Man dies of swine flu in Lucknow

A 38-year-old man died of swine flu in this Uttar Pradesh capital Wednesday, an official said.

Ram Shanker died at the Sanjay Gandhi Post Graduate Institute of Medical Sciences (SGPGIMS), where he was admitted Tuesday.


Shanker was a native of Uttar Pradesh's Jaunpur district and had arrived in Lucknow two days ago. He was staying at his brother's place in the Jankipuram locality.


In an attempt to control the spread of the virus, a team of health officials visited the residence of Shanker's brother but could not find anyone there.


'After it came to the fore that Shanker's brother family members had left for their native district Jaunpur, we contacted the health officials in Jaunpur and asked them to screen all the family members in order to check whether they have been affected by the virus or not,' A.K. Shukla, chief medical superintendent, told reporters here.

Swine flu takes another toll

LUCKNOW: Feasible weather conditions are letting the H1N1 virus spread faster in the state. The virus took a toll of another person at Chhatrapati Shahuji Maharaj Medical University (CSMMU) on Friday.

The person, named Sandeep Pandey (27), was referred from Alapur village of Ambedkarnagar on Thursday afternoon. But, according to the doctors, swine flu infection had affected is lungs badly. "He had developed bilateral pneumonia and was extremely critical... we had put him on ventilator but the infection could not be controlled," claimed Prof SN Shankhwar, chief medical superintendent, CSMMU.

It may be noted that this is the fifth swine flu death reported in the state capital in the last fortnight. Attendants of Pandey could not accept the death of their dear one so they tried to blame the medical university. But after counselling, they took his body and proceeded to their hometown.

Chief medical officer, Lucknow Dr AK Shukla stated that except for one, all the other deaths were referred cases. Still he advised that it was better to be on the safer side if the infection was to be avoided or effectively managed. "Those on any sort of medication, especially of they are children, pregnant women and elderly, and suffer from flu like symptoms along with chest pain must consult a doctor. They may also report to the government hospital for swine flu testing," he suggested.

9 test positive for swine flu in Lucknow


LUCKNOW: Nine persons have tested positive for swine flu in the city, officials said here today.

All nine hail from Lucknow and the cases have been confirmed by the Sanjay Gandhi Post Graduate Institute of Medical sciences (SGPGI), they said.

Of the nine cases, six patients are from the SGPGI itself, while two from Command hospital and one from Balrampur hospital here, they said.

Five persons have so far died of swine flu in the state in less than a month time.

Ever since the outbreak of swine flu in March 2009, UP has reported 1,313 confirmed cases, of which 26 had died.

In Lucknow, govt doctors shy away from swine flu vaccine


The swine flu season is on in full swing but the medical community, who are at the forefront of the high risk groups ? in view of their proximity to the patients ? are anything but keen to vaccinate themselves.

Worse, the hospital that saw the most cases in the city ? the SGPGI ? is yet to receive any vaccine from the Central stock for the use of its doctors and neither has it seen a demand for the same. The staff at the SGPGI are particularly vulnerable since it is the only hospital where swine flu tests are done and the most critical patients are sent there.

Four months have passed since the state has received the Central stock of vaccine for use of the doctors and paramedical staff and most of it has reached the hospitals. But few doctors who are a part of the Rapid Response Team have vaccinated themselves.

While few doctors are willing to come on record, there appears to exist an irrational fear that the vaccine might end up infecting them. Plus, the health officials dealing with swine flu patients are mostly unaware that the vaccine is meant for them. The vaccines arrived in the state in March-end and had to be distributed by April 20. While 196 vials, each containing 10 doses of intra-muscular ?flu shot? vaccine, were given to the Chief Medical Officer of Lucknow, merely 526 doses have been used so far.

THE SGPGI

The SGPGI, which has seen 15 of the 28 cases in the city ? some of the patients are staff of the institute ? has not received any stock. ?Irrespective of the fact that the hospital has not got any vaccine from the state government, I have not received any demand of vaccine from the doctors of the institute who are dealing with swine flu,? said Dr AK Bhatt, CMS, SGPGI. ?We can procure the vaccine from our own funds but we cannot force anyone to take it.?

