 Community influenza surveillance
o In week 48, there were further increases in a number of respiratory indicators in children aged <5 year, these
increases are in line with reports of increasing respiratory syncytial virus (RSV) activity.
o Three new acute respiratory outbreaks have been reported in the past seven days. One from a hospital testing
positive for RSV, one from a care home and another from a school with no test results available.
 Overall weekly influenza GP consultation rates across the UK
o In week 48, overall weekly influenza-like illness GP consultations was low in England (9.9 per 100,000), Wales (4.8
per 100,000), Scotland (5.3 per 100,000) and Northern Ireland (18.0 per 100,000).
o Weekly influenza-like illness consultations rates also remain low in week 48 through the GP In Hours Surveillance
system.
 Influenza-confirmed hospitalisations
o Five new admissions to ICU/HDU with confirmed influenza (one influenza A(H1N1pdm09), three A(unknown
subtype) and one B) were reported through the USISS mandatory ICU/HDU surveillance scheme across the UK
(128 Trusts in England) in week 47, a rate of 0.01 compared to 0.02 per 100,000 the previous week.
o Four new hospitalised confirmed influenza cases (three influenza A(H1N1pdm09) and one B) were reported
through the USISS sentinel hospital network across England (22 Trusts), a rate of 0.05 compared to 0.07 per
100,000 the previous week.
o Since week 40, one confirmed influenza admission has been reported (influenza A unknown subtype) from the five
Severe Respiratory Failure centres in England.
 All-cause mortality data
o In week 48 2015, no statistically significant excess all-cause mortality by week of death was seen through the
EuroMoMo algorithm in England overall and by age group.
o In the devolved administrations, significant excess all-cause mortality was seen in Scotland.
 Microbiological surveillance
o Two samples tested positive for influenza B through GP sentinel schemes across the UK.
o Twenty-three influenza positive detections were recorded through the DataMart scheme (eight influenza
A(H1N1)pdm09, three A(H3), one A(not subtyped) and eleven influenza B). A positivity of 1.8% was seen in week
48, a decrease from 2.0% seen in week 47, with the highest positivity in 5-14 year olds (4.8%).
 Vaccination
o Up to week 48 2015 in 73.9% GP practices reporting weekly to Immform, the provisional proportion of people in
England who had received the 2015/16 influenza vaccine in targeted groups was as follows: 38.1% in under 65
years in a clinical risk group, 37.0% in pregnant women, 66.3% in 65+ year olds, 28.4% in all 2 year olds, 29.5%
in all 3 year olds and 24.0% in all 4 year olds.
o Provisional data from the first monthly collection of influenza vaccine uptake by frontline healthcare workers show
32.4% were vaccinated by 31 October 2015 from 95.1% of Trusts, compared to 36.8% vaccinated in the previous
season by 31 October 2014.
o Provisional data from the first monthly collection of influenza vaccine uptake in children of school years 1 and 2
age show 11.8% of children of school year 1 and 11.4% of children school of school year 2 had received the
2015/16 live attenuated intranasal vaccine (LAIV) by 31 October 2015.
o Provisional data from the first monthly collection of influenza vaccine uptake in GP patients up to 31 October 2015
has been published. The report provides uptake at national, area team and CCG level.
 International situation
o Globally, influenza activity generally remained low in both hemispheres, with A(H3N2) detections predominating in
the United States and Canada.
o Influenza activity in Europe remains low and there is no indication that the influenza season has started. There has
been more A(H1N1)pdm09 detections than A(H3N2) detections.

being observed through secondary care surveillance schemes.

o During week 52 selected respiratory indicators remained high but continue to be within seasonally expected levels.
o Six new acute respiratory outbreaks have been reported in the past 7 days. Five in care homes where one tested
positive for influenza A(not subtyped), one tested positive for RSV and hMPV, one tested positive for RSV only,
one tested positive for parainfluenza and one was not tested/test results were not available). The sixth outbreak
was in a hospital which tested positive for RSV.
 Overall weekly influenza GP consultation rates across the UK
o Due to bank holidays in week 52 (ending 27 December 2015), GP surgeries were only open for four days ? data
should therefore be interpreted with caution.
o In week 52, overall weekly influenza-like illness (ILI) GP consultations was low in England (6.6 per 100,000), Wales
(6.8 per 100,000), Scotland (12.4 per 100,000) and Northern Ireland (17.0 per 100,000).
o Weekly ILI rates also remain low in week 52 through the GP In Hours Surveillance system.
 Influenza-confirmed hospitalisations
o Thirty-five new admissions to ICU/HDU with confirmed influenza (fourteen influenza A(H1N1)pdm09, eighteen
influenza A(unknown subtype) and three influenza B) were reported through the USISS mandatory ICU/HDU
surveillance scheme across the UK (129 Trusts in England) in week 52, a rate of 0.08 compared to 0.04 per
100,000 the previous week.
o Twenty-nine new hospitalised confirmed influenza cases (twenty-three influenza A(H1N1pdm09), four influenza

