Measles outbreaks confirmed in five provinces, with test positivity rate skyrocketing

The National Institute for Communicable Diseases (NICD) has urged parents to get their children vaccinated after a measles outbreak that has spread to five provinces in fewer than two months, with data revealing dismal vaccination rates.

The first outbreak was confirmed in Limpopo in October 2022.

“It is never too late to vaccinate,” the NICD’s statement reads, adding that major vaccination campaigns have commenced or are being planned countrywide...

...Between October and December, the NICD tested 2,252 samples for measles antibodies, of which 14% tested positive.

Since the first week of December, however, the positivity rate in measles cases jumped to above 20%, with new cases in Limpopo, Mpumalanga (69 cases), North West (80), Gauteng (14) and the Free State (11).

A single case was reported in the Eastern Cape, five in KwaZulu-Natal, three in the Northern Cape and four in the Western Cape. The World Health Organization (WHO) classifies a measles outbreak as three or more cases reported within 30 days in a defined district. However, the cases in these provinces were confirmed further apart than 30 days.

“The number of cases continues to increase daily as blood samples and throat swabs are submitted to the NICD for [testing],” a statement from the NICD reads.

The outbreak was first declared in Limpopo on 11 October 2022, with most patients between the ages of five and nine, while some babies and adults were also infected. Of the 131 measles cases confirmed in the province, 80% had an “unknown” vaccination status, 9% were vaccinated and 11% were unvaccinated.

In Mpumalanga, an outbreak of measles was declared in November — 63% of those infected had an unknown vaccination status, 13% were vaccinated and 24% were unvaccinated.

In North West, an outbreak was declared in December. The Lonely Park Clinic in Mafikeng reported 38 cases. Only 10 of the 80 cases confirmed in this province had been vaccinated.

In Gauteng, 14 cases were reported, and an outbreak was declared on 6 December 2022. Three cases were reported at the Ethafeni Clinic in Ekurhuleni. Five of the 14 cases were reported in Tshwane and 13 of these cases had an “unknown” vaccination status.

In the Free State, the Bethlehem Clinic reported five of the province’s 11 cases...

As the number of measles cases continues to rise, a national vaccine drive targeting the disease has started earlier in the five provinces battling the outbreak. The health ministry had initially planned to roll out the national vaccine campaign next month...

On Thursday January 12, there were 75 cases reported and before that the spread appeared to had slowed down as the cases reported between December 29 and January 6, was only one. Highlights

Since the last Situation Report, 24 new laboratory-confirmed measles cases have been detected across the country according to data up until 21 January (mid-week-03, 2023). This brings the number of measles cases in South Africa since epidemiological week 40 of 2022 to 421.
In outbreak-affected provinces, a total of 406 measles cases have been detected from week 40 of 2022 until mid-week 03 of 2023. In week 02, 2023, 16 cases were diagnosed, whilst to date, (mid-week through week 03), 14 cases have been identified.
The percentage testing positive midway through epidemiological week 03 has risen to 14% (14 positive/103 specimens tested) from a low of 9% during epidemiological week 02 (16 positive / 186 tested).
In week 03, 2023, the largest number of cases are from Gauteng Province, with 6 cases arising from clinics within a 2km radius in Ekurhuleni (3 from Daveyton Main clinic, two from Philip Moyo Clinic, one from Crystal Park Clinic and one from Bertha Nxowa Hospital). This focus of infection requires immediate action including a vaccination campaign to prevent exponential spread here, and within and beyond the province over the next few weeks.
The age distribution and origin of cases remain largely unchanged with the majority of measles cases in outbreak-affected provinces being among the 5-9-year age group (160/406, 39%), and the highest proportion of cases arising from hospitals as opposed to primary health clinics in the under-1 year age group (15/27, 59%). However, significant numbers of cases in the 10-14 year age group continue to occur (65/406, 16%), justifying the need for a vaccination campaign to include this age group.

EVENT DESCRIPTION

The Measles outbreak that was declared in Limpopo province on
11 October 2022 continues with an increasing trend.

Since the start of the outbreak on 11 October 2022 to 18 January
2023, the NICD has tested 3 326 serum samples for measles, of
which 397 (11.9%) were confirmed measles cases.

