The septic tank area has a hen coop situated very close and the lid of septic tank is not closed completely The children are often found playing in the mud in the area. The health workers also have informed that the hygiene maintenance by the mother of the children is poor.The case had eaten chapatis brought by the grandmotherwho is working as house maid in the neighbourhood except the mother of the children before the attack of the index case. Stool culture:Shigella flexnerii-positive for the index case only. Microbiology lab Vandanam.water sample taken for microbiological testing from the area. RRT lead by DSO, Alappuzha including Epidemiologist visited the area on 23-7-12. the IEC actvities strengthened in the area regading personal and sanitary hygiene. Antilarval work with abate and source reduction activity has been started on 27.07.12. IndoorThermal fogging operation has been carried out on 27.7.12. Out of 8 patient, 3 are having platlet count 14,00,00m2 , pcv 29 % and others are 2.4 % 5 serum sample were collected under process at KAPV Govt. CSF samples examined at GB pantt(children)hospital and SMHS hospital Srinagar.All samples did not grow any organisms on CSF culture. Isolation of all respiaratory tract infection patients advised. Sanitation drive started in the village through village health sanitation and nutrition Committee. Standard case management provided to all the patients at GB Pantt (children)hospital and SMHS hospital Srinagar. Health education imparted regarding spread of disease and its preventive measures. Local health staff advised to kep close vigil of the sitvation and report accordingly.               Acute Gastro Enteritis Kalyan Taluka,Khadavli PHC,PalsoliSC,Kakadpada,Kosle 20 0 30/07/2012 2012-07-31 12:26:40 1275 5 Cases with watery stools and vomiting came to Vashind PHC,Shahapur for treatment.of which 3 were severely dehydrated and all referred to SDH Shahapur.The MO PHC Vashind informed Khadavli Health workers regarding this and they started surveillance 15 more patients found .The reason for outbreak is all houses have water connections of Nalyojana and they collect water from the stand post fixed outside the house.All water stand post are at ground level and connected with rubber tube and it is kept inside a pit.When rain occurs the pit is filled with water mixed with waste materials , cow dung and human waste(obervation)and that causes mixing up with pipe line water.Leakages also observed in the pipe line. 4 water samples sent to RH Goveli for testing Patients were treated at Vashind PHC,emergency medical room opened in the village and 5 referred to SDH Shahapur.Stopped the water distribution through Pipeline and given instructions to gramsevak and padaswayamsevak to superchlorinate the well in the village and allow the people to use that water.Informed Sarpanch reg.the epidemic situation and the measures to be taken in future.RRT Visited and investigated Investigation Open Under Surveillance

Outbreak reported from village Ambarai, CHC Khuntpani due to consumption of contaminated water. 2 patients were referred to the Sadar Hospital Chaibasa for better treatment. RRT & Block team also reached to the affected locality. All cases treated with anti diarrhoeal drugs and ORS. Active surveillance done.                   Gastroenteritis Patiltola, Davandi, Bhakrondi, Armori. 14 cases 2 fatalities 24/07/2012 2012-08-02 12:38:02 154 The geo-environmental conditions of the village are conducive to various forms of vector-borne and water-borne diseases. The village is surrounded by dense forest with piles of cow dung, open-air defecation, kuchha road and houses. These conditions get worsened in rainy season leading to episodes of outbreaks either of vector-borne or of water-borne. This outbreak is probably due to the contamination of drinking water of the common well of the village that has not been chlorinated since a long. Stool sample has been collected and sent to lab. Result is awaited. All the affected patients were treated in accordance wit their symptoms and rationality. A health camp was organized for 7 days to cover the affected village and the adjacent ones. An RRT was formed to further probe the issue. The affected source was ceased. Chlorination of all the potable water sources was done. IEC activities advocating safe drinking water, hand hygiene, toilet practice, seeking medical care as and when required etc. were conducted. Investigation Open There are some health system and administrative constraints in the village, the village Patiltola is 8 KM away from SC Davandi and 16 KM away from PHC Bhakrondi which seems that the area is inaccessible to health facilities and is 25 KM away from GP Narchuli which again adds to some of administrative constraints and the people are deprived of some of their basic facilities.

Control measures were started soon after the information came from the affected area on 4.8.12.A total of 110 patients were admitted in the CHC Maurawan for treatment and most of them discharge meanwhile 18 patients were sent to the district hospital, Unnao.A team of doctors & paramedical staff is camping at the CHC Maurawan.No casuality has been detected. Out of 3malaria positive 2 are same house, where the death occurred. District RRT investigated the out break. Fever surveillance done. Entomological surveillance done by the Epidemiologist and District malariaVBD officer along with vector control measures taken eg: spraying Investigation Closed In death case, the cause of death is Malaria as per death certificate. ( falciparum mixed malaria with septicemia with jaundice with thrombocytopenia)