Summary
In week 53 2015 (ending 03 January 2016), allowing for Christmas reporting breaks, across surveillance schemes,
influenza activity has increased for several indicators, including GP ILI consultation rates, the proportion of laboratory
samples positive for influenza and influenza admissions to hospitals and ICU. There is an indication that the influenza
season has started.

to be within seasonally expected levels.
o Thirteen new acute respiratory outbreaks have been reported in the past 7 days. Twelve outbreaks were in care homes
where two tested positive for influenza (one influenza A(not subtyped) and another mixed infection of influenza A(not
subtyped). The thirteenth outbreak was from the ?other? category, in an immigration removal centre, where test results
were not available.
 Overall weekly influenza GP consultation rates across the UK
o Due to bank holidays in week 53, GP surgeries were only open for three days ? data should therefore be interpreted with
caution.
o In week 53, overall weekly influenza-like illness (ILI) GP consultations has increased in England (9.6 per 100,000), Wales
(6.5 per 100,000), Scotland (8.6 per 100,000) and Northern Ireland (21.5 per 100,000).
o Weekly ILI rates remain within seasonally expected levels in week 53 through the GP In Hours Surveillance system.
 Influenza-confirmed hospitalisations
o Sixty-two new admissions to ICU/HDU with confirmed influenza (twenty-eight influenza A(H1N1)pdm09, one A(H3), thirtytwo
influenza A(unknown subtype) and one influenza B) were reported through the USISS mandatory ICU/HDU
surveillance scheme across the UK (133 Trusts in England) in week 53, a rate of 0.13 compared to 0.11 per 100,000 the
previous week. Five deaths were also reported through this scheme.
o Thirty-eight new hospitalised confirmed influenza cases (twenty-seven influenza A(H1N1pdm09) and eleven influenza
A(not subtyped)) were reported through the USISS sentinel hospital network across England (23 Trusts), a rate of 0.41
compared to 0.37 per 100,000 the previous week.
o Since week 40, nine confirmed influenza admissions have been reported (five influenza A(H1N1)pdm09 and four
influenza A(unknown subtype) from the five Severe Respiratory Failure centres in England.
 All-cause mortality data
o Up to week 50 2015, no statistically significant excess all-cause mortality by week of death was seen through the
EuroMoMo algorithm in England overall and by age group and across the devolved administrations.
 Microbiological surveillance
o Fourteen samples tested positive for influenza (12 A(H1N1)pdm09, 2 A(untyped) and 1 B) through GP sentinel
schemes across the UK, with an overall positivity of 16.7%.
o One hundred and twenty-six influenza positive detections were recorded through the DataMart scheme (ninety-five
influenza A(H1N1)pdm09, twenty-two A(not subtyped), two A(H3) and seven influenza B). A positivity of 11.1% was
seen in week 53, an increase from 8.3% seen in week 52, with the highest positivity in 5-14 year olds (19.2%). This is
above the threshold for 2015/16 season of 7.4%.
 Vaccination
o Up to week 53 2015 in 93.1% GP practices reporting weekly to Immform, the provisional proportion of people in
England who had received the 2015/16 influenza vaccine in targeted groups was as follows: 43.6% in under 65 years in
a clinical risk group, 41.4% in pregnant women, 70.0% in 65+ year olds, 34.1% in all 2 year olds, 35.9% in all 3 year
olds and 28.9% in all 4 year olds.
o Provisional data from the second monthly collection of influenza vaccine uptake by frontline healthcare workers show
44.1% were vaccinated by 30 November 2015 from 97.0% of Trusts, compared to 48.2% vaccinated in the previous

