The reported timing below is not consistent with Ebola or Marburg infection; if the entire outbreak has taken place over the past 2-3 days, there is not time for generations of Ebola cases.

A strange disease has broken out in West Nile region claiming 4 lives and sending 64 to hospitals. Obongi County MP [Member of Parliament] Fungaroo Caps, while addressing the press at Parliament today [28 Apr 2014], said the disease has symptoms of Ebola and cholera though not yet confirmed.

"It is a contagious disease which kills very fast. Moyo district is the most affected, and particularly Obongi county," Fungaroo added. The health centre of Obongi is already congested as more patients continue to flock in. "Tents are immediately required since patients are on the open veranda."

The disease reportedly broke out on Saturday [26 Apr 2014], and the Ministry of Health was alerted, but nothing has been done so far. "We call upon all charity organizations and other NGOs to quickly help us," Fungaroo appealed. He further observed that there is no proper treatment being given to the soaring numbers of patients apart from simple dehydration interventions.

"Women are the most affected, especially in Aliba and Imara sub-counties. Worse still, there is no ambulance on the ground."

People are being transported on boda bodas [motorcycle taxis] instead, which has caused more infections [of the drivers].

The WHO Factsheets say:
- Cholera has a short incubation period, from less than one day to 5 days, and produces an enterotoxin that causes a copious, painless, watery diarrhoea that can quickly lead to severe dehydration and death if treatment is not promptly given. Vomiting also occurs in most patients.

- Ebolavirus disease (EVD) has an incubation period of from 2-21 days. It is a severe acute viral illness often characterized by the sudden onset of fever, intense weakness, muscle pain, headache, and sore throat. This is followed by vomiting, diarrhoea, rash, impaired kidney and liver function, and in some cases, both internal and external bleeding.

The difference in the diarrhoea is that with cholera it is watery, clear and much more frequent than with EVD.

The Uganda Virus Research Institute (UVRI) -- where I worked 50 years ago, when it was the East African Virus Research Institute -- has the reagents to test for both ebolaviruses and Marburg virus. But West Nile district is quite remote from the capital, Kampala and the UVRI. The West Nile district (now a region containing several districts) was where the 1st isolation of West Nile virus (WNV) was made from a fever patient. WNV causes a fever with other symptoms such as headache, body aches, joint pains, vomiting, diarrhea, or rash. Cholera and Ebola do not cause joint pains.

We would be glad to receive any follow-up reports. We received follow-up to only one of the 5 outbreaks listed below; we would still like to know what those were due to. - Mod.JW

Health experts head to Moyo as strange disease kills four

Whereas the Moyo District health team earlier on suspected a cholera or Ebola outbreak, the Ministry of Health yesterday ruled out the two and said they were conducting more investigations.

“We have taken samples from the patients and the tests are being handled by the national analytical laboratory for further confirmation but since it takes 24 hours to establish the problem, we are waiting for the final results,” said Ms Rukia Nakamatte, the Ministry of Health publicist.

She said that the ministry is sending another team today to collect more samples.

“If it was Ebola, there should have been blood oozing out of the patients since it’s a hemorrhagic fever but this has not been the case,” Ms Nakamatte said.

By press time yesterday, one more person had died bringing the number of the dead to four and 40 others admitted plus the 24 that were admitted over the weekend.

The dead and those that were admitted had symptoms of diarrhea and severe fever.

Hemorrhages are not Always a feature of Ebola Virus Disease (EVD): the condition name has been changed accordingly.

In fact, recent outbreak in W Africa showed that some of the patients affected by EVD do not develop hemorrhages at all, but have notwithstandig very high fever, diarrhea and vomit, as well as rapid progressive multiple organ failures.

For this reason, a laboratory testing on samples collected from these patients is needed to clarify the nature of the disease.

According to Pathfinder's article, cholera and Ebola have now both been ruled out through testing.

The timing on this outbreak is more consistent with a single exposure event, rather than multiple generations of H2H spread.

Moyo strange illness cases rise to 88

Moyo- The number of people affected by a strange illness in Moyo District has continued to rise, further complicating the task by health officials in discovering the actual disease.

By yesterday morning, the number of those admitted to Obongi Health Centre IV had reached 88, with more cases continuing to be reported.

Four people have so far died of the disease which manifests with symptoms of diarrhoea and vomiting.

The number of people hospitalized following an outbreak of cholera in the West Nile districts of Moyo and Adjumani is now over 92, health officials have said.

And the infection, which is mainly a sanitation problem, has caused the death of one more person, Lumisu Angelo, 45, bringing the number of the dead to 5.

Dr. Dominic Lomureju, the Moyo district health inspector, said the number of admissions at the Obongi Health Centre IV (the main health facility containing the epidemic) were now 90, with 28 cases discharged...

Ministry confirms cholera in Moyo

Kampala.

The Ministry Health has confirmed an outbreak of cholera in Moyo district that has so far killed four people and left 102 others affected.

Following confirmatory tests obtained from the Central Public Health Laboratories, the ministry in a statement said that 49 people are still admitted at Obongi Health Centre IV while 53 patients have been discharged.

The disease outbreak was first reported on April 26 after a number of patients reported at Obongi Health Centre IV with fever, abdominal pain, diarrhea and vomiting resulting in dehydration.

The outbreak is reported in Gimara and Aliba Sub-counties and West Moyo County in Lefori Sub-county.

Earlier, the disease was suspected to be Ebola but according to the Commissioner for Community Health, Dr Anthony Mbonye, blood samples from the affected persons have been tested at the Uganda Virus Research Institute and results were negative for Ebola and other related viruses as was earlier suspected.
