SUMMARY
A total of 14 098 confirmed, probable, and suspected cases of Ebola virus disease (EVD) have been reported in six
affected countries (Guinea, Liberia, Mali, Sierra Leone, Spain and the United States of America) and two previously
affected countries (Nigeria and Senegal) up to the end of 9 November. There have been 5160 reported deaths.

There is some evidence that case incidence is no longer increasing nationally in Guinea and Liberia, but steep
increases persist in Sierra Leone. A mixed picture emerges at the district level. Transmission is consistently high in
Conakry and Macenta in Guinea; Montserrado in Liberia; and in the western and northern areas of Sierra Leone.
Declines in incidence continue in Lofa in Liberia; and Kenema and Kailahun in Sierra Leone. Cases and deaths
continue to be under-reported in this outbreak.

In Mali, there have been 4 reported confirmed and probable cases, and 4 deaths. The most recent cases are not
related to the first EVD-positive patient in Mali, who died on 24 October.

Interventions to contain the disease in the three most affected countries include isolating and treating patients,
identifying their contacts and conducting burials in a safe and dignified manner. In the three most affected
countries, 19 of 53 planned Ebola Treatment Centres are now open. A total of 140 trained burial teams are on the
ground, and more than 4400 burials have reportedly been conducted in a safe and dignified manner since the
outbreak began. Samples from all 53 Ebola-affected districts can be sent to a laboratory within 24 hours by road.

Situation reports are now presented in a web-based format to provide detailed information at the country level,
including data and maps, in a more interactive way.

SUMMARY
A total of 15 145 confirmed, probable, and suspected cases of Ebola virus disease (EVD) have been reported in six affected countries (Guinea, Liberia, Mali, Sierra Leone, Spain and the United States of America) and two previously affected countries (Nigeria and Senegal) up to the end of 16 November. There have been 5420 reported deaths. Cases and deaths continue to be under-reported in this outbreak.

In the three countries with widespread and intense transmission, reported case incidence is no longer increasing nationally in Guinea and Liberia, but is still increasing in Sierra Leone. The outbreaks in Guinea and Liberia now appear to be driven by intense transmission in several key districts, whereas transmission is intense throughout the north and west of Sierra Leone. The number of new cases is highest in N?Zerekore in Guinea; Montserrado in Liberia; and in the western and northern areas of Sierra Leone, particularly the capital, Freetown, and nearby Port Loko. Lofa in Liberia, and Kenema and Kailahun in Sierra Leone have now reported no or a very low number of new cases for several weeks.

In Mali, there have been 6 reported confirmed and probable cases, and 5 deaths. The most recent cases have occurred in the Malian capital Bamako, and are not related to the country?s first EVD-positive patient, who died on 24 October. All identified contacts connected with this initial case have now completed 21-day follow-up.

The response to the EVD outbreak continues to evolve in the three most affected countries. Over 1000 beds are now operational in 18 Ebola Treatment Centres. As this number increases, so does the capacity to isolate patients and prevent further transmission of the disease. The capacity to conduct burials in a safe and dignified manner of patients who have died from EVD is a crucial part of halting EVD transmission. More than 4800 safe and dignified burials have taken place since the outbreak began. Accurate, timely laboratory diagnosis of EVD cases is also an integral part of the response. Samples from all 53 EVD-affected districts of the three most-affected countries can be transported to a laboratory for testing within 24 hours of sample collection.

A total of 15 935 confirmed, probable, and suspected cases of Ebola virus disease (EVD) have been reported in six affected countries (Guinea, Liberia, Mali, Sierra Leone, Spain and the United States of America) and two previously affected countries (Nigeria and Senegal) up to the end of 23 November. There have been 5689 reported deaths. Cases and deaths continue to be under-reported in this outbreak. Reported case incidence is stable in Guinea (148 confirmed cases reported in the week to 23 November), stable or declining in Liberia (67 new confirmed cases in the week to 23 November), and may still be rising in Sierra Leone (385 new confirmed cases in the week to 23 November). The total number of cases reported in Sierra Leone since the outbreak began will soon eclipse the number reported from Liberia. The case fatality rate across the three most-affected countries in patients with a recorded definitive outcome is approximately 60%. Three health-care workers were reported infected with EVD in Guinea in the week to 23 November.

