EVENT DESCRIPTION

South Africa continues to report confirmed cases of
measles, nearly reaching the 1 000 mark since the
outbreak began in 11 October 2022. No measles related
deaths have been reported.

In the past two weeks (ending 18 April), there have been
20 laboratory-confirmed measles cases detected across
the country, of which the majority were from Limpopo
province (12). The percentage of samples testing positive
increased from 20% of 99 samples tested in week 14 to
22% of 79 samples tested in week 15. The measles strain
detected in Limpopo province and North West province
is genotype D8 which is similar to the strain in Zimbabwe
in the 2022 outbreak. No new cases were reported in
Free State, KwaZulu-Natal, Mpumalanga, Western Cape,
Northern Cape and North West provinces in the same
reporting period.

Since the start of the outbreak on 11 October 2022 to 18
April 2023, the NICD has tested 5 798 serum samples for
measles, of which 970 (17%) were confirmed positive, of
which 964 (99%) were reported from the eight outbreak
affected provinces.

The distribution of confirmed cases among the affected
provinces include; Limpopo (405 cases), Mpumalanga
(108 cases), North West (216 cases), Gauteng (165
cases), Free State (29 cases), Western Cape (14),
KwaZulu-Natal (20) and Northern Cape (7), with Limpopo
and Mpumalanga reporting 53% of all cases.

The age of reported cases across the eight affected
provinces ranges from two months to 67 years. Children
between the 5-9 years age group are the most affected
411 (43%), followed by 228 (24%) in the 1-4 years age
group and 188 (20%) in the 10-14 year group. Disease
severity has been observed within the less than 1-year
age group as evidenced by majority of health facility
admissions in this age-group (56%).

The highest attack rate has been observed among the
5-9 (8.4 per 100 000 population) and 1-4 years (5.7 per
100 000 population) age groups.

Vaccination status for laboratory-confirmed measles
cases in provinces with a declared measles outbreak
indicates that 105 (11%) of the 964 cases have known
vaccination status, while 123 (13%) have an unknown
vaccination history. The age groups with the highest
number of vaccinated cases are those aged 1-4 years
and those aged 5-9 years.

PUBLIC HEALTH ACTIONS

The South African Health authority are supporting the
following activities in response to the measles outbreak.
The public health response to the measles outbreak in
provinces is targeted at improving measles immunity in all
affected provinces.

Strengthening surveillance and coordination of
the response to the outbreak implemented at both
provincial and district levels is ongoing.

Provinces have conducted vaccination campaigns in
the affected areas. Routine immunization should be
reinforced as the outbreak is an opportunity to identify
and correct vaccination programme weaknesses.

Routine Immunization has been strengthened in
affected districts and provinces

Follow up on laboratory-confirmed measles cases for
case management support

Follow up and vaccinate the contacts of laboratory
confirmed cases.

Continue to support the response with additional
logistics including, medicines and medical
consumables for case management.

SITUATION INTERPRETATION

Overall, the incidence of measles appears to be decreasing across South Africa. However, Limpopo province has remained stable and largely contributed to the increased number of cases seen in the previous week. Continuous surveillance for measles cases in all provinces is recommended.

Measles outbreak update: Most cases still recorded in Limpopo
According to the latest NICD data on the measles outbreak in SA, most cases appear to still be concentrated in Limpopo.

Since a measles outbreak was declared in Limpopo in October 2022, the National Health Department has declared outbreaks in all provinces, except the Eastern Cape.

Limpopo latest: Eight new measles cases

According to the latest data (which show laboratory-confirmed cases up until 3 June 2023), the National Institute for Communicable Diseases (NICD) reported that eight new cases have been confirmed in Limpopo, with a sporadic case in Gauteng.

In the week ending on 25 May 2023, a total of 28 laboratory-confirmed measles cases were detected across the country, of which 22 cases were from Limpopo.

