Some 1500 pigs were found dead over the last 3 days of causes as yet

The local animal health center said this could be a new disease with
symptoms similar to those of "plasmodium congestion" [see commentary].

[Any available data on the husbandry, breed, age, epidemiology,
clinical symptoms, pathological changes, and laboratory
investigations of the affected pig population will be welcomed.
Exclusion of highly infectious diseases, such as FMD (foot-and-mouth
disease), classical swine fever, and African swine fever, as well as
highly pathogenic PRRS (porcine reproductive and respiratory
syndrome), deserves to be prioritized.

The applied term "plasmodium congestion" as a disease entity in pigs
is neither clear nor known to this moderator. A quick Internet search
has revealed that this term is applied in Viet Nam in relation to a
disease in poultry, probably caused by _Plasmodium_ spp, but also
named "fowl cholera." A question of terminology, language
differences, otherwise?! 'Congestion', in various organs -- including
the brain -- is one of the common changes seen in cases of plasmodial
infections in chicken. Clarifications, if available, will be helpful.

There have been numerous reports of pigs dying ever since Qinghai. Different countries call the illness that is killing the farm animals by different names. On some of the pigs, tests have been run and they have tested positive for H1N1. I believe both Vietnam and Indonesia have admitted to H5N1 positive tests on pigs in their countries in years past.

The provincial People?s Committee reported Wednesday that the recent epidemic caused by the Asia 1 virus type, whose origins are yet to be traced, had spread to the five districts of Dakrong, Gio Linh, Trieu Phong, Cam Lo and Huong Hoa.
The worst-hit area was Dakrong with over 470 out of 650 animals contracting the disease.
Tests by the Animal Health Department have confirmed that the cattle have been afflicted by the Asia 1 type, and not the commonly known O type.
Early last month, 55 cows were transported from the northern province of Thanh Hoa to Quang Tri by the local Thuan Thanh Ltd. Company for a provincial poverty-elimination project.
Though the province?s Agriculture and Rural Development Department had advised the firm to quarantine the cattle for 15-21 days, the company ignored the advice and distributed the animals to farmers.
The company claims that the animals had been inoculated against foot-and-mouth and other diseases before transported to Quang Tri.
So far the provincial authority has instructed relevant agencies to quickly quarantine, sterilize infected cattle and areas, and control the outbreak.
Local inspection agencies have been tasked with investigating the case further.
Nguyen Quang Vinh, head of the provincial animal health department, said dealing with this virus type, which spreads fast, would cost hundreds of thousands of dollars for the treatment and inoculation of infected animals.
The provincial authority would allot some VND7-8 billion (US$434,000-496,000) to support affected farmers and stamp out the epidemic by July 10.
In related news, some 1,500 pigs were found dead over the last three days of causes as yet unknown in the central province of Quang Nam.
The local animal health center said this could be a new disease with symptoms similar to that of plasmodium congestion.
Foot-and-mouth is a highly contagious disease affecting animals with divided hooves, in which the animal develops ulcers in the mouth and near the hooves.:tiphat:

Local authorities in a central province are crying for help as a
rampant disease has wiped out 20 000 pigs in under 2 weeks.

The pigs have reportedly perished due to porcine reproductive and
respiratory syndrome (PRRS), also known as blue ear disease, which
broke out in Quang Nam province early this month [July 2007].

At a meeting Sunday [15 Jul 2007] with the provincial administration,
Health Minister Cao Duc Phat called for the mobilization of all
available resources to curb the outbreak. He instructed agencies
concerned to keep a close eye on epidemic-prone areas and carry out
inoculation campaigns. He also stressed the importance of preventing
the disease from spreading to other provinces.

Some 250 000 doses of blue ear vaccine would be earmarked for Quang
Nam this week [16-22 Jul 2007], said Bui Quang Anh, head of the
ministry's Animal Health Department. Sterilization equipment would
also be beefed up, he added.

Nguyen Thanh Quang, head of Quang Nam's Department of Agriculture and
Rural Development, admitted the province was in desperate need of
around VND 10 billion (around USD 626 000) to help affected breeders
who have lost their pigs to the epidemic.