Response activities continue to intensify in line with the UNMEER aim to isolate 70% of EVD cases and safely bury 70% of EVD-related deaths by 1 December. Guinea isolates over 70% of all reported cases of EVD, and has more than 80% of required safe burial teams. Progress on isolation and safe burials has apparently been slower in parts of Liberia and Sierra Leone, although uncertainties in data preclude firm conclusions. At a national level, both countries are apparently unable to isolate 70% of patients, although data on isolation is up to 3 weeks out of date. Every EVD-affected district in the three intense-transmission countries has access to a laboratory for case confirmation within 24 hours of sample collection. All three countries report that more than 80% of registered contacts associated with known cases of EVD are traced, though the low mean number of contacts registered per case suggests that contact tracing is still a challenge in areas of intense transmission.

There have been 6070 reported deaths. Reported case incidence is slightly increasing in Guinea (77 confirmed cases reported in the week to 30 November), stable or declining in Liberia (43 new confirmed cases in the 5 days to 28 November), and is still rising in Sierra Leone (537 new confirmed cases in the week to 30 November). The case fatality rate across the three most-affected countries in all cases with a recorded definitive outcome is 72%; in hospitalized patients the case fatality rate is 60%.

Response activities in the three intense-transmission countries continue to intensify in line with the UNMEER aim to isolate and treat 70% of EVD cases, and safely bury 70% of EVD-related deaths by 1 December, with the ultimate goal of reaching 100% by 1 January. At a national level, there is now sufficient bed capacity in EVD treatment facilities to treat and isolate all reported EVD cases in each of the three countries, although the uneven distribution of beds and cases means there are serious shortfalls in some areas. Similarly, each country now has sufficient and widespread capacity to bury all reported EVD-related deaths; however, because not all EVD-related deaths are reported, and many reported burials are of non-EVD-related deaths, it is possible that the 70% target has not been met in some areas. Every EVD-affected district in the three intense-transmission countries has access to a laboratory for confirmation within 24 hours of sample collection. All three countries report that more than 85% of registered contacts associated with known cases of EVD are being traced, although contact tracing is still a challenge in areas of intense transmission. Increasing capacity for contact tracing in areas with low levels of transmission will be necessary to end local chains of transmission.

Response activities in the three intense-transmission countries continue to progress in line with the UNMEER aim to isolate and treat 100% of EVD cases and safely bury 100% of EVD-related deaths by 1 January. At a national level, there is now sufficient bed capacity in EVD treatment facilities to treat and isolate all reported EVD cases in each of the three intense-transmission countries, although the uneven distribution of beds and cases means there are serious shortfalls in some areas. Similarly, each country has sufficient and widespread capacity to bury all reported EVD-related deaths; however, because not all EVD-related deaths are reported, and many reported burials are of non-EVD-related deaths, it is possible that some areas still have insufficient burial capacity. Every district that has reported a case of EVD in the three intense-transmission countries has access to a laboratory within 24 hours from sample collection. All three countries report that more than 80% of registered contacts associated with known cases of EVD are being traced, although contact tracing is still a challenge in areas of intense transmission and in areas of community resistance. Rapidly increasing capacity for case finding and contact tracing in areas with low and moderate levels of transmission will be necessary to end local chains of transmission.

At a national level, the capacity to isolate and treat EVD patients has improved in all three countries since the commencement of the emergency response. While every country has sufficient capacity to isolate patients, the uneven geographical distribution of beds and cases means shortfalls persist in some districts. Each country has sufficient capacity to bury all people known to have died from Ebola, although it is possible that capacity is inadequate in some districts. The number of trained burial teams has significantly grown in each of the three countries in the past month. Every district that has reported a case of EVD in the three countries has access to a laboratory within 24 hours from sample collection. All three countries report that more than 90% of registered contacts associated with known cases of EVD are being traced, although there are discrepancies at the district level. The number of contacts traced per EVD case remains low in many districts.

