Gorakhpur (UP), Sept 20 (PTI) Eleven children have succumbed to Japanese Encephalitis in last two days, taking the toll due to the viral infection in the district since January this year to 311, a senior health official said today.

Thirty-three patients of suspected encephalitis have been admitted in BRD Medical College yesterday, Additional Director (Health) L P Rawat said.

236 patients are being treated at the medical college and 40 in other government hospitals.

He said a total of 1,521 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome (AES) were admitted this year in various hospitals, out of which 311 died.

They include mostly children from Gorakhpur, Mahrajganj, Siddharthanagar, Deoria, Kushinagar districts and adjoining Bihar and Nepal, he said.

The deadly Japanese Encephalitis fever is caused by a virus found in pigs and wild and transferred to humans by mosquito bite.

Gorakhpur (UP), Sept 22 Seven more children succumbed to the dreaded encephalitis during past two days at BRD Medical College Hospital here, taking the death toll to 318 since January this year, official sources said.

According to additional director health Dr LP Rawat 44 new patients of suspected encephalitis were admitted to the hospital during the period.

Rawat said that 280 patients are being treated at BRD Medical College and 36 in other government hospitals of the region.

He said that a total of 1565 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome were admitted this year to BRD Medical College and other hospitals of the region, of this 318 died. - (Agencies)

As many as 29 new patients of suspected encephalitis were admitted to the hospital during last 24 hours.

Gorakhpur (UP), Oct 1 (PTI) Six more children succumbed to encephalitis here, taking the toll in the viral fever in the district this year to 356.

The fresh deaths took place yesterday at the BRD Medical College Hospital here. Forty-one new patients of suspected encephalitis have also been admitted to the hospital.

Additional Director Health L P Rawat said 258 patients are being treated at BRD Medical College Hospital and 32 in other government hospitals in the region.

Uttar Pradesh has been in the grip of the deadly disease for the last two decades. The disease is almost endemic to the region.

Many children have been admitted to the B R D Medical College.

The relatives of the patients are complaining of lack of doctors and medicines in the hospital.

Bhupendra Singh said that the doctors are not providing treatment to the patient since three days.

?The condition of this hospital is very bad as there are three to four patients on one bed. There are no doctors to look after the patients. We have to buy medicines from the markets, as there are no medicines available in the hospital. Everyday, around five to six infants die in this hospital. The relatives are feeling helpless,? said Singh.

Meanwhile, Dr. Rakesh Saxena, Principal of the B R D Medical College, said they are trying their level best to administer proper treatment to the patients.

?The news of patients buying medicines from outside the hospital is absolutely incorrect.

Our medical is well equipped with medicines and the Encephalitis patients are being treated with those medicines only. Further, I agree that there are two patients on one bed but saying that there are five to six patients on one bed is totally false,? said Dr. Saxena.

He added that the patients are being shifted to the new wards where they will be given individual beds.

This disease is almost endemic to the region and immediate measures are required to restrain the spread of the infection any further.

I'm not sure if this should be posted here, but thought this might be of interest. Looks like a similar AES outbreak in 2001 in West Bengal that couldn't be diagnosed.

Panic over mystery disease outbreak
'An outbreak of a mystery brain disease in north-east India has killed at least 30 people and created panic among local residents.

Officials in Siliguri, the second largest city in West Bengal, say more than 400 people have been admitted to hospital.

Although doctors have been unable to identify the mosquito-borne disease, the symptoms - high fever, headache and nausea - resemble those of Japanese Encephalitis.

There are also large pig farms close to residential areas, which would aid the spread of the virus.

Many of the victims have been children, another common hallmark of the disease.

Mosquitoes

Japanese Encephalitis causes severe swelling in the brain and is passed from animals to humans by mosquito bites.

It is prevalent in agricultural areas throughout south and east Asia, killing about 10,000 people each year.

