    Medan - Kasus flu burung terjadi lagi di Sumatera Utara (Sumut). Seorang anak berusia 10 tahun terpaksa dirawat di Rumah Umum Pusat (RSUP) Adam Malik Medan karena diduga suspect flu burung.

    Khairul, 10 tahun, penduduk Jl. Abdul Hamid, Desa Sungai Sigiling, Kec. Padang Hilir, Kota Tebing Tinggi, Sumatera Utara, saat ini dirawat intensif di ruang khusus suspect flu burung RS. Adam Malik Medan, Jl. Bungalaw, Medan.

    Humas RS. Adam Malik Medan Sinar Ginting mengatakan, sejak di bawa ke RS Adam Malik, Senin (11/2/2008) pagi, kondisi putra pasangan Enden dan Ani ini mulai membaik.

    "Secara umum kesehatan pasien semakin membaik. Kita telah memeriksa sampel darah, semuanya normal," kata Ginting, Selasa (12/2/2008).

    Namun menurut Ginting, untuk memastikan darah Khairul, pihaknya RS Adam Malik telah mengirim sampel darah ke Pusat Penelitian dan Pengembangan Kesehatan (Puslitbangkes) Departemen Kesehatan di Jakarta.

    "Kita masih menunggu hasil laboratorium untuk memastikan hasil sampel darah pasien. Bila hasil laboratorium menunjukkan sampel darah baik, pasien akan diperbolehkan pulang," kata Ginting.

    Sementara ibu Khairul, Ani mengatakan, anaknya mengalami demam tinggi setelah kontak dengan empat ekor ayam mereka yang mati secara tiba-tiba pada Sabtu lalu (9/2/2008).

    "Saya sempat memberinya obat demam beli dari kios. Tapi demamnya tetap tinggi," kata Ani.

    Karena menduga demam biasa, Ani membolehkan Khairul pergi ke sekolah pada Senin kamarin. Namun menjelang siang, Khairul pulang diantar oleh gurunya karena demam tinggi. Kemudian Ani melapor ke Kepala Desa
    setempat dan Luruh. Pihak Dinas Kesehatan Kecamatan Padang Hilir kemudian datang dan memeriksa sample ayam dan menyatakan ayam tersebut positif terjangkit flu burung.

        The Ministry of Health of Indonesia has announced a new case of human infection of H5N1 avian influenza. A 15-year-old female from West Jakarta, Jakarta Province developed symptoms on 2 February, was hospitalized on 8 February and is currently in hospital in a critical condition. The case is the daughter of a previously confirmed case, the 38-year-old female from West Jakarta, Jakarta Province who developed symptoms on 23 January.
        Investigations into the source of her infection are ongoing. However, she was exposed to her sick mother on 27-28 January and spent time in a neighbourbood where chickens and other birds were found. Samples from these birds have been taken and are undergoing tests to determine whether they may have been the source of infection.
        Of the 127 cases confirmed to date in Indonesia, 103 have been fatal.

                  Sa, a 38 year old man from Kali Deres sub district in West Jakarta experienced onset of illness on January 24, 2008 with symptoms of coughing, fever, runny nose, and sore throat. On January 26 2008, he was admitted to Tangerang MH Thamrin Hospital, then on january 29 2008 he was referred to Usada Insani Hospital. On February 1, 2008 Sa was referred to Persahabatan Hospital, where he is still being treated. A week ago, Sa visited his parents in Cagak village whose parents is neighbour of a duck farmer.

                      The spokesperson said that the girl first showed the symptoms of the H5N1 virus on Feb. 2 and was hospitalized on Feb. 8.                               Indonesia's last known family cluster of H5N1 cases was reported in January 2007, when an 18-year-old boy's H5N1 illness was confirmed after his mother died of the disease. The mother was from Tangerang, a suburb of Jakarta. The son's place of residence was not listed, but a Jan 15, 2007, WHO statement on his illness said the son and his mother probably had the same environmental exposure to the virus.