A total of 20 206 confirmed, probable, and suspected cases of Ebola virus disease (EVD) have been reported in four affected countries (Guinea, Liberia, Mali and Sierra Leone) and four previously affected countries (Nigeria, Senegal, Spain and the United States of America) in the seven days to 28 December (week 52). There have been 7905 reported deaths (case definitions are provided in Annex 1). On 29 December, the United Kingdom reported its first confirmed EVD case. Reported case incidence has fluctuated between 70 and 160 confirmed cases in Guinea over the past 15 weeks. In Liberia, case incidence has mostly declined in the past six weeks. In Sierra Leone, there are signs that the increase in incidence has slowed, although the country?s west is now experiencing the most intense transmission of all the affected countries. The reported case fatality rate in the three intense-transmission countries among all cases for whom a definitive outcome is known is 71%.

Interventions in the three countries continue to progress in line with the UN Mission for Ebola Emergency Response aim to conduct 100% of burials safely and with dignity, and to isolate and treat 100% of EVD cases by 1 January, 2015. Every country has sufficient capacity to isolate patients, but the uneven geographical distribution of beds and cases means shortfalls persist in some districts. In the past month, the average number of beds per reported patient has grown from 6.6 to 13.9 in Liberia, and 1.4 to 3.6 in Sierra Leone. In Guinea, it has fallen slightly from 2.3 to 1.9 beds per patient, reflecting a small increase in probable and confirmed cases. Each country has sufficient capacity to bury all people known to have died from Ebola, yet the under-reporting of deaths is a persistent challenge. The number of trained safe burial teams has significantly grown in the past month ? from 34 to 64 in Guinea, 56 to 89 in Liberia, and 50 to 101 in Sierra Leone. This is close to the capacity needed in each country. All three countries report that more than 90% of registered contacts associated with known cases of EVD are being traced, although the number of contacts traced per EVD case remains low in many districts. Social mobilization is a vital component of an effective response. Engaging communities promotes burial practices that are safe and culturally acceptable, and the isolation and appropriate treatment of patients with symptoms of EVD.

? Reported case incidence continues to fluctuate in Guinea, with no identifiable downward trend. Ebola virus disease (EVD) continues to spread geographically within the country, with the prefecture of Fria reporting 2 confirmed cases for the first time. Case incidence has declined to low levels in Liberia. There are signs that incidence has levelled off in Sierra Leone, although transmission remains intense in the west of the country.
? The UN Mission for Ebola Emergency Response (UNMEER) set twin targets of isolating and treating 100% of EVD cases, and conducting 100% of burials safely and with dignity by 1 January, 2015, in Guinea, Liberia, and Sierra Leone.
? Each of the intense-transmission countries has sufficient capacity to isolate and treat patients, with more than 2 treatment beds per reported confirmed and probable case. An increasing emphasis will be put on the rapid deployment of smaller treatment facilities to ensure that capacity is matched with demand in each area.
? Similarly, each country has sufficient capacity to bury all people known to have died from EVD, though the under-reporting of deaths means that the UNMEER target of 100% safe burial was not met.
? In addition to the two UNMMER targets, there are several other crucial aspects of the response, including rigorous contact tracing, access to laboratory services, and community engagement.
? Guinea, Liberia and Sierra Leone report that more than 90% of registered contacts are monitored, though the number of contacts traced per EVD case remains lower than expected in many districts. In areas where transmission has been driven down to low levels, rigorous contact tracing will be essential to break chains of transmission.
? There are currently 23 laboratories providing case-confirmation services in the three intense-transmission countries. Five more laboratories are planned in order to meet demand.