As of 18 January, 382 confirmed cases have been reported in
the five provinces with active measles outbreaks: Limpopo
(145 cases), North West (125 cases), Mpumalanga (79 cases),
Gauteng (18 cases), and Free State (15 cases), with Limpopo and
North West accounting for 71% of all reported cases. No measles
related deaths have been reported.

The age of reported cases across the five affected provinces
ranges from two months to 60 years. Children between the 5-9
years age group are the most affected 153 (40%), followed by
112 (29%) in the 1-4 years age group and 64 (17%) in the 10-14
year group. Disease severity has been observed within the less
than 1-year age group as evidenced by majority of health facility
admissions in this age-group (56%).

The highest attack rate has been observed among the 5-9 (5.9 per
100 000 population) and 1-4 years (5.0 per 100 000 population)
age groups.

Vaccination status for laboratory-confirmed measles cases
in provinces with a declared measles outbreak indicates that
75 (20%) have known vaccination status, while 175 have an
unknown vaccination history.

EVENT DESCRIPTION

The South African Health authority are supporting the following
activities in response to the measles outbreak. The public health
response to the measles outbreak in provinces is targeted at
improving measles immunity in all affected provinces.

Provinces are currently conducting vaccination campaigns
in the affected areas. Routine immunization should be
reinforced as the outbreak is an opportunity to identify and
correct vaccination programme weaknesses.

Routine Immunization has been strengthened in affected
districts and provinces

Strengthening surveillance and coordination of the response
to the outbreak implemented at both provincial and district
levels is ongoing.

Follow up on laboratory-confirmed measles cases for case
management support

Follow up and vaccinate the contacts of laboratory confirmed
cases.

Continue to support the response with additional logistics
including, medicines and medical consumables for case
management.

SITUATION INTERPRETATION

South Africa has seen a declining trend in the number of
laboratory-confirmed measles cases. Even when this may signify
decreasing transmission rates, other reasons for the decline
in cases may have arisen through decreased health seeking
behaviour. Clinicians should be on high alert as the opening of
schools may lead to localised outbreaks where vaccination rates
are sub-optimal. Vaccination strengthening efforts must continue
to get the outbreak under control.

The NICD has currently tested 3156 serum samples for measles of which 459 (14.5%) were confirmed positive from epidemiological week 40, 2022. In the past weeks (week 03 up until mid-week 04, 2023), there have been 43 new laboratory-confirmed measles cases detected across the country.
In outbreak-affected provinces, a total of 441 measles cases have been detected from week 40 of 2022 until week 04 of 2023.
In Gauteng Province, in weeks 03 and 04, 2023, 6 cases were notified at clinics in Daveyton, Ekurhuleni (3 from Daveyton Main clinic, two from Philip Moyo Clinic, one from Crystal Park Clinic) and 3 from the Germiston area (1 from Bertha Nxowa Hospital, one from Germiston City clinic and 1 from Wannenburg clinic). A single case was identified in Soshanguve, City of Tshwane.
In North West province, in weeks 03 and 04, 12 of 15 cases originate from Mahikeng (Lonely Park, Mocoseng and Setlopo clinics, and Mahikeng provincial hospital). Two cases are reported from Ramabesa clinic and Makapanstad health Centre in Bojanala
Amongst the 11 cases reported from Limpopo province, in weeks 03 and 04, three are from Giyani (Mapayeni clinic) and two from Makhado (Louis Trichardt Hospital and Clinic)

Measles cases in Gauteng have climbed to 37.

The provincial health department said Ekurhuleni had 25 infections, Tshwane six, Johannesburg four, and Sedibeng had two infections.

The recent developments come amid the department's measles vaccination campaign in which children between 6 months and 15 years are immunised at community health care centres...

The department set an ambitious target to immunise 4 million children in Gauteng.

On Friday, it had vaccinated 436 884 children in the five cities with 124 010 children vaccinated in Ekurhuleni, 153 383 in Johannesburg, 21 267 in Sedibeng, 87 180 in Tshwane and 51 044 in West Rand...

Cape Town – Officials from the City of Cape Town’s health department are joining a national campaign to curb a measles outbreak.

The campaign, from 6 February to 31 March, comes in the wake of a measles outbreak in five provinces.

