I cannot understand: if the child was affected by dengue fever (not dengue hemorrhagic fever), why does the hospital put it on tamiflu? and why do the x-ray images portrait a fast pneumonia remission? Does the hospital require to external lab to test children for dengue virus?

Dengue: clinical differential diagnosis of flu at the onset of illness( even in 1918.. some doctors tought about dengue ) .
I don't know if indonesian hospital make systematically serology for dengue ( I will search ), because it 's a common disease.
when there is no available laboratory, "therapeutic test" is made ( don't know if it's the same world in english ) .

We had one documented case with Dengue and bird FLU ( a young man )

In Thailand there was a patient with dengue fever symptoms but later he was stated bird flu positive after specific test was done. Dengue was discounted.

I think that rather a specific test for dengue fever, one may also try to look at routine blood screening panel (ie. blood cells count, platelets count, Proteic C, complements, serum proteins (albumin - globulin: low protein level in sera may indicate a intravascular coagulation due to blood vessels increased porosity), uric acid level; increased hematocrite...)...

While the age distribution of the patients listed above are certainly consistent with dengue, there are other inconsistencies in this thread that do not add up to a compelling picture that the "Air Batu 13" were dengue patients.

1) Why is dengue exploding now? The typical rainy season starts in fall (October through December - varies by report) and continues though spring (March - April). Has there been recent heavy rainfall that would support local amplification of the mosquito vector? I could not find any reports of recent heavy rains or flooding in N Sumatra.

2) While dengue commonly presents with high fever and severe muscle aches, lower respiratory symptoms are not typically associated with this illness. The 13 patients are described as having respiratory symptoms:"Another 13 people had been admitted to the hospital with "high temperatures and respiratory problems,'' she said." (From post #4 in this thread).

The exception is late stage dengue hemorrhagic fever/dengue shock syndrome where patients may have severe pleural effusion combined with adult respiratory distress syndrome.

There is a lot of confusion. It is not possible to dissect the information correctly by this way.

An urgent clarification is needed.

While the age distribution of the patients listed above are certainly consistent with dengue, there are other inconsistencies in this thread that do not add up to a compelling picture that the "Air Batu 13" were dengue patients.

1) Why is dengue exploding now? The typical rainy season starts in fall (October through December - varies by report) and continues though spring (March - April). Has there been recent heavy rainfall that would support local amplification of the mosquito vector? I could not find any reports of recent heavy rains or flooding in N Sumatra.

2) While dengue commonly presents with high fever and severe muscle aches, lower respiratory symptoms are not typically associated with this illness. The 13 patients are described as having respiratory symptoms:"Another 13 people had been admitted to the hospital with "high temperatures and respiratory problems,'' she said." (From post #4 in this thread).

The exception is late stage dengue hemorrhagic fever/dengue shock syndrome where patients may have severe pleural effusion combined with adult respiratory distress syndrome.

There are few reports of this and patients do not have brief hospitalizations, most die :

In dengue shock syndrome, an acute increase in capillary permeability results in leakage of plasma into the interstitial space. Pleural effusion is commonly seen in dengue shock syndrome. We report three cases of dengue-associated adult respiratory distress syndrome (ARDS) in children, in all of whom dengue haemorrhagic fever, presenting with grade 3 or grade 4 dengue shock syndrome with disseminated intravascular coagulopathy, was confirmed. The criteria for the diagnosis of ARDS were based on the expanded definition of ARDS by Murray et al. Treatment consisted of fluid resuscitation, correction of coagulopathy and mechanical ventilation. All three children had multi-organ impairment, but it was more severe in the two who died. The one survivor was well at discharge.

We may be almost certain of HK lab capability to detect Dengue fever virus.

Thirteen people in Indonesia suspected of having bird flu have tested
negative for the feared disease, the country's health ministry said
Saturday [9 Aug 2008]. Experts from the World Health Organisation
(WHO) arrived Friday in the affected village in North Sumatra to help
investigate a possible outbreak after 3 people died, and the 13 were
admitted to hospital. "All specimens collected from suspect cases
have given negative results. They are all recovered," I Nyoman
Kandun, director general of the ministry's communicable diseases
department said on a text message.

Officials and residents in Asahan district in North Sumatra province
said villagers began showing symptoms of avian flu after a large
number of chickens died suddenly last week in Air Batu village. The
local husbandry office took preventive action this week by
slaughtering and burning some 400 chickens and ducks. The ministry,
which has stopped giving regular bird flu updates, announced earlier
this week that the human toll from avian influenza in Indonesia had
risen to 112 following the recent death of a 19-year-old man.

The deadly H5N1 strain of bird flu has killed more than 240 people
worldwide since late 2003. The virus typically spreads from bird to
human through direct contact, but experts fear it could mutate into a
form easily transmissible between humans, with the potential to kill
millions in a pandemic.

[Contrary to a statement by local health officials on Wed 6 Aug 2008
(see ProMED-mail reference below), the suspected human cases of avian
H5N1 influenza in North Sumatra have not been confirmed by the
Indonesian Ministry of Health. The World Health Organisation
cumulative table of human cases still lists the number of confirmed
avian influenza fatalities in Indonesia as 110, rather than the 112
fatalities recorded by the Indonesian Ministry of Health.

Until further notice, ProMED-mail will not report suspected human

by the Indonesian Ministry of Health.

A Sufferer DBD was killed, 3 were treated in Tanjungbalai
July 16th, 2008

A person was killed and 3 other still underwent the intensive maintenance in RSU Dr T Mansyur Kota Tanjungbalai resulting from Dengue Dengue Fever. Casualties the Virgin Nasution, (5) the resident Street Utama, Kel Pulo Simardan, of Kec Datuk Bandar, Kota Tanjungbalai was killed after several hours underwent the maintenance, on Sunday (13/7). Casualties were carried to the hospital with the HB condition 10,2 gram percent, trambosit 60.000/mm3 and Hr 38 percent.

In the meantime that still was undergoing the maintenance, Chelsea Nanda, (2) the resident Street Garuda, Kel Beting Kwala Kapias, of Kec Teluk Nibung. Afterwards the older brother was siblings, Agnes, (and Natalie,13, the resident Street DTM Abdullah Kampung Baru, Kel TB Kota III, of Kec Tanjung Balai Utara. ?Sudah almost a week was treated in sini?, people say old the sibling older brother, M. Malau, (44) in the room of the child. Said, initially his two children suffered the high fever was accompanied vomited again and again. To avoid the occurrence of the matter was not wanted, then both of them were at once shifted took medicine to RSU Dr T Mansyur and it was recommended to undergo the maintenance inap.