SUMMARY
 Guinea reported its lowest weekly total of new confirmed Ebola virus disease (EVD) cases since the week
ending 17 August 2014. Case numbers remain low in Liberia, with no confirmed cases nationally for the final 2
days of the week ending 11 January, and the lowest weekly total of confirmed cases since the first week of
June 2014. Sierra Leone has now reported a decline in case incidence for the second week running, and
recorded its lowest weekly total of new confirmed cases since the week ending 31 August 2014.
 Each of the intense-transmission countries has sufficient capacity to isolate and treat patients, with more than
2 treatment beds per reported confirmed and probable case. However, the uneven geographical distribution
of beds and cases, and the under-reporting of cases, means that not all EVD cases are isolated in several
areas.
 Similarly, each country has sufficient capacity to bury all people known to have died from EVD. However, the
under-reporting of deaths means that not all burials are done safely.
 Guinea, Liberia and Sierra Leone report that between 84% and 99% of registered contacts are monitored,
though the number of contacts traced per EVD case remains lower than expected in many districts. In areas
where transmission has been driven down to low levels, rigorous contact tracing will be essential to break
chains of transmission. In the week to January 11, 15% of new confirmed cases in Guinea arose from known
contacts (equivalent information is not yet available for Liberia and Sierra Leone).
 There are currently 27 laboratories providing case-confirmation services in the 3 intense-transmission
countries. Four more laboratories are planned in order to meet demand.
 Case fatality among hospitalized patients (calculated from all hospitalized patients with a reported definitive
outcome) is between 57% and 60% in the 3 intense-transmission countries.
 A total of 825 health-care worker infections have been reported in the 3 intense-transmission countries; there
have been 493 reported deaths.
 Many elements of the response to the Ebola outbreak, from safe burials to contact tracing, rely on actively
engaging affected communities to take ownership of the response. At present, 33 of 38 (87%) of districts in

Ebola Situation Report - 1 April 2015

SUMMARY

A total of 82 new confirmed cases of Ebola virus disease (EVD) were reported in the week to 29 March, a slight increase compared with 79 cases the previous week. Case incidence in Guinea increased to 57, compared with 45 the previous week. This offset a fourth consecutive weekly fall in case incidence in Sierra Leone, which reported 25 confirmed cases. Liberia reported no confirmed cases over the same period.

In addition to an increase in case incidence in Guinea, the geographic area of transmission also increased. A total of 7 Guinean prefectures reported at least one confirmed case in the week to 29 March, compared with 3 the previous week. Two of the 7 prefectures that reported a new confirmed case, Fria and Siguiri, did so for the first time in over 50 days. Siguiri, which borders Mali, is the first prefecture outside the western area of Guinea to report a confirmed case for over 30 days.

In Sierra Leone, cases were reported from 5 northern and western districts around and including the capital Freetown, which reported 10 new confirmed cases. The neighbouring districts of Bombali (1 case), Kambia (5 cases), Port Loko (6 cases) and Western Rural (3 cases) also reported cases.

Response indicators for Guinea present a mixed picture. Of 35 confirmed deaths from EVD in the week to 29 March, 15 (43%) were identified post-mortem in the community, compared with 10 of 37 (27%) the previous week. This increase may be attributable to improved access to communities in Forecariah prefecture. The proportion of confirmed cases that arose among registered contacts increased from 38% in the week to 15 March to 53% in week to 22 March. A total of 20 unsafe burials were reported in the week to 29 March, compared with 26 the previous week.
A 45-day state of health emergency has been declared in the Guinean prefectures of Forecariah, Coyah, Dubreka, Boffa, and Kindia. The capital, Conakry, will also be subject to emergency measures, which include the restriction of movement in areas of transmission, the temporary closure and quarantine of private hospitals and clinics where EVD cases have been detected, and limitation of burial participation to close relatives only. All corpses will be tested for EVD during the 45-day emergency period.

