French Polynesian health authorities are stepping up their monitoring of dengue fever cases, with a focus on border control and screening of international passengers, in the light of a regional outbreak of the serotype 4 of the potentially deadly hemorrhagic mosquito-borne fever.

The move was mainly motivated by a recent drastic surge in the number of cases, especially in Fiji (with a current figure that was well over 200 at the weekend and could rapidly double), Palau, Kiribati, American Samoa and neighboring Cook Islands.


As a precautionary measure, the French Pacific territory's health department said it was now increasing its awareness campaign with the local population, but also passengers arriving on international flights.

Another measure is to recommend any person showing signs of dengue and who would have traveled overseas in the 14 days preceding the appearance of symptoms (headache, muscular and joint pains, fever, and nausea) to immediately seek medical advice.

French Polynesia has previously been affected by severe dengue epidemics, but those were of the serotype 1.

The main worry was that the local population, if exposed to the serotype 4, would be even more vulnerable.

Dr. Henri-Pierre Mallet, head of infectious diseases department at the Prevention Unit of the Health Department, said in his latest bulletin that 150 cases of (type 1) dengue fever had been present almost on an endemic basis since the beginning of this year.

Twelve new cases (still of type 1 dengue) appeared in August, a situation Dr. Mallet regards as "stable."

In New Caledonia, where the serotype 1 of dengue fever also prevails, close to 1,000 cases have been detected since the beginning of this year.


There, the main worry is that even though the past three months are usually regarded as not conducive to the development of mosquito larvae (mainly because of the drier and cooler weather conditions), the number of cases, during the same period, has still progressed at a rate of about 15 cases per month.

With the return, in September, of a warmer and more humid climate in the region, health authorities fear the existing ground would be particularly fertile for a fully-fledged Type 1 epidemic.

In New Caledonia, authorities are also worried of a potentially disastrous combination of the existing Type 1 and the surrounding Type 4 viruses, because just like in French Polynesia, the population would be extremely exposed and vulnerable.


Throughout this year, in New Caledonia, spraying has taken place, especially in Noum?a and its suburbs, here most of the French territory's population resides.

There have also been numerous awareness campaigns and the deployment of health department and municipal teams to clean up compounds (even private dwellings) and remove potential breeding sites (such as old tires, old cups, old pots containing still water) and cut overgrown grass and bushes.

The World Health Organisation is calling for more funds to fight dengue fever, arguing it has reached pandemic proportions in the Pacific.
Thousands of people in the region are infected by dengue, which is carried by mosquitoes and can be fatal if left untreated.
Fiji, Samoa, American Samoa, and many other nations are currently experiencing dengue outbreaks.
The WHO?s Pacific representative, Dr Kevin Palmer, says the region needs to adopt a multi-sector approach to fight the disease.
?There?s more than 1600 [people], and those are the reported cases. So if you multiply that number by five, you get 5- to 6000 people. This certainly has a major impact on the workforce, on the health system. Dengue is really the pandemic disease of the Pacific and we need to do something about it, invest the similar kind of money that was put into bird flu and put that back into dengue.?

The French Polynesia Health Ministry has issued vigilance alert to residents and visitors arriving in Tahiti from other <?XML:NAMESPACE PREFIX = ST1 /><ST1:PLACE w:st="on"><ST1:PLACENAME w:st="on">Pacific</ST1:PLACENAME> <ST1:PLACETYPE w:st="on">Islands</ST1:PLACETYPE></ST1:PLACE> as the World Health Organization declares a dengue pandemic in the region.

Tahiti's public health officials noted that the serotype 4 dengue involved in the current outbreak in the region was last detected in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE> in major epidemic numbers in 2001. More than 33,000 people were diagnosed with the mosquito-borne fever seven years ago, causing the death of eight children.
The World Health Organization (WHO) today in <ST1:COUNTRY-REGION w:st="on">Fiji</ST1:COUNTRY-REGION> declared a dengue pandemic in the Pacific region, calling for a coordinated regional response to the situation as thousands of cases of the virus have been confirmed and the number continues to rise, Radio <ST1:COUNTRY-REGION w:st="on"><ST1:PLACE w:st="on">Australia</ST1:PLACE></ST1:COUNTRY-REGION> reported.

