[h=3]Abstract[/h] [h=4]BACKGROUND:[/h]  We examined influenza vaccine effectiveness (VE) in the 2013-2014 season in which influenza A (pH1N1) predominated. In previous years, in which A (H3N2) predominated, VE was low and negatively affected by prior year vaccination.
[h=4]METHODS:[/h]   We enrolled and followed 232 households with 1049 members, including 618 children; specimens were collected from subjects with acute respiratory illnesses. VE in preventing laboratory-confirmed influenza A (pH1N1) was estimated in adjusted models. Pre-season hemagglutination-inhibition and neuraminidase-inhibition antibody titers were determined to assess susceptibility.
[h=4]RESULTS:[/h]  Influenza A (pH1N1) was identified in 25 (10.8%) households and 47 (4.5%) individuals. Adjusted VE against infection with influenza A (pH1N1) virus was 66% (95% CI, 23% to 85%), with similar point estimates in children and adults, and against both community and household-acquired infections. VE did not appear different for live-attenuated and inactivated vaccines among children age 2-8 years, although numbers were small. VE was similar for subjects vaccinated in both current and prior seasons and those vaccinated in the current season only; susceptibility titers were consistent with this observation.
[h=4]CONCLUSIONS:[/h]   Findings, including substantial significant VE and lack of negative effect of repeated vaccination on VE, were in contrast to those seen in prior seasons where A (H3N2) predominated.
