If the population of Uttar Pradesh is about 190,000,000 people, only 26 fever deaths per 48 hours in that area seems actually a bit low. That would be about 5,000 per year.

With Uttar Pradesh having about 2/3 the population of the United States, the 18,000 H1N1 deaths in the US would correspond to about 12,000 H1N1 deaths alone. And that doesn't count the thousands of malaria and dengue deaths, which do seem to make up the majority of the "fever". I would expect that annual death toll due to fever in Uttar Pradesh to be closer to 100,000 or more.

Or do we only get reports from urban areas?

The Jagran reports from around 40 cities and gives anything from 1-20 stories per day from each - I would doubt that the reports above are the sum total of disease fatalities in UP, but they serve to let us know what's going on - dengue, malaria, fever and meningitis. I'd say your estimate of 20 unreported for each reported is as good as any.