But doctors in Siliguri say they still cannot be certain about the identity of the latest outbreak. "

No laboratory diagnostic criteria were specified in 10 studies [5,16-18,33,34,37-40]. Most studies that specified laboratory diagnostic criteria relied on a combination of complement fixation and haemagglutination inhibition in CSF and/or serum, using either single or paired specimens from different phases of each patient's infection. Evaluating antibody titres may be problematic however, especially in young children as development of antibody, particularly in CSF may occur late [11]. Serum antibodies may indicate an associated infection rather than a causative CNS infection and could potentially lead to a falsely high incidence rate if not taken into account.

Despite extensive microbiological investigation, there was always a significant proportion of patients with no causative agent identified (28?85% unconfirmed). Two principle explanations for this might be firstly that the screening techniques used were not adequate to detect all possible causative agents, and secondly that a proportion of encephalitides, such as those due to VZV, measles virus or Mycoplasma pneumoniae, are post infectious and the causative agent might not be identifiable by the screening tests employed. In addition not all encephalitis cases are caused by microorganisms. The proportion of aetiologically confirmed acute encephalitis cases has varied in the past from 15?100% [23,25,28,30]. The commonest identified cause was HSV, with a relatively consistent incidence across the globe probably reflecting a lack of geographical specificity for herpes viruses.

Incidence of Encephalitis in Tropical Countries

In recent years, the epidemiology and distribution of JE has changed [52]. The disease incidence is decreasing in China, Japan and Korea, and Thailand where it appears to be associated with widespread vaccination campaigns; in contrast it appears to be increasing in parts of Bangladesh, Burma, India, Nepal, and Vietnam [16,23,25,31,53,54]. Although the reasons for these changes are not clear, they may include the adoption of rice cultivation, establishment of larger pig farms and promotion of pig-breeding as a food source, and possibly climate changes [52]. Data from tropical countries successfully implementing vaccination, would suggest that even where JE incidence has fallen as a result, the minimum overall incidence of viral acute encephalitis in JE endemic areas is similar to that in Western industrialised countries [26,27,31], with the reported incidence of JE in the tropics ranging from 2 to 15 per 100,000.

Conclusion

From the 25 studies reviewed in detail, in the most robust, prospective studies conducted in Western industrialised and tropical countries a minimum incidence of 10.5 per 100,000 AES cases is reported for children and 2.2 per 100,000 for adults and 6.34 per 100,000 for all ages. For ease of use in protocols and as benchmarks we therefore suggest that the 2 per 100,000 be used as the minimum incidence for adults, and 10 per 100,000 for children and 6 per 100,000 for all age groups for use in future WHO AES surveillance standards.

The fact that the aetiology of acute encephalitis remains unknown in the majority of patients [15] represents a diagnostic challenge for the future. The increasing incidence of encephalitis due to emerging agents, such as JEV, WNV [19], TBEV [15], and Mycoplasma pneumoniae [49], will present additional challenges. This review shows that in most studies conducted in both Western industrialised and tropical countries, an overall incidence of acute encephalitis of between 1 and 15 per 100,000 is reported, with higher incidences in children than adults. There were no prospective studies from tropical settings looking at the incidence of acute encephalitis in children only...."

NEW DELHI, Oct. 10 (Xinhua) -- With four deaths, the toll of Japanese Encephalitis victims has now been raised to 379 in the northern Indian state of Uttar Pradesh, a senior Indian Health official said Saturday.

"A total of 2,182 patients suffering from the deadly vector-borne disease were admitted to Baba Raghav Das Medical College in the recent past in Gorakhpur in the eastern part of the state. According to latest reports, 379 patients have died so far this year. The condition of some patients are also serious and the toll may mount," the official said.

Gorakhpur is the disease's epicenter, he said, adding that the disease killed 1,000 last year.

Gorakhpur (UP), Oct 18 (PTI) Two more children succumbed to encephalitis at BRD Medical College Hospital here, raising the death toll in the disease to 409 since January this year, a health official said today.

Additional Director, Health, L P Rawat, said while two children died in the past two days, 49 fresh cases of suspected encephalitis were admitted to the hospital.

Currently, 257 patients of the brain fever are under treatment at BRD hospital, while 33 others are admitted to various government hospitals in the region, he said.

A total of 2428 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome (AES) have been admitted to different hospitals this year, he added.