So far, however, there have been no reports of the potential deadly bird flu virus in the South Pacific.
There is no vaccine and no cure for the dengue, which has the potential of becoming a deadly hemorrhagic virus. However, on a worldwide basis, more than 500 million people are infected yearly with malaria, compared with "just" 50 million people for dengue, according to Olivia Judson, an evolutionary biologist who is a research fellow in biology at <ST1:PLACENAME w:st="on">Imperial</ST1:PLACENAME> <ST1:PLACETYPE w:st="on">College</ST1:PLACETYPE> in <ST1:PLACE w:st="on"><ST1:CITY w:st="on">London</ST1:CITY></ST1:PLACE>.
"As diseases go, it's (the dengue's) not terribly dangerous either: the death rate from dengue hemorrhagic fever is around 2.5 percent," she wrote on Sept. 9 in her New York Times Op-Ed blog, The Wild Side.
Describing the dengue as "on the rise . . . an up-and-coming virus,? Judson proposes using genetic engineering of mosquitoes "as a way to stop the spread of the dengue fever."
The dengue, which is not contagious, is passed from person to person through the bite of a virus-carrying Aedes aegypti mosquito, which bites only during the day. But there is no cross immunization, so someone who has had the Type 1 dengue is not immune from the Type 4 version.
The Health Ministry in Tahiti reported a continued "stable" dengue situation through the end of August in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE> with only 150 confirmed cases since Jan. 1 and 12 cases during August. Only the much milder Type 1 form of the dengue has been detected in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE>, requiring the hospitalization of only three people. One was a child under five years of age; the second was a child under 10 and the third was an adult over the age of 80.
What concerns public and private health officials in Tahiti is the potential threat of a Type 4 dengue epidemic for some 155,000 people in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE> should the more lethal version arrive for the first time since 2001. The people threatened are those who never had the Type 4 dengue so, therefore, have not become immune.
The more lethal version could arrive in Tahiti from tourists or returning residents after visits to the current <ST1:PLACE w:st="on"><ST1:PLACENAME w:st="on">Pacific</ST1:PLACENAME> <ST1:PLACETYPE w:st="on">Islands</ST1:PLACETYPE></ST1:PLACE> with known type 4 dengue cases. <ST1:COUNTRY-REGION w:st="on">Kiribati</ST1:COUNTRY-REGION>, <ST1:CITY w:st="on">Samoa</ST1:CITY>, <ST1:STATE w:st="on">American Samoa</ST1:STATE> and the <ST1:PLACE w:st="on">Cook Islands</ST1:PLACE> have reported Type 4 dengue cases. The only one of those islands that Tahiti has a direct link with is the <ST1:PLACE w:st="on">Cook Islands</ST1:PLACE>. Air Tahiti operates regular weekly flights between Rarotonga and <ST1:PLACE w:st="on"><ST1:CITY w:st="on">Papeete</ST1:CITY></ST1:PLACE>.
The Cook Islands has reported only one Type 4 case of dengue fever that was imported by a passenger from elsewhere, according to <ST1:PLACE w:st="on">Tahiti</ST1:PLACE>'s health ministry.
The most recent period of a dengue epidemic in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE> was in 2006/2007 when more than 2,000 confirmed cases of the Type 1 fever were detected, the health ministry said. The situation has remained "relatively calm" in <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE> since the end of 2007, according to the ministry.
The latest official statistics through Aug. 31 show that since Jan. 1 there were 110 Type 1 dengue cases on the <ST1:PLACETYPE w:st="on">island</ST1:PLACETYPE> of <ST1:PLACENAME w:st="on">Tahiti</ST1:PLACENAME>, or 73% of the 150 confirmed cases throughout <ST1:PLACE w:st="on">French Polynesia</ST1:PLACE>. In the Leeward Islands, Raiatea had 17 cases, <ST1:PLACE w:st="on">Bora Bora</ST1:PLACE> had seven cases and Taha'a had three cases. Tahiti's sister <ST1:PLACE w:st="on"><ST1:PLACETYPE w:st="on">island</ST1:PLACETYPE> of <ST1:PLACENAME w:st="on">Moorea</ST1:PLACENAME></ST1:PLACE> had one case and the Tuamotu atoll of Fakarava had one case. Five cases were considered indeterminate.
