Методика ШВЛ у дорослих: SIMV, вентиляція яка контролюється за тиском, хвилинний об'єм - якщо є можливість легка гіпервентиляція, РаСО2 33-35 мм.рт. ст., якщо немає можливості моніторингу РаСО2 виходити з потреби хворого (почуття дихального комфорту, відсутність боротьби з дихальним респіратором), орієнтовно для хворого вагою 70 кг - 10-12 л/хв.; дихальний об'єм для дорослих 5-7 мл/кг; для дітей 4-5 мл/кг; позитивний тиск на видоху 8-10 см.вод.ст.; для дітей 4-6 см.вод.ст; якщо є можливість контролю підтримувати тиск плато до 30 см.водн.ст (для дорослих). Протягом години після переведення на ШВЛ концентрація кисню до 100% , після години до 50% (за винятками тяжкої гіпоксії та гіпоксемії, які не корегуються іншими методами); у випадках рефрактерної гіпоксемії не зважаючи на FіО2 0,5 зміна положення хворого на живіт тривалістю на 1-2 години; у випадках рефрактерної гіпоксемії - рекротуючий маневр (роздуття легень) та інвертований режим ШВЛ (співвідношення вдих до видиху 1:1). Протипоказанням до переводу на живіт є артеріальна гіпотензія. За наявністю відповідного апарату ШВЛ - перед інтубацією та перед переводом на самостійне дихання - застосування неінвазивного методу ШВЛ. Хворих з тяжкою легеневою недостатністю (окрім дітей), у яких ШВЛ більше 3 діб - трахеостомія. Систематичний рентгенологічний контроль, фібробронхоскопія (систематична), посів харкотиння з метою визначення чутливості збудника до антибіотиків, у випадках тяжкої бактеріальної пневмонії - внутрішньотрахеальне систематичне введення антибіотиків до яких чутливий збудник. Режим седації (пропопофол, оксибутират натрію, сібазон) - неадекватна поведінка хворого, боротьба з апаратом, відсутність адаптації дихання хворого до апарату ШВЛ, у випадках коли за допомогою седативних препаратів не вдається адаптувати дихання хворого до апарату - застосовувати м'язові релаксанти, ардуан, фентанил, при переводі на самостійне дихання - застосовувати режими допоміжного дихання, режим СРАР та високочастотної вентиляції.

Ukraine closes all schools to fight swine flu

KIEV, Ukraine ? Urging its citizens not to panic, Ukraine on Monday closed down all schools nationwide for a week to avoid the spread of swine flu and suggested that nightclubs, cinemas and food markets in the west also shut down.

The World Health Organization said Monday there was no evidence that Ukraine had a bad outbreak of swine flu but at the government's request it had sent a health team in to help the country cope.

"But this is not an indication that the situation is severe," said WHO spokeswoman Liuba Negru. "The information we have gotten (from the government), we have to double-check it and make sure it is real, evidence-based information."

Ukraine's Health Ministry said Monday that 67 people in the nation of 40 million have died of flu, but did not say how many of those deaths were related to swine flu. Worldwide, outbreaks of regular seasonal flu claim 50,000 lives each year.

Nevertheless, all schools have been closed for a week across Ukraine, even in the capital, Kiev, where there have been no confirmed cases of swine flu.

In western Ukraine, local authorities advised people to travel only when necessary, a Health Ministry spokeswoman said.

All outdoor markets have been closed in the western region of Lviv, where the governor also urged cinemas, cafes, nightclubs and theaters to shut down until further notice.

Some observers, including the speaker of the parliament, Vladimir Litvin, suggested that these measures are the result of political wrangling ahead of the country's presidential election in January. The pivotal vote could overturn the 2004 Orange Revolution that swept a pro-Western government to power.

"We are seeing a political competition to see who will be the first to lead this process (of fighting swine flu)," Litvin said, according to the UNIAN news agency.

Prime Minister Yulia Tymoshenko met a Swiss shipment of anti-viral drugs at the Kiev airport on Monday.

"The government has declared the situation an epidemic, but there is absolutely no need to panic," she declared on national television.

Her main rival, President Viktor Yushchenko, said thousands were infected and called for assistance from NATO, the European Commission, the United States, Russia and other countries.

Konstantin Bondarenko, director of the Gorshenin Institute, a political consultancy, said that Tymoshenko has the most to lose from public sentiment over the outbreak, as state health officials answer to her.

"Right now all the candidates are weighing their political options, looking around for a theme, and this is a very hot topic right now. The panic is there, and they are acting on it," Bondarenko said.

After receiving the shipment of 300,000 doses of Tamiflu at Kiev's Borispol airport, Tymoshenko said her government plans to increase its hoard of the drug by another 300,000 to 950,000 doses.

"This is the supply that will reliably protect Ukraine," Tymoshenko said, ITAR-Tass news agency reported.