I'm not sure if this should be posted here, but thought this might be of interest. Looks like a similar AES outbreak in 2001 in West Bengal that couldn't be diagnosed.

Excellent find, Emily. Of note is that in that 2001 outbreak, there were dead doctors. That isn't something happening in any of these UP outbreaks.

But it is clear that most encephalitis (worldwide) is due to unknown causes and yet to be detected viruses. There is probably opportunity for a lot of research here.

13 children succumb to encephalitis

Gorakhpur, Oct 20 (PTI) As many as 13 children succumbed to deadly Japanese encephalitis in the past three days in this eastern district of Uttar Pradesh, raising the death toll from the disease to 422 this year.

Forty-seven fresh cases also surfaced in the district during the period, Additional Director (Health) Dr L P Rawat said here today.

Altogether 227 people afflicted with the viral infection were undergoing treatment at BRD Medical College and 70 in other hospitals, he said.

Gorakhpur (UP), Oct 27 (PTI) Four more children succumbed to deadly encephalitis taking the death toll to 441 while 31 fresh cases were reported in the region, officials said today.

Two children were from Gorakhpur who succumbed to the deadly disease, while one each from Sant Kabirnagar and Mahrajganj districts.

Gorakhpur (UP), Nov 4 (PTI) With three more children succumbing to encephalitis here today, the toll due to the disease has reached 481 in eastern districts of Uttar Pradesh since January this year, according to official sources said.

Fourteen new patients suffering from the fever have been admitted to various hospitals in the region, Additional Director Health L P Rawat said here.

He said a total of 2,787 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome (AES) were admitted this year to BRD Medical College and other hospitals of the region. Out of them, 481 have died.

Mahrajganj (UP) Nov 10 (PTI) Seven more children have succumbed to Japanese encephalitis in eastern Uttar Pradesh, taking the toll in the deadly brain fever to 30 this month.

The deaths were reported in Mahrajganj, Kushinagar and Gorakhpur districts yesterday, Additional Director (Health) L P Rawat said.

He said 16 new patients suffering from the disease have been admitted to the B R D Medical College in Gorakhpur.

Of the total 2,839 cases of Japanese Encephalitis and Acute Encephalitis Syndrome reported from the region this year, 496 patients have died, Rawat said.

Gorakhpur(UP): Three more children succumbed to the deadly encephalitis during last two days here taking the toll due to different types of brain fever to 506 in eastern districts, since January this year, sources said.

As many as 12 new patients suffering from the fever have been admitted to various hospitals in the region during the period, Additional Director Health LP Rawat said adding that one patient died on Wednesday while two patients succumbed today.

He said that a total of 2,889 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome were admitted this year to BRD Medical College and other hospitals of the region. Of this, 506 died.

The Additional Director, health said that 137 patients suffering from encephalitis are undergoing treatment at BRD MCH, while seven are admitted in government hospitals of adjoining districts.

Gorakhpur (UP): Nine more people, including six children, have succumbed to encephalitis here taking the toll to 515.

A total of nine patients have died since last Friday, official sources said, adding the toll due to different types of brain fever has reached 515 in eastern districts since January this year.

Sixteen new patients suffering from the disease have been admitted to various government hospitals here and Basti division since yesterday, additional director health LP Rawat said.

He said a total of 2,947 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome (AES) were admitted this year to BRD Medical College and other hospitals of the region. Of this, 515 patients died.

Gorakhpur (UP), Nov 19 (PTI) Six more children succumbed to encephalitis here during the last two days, taking the toll due to the deadly disease in eastern UP to 521 since early this year, an official said today.

Two children died on Tuesday while four succumbed on the following day at BRD Medical College here, Additional Director (Health) L P Rawat said.

Fifteen new cases have been reported from various government hospitals of Gorakhpur and Basti divisions during the last two days, he added.

A total of 2978 patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome were admitted this year at various hospitals in the region out of which 521 have died, Rawat said.

He said 96 patients suffering from the disease were undergoing treatment at BRD Medical College while nine are admitted in government hospitals of adjoining districts.