Quang also admitted that monitoring the illicit slaughter and
transport of infected pigs would be difficult due to the lack of
personnel.

Insufficient financial aid for a mass cull of infected pigs was also
another hurdle, he said, pointing out that breeders in need of cash
would sell their pigs with or without the disease.

To date, blue ear has gripped 8 districts in Quang Nam and has killed
up to 4000 pigs per day.

The disease is caused by the Lelystad virus [see comment], which
infects all types of pigs, irrespective of health, breeding methods,
or size.

[Viet Nam sent to the OIE (World Organization for Animal Health) an
"immediate notification" on the PRRS epizootic on 11 Apr 2007,
indicating that the disease had started on 12 Mar 2007 and was
confirmed on 26 Mar 2007. The notification mentioned "introduction of
new live animals" and "legal movement of animals" as the sources of
infection. The 6 reported outbreaks included 372 susceptible animals,
of which 133 were clinically affected, 87 died, and 105 were
destroyed. (Apparent morbidity rate 35.75 percent, apparent mortality
rate 23.39 percent, and apparent case fatality rate 65.41 percent).

A follow-up report, sent on 20 Apr 2007, provided data on 7
additional outbreaks, including 2452 susceptible animals, of which
1140 were clinically affected, 580 died, and 184 were destroyed.
(Apparent morbidity rate 46.49 percent, apparent mortality rate 23.65
percent, and apparent case fatality rate 50.88 percent). Both

Since the summer of 2006, a disease with rather similar
characteristics has been devastating China's pig population,
eventually reported from more than 20 provinces with very serious
economic losses. The disease, initially named "pig high fever
disease", was later reported to be caused by a highly pathogenic PRRS
(blue ear disease) virus, though other possible disease agents may
have played their role. A recent paper included a detailed
description of the 2006 Chinese outbreaks and provided the results of
a whole-genome analysis of the isolated viruses, revealing that these
highly pathogenic PRRSV isolates are grouped into type II. (See

Phylogenetic analysis of PRRSV isolates from different geographical
regions worldwide, indicate the existence of 2 major genotypes: type
I representing the European prototype (Lelystad virus, LV), and type
II with a North American strain as a prototype. It will be surprising
if the Vietnamese PRRS virus is found to be of a different genotype
than the Chinese isolates, as might be concluded from the newswire
above mentioning the Lelystad virus (genotype I) as the causative
disease agent in Viet Nam. Laboratory findings and sequencing results
will be helpful.

PRRS is included in OIE's list of pig diseases; the exceptional
pathogenicity and infectivity of the virus(es) currently circulating
in China and Viet Nam, deserve international alertness.

The Vietnamese Ministry of Health has instructed local health
watchdogs to curb the spread of a deadly pig-borne disease that has
killed 2 people since early this year [2007].

The Viet Nam Institute of Tropical and Contagious Diseases reported
Wednesday [18 Jul 2007] it had admitted 21 patients with the deadly
disease, caused by the _Streptococcus suis_ bacterium, since early
2007. Of the 21, 2 have perished and 2 others were still in critical
condition, the institute said.

_S. suis_ is a bacterium found in many parts of the world where pigs
are raised. It is most prevalent in domesticated pigs, but is also
occasionally found in wild boars, horses, dogs, cats, and birds.

People who slaughter sick pigs or process and sell the contaminated
pork would be susceptible to the disease, said Nguyen Huy Nga, head
of the Ministry of Health Prevention Bureau. They usually manifest
high fever, malaise, nausea, and vomiting, followed by meningitis,
subcutaneous hemorrhage, toxic shock, and coma in severe cases.

[Note that this is an entirely different disease from the "blue ear"
epizootic that is currently decimating Vietnamese swine, and to which
no human cases have been ascribed. - Mod.JW]

[Human _S. suis_ infection is a potentially fatal zoonosis that
occurs mainly among those who slaughter infected pigs. Eating cooked
meat or organs from infected pigs poses no risk and person-to-person
spread does not seem to occur. In humans, the organism has been
reported to cause most commonly meningitis, but also toxic shock --
if the pathogen produces the appropriate toxins --, and less
commonly, endocarditis.