Viktor Yanukovych, the Regions' Party candidate for the presidency, has not yet commented on the swine flu uproar. Yanukovych, who was beaten in 2005 by Yushchenko, is leading in the polls with a platform that emphasizes closer ties with Russia.

During the past five years of Yushchenko's presidency, relations with Moscow reached historic lows. Yushchenko's approval ratings at home have fallen to single digits in the wake of the economic crisis, which hit Ukraine hard, and years of political gridlock with Tymoshenko.

KIEV -- Reports from Ukraine are saying that the country, which has been hit with 190,000 swine flu cases to date, is not ready for the epidemic.

The H1N1 virus could also become a political issue ahead of the January presidential elections.

President Viktor Yushchenko has asked for emergency assistance from the United States, EU, NATO and neighboring countries.

On Monday, the Ministry of Health in Kiev explained that only 22 of 67 victims of respiratory infections were in fact infected with swine flu.

Ukrainian Prime Minister Yulia Tymoshenko stated that the "people were dying from common flu and respiratory infections".

The neighboring countries have responded to the Ukrainian government appeal's for help.

Additionally, a Swiss plane carrying 16 tons of antivirus medication landed at the airport in Kiev last night.

?There are 300,000 units of Tamiflu, which means that 300,000 people will receive emergency medical help. It is also the amount that will meet the daily needs in all of Ukraine,? said the prime minister.

Tymoshenko also stated that her country would buy the same amount of Tamiflu for USD 8,7 per package, but added the medication would not be distributed via pharmacies, because some of them took advantage of the situation and were selling the drug at ten times its original price.

Meanwhile, reports said that half of the 190,000 swine flu cases were children. The Ministry of Health stated that four doctors succumbed to the disease.

The epidemic broke out last week in the western part of the country and the government reacted by closing off nine regions.

The state of emergency has not been introduced yet, however, but school children have been sent home for three weeks.

All mass gatherings have been canceled, including concerts, and movie shows, while traveling from one region to another has been restricted.

Tymoshenko suspended her election campaign two days ago, but she in the meantime stated that the epidemic would not jeopardize the holding of the presidential elections.

I would reminder to readers that during Hong Kong summer wave of pandemic H1N1 2009 influenza, a lot of health care workers fell ill with flu but promptly treated recoverd quickly.

I think that comprehensive evaluation of Hong Kong experience in managing influenza epidemic should be seen as prototypical for other regions and country, even those in short supplies for drugs or vaccines.

Deaths in Ukraine fuel swine flu fears

KIEV ? Ukraine raised its death toll from flu and respiratory problems on Monday, while Kiev's mayor suggested a Champions League match should be played without spectators amid a swine flu scare.

Kiev had made an urgent appeal to world powers for help battling the spread of the A(H1N1) virus on Sunday, after declaring 60 people had died of respiratory complications and flu.

The health ministry put this figure at 67 on Monday, without detailing when the deaths took place or explaining the jump in the toll.

"Sixty-seven people have died," deputy health minister Vasyl Lazoryshynets said, adding that this figure "did not differ much" from previous years.

He said 255,000 cases of flu and acute respiratory illness had been registered among the country's population of 46 million. Of those 15,000 people were hospitalised, he said.

The Ukrainian government's announcement last week that it was implementing the toughest measures yet in Europe to combat swine flu led to panic, sparking a run on pharmacies.

Authorities in Ukraine's eastern Donetsk region have ordered employees of medical centers, shopping centres and public transport to wear masks over the next three days, Interfax news agency reported Monday.

Kiev's mayor meanwhile suggested the Champions League match of Inter Milan and Dynamo Kiev later in the week should take place in a stadium empty of football fans to prevent contagion.

According to the health ministry, only 22 cases, including one death, have been confirmed as the A(H1N1) virus, widely known as swine flu. The country's National Security Council however puts the number of dead at four.

Alarm over swine flu has become a political issue in Ukraine as the country gears up for January's presidential election, with each candidate striving to take the lead in managing the health scare.

Prime Minister Yulia Tymoshenko ordered a three-week shutdown of schools and universities, while her rival, President Viktor Yushchenko, asked the United States, the European Union and NATO for drugs and equipment to fight the virus' spread.

Nov. 2 (Bloomberg) -- The World Health Organization sent a team of experts to Ukraine today to investigate an outbreak of respiratory disease that?s sickened a quarter of a million people and left pharmacies without masks or flu remedies.

A group of epidemiologists, physicians, laboratory technicians and communications advisers is scheduled to arrive tonight in Kiev, Gregory Hartl, a WHO spokesman in Geneva, said in a telephone interview today.

