Influenza surveillance programmes:
The data presented in this report are generated from three influenza surveillance programmes: the Viral Watch [influenza-like illness(ILI)], Severe Acute Respiratory Illness (SARI) and the respiratory consultations and hospitalisations surveillance system.
The Viral Watch is a sentinel influenza surveillance programme started in 1984 in Gauteng and expanded from 2005 onward to include all 9 provinces in South Africa. The majority (90%) of the sentinel sites are general practitioners. Respiratory specimens (throat or nasal swabs) are collected from patients of all ages meeting the ILI case definition which is an acute respiratory illness with a measured temperature of ≥ 38 ˚C and cough, with onset within the past 7 days prior to consultation. Specimens are tested for the presence of influenza and other respiratory viruses by RT-PCR.
The Severe Acute Respiratory Illness (SARI) surveillance program is a prospective sentinel hospital-based surveillance program. It was established in 2009 and is currently conducted at 5 sentinel sites (public hospitals) in 4 provinces of South Africa. Hospitalised patients meeting the surveillance case definition of acute respiratory illness are prospectively enrolled. Clinical and epidemiologic data are collected using standardized questionnaires. Information on in-hospital management and outcome is collected. Upper respiratory tract samples (oropharyngeal and nasopharyngeal swabs in cases ≥5 years old or nasopharyngeal aspirate in cases < 5 years of age) are tested for the presence of influenza and other respiratory viruses using RT reverse transcriptase PCR.
The respiratory consultations and hospitalisations surveillance system collects anonymous influenza and pneumonia associated outpatient consultations and hospitalisations data from one private hospital group in 7 provinces (Gauteng, North West, Free State, Mpumalanga, Eastern and Western Cape and KwaZulu- Natal). These data on the number of consultations and hospitalisations are compared to the influenza season as described by the viral watch and SARI programmes.

The 2012 influenza season has not started as yet, though sporadic influenza detections are being made.
This year to date (week 19, ending 13 May), 1726 patients with SARI have been enrolled at the 5 sentinel sites, and one influenza A(H1N1)pdm09, and nine influenza B were detected from patients in Gauteng and KwaZulu-Natal.
Three hundred and two specimens have been received from Viral Watch sites in this time period, and influenza A (unsubtyped) was detected in one patient, influenza A(H1N1)pdm09 in one patient, influenza A(H3N2) in six, and influenza B in 16 patients. Seven of these patients were known to have travelled to the northern hemisphere.

Please note that these data are from sentinel sites and reflect trends in the areas with participating sites. Numbers reported reflect numbers of patients enrolled into the surveillance programmes and do not reflect total numbers of patients in the community.

This year to date (week 20, ending 20 May), 1811 patients with SARI have been enrolled at the 5 sentinel sites, and one influenza A(H1N1)pdm09, and 12 influenza B viruses were identified from patients in Gauteng and KwaZulu-Natal.
Three hundred and fifty-five specimens have been received from Viral Watch sites in this time period, and influenza A (unsubtyped) was detected in two patients, influenza A(H1N1)pdm09 in one patient, influenza A(H3N2) in nine, and influenza B in 17 patients. Sporadic influenza detections continue to be made through both the Viral Watch and SARI surveillance programmes. The average onset of the influenza season in South Africa over the past 28 years has been week 23(the first week in June), but has ranged from as early as the last week in April and as late as the second week in July. The influenza season is considered to have started when the detection rate rises above 10% and stays there for two weeks or more. The detection rate from the Viral Watch Programme has risen to 21% in week 21.
This year to date (week 21, ending 27 May), 1866 patients with SARI have been enrolled at the 5 sentinel sites, and one influenza A(H1N1)pdm09, one A(H3N2) and 13 influenza B viruses were identified from patients in Gauteng and KwaZulu-Natal.
Four hundred and fifteen specimens have been received from Viral Watch sites in all nine provinces in this time period, and influenza A (unsubtyped) was detected in one patient, influenza A(H1N1)pdm09 in two, influenza A(H3N2) in 11, and influenza B in 26 patients.

The detection rate from the Viral Watch Programme rose to 21% in week 21, and remained above 10% in week 22. If this trend is sustained then this will indicate that the influenza season has started.