Siblings tested positive for H1N1 in Lucknow

Two brothers, school students of primary section, were tested positive for swine flu here, officials said today.

The siblings, studying at St Fransis school here were admitted to a hospital after they reported ill, they said adding all necessary precautions has been taken to treat them.

Till now 25 cases of swine flu and five deaths from it have been reported from the state capital.

Even since the outbreak of H1N1 virus in March 2009, UP has reported 1313 lab confirmed cases of these 26 had died so far.

Swine flu scare in city: Health dept on its toes



ALLAHABAD: Alarmed over the swine flu scare in the city, the district health officials have swung into action.

The swab sample of one suspected case reported in the city hospital has been sent to SGPGI Lucknow for test.If tested positive and in case the symptoms are mild, the patient would be given the option of seeking the treatment either at a hospital or at home.

A separate 10-bed swine flu ward has been set up at MLN district hospital, health officials said. Besides, medicines, masks and apparatus needed to tackle the disease would be made available at the hospitals within 24 hours, they added.

People can avail any information about swine flu from the office of chief medical officer. District health officials, however, said people should not panic as no swine flu case as been confirmed in the district.

District surveillance officer Dr OP Bhaskar told TOI that the health department was keeping a close watch on the situation and added that they would sensitise people on precautions to be taken to avoid catching swine flu.

The health officials said people can also visit the nearest health centres if they have any query regarding the flu or can call up the medical officer at informative/epidemic cell.

"Any person with flu like symptoms such as fever, cough, sore throat, cold, running nose etc, should go to a doctor for test," said a health official. Though no case has been confirmed in the city, people have been advised to stay away from crowded places.

Last year, 28 swine flu cases were reported in the city while one person died from swine flu.

People who have travelled from the affected cities/states recently and show symptoms of influenza A (H1N1) like fever, cough, sore throat and breathing problems should immediately contact a doctor.

3 more H1N1 cases reported from UP; toll reaches 145

LUCKNOW/ALLAHABAD/KANPUR/VARANASI: With three more cases of swine flu -- two from Lucknow and one from Allahabad detected on Friday -- the total number of persons found infected with H1N1 virus in the state has reached to 145 this year. Of this, seven victims have already lost their lives.

In Lucknow, one Chaitali Biswas (30) and Aman Akash (20) tested positive for the virus at the Sanjay Gandhi Post Graduate Institute of Medical Sciences (SGPGIMS) on Friday. A resident of Kolkata, Chaitali was on a visit to SGPGI where she went for a routine check up following symptoms of cold and fever. Akash happens to be the resident of Lucknow. Chief medical officer Dr AK Shukla said that the two have been quarantined.

In all, over a dozen patients are under close observation at SGPGI alone. This include five patients who have been admitted to the Tertiary Care Institute. Medical superintendent, SGPGIMS, Dr AK Bhatt said that the patients were being constantly monitored. Another seven patients have been quarantined and provided medicine `Tamiflu' and masks.

Meanwhile, Allahabad registered its first victim of swine flu on Friday when a 63-year-old woman hailing from Pratapgarh was found positive at SGPGI. The woman was further admitted to Army Hospital in an isolated ward where a team of doctors are treating her. She was administered with Tamiflu and the mask.

Medical officer, epidemic cell, Dr Rajendra Kumar told TOI that the woman had approached the hospital authorities three days back and doctors had sent her swab sample to SGPGI, Lucknow for test. On Friday, the woman was found positive for swine flu.

No case of swine flu has been detected from Varanasi this year. Ironically, the city, which receives a number of foreign tourists is yet to initiate screening of passengers at its airport.

"So far we have not received any instructions to start the screening at airport though sporadic cases of infectious swine flu are pouring from central and western part of the state,'' said Dr Shaukat Kamal, assistant director, National Institute of Communicable Disease (NICD), Varanasi and nodal officer for swine flu in the region while talking to TOI on Friday. Only the health staffs and paramedics involved in the control programme have been vaccinated on a precautionary note in the region, he added.

