Influenza-Associated Pediatric Mortality
Three influenza-associated pediatric deaths were reported to CDC during week 10. Two deaths were associated with a 2009 H1N1 virus and occurred during weeks 52 and 7 (weeks ending December 28, 2013 and February 15, 2014). A total of 68 influenza-associated pediatric deaths have been reported during the 2013-2014 season from Chicago [1], New York City [1] and 28 states (AR [4], AZ [1], CA [6]; FL [3], GA [1]; IA [1]; IL [1]; KS [2], KY [1]; LA [5]; MA [2]; MD [1]; ME [1]; MI [2], MS [1], NC [5]; NE [1], NV [1], OK [2]; OR [1], PA [1], SC [2], TN [4]; TX [11]; UT [2]; VA [1]; WI [1]; and WV [2]).

Influenza-associated Hospitalizations:
Between October 1, 2013 and March 8, 2014, 7,910 laboratory-confirmed influenza-associated hospitalizations were reported. This is a rate of 29.2 per 100,000 population. The highest rate of hospitalization remains among adults aged ≥65 years, followed by the 50-64 years and 0-4 years age groups. People 18-64 years accounted for more than 60% of reported hospitalized cases. Among all hospitalizations, 7,434 (94.0%) were associated with influenza A, 397 (5.0%) with influenza B, 29 (0.4%) with influenza A and B co-infection, and 50 (0.6%) had no virus type information. Among those with influenza A subtype information, 77 (2.2%) were H3 and 3,415 (97.8%) were 2009 H1N1.

Clinical findings are preliminary and based on approximately 44% of cases with complete medical chart abstraction. The most commonly reported underlying medical conditions among adults were obesity, metabolic disorders, cardiovascular disease, and chronic lung disease (excluding asthma). Approximately 12% of hospitalized adults had no identified underlying medical conditions. The most commonly reported underlying medical conditions in children were asthma, neurologic disorders, obesity, cardiovascular disease, and chronic lung disease (excluding asthma). Approximately 44% of hospitalized children had no identified underlying medical conditions. Among 486 hospitalized women of childbearing age (15-44 years), 104 (21.4%) were pregnant.

Influenza A (H3N2) [170]:
All 170 influenza A (H3N2) viruses tested have been characterized as A/Texas/50/2012-like, the influenza A (H3N2) component of the 2013-2014 Northern Hemisphere influenza vaccine.

Influenza B [83]: 53 (64%) of the 83 influenza B viruses tested belong to B/Yamagata/16/88-lineage and the remaining 30 (36%) influenza B viruses tested belong to B/Victoria/02/87 lineage.
Yamagata Lineage [53]: 53 influenza B/Yamagata-lineage viruses were characterized as B/ Massachusetts/2/2012-like, which is included as an influenza B component of the 2013-2014 Northern Hemisphere trivalent and quadrivalent influenza vaccines.
Victoria Lineage [30]: 30 influenza B/Victoria-lineage viruses were characterized as B/Brisbane/60/2008-like, which is included as an influenza B component of the 2013-2014 Northern Hemisphere quadrivalent influenza vaccine.

Composition of the 2014-2015 Influenza Vaccine
The World Health Organization (WHO) has recommended vaccine viruses for the 2014-2015 Northern Hemisphere vaccines, and the Food and Drug Administration’s Vaccines and Related Biological Products Advisory Committee (VRBPAC) has made recommendations for the composition of the 2014-2015 influenza vaccines to be used in the United States. Both agencies recommend that trivalent vaccines contain an A/California/7/2009-like (2009 H1N1) virus, an A/Texas/50/2012-like (H3N2) virus, and a B/Massachusetts/2/2012-like (B/Yamagata lineage) virus. It is recommended that quadrivalent vaccines containing an additional influenza B virus contain a B/Brisbane/60/2008-like (B/Victoria lineage) virus in addition to the viruses recommended for the trivalent vaccines. These recommendations were based on global influenza virus surveillance data related to epidemiology and antigenic characteristics, serological responses to 2013-2014 seasonal vaccines, and the availability of candidate strains and reagents.

Among those with influenza A subtype information, 98 (2.6%) were H3 and 3,606 (97.4%) were 2009 H1N1.

Influenza-Associated Hospitalizations
Between October 1, 2013 and April 12, 2014, 8,995 laboratory-confirmed influenza-associated hospitalizations were reported. This is a rate of 33.2 per 100,000 population. The highest rate of hospitalization is among adults aged ≥65 years, with a steady increase in weekly rates observed in the past few weeks. Nonetheless. people aged 18-64 years still accounted for approximately 60% of reported hospitalized cases. Among all hospitalizations, 8,171 (90.8%) were associated with influenza A, 730 (8.1%) with influenza B, 43 (0.5%) with influenza A and B co-infection, and 51 (0.6%) had no virus type information. Among those with influenza A subtype information, 153 (3.8%) were H3 and 3,872 (96.2%) were 2009 H1N1.

Clinical findings are preliminary and based on approximately 60% of cases with complete medical chart abstraction. The most commonly reported underlying medical conditions among adults were obesity, metabolic disorders, cardiovascular disease, and chronic lung disease (excluding asthma). Approximately 12% of hospitalized adults had no identified underlying medical conditions. The most commonly reported underlying medical conditions in children were asthma, neurologic disorders, chronic lung disease (excluding asthma), and cardiovascular disease. Approximately 44% of hospitalized children had no identified underlying medical conditions. Among 730 hospitalized women of childbearing age (15-44 years), 167 (23%) were pregnant.

Influenza-Associated Hospitalizations
Between October 1, 2013 and April 30, 2014, 9,632 laboratory-confirmed influenza-associated hospitalizations were reported. This is a rate of 35.6 per 100,000 population. The highest rate of hospitalization is among adults aged ≥65 years, although those aged 18-64 years account for approximately 60% of reported hospitalized cases. Among all hospitalizations, 8,493 (88.2%) were associated with influenza A, 1,047 (10.9%) with influenza B, 43 (0.4%) with influenza A and B co-infection, and 49 (0.5%) had no virus type information. Among those with influenza A subtype information, 268 (6.0%) were H3 and 4,213 (94.0%) were 2009 H1N1.

Clinical findings are preliminary and based on 70% of cases with complete medical chart abstraction. The most commonly reported underlying medical conditions among adults were obesity, metabolic disorders, cardiovascular disease, and chronic lung disease (excluding asthma). Approximately 11% of hospitalized adults had no identified underlying medical conditions. The most commonly reported underlying medical conditions in children were asthma, neurologic disorders, cardiovascular disease, and chronic lung disease (excluding asthma). Approximately 43% of hospitalized children had no identified underlying medical conditions. Among 869 hospitalized women of childbearing age (15-44 years), 195 (22.4%) were pregnant.

Novel Influenza A Viruses:

One new human infection with a novel influenza A virus was reported by Ohio. The person was infected with an influenza A (H3N2) variant (H3N2v) virus and was not hospitalized as a result of their illness. No ongoing human-to-human transmission has been identified and the case reported close contact with swine in the week prior to illness onset. Public health and agriculture officials are investigating the extent of disease among humans and swine and no increases in influenza-like illness in the community have been reported. Genetic sequencing of the H3N2v virus conducted at CDC showed that this virus has the nucleoprotein (NP) and matrix (M) gene from the 2009 H1N1 virus, and was similar to the H3N2v virus identified previously this summer. A total of two H3N2v cases have been reported in 2014 and are not epidemiologically linked.

Early identification and investigation of human infections with novel influenza A viruses is critical in order to evaluate the extent of the outbreak and possible human-to-human transmission.
