SUMMARY

A total of 150 new confirmed cases of Ebola virus disease (EVD) were reported in the week to 15 March, compared with 116 the previous week. There were 95 new confirmed cases reported in Guinea: the highest weekly total for the country in 2015. Sierra Leone reported 55 new confirmed cases over the same period: the country?s lowest weekly total since late June 2014. Liberia reported no new confirmed cases for the third consecutive week. March 15 was day 12 since the final patient in Liberia had a second negative test for EVD (42 days must elapse before transmission can be considered to have ended).

A total of 12 districts in Guinea and Sierra Leone reported a confirmed case in the week to 15 March, all of which lie on a geographically contiguous arc in and around Conakry to the north and Freetown to the south. An additional 4 districts have reported a confirmed case in the past 21 days: Kono and Tonkolili in central and eastern Sierra Leone, and Lola and Macenta in eastern Guinea.

Though transmission is currently confined to a relatively narrow geographic corridor, the population is highly mobile, with a great deal of movement throughout surrounding districts and countries. Limiting the movements of cases and contacts is challenging but essential to prevent the seeding of new outbreaks.

Key response indicators for Guinea suggest that there remain significant challenges to overcome before transmission is brought under control. Of 49 total reported EVD deaths in the week to 15 March, almost half (23) were identified post-mortem in the community. In the week to 8 March, a low proportion (28%) of confirmed cases arose from registered contacts, and there were a reported 18 unsafe burials. Taken together, these indicators suggest that the outbreak in Guinea is still being driven by unknown chains of transmission.

In the week to 15 March a total of 125 suspected cases of EVD were reported in Liberia, none of whom tested positive for EVD. All contacts associated with the last known chain of transmission have now completed 21-day follow-up.

By contrast with Guinea, key response indicators for Sierra Leone present a more promising outlook. In the week to 8 March over two-thirds (67%) of confirmed cases came from registered contacts, whilst in the week to 15 March, 6 of 62 total EVD-confirmed deaths were identified post-mortem in the community. There was 1 reported unsafe burial over the same period. However, there are still areas where most new cases arise from unknown chains of transmission. Kambia, a district north of Freetown on the border with the Guinean prefecture of Forecariah, reported 7 new cases in the week to 8 March, 5 of which came from post-mortem testing of people who had died in the community and who were not known to be contacts of a previous case.

COUNTRIES WITH WIDESPREAD AND INTENSE TRANSMISSION

There have been over 25 000 reported confirmed, probable, and suspected cases of EVD in Guinea, Liberia and Sierra Leone (table 1), with over 10 000 reported deaths (outcomes for many cases are unknown). A total of 95 new confirmed cases were reported in Guinea, 0 in Liberia, and 55 in Sierra Leone in the 7 days to 15 March.

The total number of confirmed and probable cases is similar in males and females (table 3). Compared with children (people aged 14 years and under), people aged 15 to 44 are approximately three times more likely to be affected. People aged 45 and over are nearly four times more likely to be affected than children.

A total of 852 confirmed health worker infections have been reported in Guinea, Liberia, and Sierra Leone; there have been 492 reported deaths (table 5).

SIERRA LEONE
  • Key performance indicators for the EVD response in Sierra Leone are shown in table 6.
  • A total of 55 confirmed cases were reported in the week to 15 March, compared with 58 the previous week. This is the lowest weekly total since late June 2014.
  • Cases were reported from 6 northern and western districts around and including the capital Freetown, which reported 29 new confirmed cases. The neighbouring districts of Bombali (6 cases), Kambia (4 cases), Port Loko (11 cases) and Western Rural (3 cases) also reported cases.
  • Two other districts, Kono and Tonkolili, have reported a confirmed case within the past 21 days.
  • More than two-thirds (39 of 58: 67%) of confirmed EVD cases arose among known contacts in the week to 8 March; the number of confirmed cases identified after post-mortem testing of dead bodies found in the community fell to 6 in the week to 15 March. According to the National Ebola Response Centre, 92% of credible reports of potential EVD cases were investigated within 24 hours in the week to 8 March. Potential cases were identified through contact tracing or case finding, or from reports to a dedicated national Ebola alert hotline.
  • Locations of the 20 operational Ebola treatment centres (ETCs) in Sierra Leone are shown in figure 6. A total of 12 of 17 assessed ETCs met minimum standards for IPC, along with 9 of 15 community care centres.

