As on 04.09.2014, 15951 passengers have been screened [Low risk: 15909; Medium Risk: 07 and High Risk: 35]. Today, no passenger has been isolated at the quarantine facility.

So Far 21799 Passengers Screened For Ebola

1230 passengers have exited the Surveillance after follow up for 1 month

Global Ebola Virus Disease (EVD) epidemiological situation in EVD affected countries and travel-associated spread was reviewed by the Ministry of Health and Family Welfare. A detailed account of measures being undertaken for strengthening preparedness and activities at points of entry, IDSP, laboratory was assessed.

Airport and port health organizations at ports of entry nationwide are actively performing disease screening on international travelers arriving from the countries impacted by the Ebola outbreaks. Screening measures include travel history, taking body temperature and examination of high risk travelers by detailed medical examination. From August 1st to October 12, 2014, a total of 21799 passengers have been screened [Low risk: 21737; Medium Risk: 07 and High Risk: 55] have been screened at 18 airports upon arrival into the India. As on date, a cumulative number of 1128 passengers are being tracked by IDSP. Most of them are in the States of Maharashtra, Kerala, Tamil Nadu, Gujarat, West Bengal and Delhi. 1230 passengers have exited the surveillance after follow up for 1 month. So far, over 1000 travelers identified as suspected cases, 96 high risk have been tested but all been excluded after negative testing.

The Ministry of Health & Family Welfare has issued guidelines for travelers, immigration officials, airlines crew and other Ebola Virus Disease (EVD) and public health measures being undertaken including the personal protective equipment. Immigration officials and airlines crew at 18 international airports where travelers from EVD affected countries can arrive via gulf have been trained.

The Control Room in the Ministry of Health & Family Welfare has received 790 calls since its inception on 09/08/2014 morning and 01 call received in the past 24 hours. Most of the callers have sought information on signs and symptoms of the disease and travel related risk of acquiring the infection.

Indian Medical Association has been apprised to create awareness among its members on Ebola Virus Disease.

One hospital in each state has been identified in each state for admitting the suspects and having a dedicated isolation facility for management of EVD cases. The Health Ministry has strengthened Ebola preparedness and is currently planning specific training exercises on surveillance and response measures in states where 18 international ports of entry are located and which are considered as priority areas. These exercises will be focused primarily on the role of health care workers in these local hospitals, contact tracing, laboratory sample collection and transportation to the identified labs, disinfection procedures and personal protective gears for health workers. The Health Ministry has been providing technical and material support to states for preparing health facilities with dedicated isolation wards, enabling them to provide [Ebola] case management in accordance with the clinical practice guidelines for diagnosis, treatment and care, and infection control guidelines.

Cabinet Secretary reviews preparedness in the wake of emerging global crisis on Ebola

Preparatory and surveillance measures are put in place

The Cabinet Secretary Shri Ajit Seth chaired a high level meeting here today to review the state of preparedness in the context of emerging global crisis on Ebola. In a video conferencing with Chief Secretaries/ Health Secretaries of 19 states/UTs which have international airport/ seaport, he took stock of the measures being undertaken for strengthening the preparedness, surveillance and response system in the respective states.

Health Secretary, Home Secretary, Shipping Secretary and Secretaries/ representatives of D/o Health Research, Civil Aviation and External Affairs were also present.

The protocol/ preparedness measures including infection control practices to be followed by the States/UTs have already been communicated to the states. The state Governments informed during the meeting that these protocols have been put in place and all inbound passengers are being screened.

Till 15.10.2014, 22150 passengers have been screened of which 7 were identified to be at medium risk and 56 at high risk while the others were categorised at low risk. Low risk passengers are given general advice on the virus while medium and high risk passengers are kept under observation for a further period of 30 days and their blood samples tested. A tracking system has been put in place under the Integrated Disease Surveillance Programme (IDSP). No case of Ebola has so far been reported in the country although WHO has reported about 9000 cases and 4493 deaths globally.

State Governments informed that in accordance with the protocol, they have identified hospitals and isolation ward for Ebola treatment. Central teams have been asked to inspect the adequacy of preparations in these hospitals. A training of trainers is being organised by the Health Ministry from 19th-21st October, 2014 at Delhi. This training will include demonstrations and mock drills.

National Institute of Virology at Pune is fully equipped to handle the testing work relating to Ebola virus and the National Centre for Diseases Control (NCDC), Delhi, for molecular diagnosis. 10 other laboratories in various places including Delhi, Dibrugarh, Port Blair, Kolkata, Manipal, Lucknow, Guwahati, Bengaluru, Chandigarh, Thiruvananthapuram, Bhubaneswar, Chennai, Jaipur and Agra will be strengthened to handle this work..