Ukraine faces an outbreak of flu-like illness that?s killed at least 67 people and infected 255,000, according to the country?s first deputy health minister Vasyl Lazoryshynets. About 22 patients tested positive for swine flu, Lazoryshynets said. It?s ?difficult to tell? whether the pandemic H1N1 virus is responsible for all the cases, according to Hartl.

?There are a lot of unknowns,? he said. The crew of experts will collect samples from patients and send them to the WHO?s influenza collaborating center in London for diagnosis. The WHO may have more information on Nov. 4, Hartl said.

Ukraine?s government has closed schools and banned public events. In four of Kiev?s 10 districts, a majority of drugstores posted handwritten signs in their windows that read ?no masks.? A central information line for the city?s drugstores said yesterday most stores had run out of protective masks or flu and cold remedies such as paracetamol, called acetaminophen in the U.S., and new supplies were expected later in the week.

Onions, Garlic and Lemons

The streets of Lviv, the city near the Polish border that?s reported the most cases, were empty yesterday, with museums, restaurants and stores closed, said Halyna Shymanska, a 47-year- old doctor. Shymanska, who runs a private ultrasound diagnostics office in nearby Truskavets, said pharmacies have run out of masks as well as antivirals and basic flu medicines.

?People are relying on folk remedies like onion and garlic,? she said in a telephone interview.

Garlic and lemons, also used as a flu remedy, are now scarce in the town of Cherasky, in central Ukraine. The price of a kilogram (2.2 pounds) of lemons there has quadrupled to 50 hryvnia ($6.10) in the past week.

About 15,000 people are being treated at hospitals across Ukraine, Lazoryshynets said at a press conference in Kiev today.

Ukraine has asked the U.S., the European Union, NATO and neighbors for anti-flu drugs. Poland and Slovakia sent protective masks and Roche Holding AG?s drug Tamiflu to Ukraine after President Viktor Yushchenko said the country couldn?t fight an outbreak of pandemic influenza alone, according to a statement from the government.

?A Bit of a Flap?

H1N1, a new flu strain that?s evolved in pigs, humans and birds, has sickened at least 440,000 people and killed over 5,700 since it was discovered in Mexico and the U.S. in April, the World Health Organization said last week. The WHO estimates seasonal flu causes up to 500,000 deaths a year.

?It?s a bit like Mexico in the beginning,? said John Oxford, professor of virology at Queen Mary?s School of Medicine and Dentistry in London, adding that Ukrainian authorities may not have the scientific resources they need to observe the outbreak closely. ?One?s first reaction is that not due to any fault of their own, they?re getting into a bit of a flap.?

It?s unlikely that the virus has mutated and become more deadly, according to Oxford. ?It doesn?t fit with the experience of other countries at the moment, so why should the Ukraine be different,? he said.

An analysis of early data on the outbreak suggests severe cases and deaths in Ukraine occurred among adults aged between the ages of 20 and 50, according to the Geneva-based WHO.

The U.K. Health Protection Agency estimated on Oct. 29 that about 521,000 of the country?s 61 million residents have gotten the virus since the pandemic began, and 137 of them have died. That?s a death rate of 0.03 percent, similar to the one Ukraine?s numbers indicate.

Across the Black Sea, Bulgaria also said it was hit by a flu epidemic, closed schools and banned gatherings, the Sofia News Agency reported, citing Angel Kunchev, head of the health ministry?s department of control of communicable diseases.

The U.S. Embassy is aware of the disease outbreak being widely reported in Western Ukraine. According to press reports, the Ministry of Health has confirmed that the disease in question is 2009-H1N1 influenza, sometimes referred to as novel H1N1 or swine flu. The Embassy is monitoring the situation and remains in contact with Ukrainian public health authorities responding to it. According to the press, Ukrainian authorities have put several precautionary measures in place. For example, the Embassy understands that the current school vacation will be extended for three weeks; authorities have asked that public gatherings not take place; and pregnant women are being advised to defer non-emergency visits to hospitals where patients suffering from flu-like symptoms are being treated. Additionally, some press reports have said that restrictions limiting the "unnecessary" movement between regions will be in place for the next three weeks, although no details of how officials will determine the urgency of travel have been released. Individuals reporting flu-like symptoms are being advised to limit contact with other people, and, in some cases, are being treated in isolation units at local hospitals. However, public health officials have told the Embassy that as of October 30, no general quarantine is in effect.


If you are quarantined or placed in isolation due to 2009-H1N1 influenza, please contact the Embassy at the number below and let us know, so we can monitor your situation. However, please be aware that the Embassy cannot demand your immediate release if you have been detained or quarantined in accordance with local public health and legal authorities.


Due to legal restrictions and a lack of sufficient resources, the Embassy is not in a position to provide private citizens with pandemic-related supplies, medications or medical treatment, including vaccines, and cannot provide specific medical advice. Questions and concerns about influenza or other illnesses should be directed to a medical professional. On 28 Oct 2009, the Ministry of Health of Ukraine informed WHO, through its
Country Office in Ukraine, about an unusually high level of activity of
acute respiratory illness in the western part of the country, associated
with an increased number of hospital admissions and fatalities.