It may be mentioned here that around 20,000 air passengers of international flights were screened for suspected swine flu cases at Babatpur Airport during the period of nine months (May 2009 to January 2010).

Dr RS Verma, chief medical officer, Varanasi, claimed that as many as two isolation wards (each with 10-beds) at DDU and SPG hospitals have been reserved for swine flu cases while a stock of around 7,500 Tamiflu tablets are ready to meet any emergency situation. Likewise, as many as 45 swine flu protection kit with gloves and face masks apart from 15 sample collection kits are also available with the department, he added.

No swine flu case was reported from Kanpur, even though one suspected case of swine flu was reported in July. The patient, thankfully, tested negative.

Seven new cases of swine flu

LUCKNOW: Seven new H1N1 positive cases were reported in Lucknow during the weekend. While four of the total cases hailed from Sanjay GandhiPost Graduate Institute of Medical Sciences (SGPGIMS), two others were referred to Command Hospital from other centres. The seventh case, who hails from Gonda district, is undergoing treatment at SGPGIMS.

Talking to reporters, chief medical Officer, Lucknow, Dr AK Shukla claimed that barring the patients at SGPGIMS, none of the patients undergoing treatment in Lucknow are natives. "They all are referred cases and have been put on medicines... so there is no need for panic,'' he told TOI.

Giving details of the patients, officials at SGPGI said that sister duo Manjulata Kamal and Roshni Singh have been gripped by the swine flu virus. Residents of a type-III house in PGI, the sisters have not been admitted in the hospital. Komal Vishwas, who lives in a type-II house on PGI campus, is the third case at the medical institution.

Doctors attending the patients claim that the three persons are fine. "They have been isolated inside their houses and have been given tamiflu... all three should be fine in 3-4 days,'' said one of them. The fourth case at PGI is a patient named Usha who is undergoing treatment at the liver transplant unit.

When asked to comment, chief medical superintendent Dr AK Bhatt said, "we are taking all the necessary precautions defined in the prevention and treatment protocol issued by the health ministry... PGI staff and their dependents have the option of getting themselves vaccinated.''

This was almost eight hours since his swab samples were tested at SGPGI. It was only he who knew as to after how many days he went for the check up after showing up symptoms of a possible swine flu. And, with how many people he came in contact with all this time. ``We immediately asked him to go to his house and avoid contact with people,'' said chief medical officer, Lucknow, Dr AK Shukla. Ironically, the health authorities were not having Akash's address till that time.

The instance only raises the big question: Are authorities prepared to tackle the menace of swine flu? Nearly 145 cases within a period of eight months may be a projection of the virus not spreading its tentacles too wide in UP (In Lucknow alone, more than 50 cases of swine flu cases have been reported during the same period). But it is the absence of enough preventive measures which has been a cause of concern.

Vaccination, to begin with, has largely been limited to the medicos and paramedical personnel who are exposed to the H1N1 suspected/infected patients. There is complete absence of rigorous immunisation drive against the virus which floats in the air once an H1N1 infected person sneezes or coughs. In fact, at times, the carrier himself may not be even aware of the infection while he drives or walks around the city. All that one can do is to voluntarily get a vaccine administered. Cost of the vaccine, however, appears to be a limiting factor. While a single injectable vial could make you cough out Rs 3,000, the nasal variant costs around Rs 800 (A single vial may be used for five persons). Unfortunately, the vaccines provide immunity against the virus for a period of one year only.

Besides vaccination, it's the absence of a rigorous surveillance mechanism which forms a major constraint. Health authorities insist of having a rapid response team (RRT) to screen people around the person who possibly came in contact with the infected person. ``It's not a water borne disease that we can demarcate localities which are vulnerable to the disease. The virus spreads out in the air itself,'' said a senior health official.

Screening of passengers migrating into the city too has been found wanting. Take for example the case of Amausi airport, which happens to be the most crucial gateway to national and international tourists. Airport sources claimed that a system of screening is yet to be put in place. Airport director, Atul Dixit said that they have a medical instruction (MI) room as for that matter. ``As soon as any passenger complains of any such problem we send him/her to hospital immediately. It is therefore up to the medicos to decide if the passenger needs to get any test done or not, he said.