The last confirmed case in Liberia passed away on 27 March. Investigations are ongoing to establish the origin of infection. A total of 185 contacts associated with the case are being monitored twice a day. Heightened vigilance is being maintained throughout the country. In the week to 29 March, 278 laboratory samples were tested for EVD, with no confirmed cases.
In the week to 29 March, 67% of confirmed cases in Sierra Leone came from registered contacts, compared with 84% the previous week. There was one report of an unsafe burial over the same period. The proportion of confirmed deaths from EVD that were identified in the community increased slightly, from 7 of 56 (13%) in the previous week to 8 of 52 (15%) in the week to 29 March. Heightened surveillance is being maintained: over 100 suspected cases were reported in the week to 29 March, compared with 57 the previous week. The majority of suspected cases (52) were reported during the final 2 days of the 3-day stay-at-home.

There were 8 new health worker infections in the week to 29 March: 7 in Guinea, and 1 in Sierra Leone. This brings the total number of health worker infections reported across the three most-affected countries since the start of the outbreak to 861, with 495 deaths.

SUMMARY

The decline in confirmed cases of Ebola virus disease (EVD) has halted over the last three weeks. To accelerate the decline towards zero cases will require stronger community engagement, improved contact tracing and earlier case identification. In the week to 19 April, a total of 33 confirmed cases was reported, compared with 37 and 30 in the preceding weeks.

In the week to 19 April, Guinea reported 21 confirmed cases, compared with 28 cases the previous week. Sierra Leone reported 12 confirmed cases, compared with 9 cases reported the previous week. Liberia reported no confirmed cases.

A total of 4 Guinean prefectures reported at least one confirmed case in the week to 19 April, compared with 5 the previous week. Transmission remains confined to the west of the country and is primarily focused on the prefecture of Forecariah, bordering Sierra Leone, which reported 86% of national confirmed cases.

In Sierra Leone, Western Area Urban, which includes the capital Freetown, reported 6 confirmed cases, which represent half of the cases reported nationally, a slight increase from 4 cases the previous week. Koinadugu, which borders Guinea to the north, reported 1 new confirmed case of unknown origin. Other districts reporting new confirmed cases were Kambia (4 cases) and Port Loko (1 case) in the west of the country. In the week to 19 April, 4 districts reported at least one confirmed case compared to 3 districts the previous week.

Response indicators from Sierra Leone present a mixed picture. The number of EVD-positive deaths that were identified in the community after post-mortem testing was 3 (1 in Kambia, 1 in Western Area Urban, 1 in Koinadugu) in the week to 19 April and the percentage of new cases arising from known contacts was below 50% in the week to 12 April (44%). Laboratory indicators reflect heightened vigilance: large numbers of samples were collected (1467) and less than 1% these tested EVD-positive (12 of 1467 samples).

Response indicators for Guinea also present a mixed picture. A total of 6 deaths from EVD in the week to 19 April were identified post-mortem in the community compared with 8 deaths the previous week. The percentage of confirmed cases that arose among registered contacts remained below 50% for the third consecutive week, at 46% in week to 12 April (the most recent week for which data are available), but an increase on 29% reported in the previous week. Laboratory indicators improved with the number of laboratory samples tested increasing for a fifth consecutive week to 565 samples tested in the week to 19 April and the number of positive samples dropping to 6% compared to 10% the previous week.

Community engagement appears to be steadily improving in Guinea and Sierra Leone, but more needs to be done to identify all chains of transmission. A case-finding and community awareness-raising campaign took place in the Guinean prefecture of Forecariah from 12 to 15 April, identifying 12 new confirmed cases, 7 of these from post-mortem testing. This represents 52% (12 of 23) confirmed cases reported in Forecariah during the period 12 to 18 April. A similar campaign is planned for the prefecture of Coyah (expected launch date 24 April) and later in the prefectures of Dubreka, Conakry, Kindia and Boffa.

The last confirmed case in Liberia died on 27 March and was buried on 28 March. Heightened vigilance is being maintained throughout the country. In the 5 days to 19 April, 212 new laboratory samples were tested for EVD, with no confirmed cases. Forty-two days will have elapsed since burial of the last confirmed case on 9 May.
There were no new health worker infections reported in the week to 19 April.