The Western Cape has not experienced a measles outbreak, but there have been five reported cases in the province, the City said.

Highlights

The NICD has tested 3279 serum samples for measles since epidemiological week 40, 2022, of which 506 (15.4%) were confirmed positive. In outbreak-affected provinces, 484 cases have been reported since week 40, 2022. In the past weeks (week 04 up until mid-week 05, 2023), there have been 58 laboratory-confirmed measles cases detected across the country of which 56 are from outbreak-affected provinces.
In week 04, the percentage of samples testing positive (PTP) increased to 25% of the 330 samples tested, compared with a lower PTP (under 10%) in week 03, 2023. Whilst the PTP for week 05 looks lower, the data for epidemiological week 05 will be reviewed in the next situation report when all samples collected during week 05 have been tested.
All Gauteng cases, except two, originate from Ekurhuleni (Daveyton and Germiston). The two cases not from Ekurhuleni are from Tshwane (Hammanskraal) and Johannesburg (Olievenhoutbosch).

The NICD has tested 3694 serum samples for measles since epidemiological week 40, 2022, of which 560 (15.1%) were confirmed positive. In outbreak-affected provinces, 537 cases have been reported since week 40, 2022. In the past weeks (week 05 up until mid-week 06, 2023), 76 laboratory-confirmed measles cases were detected across the country, of which 73 are from outbreak-affected provinces.
In week 05, the percentage of samples testing positive (PTP) increased to 20% of the 251 samples tested, compared with a lower PTP (13%) in week 04, 2023. The PTP for week 06 has further increased to 25%.

The NICD has tested 4019 serum samples for measles since epidemiological week 40, 2022, of which 615 (15.3%) were confirmed positive. In outbreak-affected provinces, 586 cases have been reported since week 40, 2022. An outbreak has been declared in the Western Cape Province following the detection of four laboratory-confirmed measles in the City of Cape Town between 24 January 2023 to 17 February 2023. All cases are epidemiologically unlinked; however, two cases aged 2 and 4 years, are from the Southern Subdistrict and presented at facilities only 4km from each other (Grassy Park and Sea Winds CHC). The third case (10 years old) is from Khayelitsha and the fourth is likely a sporadic case in a 58-year-old commercial pilot. The Western Cape commenced with their vaccination campaign targeting 0-15 year-olds on 6th February 2023. These cases are not reflected in this sitrep as they have occurred outside the reporting interval for this week.

The NICD has tested 4250 serum samples for measles since epidemiological week 40, 2022, of which 665 (15.6%) were confirmed positive. In outbreak-affected provinces, 641 cases have been reported since week 40, 2022. In the past weeks (week 07 up until mid-week 08, 2023), 72 laboratory-confirmed measles cases were detected across the country, of which 68 are from outbreak-affected provinces.
The percentage of samples testing positive (PTP) increased slightly from 18% of 298 samples tested in week 06 to 20% of the 284 samples tested in week 07
Since the declaration of the outbreak in the Western Cape in epidemiological week 7, no new cases have been reported.
An outbreak has been declared in the Northern Cape Province following the detection of three laboratory-confirmed cases in the Francis Baard district. Three laboratory-confirmed measles cases were reported in a single facility in Kimberley on the 28th of February, taking the total number of cases to 4. The measles campaign began on the 6th of February 2023. These cases are not reflected in this sitrep as they occurred outside this week’s reporting interval.

From epidemiological week 40, 2022 (ending 8 October 2022) to mid-week 08, 2023 (ending 25 February 2023) the NICD has tested 4250 serum samples for measles of which 665 (15.6%) were confirmed measles cases. The percentage of samples testing positive (PTP) decreased slightly from 20% of 305 samples tested in week 07 to 18% of the 274 samples tested in week 08.
A measles outbreak has now been declared in all the provinces in South Africa except for the Eastern Cape.
The measles strain detected in Limpopo province and North West province is genotype D8, similar to the strain in Zimbabwe in the 2022 outbreak.
In the provinces where an outbreak has been declared, the most affected age groups are still the 5–9-year-olds (42%) with a considerable proportion of cases reported among the 1-4 (25%) and 10-14 age groups (19%). Vaccination campaigns should therefore also include children aged 10 to 14.

