher than confirmed cases, but the concerns in Argentina center both on the state of the health care system in general, as well as the rapid increase in fatal infections, including those that rapidly deteriorate while develping symptoms similar to infected patients in the 1918 pandemic. Moreover, like the 1918 pandemic, the vast majority of infections and fatalities are in previously healthy young adults, and the H1N1 is a swine flu that has recently begun to spread throughout the human population.

Clinicians responsible for the care of patients should take note of this comment by Dr. Niman. Those of us within areas where the pandemic strain is circulating who encounter a patient with and influenza like illness whether they practice in the northern or southern hemisphere should understand that the overwhelming odds favor the fact that they are dealing with a person infected with novel H1N1.

Under these conditions, there is no need to seek confirmation before instituting efficacious therapy. To do so places the patient at unnecessary risk and is inconsistent with recommendations from the US CDC's 2005 Pandemic Influenza Plan that contemplated just this situation.

What their guidelines state is consistent with the above comment. This guideline was issued at a time when there was no pandemic and no political pressure one way or another, unlike today.

What is important for all clinicians to understand now is that the world is engulfed by a novel influenza strain that is in the exponential phase of spread across the planet in all regions at the same time. We are in the beginning of what appears to be a massive and prolonged influenza wave that due to the mobility of our population has managed to spread itself around the globe within a few months rather than in the past when this required significantly longer.

Clinicians everywhere need to bear this in mind. If the novel strain is not in your area now, it will be soon. If it is in your region already, then virtually every LIL that you will be seeing is due to the novel strain. There is no reason nor is there time to bother with testing. If you have Tamiflu available, then by all means use it since it is likely that in the near future the novel strain will acquire to 274 polymorphism that gives it Tamiflu resistance. Therefore it makes little sense to hoard this drug now in the hopes that it might still be effective later on if the novel strain becomes more lethal.

Number of H1N1 cases, deaths is much higher than officially reported
The number of H1N1 cases in Argentina already surpasses 10,000 and those cases are concentrated in the city of Buenos Aires and in Buenos Aires Province.
The number of deaths as a result of the virus is also much higher than official government reports have said, and there have been petitions to declare a National Sanitary Emergency.
According to officials, many cases have gone unregistered. The registered cases are those that are the most complicated, but health insurance companies and certain medical centers do not report many cases.
According to Doctors Without Borders, the good news is that the virus is not so lethal as previously thought, but prevention is necessary as well as an early diagnosis.

Influenza A: Two new deaths in Santa Fe

Influenza A: Two new deaths in Santa Fe

Mientras cada vez más provincias se suman al cese de actividades que impliquen una aglomeración de personas, PERFIL averiguó que la declaración de la emergencia sanitaria por la Gripe A es un hecho después de las elecciones (sería el lunes) y que el Ministerio de Salud pidió una reunión del Comité de Crisis para la semana que viene, en la que se evaluará tomar medidas de distanciamiento social, similares a las de México: se cerrarían colegios, shoppings, cines y teatros.
Lo que se viene. Un integrante del Comité de Crisis que lidera Sergio Massa –que pidió la reserva de su identidad– le confirmó a PERFIL que en la mesa de discusión figuran tanto la declaración de la emergencia sanitaria (lo que implica una mayor disponibilidad de recursos humanos y económicos) como de un aislamiento social al estilo de lo que ocurrió en México. Y estima que, de decidirse esto último, la medida será total.
“La verdad es que estamos esperando que se declare la emergencia sanitaria, pero eso lo decide el Comité de Crisis. Se tendría que haber hecho ayer”, dijo en forma metafórica Jorge San Juan, jefe de Terapia Intensiva del Hospital Muñiz e integrante del Comité de Expertos que asesora al de Crisis. En cuanto al aislamiento social, agregó: “Nosotros ya se lo sugerimos a los ministros varias veces. Se vio que cuando se tomó la medida en México, sirvió”.
Si bien anoche la ministra Graciela Ocaña participó de la conferencia de prensa en la que el Ministerio de Salud brinda el parte diario sobre la situación de la enfermedad, durante todo el día de ayer resonó con fuerza la versión de que ya presentó la renuncia a su cargo, el cual dejaría luego de los anuncios del endurecimiento de las medidas sanitarias