SIERRA LEONE

Key performance indicators for the EVD response in Sierra Leone are shown in table 6.
A total of 25 confirmed cases were reported in the week to 29 March, compared with 33 the previous week. This is the fourth consecutive weekly decrease and the lowest weekly total since the final week of May 2014.

Cases were reported from 5 northern and western districts around and including the capital Freetown, which reported 10 new confirmed cases. The neighbouring districts of Bombali (1 case), Kambia (5 cases), Port Loko (6 cases) and Western Rural (3 cases) also reported cases (figure 2; figure 5; figure 6).

The proportion of confirmed EVD cases that arose among contacts decreased to 67%, from 84% the previous week (table 6). The proportion of confirmed cases identified after post-mortem testing of dead bodies found in the community increased slightly, from 7 of 56 (13%) in the week to 22 March to 8 of 52 (15%). Of those 8, half were reported from the district of Kambia, where community engagement has proved challenging.

According to the National Ebola Response Centre, 95% of 451 credible reports of potential EVD cases were investigated within 24 hours in the week to 22 March: an increase compared with the previous week. Most (87%) of alerts come from Bombali (39), Port Loko (35), and Western Area (317). The district of Kambia recorded 10 alerts. Potential cases were identified through contact tracing or case finding, or from reports to a dedicated national Ebola alert hotline. Heightened vigilance is being maintained throughout the country: of 1606 new samples tested in the week to 29 March, 2% were EVD positive. Over 100 suspected cases were reported during the same period, compared with 57 the previous week. The majority of suspected cases (52) were reported during the final 2 days of the country’s 3-day stay-at-home.
In the context of the 3-day stay-at-home period in Sierra Leone that ended on 29 March, surveillance has been increased at 53 border crossing points between Guinea and Sierra Leone. Guinean authorities indicated that the prefecture of Forecariah, which borders Kambia, will also have a stay-at-home period to reinforce the efforts of Sierra Leonean authorities.

  • Key performance indicators for the EVD response in Sierra Leone are shown in table 6.
  • A total of 8 confirmed cases were reported in the week to 17 May, compared with 2 the previous week (figure 5), ending a sequence of 3 consecutive falls in weekly case incidence.
  • Three districts reported confirmed cases in the week to 17 May, compared with a single district the previous week (figure 3, figure 6). Of 14 districts in Sierra Leone that have ever reported a confirmed case since the beginning of the outbreak, 9 have not reported a confirmed case for over 6 weeks (figure 6).
  • Half of the cases reported from Sierra Leone occurred in three neighbourhoods in the north of the capital, Freetown (table 3, figure 2). Of these, three have a proven epidemiological link to a previous case. The remaining case, reported from Freetown, was identified after post-mortem testing of a community death. The source of infection is unknown, but the case was found in the Moa Wharf area.
  • The district of Port Loko, just to the north of Freetown, reported cases for the first time in over 3 weeks. All 3 cases were reported from the Chiefdom of Kaffu Bullom, and all were registered contacts with an epidemiological link to a previous case reported from the district of Kambia.
  • The district of Kambia, which borders the Guinean prefecture of Forecariah to the north and Port Loko to the south, and which reported zero cases in the week to 10 May, reported a case from the Chiefdom of Magbema in the week to 17 May. The case was a registered contact of a previous case.
  • A total of 599 contacts are currently being monitored daily in 3 districts: 47 in Port Loko, 82 in Kambia, and 470 in Western Area Urban, which includes Freetown.
  • There were 415 credible alerts of sick people with possible Ebola-like symptoms received in the week to 10 May (the most recent week for which data are available). Of these 415, 404 (97%) were responded to within 24 hours. Over 90% of alerts were reported by the public.
  • After 5 consecutive weeks without reporting a health worker infection, a Sierra Leone national working at an Ebola treatment centre near Freetown was laboratory confirmed as EVD positive on 14 May. The national was working at the same facility as the recent Italian health worker case. Investigations are ongoing into how both health workers came to be exposed to EVD.
  • Laboratory indicators continue to reflect a heightened degree of vigilance, with 1654 new samples tested in the week to 17 May. Less than 1% of samples tested positive.