This year to date (week 22, ending 3 June), 1929 patients with SARI have been enrolled at the 5 sentinel sites, and one influenza A(H1N1)pdm09, one A(H3N2) and 13 influenza B viruses were identified from patients in Gauteng and KwaZulu-Natal.

Four hundred and sixty-eight specimens have been received from Viral Watch sites in all nine provinces in this time period, and influenza A (unsubtyped) was detected in one patient, influenza A(H1N1)pdm09 in two, influenza A(H3N2) in 15, and influenza B in 27 patients. In addition other respiratory viruses have been detected in the specimens of 197 patients negative for influenza. These include adenovirus, enterovirus, human metapneumovirus, parainfluenza, respiratory syncytial and rhinovirus.

The 2012 influenza season started in week 21 (week ending 27 May) when the detection rate from the Viral Watch Programme rose to 21%, and remained above 10% in the following weeks. This year to date (week 24, ending 17 June), 611 specimens have been received from Viral Watch sites in all nine provinces. Influenza A (unsubtyped) was detected in one patient, influenza A(H1N1)pdm09 in two, influenza A(H3N2) in 38, and influenza B in 44 patients. Other respiratory viruses have been detected in the specimens of 19 patients negative for influenza in week 24. The majority of these were adenovirus followed by rhinovirus and respiratory syncytial virus.
In this time period 2162 patients with SARI have been enrolled at the 5 sentinel sites, and one influenza A(H1N1)pdm09, six A(H3N2) and 15 influenza B viruses were identified from patients in Gauteng and KwaZulu-Natal. The detection rate for SARI in week 24 was 4%.

Influenza A (unsubtyped) was detected in six patients, influenza A(H1N1)pdm09 in two, influenza A(H3N2) in 60, and influenza B in 62 patients. Other respiratory viruses have been detected in the specimens of 18 patients negative for influenza in week 25. The majority of these were rhinovirus followed by adenovirus and respiratory syncytial virus.
In this time period 2287 patients with SARI have been tested at the 5 sentinel sites, and one influenza A(H1N1)pdm09, 15 A(H3N2) and 16 influenza B viruses were identified from patients in Gauteng and KwaZulu-Natal. There are a number of specimens collected during week 25 awaiting results.

This year to date (week 31, ending 5 August), 1337 specimens have been received from Viral Watch sites in all nine provinces. Influenza A (unsubtyped) was detected in 43 patients, influenza A(H1N1)pdm09 in two, influenza A(H3N2) in 227, and influenza B in 150 patients.
In this time period 3073 patients with SARI have been tested at the 5 sentinel sites, and influenza A(unsubtyped) was detected in four patients, influenza A(H1N1)pdm09 in one, influenza A(H3N2) in 67, and influenza B in 34 patients in all four provinces with sentinel sites. There are a number of specimens collected during week 31 awaiting results.

This year to date (week 33, ending 19 August), 1521 specimens have been received from Viral Watch sites in all nine provinces. Influenza A (unsubtyped) was detected in 13 patients, influenza A(H1N1)pdm09 in three, influenza A(H3N2) in 348, and influenza B in 206 patients.

In this time period 3266 patients with SARI have been tested at the 5 sentinel sites, and influenza A(unsubtyped) was detected in four patients, influenza A(H1N1)pdm09 in one, influenza A(H3N2) in 84, and influenza B in 45 patients in all four provinces with sentinel sites. There are a number of specimens collected during week 33 awaiting results.

This year to date (week 35, ending 2 September), 1712 specimens have been received from Viral Watch sites in all nine provinces. Influenza A (unsubtyped) was detected in 9 patients, influenza A(H1N1)pdm09 in four, influenza A(H3N2) in 403, and influenza B in 267 patients.

In this time period 3587 patients with SARI have been tested at the 5 sentinel sites, and influenza A(unsubtyped) was detected in five patients, influenza A(H1N1)pdm09 in one, influenza A(H3N2) in 106, and influenza B in 79 patients in all four provinces with sentinel sites. There are a number of specimens collected during week 35 awaiting results.