                                Kematian akibat flu burung di Indonesia sudah mencapai angka yang mengkuatirkan. Dari 124 kasus yang sudah terjadi, sebanyak 102 di antaranya meninggal dunia. Itu berarti tingkat kefatalan penyakit ini sudah mencapai lebih dari 80 persen. Artinya, hanya 2 dari 10 orang penderitanya dapat dipastikan akan selamat. Sisanya secara statistik akan meninggal dunia.
                                Penyebaran penyakit flu burung pada manusia di awal tahun 2008 memang luar biasa, dibandingkan dengan periode yang sama pada tahun sebelumnya. Setelah kejadian kasus sempat menurun di tahu lalu, kini baru di awal tahun sejumlah kasus fatal sudah terjadi Senin (11/2) seorang bocah dari T. Tinggi terindikasi penyakit ini dibawa ke RS HAM Medan.

                                Pemerintah sendiri masih tetap dengan cara lama menghadapi masalah ini. Terkesan gamang. Departemen Kesehatan menyatakan bahwa memang belum terbukti adanya penularan dari manusia ke manusia. Tetapi dengan semakin banyaknya kasus yang bersifat kluster, ini menandakan bahwa kemungkinan penularan bukan tidak ada. Hanya masalah waktu saja untuk membuktikan hal itu.

                                Sementara itu, Departemen Pertanian sendiri juga menyatakan bahwa mereka tidak bisa bertindak atas kejadian yang menimpa manusia. Mereka hanya bertugas pada kawasannya saja, yaitu unggas. Namun persoalan yang sangat tidak bisa diselesaikan pemerintah adalah bagaimana supaya unggas yang menjadi sarana penularan kasus ini tidak lagi bersentuhan langsung dengan manusia. Di berbagai tempat, unggas memang sangat dekat dengan pemukiman. Hal ini disebabkan karena nilai ekonominya yang sangat penting bagi masyarakat.

                                Memang akibat dari kasus ini sangat besar. Pembiaran terjadinya kasus ini tanpa mengambil tindakan nyata seolah membiarkan terjadinya teror diam (silent terror) di tengah masyarakat. Mutasi virus penyakit ini akan mudah terjadi karena unggas yang menjadi sumber penularan selalu ada. Sementara sifatnya yang semakin resisten akan mudah terjadi juga karena terjadinya hubungan yang mudah antara berbagai hewan lain yang berada di sekitar kawasan unggas.

                                Dan yang paling mengkuatirkan adalah jaminan mengenai obat yang kini dimiliki oleh pemerintah. Ada kekuatiran bahwa karena munculnya kasus demi kasus, masyarakat akhirnya memilih mengkonsumsi sendiri obat untuk penyakit ini. Dikuatirkan bahwa jika masalah yang sebenarnya terjadi, penyakitnya justru tidak akan dapat disembuhkan.

                                Kesigapan lembaga donor bukan tidak ada. Badan Kesehatan Dunia sudah memberikan konsultasi. Bahkan laboratorium penyelidikan flu burung di Sumatera Utara pun sudah didanai. Uni Eropa sudah pula menyediakan dana. Kini hanya bagaimana menggerakkan semua sumber daya itu sehingga bisa maksimal.

                                Pemerintah seharusnya harus lebih mengintensifkan berbagai sarana promosi kesehatan pada masyarakat sampai pada tingkat lokal. Perubahan perilaku masyarakat untuk mau menjaga diri untuk hidup sehat adalah sebuah proses panjang. Andaikan sejak kasus ini terjadi pertama sekali di tahun 2005 pemerintah langsung mengambil tindakan gerak cepat berupa pemberdayaan masyarakat, maka niscaya kasus ini tidak akan terus meningkat. Hanya dalam waktu 2 tahun saja, Vietnam, misalnya, berhasi mengeliminir kasus baru sampai 0 persen.

                                Yang tidak kalah penting adalah penegakan peraturan. Gaung mengenai pemusnahan unggas entah sampai di mana kini. Padahal, meski kontroversial, tetapi jika dengan kompensasi yang memadai, pemusnahan unggas adalah satu-satunya cara yang paling efektif untuk meredam penyebaran teror diam ini di tengah-tengah kita. Tindakan segera harus diambil. (***)