 Influenza-confirmed hospitalisations
o Fifty-one new admissions to ICU/HDU with confirmed influenza (twenty-seven influenza A(H1N1)pdm09, twenty-two

 GP consultation rates for combined flu and flu-like illness (flu/FLI) remain below the 2015/16 pre-epidemic Northern Ireland threshold1 at 30.5 per 100,000 population in week 2, 2016.
 The OOH consultation rate for flu/FLI has remained stable at 8.5 per 100,000 population overall, with increases in some age groups.
 RSV activity has further decreased in week 2 and is lower than the same period during last season. 
There were ten admissions to ICU with confirmed influenza reported in week 2, 2016.
 There were three deaths in ICU patients with laboratory confirmed influenza reported in week 2, 2016.
 In week 2, 2016 significant all-cause excess mortality was reported through the EuroMOMO algorithm.
 There were no confirmed influenza outbreaks reported to PHA in week 2, 2016.

Comment
Data are collected on laboratory confirmed influenza patients and deaths in critical care (level 2
and level 3).
During week 2, there were ten admissions to ICU confirmed with influenza reported to the PHA -
six with influenza A (H1N1)pdm09 and four with influenza A untyped (typing awaited). There
have been a total of 27 admissions to ICU with confirmed influenza reported this season to date,
of which 21 have been confirmed as influenza A (H1N1)pdm09 and 6 as influenza A untyped
(typing awaited) (Figure 9).
Up to week 2, 2016, of the 27 ICU patients with confirmed influenza 18 had co-morbidities.
Provisional data shows that 16 of the 27 cases met the criteria for influenza vaccine and 5 had
received it (Table 4).
There were three deaths in ICU patients with laboratory confirmed influenza reported since the
last bulletin. To date, there have been 6 deaths in ICU patients with laboratory confirmed
influenza.

Mortality Data
Weekly mortality data is provided from Northern Ireland Statistics and Research Agency. The
data relates to the number of deaths from selected respiratory infections (some of which may be
attributable to influenza, and other respiratory infections or complications thereof) registered
each week in Northern Ireland. This is not necessarily the same as the number of deaths
occurring in that period. Searches of the medical certificates of the cause of death are performed
using a number of keywords that could be associated with influenza (bronchiolitis, bronchitis,
influenza and pneumonia). Death registrations containing these keywords are presented as a
proportion of all registered deaths.

Comment
During week 2, the proportion of registered deaths from specific respiratory infections decreased
to 29% from 32% in week 1.
In week 2, there were 348 registered deaths, of which 100 related to specific respiratory
infections (29%). The proportion of deaths attributed to specific respiratory infections is lower
than the same period in both 2014/15 and 2013/14.


EuroMOMO
Significant excess all-cause mortality was reported for week 2 in Northern Ireland. Significant
excess all-cause mortality has also been reported retrospectively for weeks 52, 53 and 1.
Therefore including the previous report of excess all-cause mortality in week 49, excess allcause
mortality has been reported in five weeks of this influenza season.

Summary

In week 04 2016 (ending 31 January 2016), influenza indicators continued to increase across surveillance schemes,

to hospitals and ICU. The Department of Health has issued an alert on the prescription of antiviral medicines to the
health service. Updated guidance on antiviral prescribing in secondary care when influenza A(H1N1)pdm09 is the

