    The first outbreak of the disease Rift Valley dates back to Madagascar in 1979. Being a cyclical viral disease, RVF was subsequently struck the Big Island on three occasions: 1990-1991, 2007-2008 and 2008-2009.

    According to the report given yesterday at a press conference by the Director General of Livestock, Lezoma Tsiry Andriamahatola, 1,934 cases are currently confirmed over the period since the end of February in the 4 regions affected by the 'epidemic. Vatovavy Fitovinany holds the record, with 1,131 cases, or 70% of the total. Atsimo-Andrefana comes in second with 583 cases, followed by Diana with 201 cases and Atsinanana with 19 cases. The death rate seems worrying, of these 1,934 confirmed cases, 634 cattle died of the disease. In addition, no cases of transmission of the disease to humans have yet been recorded. The high number of cases in Vatovavy Fitovinany is explained by the humidity of the area, where mosquitoes are numerous. The latter transmit Rift Valley Fever through their bites.


    Faced with the situation, the government has taken the necessary measures. "We started with raising farmers' awareness of the attitudes that must be adopted to curb the spread of the disease",explained the DG of Livestock. In particular, the clearing of areas of contamination and the planting of cypresses and tomatoes to keep mosquitoes away. Administratively, the regional authorities are taking measures to isolate contaminated animals, including quarantine that prevents any inter-communal movement of cattle and small ruminants. As there is no specific treatment for the disease, the government is currently working to bring drugs such as anti-parasites, vitamins and tonics to these areas. Vaccines to prevent the RVF outbreak in ruminants are also available and a free vaccination campaign will be carried out to deal with the emergency. Veterinarians and those who perform meat inspections are provided with protection kits.

    Better future

    But the Malagasy government is not limited to emergency actions. For a better future for the beef industry, the authorities are moving forward with the traceability and electronic identification project for ruminants in the southern part of the island. An interministerial dialogue aimed at involving all public actors in the beef industry and synchronizing their actions took place on April 13, 2021. This dialogue initiated the Ministry of Agriculture, Livestock and Fisheries (MAEP), the Ministry of the Interior and Decentralization (MID) and the Ministry of National Defense (MDN) or the Gendarmerie.?All stakeholders are working to complete this project.       Rift Valley Fever - a multidisciplinary team at the heart of an investigative mission

      A team of virologists, epidemiologists and entomologists from the Institut Pasteur de Madagascar (IPM) and the Ministry of Public Health carried out a joint mission to investigate cases of Rift Valley Fever (RVF), from April 22 to May 6, 2021, in Mananjary, Vatovavy Fitovinany region.

      In Madagascar, an epizootic (epidemic affecting animals) of RVF was declared by the Ministry of Agriculture, Livestock and Fisheries on April 2, 2021. Cases of RVF have been observed in the cattle herd of 'at least five regions of Madagascar: Atsimo Andrefana, Diana, Atsimo Atsinanana, Alaotra Mangoro and, the most affected region, Vatovavy Fitovinany. The circulation of the RVF virus was confirmed by the Virology Unit of the IPM through analyzes carried out in Antananarivo on samples sent from the five regions.

      The RVF virus can be transmitted to humans through the bites of infected mosquitoes or through direct contact with the blood or organs of infected animals. Infected people can be asymptomatic, have a mild form, or have a severe form.

      An investigative mission carried out thanks to the financial support of USAID within the framework of the RISE project (Research, Innovation, Surveillance and Innovation), was therefore organized with the objective of documenting the situation at the level of the district of Mananjary. in order to assess the risks of transmission to humans and to propose suitable prevention and control measures.

      The IPM and Ministry of Public Health teams were supported by the managers of the basic health centers (CSB) concerned, the mayors and local veterinary agents. They carried out epidemiological surveys (case finding) in the human population and in the bovine population, virological analyzes and an entomological survey. They also carried out mass sensitization on the prevention of human RVF infections and training of CSB leaders and community workers on disease surveillance.

      A total of 103 individuals suspected of RVF by CSB officials or having been in direct contact with animals suspected or confirmed of RVF were contacted and offered diagnostic tests for malaria (RDT) and RVF. Part of the RT-PCR analyzes for recent RVF virus infection were performed on site in the IPM mobile laboratory and test results were available within hours. Thus, the team of the Virology Unit of the IPM was able to diagnose three human cases of RVF out of 103 individuals whose blood samples were tested on site. Analysis of the remainder of the samples will be performed at the IPM. The team from the Ministry of Public Health provided care for confirmed RVF patients. In addition, among the 103 people tested for malaria, 23 were positive and received adequate treatment according to the recommendations of the Ministry of Public Health.

      Collections of mosquitoes at their different stages of development were carried out by the team of the Medical Entomology Unit of the IPM in three fokontany, the choice of which was dictated by the results of epidemiological surveys and virological studies. A total of 2806 adult mosquitoes, belonging to more than twenty species, were collected day and night inside or outside the homes using different trapping systems. Mosquitoes known to be proven or suspected vectors of RVF in Madagascar have been identified, sometimes in high densities.

      Additional analyzes on the epidemiological data collected and the mosquitoes collected are underway at the IPM in order to be able to formulate recommendations to guide strategies to respond to the epidemic.