The Viet Nam Institute of Tropical and Contagious Diseases is a Hanoi
Hospital to which the 21 patients were admitted (see

It is not stated if the Vietnamese cases were epidemiologically
linked, or if the isolates were genotypically related.

Most cases of _S. suis_ infection have been sporadic, but a human
outbreak has been reported from China (see

Pig infection

According to the data of Iowa State University Centre for Food
Security and Public health, _S. suis_ is carried asymptomatically by
many pigs, but more virulent strains can cause serious disease. _S.
suis_ infection in pigs may cause -- in the animals -- sporadic cases
of polyarthritis or peracute meningitis and septicemia in suckling
pigs. More often, _S. suis_ causes acute meningitis, with high
mortality rates, in recently weaned and growing pigs. The symptoms
may include depression, anorexia, fever, trembling, incoordination,
opisthotonos, convulsions, and blindness. Labyrinthitis, otitis
interna, and otitis media can lead to deafness and possibly
vestibular dysfunction. In addition, _S. suis_ sometimes causes
pneumonia, endocarditis, myocarditis, or abortions in pigs.

Experts of the United Nations Food and Agriculture Organisation (FAO)
have announced that their study of porcine reproductive and
respiratory syndrome (PRRS), or green-ear disease [see comment], in
pigs in the central province of Quang Nam, Viet Nam, shows that the
green-ear virus there now is stronger than the one from the previous
epizootic in the northern province of Hai Duong in March [reportedly
starting in the northern province of Hai Duong on 12 Mar 2007,
spreading to other northern provinces, such as Hung Yen, Bac Ninh,
Son La, Thai Binh, Hanoi, and Bac Giang. See ProMED-mail

Speaking to VietNamNet, Jeffrey Gilbert, senior technical advisor of
FAO, said that when they carried out the study in Quang Nam province
FAO's experts discovered a difference between this type of virus and
those of Europe and the Americas.

"It seems that this type of virus is much stronger than those of
Europe and the Americas. It is nearly like the green-ear virus in
China. However, we need to do further research to know whether this
type is similar to ones in China or in other countries," said Mr.
Gilbert.

Hoang Van Nam, Vice Head of the Veterinary Agency, said that the
agency was about to send a sample of the virus that is causing
green-ear disease in pigs in Quang Nam to an international laboratory
for testing.

So far, green-ear epidemic has been reported in more than 40
communes, wards, and towns of 6 districts in Quang Nam province, with
a total of 18 000 ill pigs. More than 800 pigs have died or been
culled.

Last weekend [14-15 Jul 2007], the Minister of Agriculture and Rural
Development went to Quang Nam to instruct local agencies on how to
control the disease. The Head of the Veterinary Agency, Bui Quang
Anh, is now in the central region to directly guide [personnel] to
combat foot-and-mouth disease (FMD), green-ear, and cholera in pigs.
[This implies that the porcine population in Quang Nam is currently
affected by 3 diseases: FMD, PRRS, and hog cholera, which is
synonymous with classical swine fever (CSF). - Mod.AS]

[The enigma of the porcine disease in Viet Nam -- probably the same
epizootic plaguing China since mid-2006, initially called a
multifactorial "pig high fever disease", later "highly pathogenic
PRRS", and in North Viet Nam, bordering China, since March 2007 PRRS
(blue ear disease) -- continues to surprise, now coloured "green" (a
subjective colour interpretation by observers?). Now, it has spread
southwards to Quang Nam; its final decoding is pending.

The following clinical description of the Chinese "highly pathogenic
PRRS", derived from the recent paper by Tian et al, as cited in
ProMED-mail 20070622.2017, may be useful in comparing the Chinese and
Vietnamese disease entities:

"Initially, the "high fever" was suspected to be hog cholera or
African swine fever (ASF). The representative sick pigs had the
following common clinical symptoms: rubefaction, blood spots,
petechiae, erythematous blanching rashes, and pimples, frequently
observed in ears (see figure 2D, which can be enlarged in the on-line
paper!), mouth, nose, back, and the inner thigh. Other common
symptoms included high fever (40-42 deg C / 104-107.6 deg F),
depression, anorexia, cough, asthma, lameness, shivering, disorder in
the respiratory tract, and diarrhea. Infected pigs could be divided
into two groups: group one, which appeared fat and healthy; and group
two with thin and debilitated features, ultimately leading to death.
To our surprise, many grown pigs also died during this epidemic
period, which is unlike the case for typical PRRSV infection.

