Neither country has had human activity in the regions in question (Vietnam H5n1 activity has been largely in the north). However, H5N1 in South Korea is likely, based on migration patterns, although South Korea has yet to report human H5N1 with pneumonia.

All that is clear is that the fatal infection took a sudden turn for the worse, with death 7 hours after the patient was put on a respirator (which was 4 hours after admission).

Agreed, but this part of post #1 led me to suspect that the 33 YO travelled to (or returned home to) his wife's city of Can Tho from S. Korea.

I think there are many reasons to lean toward travel from Korea to Vietnam, but details are sketchy. The term "foreigner" may just refer to nationality, since the story originated in Vietnamese press. Thus, the husband could be staying with his wife in Vietnam and his visting father returned to Korea.

However, since both patients with symptoms are Korean and the son may have recently arrived in southern Vietnam, a Korean orgin is possible (and probably likely).

Of course just a little more detail would resolve these issues, which is why I was searching through the Vietnamese-language press for more clues.

Unfortunately, even though I suspect the infection originated in South Korea, I do not expect to hear anything from South Korea unless the son tests positive in Vietnam (I haven't seen any mention of this in the South Korean press).

There is news in the Vietnamese press, but the online translator appears to be down.

There really are few benefits of testing, when only one person is involved. Most countries that report H5N1 in humans for the first time involve clusters. No testing means no report. That is why many index cases in clusters are not official cases (no sample is collected for testing). This is true for Cambodia, Indonesia, China. Initial negatives were also reported for Turkey, Iraq, and Azerbaijan.

South Korea has not reported H5N1 in humans, although they had a Qinghai outbreak last year at this time on MANY farms (follwed by infectios in Japan), and likely have H5N1 infections again this year.

No testing means no report.
If moustly will not be done enaugh testing, or it will remain secret, and there are no means for WHO to get a report in time, than we have scarce probabilities to stop the pandemic at the begining.

Than, probably this is not the begining, only a farm/food family cluster infection.

A South Korean man died of pneumonia in Can Tho city Monday [5 Nov
2007], local doctors, who suspect he had contracted bird flu, said.

The man was taken to the city General Hospital at 3:00 am Monday [5
Nov 2007] with high fever and breathing problems. He died at 2:00 pm.

The doctors have sent his blood samples to Pasteur Institute in Ho
Chi Minh City to test for the H5N1 strain of bird flu virus, which
has caused 100 infections and 46 deaths in Viet Nam in the last few years.

The man's family said his father in Korea too was suffering from
pneumonia-like symptoms.

The doctors have sent his blood samples to Pasteur Institute in Ho Chi Minh City to test for the H5N1 strain of bird flu virus, which has caused 100 infections and 46 deaths in Vietnam in the last few years.
The man?s family said his father in Taiwan too was suffering from pneumonia-like symptoms.

This is a follow up to ProMed - Avian influenza, human (148): Viet

The 32-year-old Taiwanese man, who died of suspected avian influenza
in Viet Nam on 5 Nov 2007, went to Viet Nam on 31 Oct 2007. According
to information received from the Ho Chi Min City Pasteur Institute,
the man was admitted to hospital on 2 Nov 2007, presenting with fever
of 38.5 degrees C [101.3 degrees F], cough, sore throat, and dyspnea.
Additional information from his wife indicated that he had abdominal
pain associated with severe uncontrollable watery diarrhea. The man
also had a history of chronic diarrhea and alcoholic hepatitis. No
additional information on the final diagnosis has been received.
However, avian influenza had been ruled out.

The father of the suspected case of avian influenza was admitted to
hospital in Taiwan on 6 Oct 2007. He was a 58-year-old man with long
history of smoking. In October, he was admitted because of increased
frequency of cough. Chest x-ray showed left upper lobe, posterior
segment consolidation that is suspicious of tuberculosis. After
treatment with antibiotics, the patient was discharged. He has been
well since discharge.

At this time, we do not believe that there is any epidemiological
association between the illnesses of the father and son.

[ProMED-mail welcomes this clarification which clearly establishes
that the 32-year-old patient in Viet Nam originated from Taiwan (not
Korea) and furthermore confirms that he has been eliminated as a
suspected human case of H5N1 avian influenza. The father of the
suspected Vietnamese case is resident in Taiwan and has recovered
from an unrelated respiratory illness. There is considered to be no
epidemiological link between the illnesses of the father and the son.
Neither are now suspected human cases of avian influenza and no
potential clustering of cases has occurred. - Mod.CP]