The NICD stressed that the Limpopo province continues to contribute a significant number of cases, primarily among children aged five to nine.

As per the latest situation report dated 9 June, the NICD tested 6264 serum samples for measles since October 2022, of which 17% (or 1060 cases) were confirmed positive.

To date, Limpopo has recorded 476 cases; 109 in Mpumalanga; 217 in the North West; 176 in Gauteng; 32 in the Free State; 16 in the Western Cape; 21 in KwaZulu-Natal and seven in the Northern Cape...​

Highlights

The NICD has tested 6460 serum samples for measles since epidemiological week 40, 2022, of which 1100 (17%) were confirmed positive. In the past week (week 25 up until week 26, 01/07/2023) there have been 10 laboratory-confirmed measles cases detected across the country, most of which were from Limpopo (6).
The percentage of samples testing positive (PTP) increased from 17% (7/41) of samples tested in week 25 to 43% (3/7) of samples tested in week 26.
In week 25 (week ending 25/06/2023), Limpopo province reported a total of five (5) new measles cases, while sporadic cases were reported in Gauteng (1) and Western Cape (1).
To declare the measles outbreak over, the following criteria should be achieved: Absence of newly detected Measles cases for more than two incubation periods (42 days). The outbreak was declared over for the Northern Cape in week 15, North West in week 24 and Free State in week 25. Unfortunately, due to the ongoing sporadic cases, the outbreak cannot be declared over in Gauteng, Limpopo, Mpumalanga and the Western Cape.
In week 25 Limpopo province reported a total of 5 cases, Waterberg (2) Greater Sekhukhune (2) and one (1) case from Capricorn district. Only one case from the Capricorn district has been reported for week 26
The vaccination campaign in Limpopo province targeting the 5-15 years age group has come to an end with overall vaccination coverage of 56%. To achieve a higher coverage rate, the NICD is informed that a mop-up campaign will be conducted from 24 July 2023 to 31 August 2023.

Measles outbreak in South Africa declared over with lingering cases
The NICD reported only 5 laboratory-confirmed measles cases across the country.

The National Institute for Communicable Diseases (NICD) in South Africa has declared the measles outbreak officially over based on laboratory testing data up until 17 July 2023.

According to the NICD, the outbreak, which began in 2022, saw a total of 1106 confirmed measles cases reported across eight provinces.

The NICD conducted extensive testing, analysing 6 523 serum samples for measles, of which 1 114 (17%) were confirmed positive.

In the recent weeks leading up to 17 July 2023, the NICD reported only 5 laboratory-confirmed measles cases across the country.

Among these cases, three were from Gauteng, and one each from Limpopo and KwaZulu-Natal.

The NICD said the outbreak is effectively over; however, it continues to observe sporadic cases in certain provinces....​

The South African National Institute for Communicable Diseases (NICD) has called for increased surveillance of new measles cases in Gauteng, this as the province has shown an increase in reported cases over the last two weeks.

The Institute says that for five weeks the recorded just five positive measles. However, that number has since increased 28 new cases between epidemiological weeks 34-36 across the country.

In the week ending 9 September 2023, 16 new cases were announced, 11 of which were located in Gauteng...

Western Cape health officials note a spike in German measles cases
Charlene Lawrence - who is the deputy director of communicable diseases control at the Western Cape Department of Health and Wellness - said there have been 86 cases reported in the province since January this year.​

CAPE TOWN - The Western Cape has noted a spike in German measles cases in the province. Health officials said they managed to identify 32 cases of the infection in Khayelitsha in seven days alone.

Last week - officials issued an alert after recording an increase in German measles in the province.​..

An outbreak of German measles was reported at the Phillipsvale Primary School in Phillipstown in the Northern Cape.

Geoffrey van der Merwe, media liaison officer of the Northern Cape Department of Education, said the local Masibambane Clinic has so far recorded 26 confirmed laboratory cases of German Measles, which is a contagious viral infection, best known by its distinctive red rash...