UP govt review measures to control swine flu


Lucknow, Aug 20 (PTI) Uttar Pradesh Health Minister Anant Kumar Misra today directed officials of his department to take immediate steps to check swine flu cases.In a meeting with senior officials of his department, Misra instructed that functioning of three-member Rapid Response Teams in all districts should be made effective.The minister directed that control rooms should work round the clock. Affected persons should be kept in isolation in their respective homes itself and given proper treatment, he told the officials.On rising cases of Japanese Encephalitis in Gorakhpur and Basti districts, the minister said 75 lakh doses of vaccines would be provided by the Central government by this month end and a drive would be launched in Novemeber to vaccinate 70 lakh children in the age group of 1-15 yrs.

40-yr-old housewife gets swine flu

KANPUR: A swine flu case was confirmed in the city when one Sunita Jaiswal, a suspected case of H1N1 influenza, was declared positive by Sanjay Gandhi Post-graduate Institute of Medical Sciences (SGPGIMS), Lucknow, on Monday.

A 40-year-old Sunita was confirmed swine flu positive two days after she was admitted to a private hospital as a patient of bilateral pneumonia.

As informed by the authorities of the private hospital, the condition of the patient is critical. As soon as her H1N1 status was confirmed, the attendants decided to shift her to SGPGI for proper treatment. Narrating the series of events, Dr D P Agarwal, who looked looked after the patient said, "Sunita was suffering from severe cough, cold and respiratory illness for the past one week. Having symptoms similar to H1N1 influenza, she was admitted to a private hospital on August 20."

"After she was detected with bilateral extensive pneumonia, her throat swab was collected and sent to SGPGI for H1N1 test, which was confirmed positive on Monday evening," said he, adding: "The three of her relatives, including her two kids and husband, who were accompanying her, have been given Tamiflu."

Notably, in 2009-10, as many as 27 cases were reported H1N1 positive, including two cases from district Kanpur Dehat. Whereas around 303 suspected cases received Tamiflu in 2009. Sunita is 2010's first swine flu case from the city. Besides, Vineeta Devi from Etawah district was tested positive on August 20.

Health experts say as the gradual dip in the temperature would facilitate the transmission of H1N1 influenza virus, children and elderly need to take more precautions and should avoid crowded places. Dr Shailendra Tiwari, senior doctor at Ursala Horsman Memorial (UHM) Hospital, said, "There has been tremendous increase in the number of patients who are experiencing problem in breathing and other respiratory diseases. While in people showing swine flu like symptoms, constriction of the airway is the common problem."

Woman succumbs to swine flu at UHM


KANPUR: After struggling for more than five days, 30-year-old Vineeta Devi, a resident of Etawah, succumbed to swine flu on Thursday at the Ursula Horsman Memorial (UHM) hospital.

The woman had delivered a baby girl on August 13 at a private hospital in Etawah. Two days later, on August 15, she had a high fever and difficulty in breathing. Her swab test was done and, as soon as the report from SGPGI, Lucknow, confirmed she had swine flu, she was shifted to UHM on August 20.

Another patient, Akhilesh Gupta (40), a resident of Ananad Nagar, was confirmed as H1N1 positive by Sanjay Gandhi Post-Graduate Institute (SGPGI), Lucknow, on Thursday.

Swine flu cases likely to rise


KANPUR: With the fluctuating weather conditions, doctors caution against the H1N1 infection as there are chances of it getting more virulent. Health experts say that there will be spurt in swine flu cases. Hence, the masses need to be vigilant if they are suffering from common cold like symptoms.

Chief medical officer (CMO) Ashok Mishra said: "In the coming days, there will sharp increase in the number of positive cases. Testing all the cases is not possible. We have to treat the disease like common flu and strong flu like symptoms should be administered full dose of Tamiflu or Tamivir. The benefit of the anti-viral drug is that it makes illness milder and shorter and may also prevent serious influenza."

The high risk categories are children, elderly, pregnant women, patient with chronic illness, immuno-compromised persons and even those who come late for treatment. "Death is mostly due to respiratory complication. It affects the lungs resulting in viral pneumonia. The virus disrupts the respiratory mechanism by lowering the oxygen level and making the patient dependent on artificial ventilation," the CMO said.