On 30 Oct 2009, the Ministry of Health of Ukraine announced the
confirmation of pandemic (H1N1) 2009 virus infection by RT-PCR in 11 out of
30 samples obtained from patients presenting with acute respiratory illness
in 2 of the most affected regions. Tests were performed in 2 laboratories
in Kyiv, including the National Influenza Centre. Confirmatory tests will
be performed at one of the WHO Collaborating Centres for Influenza.

The situation is quickly changing with increasingly high levels of acute
respiratory illness (ARI)/influenza-like-illness (ILI) activity being
observed in Ternopil, Lviv, Ivano-Frankivsk, and Chernivtsi regions. The
higher levels of transmission in these regions corresponds to an increased
number of hospital admissions and fatalities associated with severe
manifestations of acute respiratory illness.

As of 30 Oct 2009, over 2300 individuals have been admitted to hospital,
including over 1100 children. 131 cases have required intensive care,
including 32 children. As of 31 Oct 2009, a total of 38 fatalities
associated with severe manifestations of ARI have been registered.
Preliminary epidemiological data analysis indicates that severe cases and
deaths primarily occur among previously healthy young adults aged 20-50
years. Fatal and severe cases are reported to have sought medical attention
5 to 7 days after onset of symptoms.

International experience of the (H1N1) 2009 pandemic to date, especially
from the Southern Hemisphere, has shown that poor clinical outcomes are
associated with delays in seeking health care and limited access to
supportive care. In addition, this virus has also shown its ability to
cause rapidly progressive overwhelming lung disease, which is very
difficult to treat.

Public health measures recommended by the Ministry of Health of Ukraine
across the entire country include: social distancing (school closures and
cancellation of mass gatherings); enhancement of surveillance activities;
increased respiratory hygiene; and continuation of the vaccination campaign
against seasonal influenza targeting at-risk groups.

The Government of Ukraine has activated coordination mechanisms to respond
to the rapidly evolving situation, including the harmonization of response
plans across all administrative levels. In response to the request from the
minister of health of Ukraine, WHO is deploying a multidisciplinary team of
experts to assist national authorities in mitigating the impact of the
pandemic. The team comprises the following expertise: health emergencies
coordination, case management, epidemiology, laboratory diagnostics,
logistics, and media/risk communications.

As per WHO's communication in May 2009, there is no rationale for travel
restrictions, because such measures will not prevent the spread of the
disease. Travellers can protect themselves and others by following simple
recommendations aimed at preventing the spread of infection such as
attention to respiratory hygiene. Individuals who are ill should delay
travel plans, and returning travellers who fall ill should seek appropriate
medical care. These recommendations are prudent measures which can limit
the spread of many communicable diseases and not only the pandemic (H1N1)
2009 virus.

[This is confirmation by WHO that the outbreak of acute respiratory illness
throughout Ukraine is an extension of the global influenza (H1N1) 2009
pandemic. So far in Ukraine, 11 of 30 patient samples have tested positive
for influenza pandemic (H1N1) 2009 virus. Confirmatory tests are in
progress in another WHO Collaborating Centre. As of 30 Oct 2009, a total of
2300 people (including 1100 children) have been hospitalised, and as of 31
Oct 2009, there have been 38 fatalities. The Ukrainian authorities seem to
have been unprepared for an outbreak on this scale, and WHO involvement in
the control of the outbreak will be considerable. No travel restrictions
are envisaged.

Chief Medical Officer Ukraine did not confirm the swine flu epidemic


KIEV, November 3. In Ukraine, the epidemic is not only "swine", as well as normal influenza, as well as OCR. This, according to "censor", said chief medical officer Alexander Bilovol.

"The epidemic of influenza and acute respiratory disease observed in the western regions, - said the chief medical officer in the air channel ICTV. - Especially in the 11 regions, where almost 15% exceeded the threshold.

According to him, in other regions, there is the previous level indicators that have been in 2007-2008.

Chief sanitary doctor said that as a result of the epidemic, according to latest figures, 71 people died and has 14 laboratory confirmed cases of influenza A/H1N1.

"The three people who died, identified influenza A/H1N1?, - added Bilovol.

He also said that "100% verification of this diagnosis in Ukraine give two laboratories that are equipped with modern equipment - is a central sanitary and epidemiological stations and Institute Gromashevsky.

At the same time on October 30, Minister of Health of Ukraine Vasyl Knyazevich said that the country's outbreak of highly influenza A/H1N1.

The same day, Prime Minister Yulia Tymoshenko has imposed quarantine in 9 regions of Ukraine and for 3 weeks canceled all public events in the State, as well as classes in all schools.