Outbreak overview

From epidemiological week 40, 2022 (ending 8 October 2022) to mid-week 09, 2023 (ending 4 March 2023) the NICD has tested 4608 serum samples for measles of which 727 (15.8%) were confirmed measles cases. The number of samples submitted, and the percentage of laboratory-confirmed measles-positive cases are shown in Figure 1. From epidemiological week 40 of 2022 to week 09 of 2023, 721 laboratory-confirmed cases were reported from eight provinces with declared measles outbreaks; Limpopo (255 cases), Mpumalanga (102 cases), North West (196 cases), Gauteng (107 cases), Free State (27 cases), Western Cape (10), KwaZulu-Natal (17) and Northern Cape (7) (Table 1). Situation at a glance

Sporadic cases of measles were reported in South Africa throughout 2022. In epidemiological week 40 in 2022 (ending 8 October 2022) an outbreak was declared in Limpopo province. As of 16 March 2023, confirmed cases have been reported from all provinces; eight out of nine South African provinces have declared measles outbreaks[SUP]1[/SUP]. No deaths associated with measles have been recorded. Most cases (86%) are reported among those aged under 14 years.
Community-based surveillance has been strengthened and the Ministry of Health (MoH) is conducting a mass measles vaccination campaign targeting children aged between 6 months and 15 years in all provinces.
WHO assesses the risk posed by the current outbreak as high at the national level, moderate at the regional level and low at the global level.

Description of the situation
South Africa is a measles-endemic country, with several measles outbreaks reported in recent years.
Sporadic cases were reported in all nine provinces in South Africa in 2022. As of 4 March 2023, measles outbreaks were declared in eight provinces, further to the outbreak which was declared in Limpopo province in October 2022 (Figure 1).

From epidemiological week 40, 2022 (ending 8 October 2022) to week 10, 2023 (ending 7 March 2023), the National Institute of Communicable Diseases (NICD) tested 4830 serum samples for measles, of which 772 (16%) were confirmed measles cases (Figure 2).

The 772 laboratory-confirmed measles cases were reported from the provinces of: Limpopo (275; 36%), North West (198; 26%), Gauteng (124; 16%), Mpumalanga (106; 14%), Free State (28; 3.5%), KwaZulu-Natal (17; 2%), Western Cape (11; 1.5%), Northern Cape (7; 1%) and Eastern Cape (6; 1%) (Figure 3).

The age of laboratory-confirmed measles cases ranges from two months to 60 years. The majority of cases (42%) are in the age group 5-9 years, followed by the age groups 1-4 years (25%) and 10-14 years (19%). The attack rates (per 100 000 population) are highest among age groups 1-4 years (4.7 / 100 000) and 5-9 years (6.6 / 100 000).
Out of the laboratory-confirmed cases, 80 (10%) were vaccinated with at least one dose of measles-containing vaccine (MCV), 92 (12%) were unvaccinated, and the vaccination status of 570 (79.1%) is unknown.
According to the South African National Department of Health (NDOH), national immunization coverage of the measles-containing vaccine first dose (MCV1) and second dose (MCV2) was estimated at 87% and 82% respectively in 2021, while in 2022 the respective coverage was 86% and 86%. However, historical vaccination coverage has been low, which may be contributing to the current resurgence. According to WHO-UNICEF estimates, in 2018, MCV1 coverage averaged 81% and MCV2 75%. In 2017, MCV1 and MCV2 coverage was estimated at 81% and 78%.