Experts say that for the treatment of flu, one should visit a doctor within 48 hours and if necessary go for confirmatory test and other Influenza test like rapid influenza diagnostic tests ( RIDT), immunofluorescence tests (DFA or IFA) as early as possible.

Seasonal influenza has never been a serious concern until H1N1 influenza A virus gripped the city. Five deaths were reported in one week. Doctors say, every year many people die due to seasonal influenza world wide but unnecessary panic has been created about swine flu. The H1N1 influenza has high morbidity but low mortality.

H1N1 is a new sub-type of influenza A virus resulting from genetic shift. The virus is expected to behave like seasonal influenza. Currently, limited data is available regarding its epidemiological and clinical behaviour. Moreover. the virus has not shown any change in its genetic configuration since its emergence. Seasonal influenza virus (H1N1 and H3N2) and other respiratory viruses are also causing flu side by side.

Dr Arti Lalchandani, ICU incharge at Lala Lajpat Rai ( LLR) Hospital, said: "Around 60 per cent of the people develop antibodies against the infection without showing any clinical symptom. Only 5 per cent people show clinical symptom and 3 per cent die under critical condition."

"Generally, the incubation period of the virus is 1-4 days or may be up to 7 days. The condition becomes severe when the patient feels difficulty in breathing, pain or pressure in the chest, sudden dizziness, persistent vomiting and flu-like symptoms with fever and cough," Dr Arti added.

UP: Swine flu death in Farrukhabad, govt. sounds alert
In Uttar Pradesh, an alert has been sounded in Farrukhabad after the death of a female patient of the district due to Swine Flu.

Quoting health officials reports that the girl had succumbed in a Delhi hospital. The girl probably caught the infection from the family, she had been staying with in Delhi. The blood samples of girl?s family members have been taken for investigation.

Meanwhile, similar alert has been sounded in Deoria district of eastern UP where an isolation ward has been earmarked for swine flu patients. The Chief Medical Officer of the district has said that no patient of swine flue has been detected yet in the district but the department is keeping vigil on migratory population.

Suspected swine flu patient dies in Manki village

Ghaziabad, Aug 29 (PTI) A suspected swine flu patient died in Manki village near Modinagar where already the death toll in water-borne diseases have reached 10, officials said here.

Ashima (18), who was being treated in a local hospital was rushed to Delhi after she was suspected to be affected with swine flu, they said, adding she died yesterday.

The deaths occurred in Manki and Kalchhina villages due to fever and jaundice but no case of swine flu came in my notice, Chief Medical Officer (CMO) A K Dhawan said.

District Magistrate Hredesh Kumar, who visited the affected villages, also confirmed fever and jaundice but he directed to CMO to confirm the case of swine flu.

Three persons died of viral fever and jaundice on Friday taking the total number of deaths, including seven children and one woman, to ten in the villages.

Swine flu vaccine given to 900 medicos

ALLAHABAD: Senior district health officials on Monday claimed that swine flu vaccine had been administered to all medical and para-medical staff working at all three government hospitals, primary and community health centres of the district.

District surveillance officer, Dr OP Bhaskar told TOI: "We have received cent percent results in administering medical and para-medical staff of government hospitals and medical centres". He added that a total of 900 medical and para-medical staff were given vaccines who had been examining patients at various medical units.

He added that vaccines have been administered to medical and para-medical staff as they were in contact with affected persons daily and treatment them. As a junior doctor was earlier found swine flu positive, vaccines were immediately offered to all medical and para-medical staff who were somehow left during the vaccination drive.

The top state health officials had directed surveillance officers (DSOs)/Epidemiologists across the state to administer swine flu vaccine all doctors and para-medical staff treating patients.

With city had already registered a swine flu death and three confirmed cases, the medical and para-medical staff are taking all precautionary measures to treat the patients.

The health officials in the last one and half months had sent over 47 samples for test to SGPGI, Lucknow for swine flu confirmation and only four cases were found positive.