Two of the 21 have died and two others were still in critical condition, the institute said.
Streptococcus suis is a bacterium found in many parts of the world where pigs are raised. It is most prevalent in domesticated pigs, but is also occasionally found in wild boars, horses, dogs, cats and birds.
They usually manifest high fever, nausea and vomiting, followed by meningitis, subcutaneous hemorrhage, toxic shock and coma in severe cases.
Danang issues red alert
Meanwhile, Danang City has called for drastic measures to contain the PRRS epidemic that has killed over 20,000 pigs in neighboring Quang Nam province.
The Danang Municipal Administration Friday banned the slaughter, trading and transport of pigs testing positive for PRRS in epidemic-prone areas.
It instructed concerned agencies to quarantine vulnerable areas and called for a mass cull of infected pigs.
The city would provide VND10,000 (US$0.63) per kilogram to breeders who have lost their pigs to the epidemic.
The neighboring province of Quang Ngai also reported Friday that 10 local pigs have tested positive for PRRS virus.
PRRS first hit Quang Nam early this month. So far it has plagued six districts in the province, prompting authorities to declare 52 epidemic zones. Around 4,000 pigs in Quang Nam have reportedly perished per day due to the disease.
The provincial government has decided to support affected areas with VND10 million ($625) each.

HANOI (Reuters) - Two people have died in northern Vietnam from a virus that has killed thousands of pigs in recent weeks while bird flu has returned to the central region, killing hundreds of ducks, government and media reports said.
Twenty two people, most from northern areas, have been taken to a Hanoi hospital so far this year after they fell sick from the Porcine Reproductive and Respiratory Syndrome (PRRS) virus, the Vietnam News Agency said on Monday.
Two of the infected had died, it said without giving more details.
On July 14, Agriculture Minister Cao Duc Phat said more than 16,000 pigs had been infected by the virus, also known as Lelystad virus, in the central province of Quang Nam since June 25.
"The disease in Quang Nam now is very serious, it is spreading wide and risks to infect other provinces and cities," Phat said in a letter circulated to provincial authorities nationwide, a copy of which was seen by Reuters.
He said the transport of pigs and pork from infected areas must be banned to help halt the virus spread.
The syndrome was first recognized in the United States in the mid 1980s and was called "mystery swine disease."
In some other countries including Vietnam it is referred to as "blue ear disease."
People infected by the virus suffer from rapid internal hemorrhage and high fever after they eat pork from a sick pig or inhale the air near the sick swine, doctors said.
Since late June the virus has spread to neighbouring Quang Ngai province and Danang city, infecting more than 27,000 pigs, nearly 1,500 of them had died, the Animal Health Department said.
On Sunday state-run Vietnam Television said pig raisers in Quang Nam province had thrown hundreds of dead pigs into a local river, causing serious water pollution near the UNESCO-recognised tourist town of Hoi An.

BIRD FLU KILLS DUCKS

The Animal Health Department said bird flu returned to the central region killing 220 ducks at a farm province of Quang Binh last Saturday. Tests of the dead ducks showed they had the H5N1 virus, it said.
In all, bird flu infections have been detected in ducks and chickens in two northern provinces, Quang Binh province in the centre and the southern Mekong delta province of Dong Thap.
Vietnam has detected five human infections by bird flu since May, three had been cured but two others had died, the first human casualties in the Southeast Asian country since November 2005.
State media said on Monday that the Medical Military Academy would start testing the domestically produced anti-bird flu vaccines on humans from August, with the first group involving 20 volunteers.
Globally, the H5N1 virus has killed 192 people out of 318 known cases, according to the World Health Organization, while hundreds of millions of birds have died or been slaughtered.
Experts fear the virus could mutate into a form easily transmitted from person to person and sweep the world, killing millions.