 Community influenza surveillance
o During week 04, there were further small increases in GP consultations for influenza-like illness and NHS 111 cold/flu
calls.
o Fifty-one new acute respiratory outbreaks have been reported in the past 7 days. Thirty-three outbreaks were in schools
where two tested positive for influenza A(H1N1)pdm09. Six outbreaks were from care homes where one tested positive
for influenza A(H1N1)pdm09 and another one for influenza A(not subtyped). Six outbreaks were from hospitals where
one tested positive for influenza A(H1N1)pdm09 and one for influenza A(not subtyped).
 Overall weekly influenza GP consultation rates across the UK
o In week 04, overall weekly influenza-like illness (ILI) GP consultation rate has increased and is above the baseline
threshold in England (19.7 per 100,000). ILI rates have increased in Wales (16.5 per 100,000), Scotland (16.2 per
100,000) and Northern Ireland (31.7 per 100,000).
o Through the GP In Hours surveillance system, weekly ILI rates have increased but remain within seasonally expected
levels in week 04.
 Influenza-confirmed hospitalisations
o One hundred and twelve new admissions to ICU/HDU with confirmed influenza (fifty-two influenza A(H1N1)pdm09,fiftynine
influenza A(unknown subtype) and one influenza B) were reported through the USISS mandatory ICU/HDU
surveillance scheme across the UK (137 NHS Trusts in England) in week 04, a rate of 0.23 per 100,000 compared to
0.19 per 100,000 in week 03. Eight new confirmed influenza deaths were also reported through this scheme.
o Eighty-eight new hospitalised confirmed influenza cases (fifty-five influenza A(H1N1)pdm09, three influenza A(H3N2),
twenty-six influenza A(unknown subtype) and four influenza B) were reported through the USISS sentinel hospital
network across England (23 NHS Trusts), a rate of 0.95 compared to 0.89 per 100,000 the previous week.
o Since week 40, twenty confirmed influenza admissions have been reported (fifteen influenza A(H1N1)pdm09 and five
influenza A(unknown subtype) from the six Severe Respiratory Failure centres in the UK.
 All-cause mortality data
o Up to week 04 2016 in England, excess mortality by date of death was seen in <5 year olds and 5-14 year olds in week
51 and in 15-64 year olds in week 50, 52 & 53 with the EuroMoMo algorithm. In the devolved administrations,
significant excess was seen in Northern Ireland in week 04.
No excess was seen in Wales and Scotland in week 04.
 Microbiological surveillance
o Sixty-three samples tested positive for influenza (52 A(H1N1)pdm09, 2 A(H3), 4 A(untyped) and 5 B) through GP
sentinel schemes across the UK, with an overall positivity of 32.6%.
o Two hundred and ninety influenza positive detections were recorded through the DataMart scheme (two hundred and
fifteen A(H1N1)pdm09, three A(H3), fifty-four A(not subtyped) and eighteen influenza B). A positivity of 18.9% was seen
in week 04, compared to 13.8% in week 03, with the highest positivity in 15-44 year olds (25.7%). This is above the allage
threshold for 2015/16 season of 7.4%.
 Vaccination
o Up to week 04 2016 in 87.1% GP practices reporting weekly to Immform, the provisional proportion of people in
England who had received the 2015/16 influenza vaccine in targeted groups was as follows: 45.3% in under 65 years in
a clinical risk group, 42.4% in pregnant women, 71.2% in 65+ year olds, 35.9% in all 2 year olds, 38.1% in all 3 year
olds and 30.3% in all 4 year olds.
o Provisional data from the third monthly collection of influenza vaccine uptake by frontline healthcare workers show
47.6% were vaccinated by 31 December 2015 from 95.4% of Trusts, compared to 52.6% vaccinated in the previous
season by 31 December 2014. The report is available here.
o Provisional data from the third monthly collection of influenza vaccine uptake children of school years 1 and 2 age show
the proportion of children in England who received the 2015/16 live attenuated intranasal vaccine (LAIV) from 1
September 2015 to 31 December 2015 was as follows: 50.6% in children school year 1 age (5-6 years) and 49.3% in
children school year 2 age (6-7 years).
o Provisional data from the third monthly collection of influenza vaccine uptake in GP patients up to 31 November 2015
has been published. The report provides uptake at national, area team and CCG level.
 International situation
o Globally, influenza activity has increased in some temperate countries of the Northern hemisphere. High levels of
influenza activity have been reported from some of the Western Asia countries.
o Influenza activity in Europe continues to increase.

o In week 05, overall weekly influenza-like illness (ILI) GP consultation rate has increased and is above the baseline threshold in
England (21.5 per 100,000). ILI rates have decreased in Wales (13.1 per 100,000) and Northern Ireland (22.5 per 100,000) and
increased in Scotland (19.2 per 100,000).
o Through the GP In Hours surveillance system, weekly ILI rates have increased but remain within seasonally expected levels in
week 05.
 Influenza-confirmed hospitalisations
o One hundred and forty-eight new admissions to ICU/HDU with confirmed influenza (seventy-three influenza
A(H1N1)pdm09,seventy-three influenza A(unknown subtype), one influenza A(H3N2) and one influenza B) were reported
through the USISS mandatory ICU/HDU surveillance scheme across the UK (145 NHS Trusts in England) in week 05, a rate of
0.31 per 100,000 compared to 0.23 per 100,000 in week 04. Thirteen new confirmed influenza deaths were also reported
through this scheme.
o One hundred and sixty-four new hospitalised confirmed influenza cases (one hundred and twenty-three influenza