SIERRA LEONE

A total of 15 confirmed cases were reported from 2 districts in the week to 31 May, compared with 12 cases from 3 districts the previous week (table 3, figure 2, figure 3, figure 5, figure 6). The weekly total is the highest since March.

As was the case the previous week, most (7) cases were reported from a densely populated area of the Kaffu Bullom chiefdom in Port Loko. All of the cases were registered contacts of previous cases within quarantined houses in the chiefdom (table 4). However, a cluster of 3 cases was also reported from the Bureh Kasseh Ma chiefdom of the same district, 2 of which arose from an as-yet unknown chain of transmission. These cases are not thought to be directly linked to those in Kaffu Bullom, but rather to a chain of transmission in the neighbouring district of Kambia.

After reporting its first case in more than 2 weeks the previous week, 5 cases were reported from 2 chiefdoms in Kambia during the week ending 7 June (figure 2, table 3, table 4). A total of 4 cases, all of whom were registered contacts, were reported from the Tonko Limba chiefdom. However, the single case reported from the Magbema chiefdom arose from an as-yet unknown chain of transmission, and was detected only after post-mortem testing of a community death (table 4). Community engagement has continued to be challenging in several chiefdoms in Kambia, and investigations into recent cases there have documented the occurrence of unsafe burials that have previously gone unreported. Improved communication with local communities is essential to understand and address any concerns or disincentives that prevent cases, deaths, and burials from being reported.

The Western Urban Area of Sierra Leone, which includes the capital Freetown, reported no cases for the first time since August 2014. However, there were still 195 contacts under follow-up in the district as at 7 June, and a total of 392 contacts nationally in 3 districts (Kambia and Port Loko are the other 2 districts).

No health worker infections were reported in the week to 7 June.

Laboratory indicators continue to reflect a heightened degree of vigilance, with 1752 new samples tested in the week to 7 June. Less than 1% of samples tested positive. As in Guinea, over 80% of samples were collected from dead bodies, although samples were more evenly distributed among districts: all 14 districts collected and tested sampled in the week to 7 June.

SIERRA LEONE

Key performance indicators for the EVD response in Sierra Leone are shown in table 7.

A total of 9 confirmed cases were reported from 3 districts (Kambia, Port Loko, and Western Area Urban) in the week to 5 July (table 3, figure 2, figure 3, figure 5, figure 6).

Four chiefdoms—Bramaia, Magbema, Samu and Tonko Limba—in the district of Kambia each reported a single confirmed case of EVD. All 4 cases were either registered contacts of a previous case or have an established epidemiological link to one.

Two cases were reported from two chiefdoms in Port Loko. The first, from Maforki chiefdom, was a registered contact of a previous case. The second, reported from the chiefdom of Masimera, arose from an unknown source of infection and was identified after post-mortem testing of a community death.

Three cases were reported from the Magazine Wharf area of the capital, Freetown. All three cases are registered contacts associated with previous cases in the cluster.

All but one of the 9 cases reported from Sierra Leone in the week to 5 July were either registered contacts of a previous case (5) or have an established epidemiological link to one (3). However, 3 cases were only identified as a result of post-mortem testing of community deaths. On 5 July, a total of 1521 contacts were being monitored in 3 districts: Kambia, Port Loko, and Western Area Urban (table 3).

In the week to 5 July, 98.1% of 480 credible reports of sick people with possible EVD-like symptoms were responded to within 24 hours. More than two-thirds (70%) of reports came from Freetown and the surrounding rural area. In addition, 1710 reports of deaths were received during the same period, 98.3% of which were responded to within 24 hours.