By Monday, the outbreak had spread to 11 districts and towns across Quang Nam, killing over 1,400 pigs, the local animal health agency reported.
The provincial government Monday decided to give VND200 million (US$12,390) and VND10 million ($620) to each PRRS-stricken district and commune respectively. The respective funding for the districts and communes currently outside the epidemic-prone areas is VND100 million and VND5 million.
The province has also asked the Prime Minister to provide it with initial funding of VND10 billion (nearly$620,000) to fight the epidemic and redevelop pig farming after successfully curbing the disease.
Promising treatment
The veterinary agency in Quang Nam?s Thang Binh district Monday affirmed that 300 pigs infected with PRRS in Binh Trieu commune had recovered after being treated for five three to days with a treatment process proposed by the local veterinarians.

In related news, Quang Nam?s neighboring province of Quang Ngai Monday destroyed 34 pigs infected with PRRS in Tu Nghia district, raising the number of slaughtered pigs in the province to 81.
The provincial Health Animal Department said the blue ear disease first broke out in Lai Chau hamlet in Tu Nghia district, then spread to An Hoa hamlet of Nghia Trung commune in the district.
The disease is currently threatening the neighboring commune of Hanh Trung, Nghia Hanh district.
The Central Steering Committee for Bird Flu Control and the Animal Health Department in April this year announced PRRS had infected nearly 30,000 pigs in six localities in northern Vietnam.
The disease first broke out early this year in Hai Duong province, then spread to Hung Yen, Bac Ninh, Bac Giang, and Thai Binh provinces, and Hai Phong and Hanoi cities.

HANOI, July 24 (Reuters) - Vietnam must speed investigations into a pig disease that has struck 42 people killing two, the agriculture minister said, calling for urgent measures to contain the bacteria.

Cao Duc Phat told health officials at a meeting that initial assessments showed the disease caused by Streptococcus suis bacteria had spread in the country, the Vietnam Agriculture newspaper reported on Tuesday.

The bacteria emerged only recently in the country of 85 million people, infecting 22 people in the northern provinces followed by 20 in the southern region.

Animal health authorities nationwide should increase disease surveillance, work out treatment measures for pigs and publish information to raise public awareness, Phat said.

"Since the disease involving Streptococcus suis bacteria is directly related to human health and the development of the husbandry sector so we need to act urgently, without any delay," Phat was quoted as saying at the Monday meeting.

He urged officials to check if there were any links between the Porcine Reproductive and Respiratory Syndrome (PRRS) virus, also known as Lelystad virus that has infected more than 27,000 pigs in the central region, and the Streptococcus suis bacteria.

People catch the bacteria after coming into direct contact either by hand or eating pork from a sick pig, Nguyen Hong Ha deputy head of the National Institute for Infectious and Tropical Diseases, said in a Voice of Vietnam radio broadcast.

The bacteria causes rapid internal haemorrhage and high fever and can develop meningitis, septicaemia and endocarditis in the next stage leading to death or deafness if the victim survives.

Eating pig blood pudding -- a popular dish to go with rice wine in rural Vietnam areas -- is extremely dangerous due to high density of the bacteria in blood, Ha said.

"For the sake of the community and our future we should not try to sell (the sick pig), it must be destroyed," he said.

On Monday, Minister Phat said the PRRS, also known as 'blue ear disease', had been spreading fast in the central provinces of Quang Nam, Quang Ngai and Danang city partly because officials were unable to stop trading and transport of pigs. :tiphat: Christian:tiphat:

Local vets and farmers will be taught how to protect their pigs against the disease.

Mr. Anh stressed that the disease could be kept under control. The recent serious outbreak occurred in Quang Nam because the local authorities were lax in their duty and veterinaries were warned of the danger too late.

In addition, dead pigs were thrown into rivers and dams, allowing the disease to spread, because people were ignorant of the possible consequences of such an unhealthy practice.

On Tuesday the Viet Nam Institute of Tropical and Contagious Diseases reported it had admitted 20 people with the deadly pig-borne disease since the year began.
So far two have died from the pathogen responsible, a bacterium called Streptococcus suis.