 Community influenza surveillance
o During week 08, GP influenza-like consultations reported through the syndomic surveillance system, remained stable.
o Thirty-two new acute respiratory outbreaks have been reported in the past 7 days. Six outbreaks were in schools where one
tested positive for influenza A(H1N1)pdm09. Twelve outbreaks were from care homes where one tested positive for influenza
A(H1N1)pdm09. Ten outbreaks were from hospitals where three tested positive for influenza A(H1N1)pdm09, three tested
positive for influenza A(not subtyped), one tested positive for influenza B and two tested positive for RSV. The remaining four
outbreaks were from other settings where no test results were available.
 Overall weekly influenza GP consultation rates across the UK
o In week 08, overall weekly influenza-like illness (ILI) GP consultation rate is similar and remains above the baseline threshold in
England (20.1 per 100,000). In the devolved administrations, ILI rate for Wales remained similar (12.3 per 100,000), in Scotland
a decrease in ILI rate was noted (21.7 per 100,000) and an increase was noted in Northern Ireland (37.5 per 100,000).
o Through the GP In Hours surveillance system, weekly ILI rates have remained stable in week 08.
 Influenza-confirmed hospitalisations
o One hundred and forty-four new admissions to ICU/HDU with confirmed influenza (ninety-two influenza A(H1N1)pdm09, fortyeight
influenza A(unknown subtype) , two influenza A(H3N2) and two influenza B) were reported through the USISS mandatory
ICU/HDU surveillance scheme across the UK (127 NHS Trusts in England) in week 08, a rate of 0.34 per 100,000, the same
rate as in week 07. Eleven new confirmed influenza deaths were also reported through this scheme.
o One hundred and fifty-seven new hospitalised confirmed influenza cases (one hundred and eight influenza A(H1N1)pdm09,
thirty-one influenza A(unknown subtype) and eighteen influenza B) were reported through the USISS sentinel hospital network
across England (23 NHS Trusts), a rate of 1.67 compared to 1.88 per 100,000 the previous week.
o Since week 40,forty-one confirmed influenza admissions have been reported (thirty-four influenza A(H1N1)pdm09, six influenza
A(unknown subtype) and one influenza B) from the six Severe Respiratory Failure centres in the UK.
 All-cause mortality data
o Up to week 08 2016 in England, excess mortality by date of death has been seen in 15-64 year olds in weeks 52-03 and 05-
08;
in <5 year olds in weeks 51 and 05, and 5-14 year olds in week 51 with the EuroMoMo algorithm. In the devolved
administrations, no significant excess was seen in week 08 2016.
 Microbiological surveillance
o Sixty samples tested positive for influenza (44 influenza A(H1N1)pdm09, 6 influenza A(untyped) and 10 influenza B) through
GP sentinel schemes across the UK, with an overall positivity of 33.7%, compared to 33.3% in previous week.
o Four hundred and fifty-five influenza positive detections were recorded through the DataMart scheme (two hundred and
seventy-two A(H1N1)pdm09, five A(H3), one hundred and twenty-nine A(not subtyped) and forty-nine influenza B). A positivity
of 23.2% was seen in week 08, compared to 23.1% in week 07, with the highest positivity in 15-44 year olds (29.0%). This is
above the all-age threshold for 2015/16 season of 7.4%.
 Vaccination
o Up to week 04 2016 (31 January 2016) in 98.8% GP practices reporting weekly to Immform, the provisional proportion of
people in England who had received the 2015/16 influenza vaccine in targeted groups was as follows: 45.1% in under 65
years in a clinical risk group, 42.3% in pregnant women, 71.0% in 65+ year olds, 35.4% in all 2 year olds, 37.7% in all 3 year
olds and 30.1% in all 4 year olds.

Comment: I'm not sure what is happening to the fatality totals these past two weeks. Often the total fatalities given is greater than the sum of the previous week and the additional confirmed in the current week. I assume that this is because late confirmations of fatalities from earlier in the season get added to the total. But the past two weeks the numbers just don't add up. Week 10 reported a total of 127 deaths. Week 11 added 15 deaths, but only raised the total to 139, an increase of 12. Week 12 added 12 new deaths, but did not raise the total at all. - Ro