Epidemiology of measles

Measles is caused by a virus in the paramyxovirus family. The virus infects the respiratory tract, then spreads throughout the body. Measles is a human disease and is not known to occur in animals. It can lead to major epidemics with significant morbidity and mortality, especially among vulnerable people. Among young and malnourished children, pregnant women and immunocompromised individuals, including those with HIV, cancer or treated with immunosuppressives, measles can cause serious complications, including ear infection, severe diarrhea, blindness, encephalitis, pneumonia, and death.
Transmission is primarily person-to-person by airborne respiratory droplets that disperse rapidly when an infected person coughs or sneezes. Transmission can also occur through direct contact with infected secretions. Transmission from asymptomatic exposed immune persons has not been demonstrated. The virus remains active and contagious in the air or on infected surfaces for up to two hours. A patient is infectious from four days before the start of the rash to four days after its appearance. There is no specific antiviral treatment for measles and most people recover within 2-3 weeks.
An effective and safe vaccine is available for prevention and control. The measles-containing vaccine first dose (MCV1) is given at the age of nine months, while the second dose of the measles-containing second dose (MCV2) is given at the age of 15 months. A 95% population coverage of MCV1 and MCV2 is required to stop measles circulation.
In areas with low vaccination coverage, epidemics typically occur every two to three years and usually last between two and three months, although their duration varies according to population size, crowding, and the population’s immunity status.
South Africa has had several outbreaks in recent years. Between 2003 and 2005, an outbreak occurred with 1676 cases reported. In 2009-2010, a large outbreak occurred with 18 431 documented cases. In 2017, a small outbreak was declared in Western Cape, Gauteng, and Kwazulu-Natal provinces, with a total of 186 cases. In 2019, a cluster of measles cases was reported in Cape Town affecting four siblings with a travel history to a measles-affected country.

Public health response

The Ministry of Health has implemented the costed national measles response plan with the support of WHO. The NDOH is conducting a mass measles vaccination campaign in all provinces, targeting children aged 6 months - 15 years. WHO staff have been deployed to support all aspects of the response: coordination; information management and surveillance; case management; supplies and logistics; and risk communication and community engagement.
Other response activities include:
  • Active case search, investigation and line listing of fever and rash cases;
  • Weekly measles incident management support team (IMST) meeting conducted with all provinces;
  • Weekly interim situation reports based on the results of laboratory tests being issued by the NDOH and NICD;
  • Deployment of provincial and district rapid response teams (RRTs) to affected neighbourhoods;
  • Training in preparation for the national immunization campaign, completed in all provinces;
  • A local vaccination campaign targeting children aged 6 months to 5 years, conducted in Limpopo province;
  • Training health workers on measles case identification and supportive case management to prevent measles complications and mortality
  • Strengthening of community-based surveillance for early detection of cases using community health volunteers, with support from WHO;
  • Raising community awareness via community health workers;
  • Mapping of partners and resources to support the outbreak response, supported by WHO and partners.
WHO risk assessment

Four of the provinces affected by the current outbreak (Limpopo, Mpumalanga, Northwest, and KwaZulu-Natal) border Zimbabwe, Botswana and Mozambique. The cross-border movement of populations across the Southern African Development Community (SADC) region[SUP]2[/SUP] and the weak disease surveillance health systems in many SADC countries pose a threat for further international spread.
There is an ongoing measles outbreak in neighboring Zimbabwe. Response capacities in the region are also constrained due to the concurrent readiness and response efforts to COVID-19, cholera, mpox, wild poliovirus, floods and cyclones.
Based on the currently available information, the overall risk of measles in South Africa is assessed as high for the following reasons:
  • The current measles outbreak has been declared in eight out of nine provinces, and cases have been confirmed in all provinces, with further spread likely due to a highly mobile population;
  • The 2022 measles risk assessment reported 86% of districts as having medium to high risk for measles, based on the WHO measles risk assessment tool;
  • Suboptimal national measles vaccination coverage (in 2022, the coverage was 86% for both MCV1 and MCV2, which is below the required coverage of 95% to achieve population herd immunity and thus prevent sustained transmission);
  • The presence of high rates of HIV/AIDS and TB, which can worsen the clinical picture of measles, in the South African population;
  • Floods reported in seven provinces, which have rendered sections of the population inaccessible during the ongoing measles vaccination campaign;
  • The need for technical and logistical support to provinces, as most essential services and programs (including immunization) were impacted by the COVID-19 pandemic and are only now recovering from its devastating impact.