Immigration officers have been trained and crew members of flights have also been sensitised on the measures to be taken by the airline crew and on the standard operating procedures for passenger screening at the airports by Airport Health Organisations. The Ministry of Civil Aviation has been asked to make inflight announcement regarding the virus and its symptoms. A health screening card and advisory for passengers has also been prepared and information is being collected from the passengers arriving from/transiting through the affected countries. The Chief Secretaries were requested to have proper liaison and coordination is maintained by the State health authorities with the airports. State Governments have been asked to maintain coordination with all ports, including the minor ports, so that ship crew are also screened properly.

Some State Governments expressed the need for supply of adequate number of Personal Protective Equipment (PPE) gear. 50,000 PPE gear procured earlier and available with the Central Government will be distributed to the States. State Governments have also been advised to procure PPEs based on specifications of Health Ministry. Thermal scanners are being deployed at different airports in the country.

Chief Secretaries were requested to convene meetings of the State Crisis Management Group at regular intervals for continued vigilance and preparedness. Health Ministry will continue to monitor the situation. A Control Room has been set up at the Ministry of Health & Family Welfare. The numbers are 23063205, 23061469 and 23061302.

Kerala to step up Ebola screening at airports

As the Ebola epidemic, which has claimed more than 4000 lives globally, causes concern, Kerala has stepped up screening at airports and examined over 450 passengers who arrived from various African countries. A top health department official said no one has tested positive for the deadly virus that has claimed large number of lives in Africa and in some other parts of the world.

Besides airports, the screening will soon be extended to sea ports also to check possible threat of its spread in the state, State Health Secretary K Elangovan said. "No Ebola positive case has been reported so far in the state. We have already taken strict measures to monitor the situation and stepped up screening at the airports. We are now mulling extending similar checks at seaports also," he told PTI. Of the total 469 persons screened, blood samples of two, who had fever, were collected and sent for analysis which tested negative, he said.

Ebola Treated and Cured Person from Liberia Quarantined at Airport Health Organisation Quarantine Centre, Delhi

A 26 year old male, Indian, had travelled from Liberia to India and reached Delhi on 10th November, 2014. He underwent the mandatory screening at the Delhi Airport. On interview, he gave history of febrile illness for which he was admitted to a health facility in Liberia on 11th September, 2014 and got discharged on 30th September, 2014. He carried a certificate of medical clearance from the Ministry of Health and Social Welfare, Government of Liberia mentioning that ?he has successfully undergone care and treatment related to Ebola Virus Disease and after post treatment assessment he has been declared free of any clinical signs and symptoms and confirmed negative by laboratory analysis?.

As a matter of abundant caution, he was isolated at the Airport Health Organization?s Quarantine Centre at Delhi Airport. His three blood samples tested at National Centre for Disease Control, Delhi on 10-13 November, 2014, respectively, were found negative for Ebola Virus Disease by Reverse Transcriptase Polymerase Chain Reaction (RT?PCR) tests. Therefore, as per WHO and CDC specifications, he is deemed to be cured. However, as has been reported in the past, the virus may continue to be positive in secretions like urine and semen for a longer time. Before releasing him from quarantine, a decision was taken to test his other body fluids. His semen sample tested at National Centre for Disease Control on 17th November, 2014 was found positive for Ebola Virus by RT-PCR. The tests for semen samples repeated at National Institute of Virology, Pune, on 17th November, 2014 also tested positive. This was confirmed by DG, ICMR on 18th November, 2014.

It is a known fact that during convalescence from Ebola Virus Disease, persons continue to shed virus in body fluids for variable periods. However, presence of virus in his semen samples may have the possibility of transmitting the disease through sexual route upto 90 days from time of clinical cure.

Currently, this person is not having any symptoms of the disease. However, he would be kept under isolation in the special health facility of Delhi Airport Health Organization, till such time his body fluids test negative and he is found medically fit to be discharged.

It is reiterated that the person concerned is a treated and cured case of Ebola Virus Disease. No cases of relapse of Ebola have been documented. All necessary precautions are being taken at the isolation facility. This would rule out even the remote possibility of spread of this disease by the sexual route. The situation is under control and there is no need for any alarm. However, all precautions are being taken in this regard.

Directs for strengthening preparedness and response management

Shri J P Nadda,Union Minister of Health and Family Welfare,chaired an inter-ministerial meeting to review the situation arising out of Ebola Virus Disease outbreak in West Africa, here yesterday. Senior Officers from the Ministry of External Affairs, Ministry of Shipping, Directorate General of Civil Aviation, Department of Biotechnology, Department of Health Research, Department of Animal Husbandry, Bureau of National Disaster Management Authority, Armed Forces Medical Services, AIIMS, Central Government Hospitals, NCDC, WHO and CDC Global Disease Detection Regional Centre, India along with senior officialsof the Ministry of Health and Family Welfare participated in the meeting.

