However, the only reported survivor in northern Vietnam was the sister of a fatal case (13F) who was not tested. The surviving sister (8F) initially tested negative on the rapid test, but was subsequently PCR confirmed twice, meeting the WHO definition of an H5N1 confirmed test. However, local media reports indicated the lab confirmation was inconclusive, citing the negative rapid test, and the attending physician tried to justify the lack of testing of the fatal case, even after the sister was twice PCR confirmed. The comments regarding this cluster in the north raise questions about testing and reporting of H5N1 cases in Vietnam in general.

Previous, in the spring of 2005, northern Vietnam had an increase in the number of clusters and a dramatic drop in the number of fatal cases (less than 10% CFR). Similar mild cases have been reported in Egypt in the spring of 2007, and this year all 8 confirmed cases in Egypt have been discharged or stable. The most recent confirmed case in Egypt was symptomatic for 4 days, raising concerns that the number of mild cases in Egypt this year is markedly higher than the 8 confirmed cases, since most suspect cases without a link to dead or dying poultry are not tested fro H5N1, and milder cases would commonly be diagnosed as seasonal flu.

In Vietnam, the current cluster of three cases in the south are the first reported this year, and last year the vast majority of confirmed cases were also in the north. These recent cases in the south raise additional concerns of unreported milder cases in Vietnam, as well as other countries which report high case fatality rates, such as Indonesia and China.

HANOI, March 23 (Xinhua) -- A Vietnamese 26-year-old woman from Mekong Delta province of Soc Trang died with symptoms similar to bird flu patients, the local newspaper Labor reported on Monday.

The woman, named Ly Thi Hang, was admitted to the Bac Lieu general hospital on March 16 to treat symptoms of coughing, difficulty in breathing, and pneumonia, said Nguyen Minh Tung, head of the hospital's preventive health center.

Hang's condition worsened, and she died on March 20, said Tung.

Hang ate death duck about a month ago before being taken to the hospital, said her family member.

According to Nguyen Huy Nga, Head of the Department of Preventive Health and Environment under the Ministry of Health, her blood sample will be tested to confirm the case.

Soc Trang province where the victim comes from was previously reported of being hit by avian flu and has been lifted out of the black list of infected areas in Vietnam, according to Vietnam's Department of Animal Health under the Ministry of Agriculture and Rural Development on Monday.

Vietnam has reported four human cases of bird flu so far this year, three of them died.

In 2008, bird flu killed five people in Vietnam.

Currently, Vietnam reports four provinces nationwide hit by avian flu including two provinces in Mekong Delta, namely Ca Mau and Bac Lieu, and two northern provinces of Quang Ninh and Dien Bien, according to Department of Animal Health on Monday.

Health officials have called for strengthened precautionary and preventive measures against the spread of bird flu in the Mekong Delta province of Dong Thap.
The call came after the death of a three-year-old boy from the province?s Chau Thanh District on Thursday. Nguyen Huy Nga, head of the Health Ministry?s Preventive Health Department, Friday ordered the district to treat itself as an epidemic hotspot.

Another boy of four-year-old from Chau Thanh District, identified as N.T.K.T., was transferred to the Tropical Diseases Institute Thursday with bird flu symptoms but Friday tested negative for the H5N1 virus.

According to the World Health Organization, the H5N1 virus has killed more than 250 people since 2003. Vietnam has the world?s second-highest bird flu death toll after Indonesia, with 55 deaths.

Of the 110 cases confirmed to date in Viet Nam, 55 have been fatal.

Bird flu kills Vietnamese woman - state radio

HANOI, April 24 (Reuters) - A 23-year-old woman has died from H5N1 bird flu in northern Vietnam, state-run radio said on Friday, the country's fourth human fatality this year.

The woman, from Thanh Hoa province, fell sick and died earlier this month, and tests showed she had been infected by the H5N1 virus, Voice of Vietnam said, quoting the Health Ministry.

It said dead chickens were found in the area where she lived, but it did not say if they were infected with the disease. Thanh Hoa was not in the government's bird flu watch list.

The World Health Organization, which often waits for official announcements from Vietnam's Health Ministry before confirming bird flu cases, had yet to report the woman's death.

Prior to the woman's death earlier this week, Vietnam had reported 110 cases of human infections, of whom 55 had died, the WHO's tally showed.

Avian influenza has affected millions of birds around the world, particularly in East Asia, and can be caught by humans who come into contact with infected birds.

Scientists fear the virus could mutate into one that spreads easily among people, unleashing a pandemic that could kill millions.

A 23-year-old woman from Thanh Hoa Province died of bird flu on Wednesday, Vietnam?s fourth fatality from the disease this year, an official from the Health Ministry confirmed Friday.

Nguyen Huy Nga, head of the ministry?s Bureau of Preventive Health and Environment, said the victim was Pham Thi Hao from Quan Hoa District in the central province.

Though the bureau confirmed the victim had been infected with the H5N1 avian virus no birds had been affected by the disease in the area recently.

Hao was admitted to Quan Hoa General Hospital last Thursday before being transferred to Thanh Hoa General Hospital in critical condition on Tuesday.

She was suffering from pleural and pericardial effusion and kidney failure at the time.

A team from the National Institute of Hygiene and Epidemiology and local health authorities failed to find any trace of bird flu in Thien Phu Commune, where Hao lived.

All four human bird flu cases in Vietnam this year have proved fatal.

Two families of patients died from acute respiratory failure suspected of H5N1 in Luong Van Chua, from Panh mountain village, Thanh Luong commune, and Luong Thi Lau, from Xa Cuong mountain village, Pa Thom commune.