WHO advice

Although measles is highly contagious, an effective and safe vaccine is available for prevention and control. MCV1 is given at the age of 9 months in countries with both ongoing transmission and a high risk of measles mortality among infants. These countries should administer the routine MCV2 at the age of 15-18 months. However, in South Africa, MCV1 is given at 6 months and MCV2 is given at 12 months[SUP]3[/SUP]. A 95% population coverage of MCV1 and MCV2 is required to stop measles circulation. Reaching all children with two doses of the measles vaccine is recommended to ensure immunity and prevent outbreaks, as about 15% of vaccinated children fail to develop immunity from the first dose.
Routine vaccination of children against measles, combined with mass immunization campaigns in countries with high morbidity and mortality rates, are key public health strategies to reduce burden and transmission.
There is no specific treatment for measles. Case management of measles focuses on supportive care as well as the prevention and treatment of measles complications and secondary infections. Since measles is highly contagious, patient isolation is an important intervention to prevent the further spread of the virus.
Oral rehydration salts should be used as needed to prevent dehydration. All children diagnosed with measles should receive two doses of vitamin A oral supplements, given 24 hours apart, irrespective of the timing of previous doses of vitamin A; 50 000 international units (IU) should be given to infants aged < 6 months, 100 000 IU to infants aged 6-11 months and 200 000 IU to children aged 12 months. This treatment restores low vitamin A levels in acute measles cases that occur even in well-nourished children, and can help prevent eye damage and blindness. Vitamin A supplements have also been shown to reduce the number of measles deaths.
Nutritional support is recommended to reduce the risk of malnutrition due to diarrhoea, vomiting and poor appetite associated with measles. Breastfeeding should be encouraged where appropriate.
In unimmunized or insufficiently immunized individuals, the measles vaccine may be administered within 72 hours of exposure to the measles virus to protect against disease. If the disease does subsequently develop, symptoms are usually less severe, and the duration of illness may be shortened.
WHO does not recommend any restrictions on travel or trade to or from South Africa.

Measles outbreak declared in eight provinces

Durban – According to the National Institute for Communicable Diseases (NICD) and the Department of Health the measles outbreak has been declared in all provinces in the country except for the Eastern Cape.

The department said of the 5 507 tests conducted since October 8, 2022, 911 (17%) were confirmed positive, with Limpopo currently leading in newly confirmed cases.

The recent numbers published by the departments indicate that Limpopo has 362 cases, North West 213, Gauteng 156, Mpumalanga 107, Free State 30, KwaZulu-Natal 18, Western Cape 12, Northern Cape 7 and Eastern Cape 6...

Highlights

The NICD has tested 5612 serum samples for measles since epidemiological week 40, 2022, of which 931 (17%) were confirmed positive. In outbreak-affected provinces, 925 cases have been reported since week 40, 2022. In the past weeks (week 13 up until week 14, 04/04/2023) there have been 32 laboratory-confirmed measles cases detected across the country all of which were from Limpopo, Gauteng, and North West provinces.
The percentage of samples testing positive (PTP) increased from 20% of 201 samples tested in week 12 to 26% of 111 samples in week 13.
Measles outbreak has been declared in all the provinces in South Africa except for the Eastern Cape. In week 13, no new cases were reported in Free State, KwaZulu-Natal, Mpumalanga, Northern Cape and Western Cape provinces.
The measles strain detected in Limpopo province and North West province is genotype D8 which is similar to the strain in Zimbabwe in the 2022 outbreak.
In the provinces where an outbreak has been declared, the most affected age groups are still the 5–9-year-olds (43%) with a considerable proportion of cases reported among the 1-4 (24%) and 10-14 age groups (19%).
Vaccination campaigns have so far included all children including those aged 10 to 14.
The majority of cases (67%) were reported from primary healthcare facilities, and the highest proportion of cases reported from hospitals (58%) was reported in children under the age of one.
Members of the public are urged to ensure their children are vaccinated against measles.
Nationally, the reproduction number as of 2023-03-29 was estimated to be 0.68 (0.33 – 1.3), suggesting that infection incidence is likely decreasing. There is an 85% chance that the reproduction number was below 1 as of 2023-03-29.
At the provincial level, the reproduction number as of 2023-03-29 was estimated to be 0.98 (0.7 – 1.2) in Limpopo, suggesting that infection incidence is stable. In contrast, the reproduction number as of 2023-03-29 was estimated to be 0.62 (0.32 – 1) in Mpumalanga, 0.82 (0.55 – 1) in Gauteng, and 0.65 (0.38 – 0.93) in North West, suggesting that infection incidence is likely decreasing...