The Health Minister gave directions that in order to ensure that the isolation/ quarantine facilities set up at airports adhere to all standards guidelines / protocols, a three member team will be constituted to inspect all airports to identify the gaps. It will include officials from the ministries of Health, Civil Aviation and Immigration. The three-member committee will submit a report within a week.

The Health Minister reemphasized on the importance of alertness, intensive training and capacity building of all stakeholders to be better equipped to address EVD, and directed for sensitisation of all stakeholders involved with the passenger screening.

Along with the airports, surveillance at seaports also needs to be strengthened, the Health Minister pointed out. He has called for a detailed report on the working of surveillance facility at ports.

The Health Minister desired that Health Ministry should expedite the action plan on the need to have India compliant on the core capacities required under International Health Regulations. On the core capacity of laboratory support the Health Minister emphasized the need to strengthen 10 such laboratories by the end of the month. On yet another core capacity, i.ethe hospital response, he has asked the concerned division to develop short term and long term strategies for medical management of emerging diseases such as Ebola Virus Disease. The Minister has directed the Health Ministry officials to constitute teams of experts who will visit the states and report on the preparedness of their designated hospitals, and asked them to work together towards capacity building of the states. He stated that training of the doctors and the para-medical staff needs to be a continuous exercise.

The Health Minister stated that the large medical force available with Ministry of Defence could be beneficially utilised at the time of need; he proposed that capacity building for Ebola case management should be initiated for Defence doctors and nurses to equip them better for emergency response management.

Press Note on Ebola

The 26 year old male, Indian, who had undergone Ebola treatment in Liberia and presently quarantined at Airport Health Organization Quarantine Centre, New Delhi continues to be symptom free. His urine and saliva samples tested at National Centre for Disease Control on 21/11/2014 have been found negative for Ebola Virus. However, he would continue to remain in this quarantine facility till all his samples are tested negative.

Union Secretary (Health & Family Welfare) Shri Lov Verma held video conference with Principal Secretaries of twenty four (24) States / Union Territories (UTs). The need to strengthen surveillance at ports and airports was emphasized. States were informed that Central Teams would again be visiting designated Ports and Airports to review the passenger screening. The Principal Secretaries of States/UTs were also requested to conduct surprise checks of the Airports to identify gaps in screening, if any.

As States have now trained master trainers, States were asked to conduct training of District Rapid Response Teams. A special mention was made to sensitize private practitioners and members of mass media. Further the States were requested to conduct mock drills involving the airports and isolation facilities and in particular on donning and taking off personal protective equipments.

36460 passengers screened for Ebola; 700 passengers being tracked by IDSP

Total of 36460 passengers have been screened so far for Ebola. Of these, 36379 were in low risk, eight in medium risk and 73 in high risk category. As on 4 December 2014, 700 passengers are being tracked by Integrated Disease Surveillance Programme (IDSP). Most of them are in Maharashtra, Kerala, Tamil Nadu, Gujarat, West Bengal and Delhi. 3189 passengers have exited the surveillance after follow up for a month. The IDSP is tracking such passengers who do not report any symptom at the time of arrival but are categorised as having risk of acquiring the disease because of the contact with a suspect case during their stay in an affected country. Passengers from the affected country arriving at 18 international airports and nine major ports are being screened. Crew members of flights arriving at Delhi, Mumbai, Hyderabad, Bangalore, Kochi and Trivandrum (covering 11 airlines) have been sensitized on the measures to be taken by the airline crew and on the standard operating procedures for passenger screening at airports, by the Airport Health Organisation.

The National Centre for Disease Control (NCDC), Delhi and National Institute of Virology (NIV), Pune are testing clinical samples for Ebola Virus Disease. Over 100 samples have been tested at these two laboratories using RT PCR test. Ten additional laboratories have been identified by Indian Council of Medical Research (ICMR) for testing Ebola Virus Disease, and are ready to test samples. Moreover,about 9500 Personal Protective Equipment (PPE)kits have been provided to the states by the Medical Store Organization. Additional 10000 PPE kits are being procured.

The central Ministry of Health and Family Welfare has conducted separate trainings for Master Trainers and the State Rapid Response Teams (RRT) of all states for public health preparedness and response to Ebola Virus disease. Master Trainers from 25 states and Rapid Response Teams of all the states/UTs have been trained through four Regional Training Workshops. Mock drills have been conducted with Master Trainers of 25 states.
