In 2022, vaccine-derived polioviruses (VDPVs) emerged as a potential threat to global polio eradication.[SUP]1[/SUP] The first known outbreak of VDPV infection was reported in Hispaniola in 2000–01[SUP]2[/SUP] and was associated with VDPV type 1. Since 2005, outbreaks due to VDPV type 2 have been reported in many African and Asian countries.[SUP]3[/SUP] However, VDPV type 3 has been detected sporadically in 2002 from sewage in Estonia,[SUP]4[/SUP] in 2017 from acute flaccid paralysis (AFP) cases in Somalia,[SUP]5[/SUP] in 2022 in immunocompromised children in China,[SUP]6[/SUP] and in 2023 from sewage and AFP cases in Israel and Pakistan.[SUP]1[/SUP]
From Aug 21, to Sept 27, 2024, circulating VDPV type 3 were isolated from three children with AFP living in the Kankan region, as part of the AFP surveillance system in Guinea, according to WHO standard procedures (appendix p 3).[SUP]7[/SUP] Samples were further analysed by next generation sequencing (appendix p 1).[SUP]8[/SUP] The newly characterised VDPV type 3 sequences from Guinea had VP1 nucleotide distances of 2·9%, 2·3%, and 3·1% from the type 3 Sabin strain; suggesting sustained VDPV type 3 circulation and possible silent virus transmission in the Siguiri and Kankan health districts (Kankan region). We identified a 2493U→C encoding an Ile6Thr substitution of the VP1 gene of the newly characterised VDPV type 3 sequences from Guinea, confirming then their origin from the vaccine strain. ...
