As African Medics Die of Ebola, Debate Over Drugs

A leading physician in Sierra Leone's fight against Ebola has died from the disease, an official said Wednesday, as it emerged that another top doctor last month was considered for an experimental treatment.

Ultimately, doctors decided against using the drug and he died before he could be airlifted out of the country. There was very little of the experimental treatment available, which has stoked debate about ethics on who should get it even though it hasn't been tested in humans. There is no way of knowing if the drug, known as ZMapp, made a difference in the few people who have gotten the now-exhausted supply of the drug.

Doctors considered giving ZMapp to Sheik Humarr Khan, the chief doctor treating Ebola in Sierra Leone who had come down with the deadly disease, but eventually decided against it "after taking (his) clinical and treatment conditions into account," officials at the World Health Organization said in an email to The Associated Press on Wednesday.

WHO then tried to arrange airlifting Khan out of the country, but "his condition had deteriorated too much to be transported safely." He died July 29.

Highlighting how dangerous the response of health workers is, yet another doctor in Sierra Leone, Modupeh Cole, died on Wednesday, according to Sidie Yayah Tunis, director of communications for the Ministry of Health and Sanitation. Cole, trained in the U.S., was one of the top doctors working in the Ebola isolation ward in Connaught Hospital in Freetown, the capital. He tested positive for the disease last week and was transferred to the eastern district of Kailahun, where Doctors Without Borders is running a treatment center.

Cole's sickness spread fear throughout the hospital where he worked, and staff there went on strike Friday and Saturday after learning that he had tested positive for the deadly disease. They returned to work on Sunday.

Both Cole's and Khan's deaths are a major blow to Sierra Leone's health system, which is struggling to cope with the deadly outbreak.

 For the 12th August 2014, a total of 26 new samples were received by the Kenema Laboratory: Kenema (21), Port Loko (3)and Bo (2).Eleven (11) of these samples are positive from Kenema (9) and Port Loko (2).Four (4) of the remaining samples are negative from Kenema (3) and Port Loko (1) while 11 samples arrive late and are recorded as pending Lab. result.(see table 1)
 The 4 previous Lab. pending results from Kenema(3) and Tonkolili (1) are all negative
 The cumulative number of Laboratory confirmed cases are 717 whiles confirmed death is 264 with a Case Fatality
Rate (CFR) based on confirmed cases of 36.8%.
 Fifty-three (53) patients are currently admitted at the Kenema Isolation Unit with no report received from Kailahun

Epidemiology and surveillance

Between 19 and 20 August 2014, a total of 142 new cases of Ebola virus disease (laboratory-confirmed, probable, and suspect cases) as well as 77 deaths were reported from Guinea, Liberia, Nigeria, and Sierra Leone.

Health sector response

Questions have been received in WHO Headquarters about the original proposed budget for the response and the new draft budget, which is being reviewed by partners. The increase in needed resources is based on improved data and understanding of the situation on the ground in the affected countries. The new estimation of costs is derived using a unit-cost model, built for the most intense transmission areas and reflects the average operational costs based on the current situation in the affected countries. The major assumptions for the cost estimates will be announced towards the end of next week.

WHO continues to receive reports of rumoured or suspected cases from countries around the world and systematic verification of these cases is ongoing. Countries are encouraged to continue engaging in active surveillance and preparedness activities. As of today, no new cases have been confirmed outside of Guinea, Liberia, Nigeria, or Sierra Leone.

WHO does not recommend any travel or trade restrictions be applied except in cases where individuals have been confirmed or are suspected of being infected with EVD or where individuals have had contact with cases of EVD. (Contacts do not include properly-protected health-care workers and laboratory staff.) Main highlights
 For the 25th August 2014, a total of 32 new samples were received and analysed by the CDC KenemaLaboratory:
Kenema (9), Bombali (4), Tonkolili (4), Port Loko (5), Pujehun (2) and Western Area (8). Twenty (20) of these samples
are positive, 7 are negative while 5 suspected cases have been recommended for a second drawbase on their
symptoms onset.(See table 1)
 A total of 4 samples were received and analysed by NICD MLU Lakka, Freetown Laboratory. 1 positive and 3
Negative. Data will be added to the current data.
The cumulative number of Laboratory confirmed cases are 955 whiles confirmed death is 355 with a Case Fatality
Rate (CFR) based on confirmed cases of 37.2%.

 Fifty-eight (58) patients are currently admitted at the Kenema Isolation Unit with no report received from the Kailahun
Lab &Isolation unit. Cumulative discharge is still 244
 DMOs, DSOs & clinicians are reminded to read the comment on the daily Laboratory result for your action
 Data cleaning is on-going, therefore the figures in table 1 will keep on changing as the cleaning continues

Ebola outbreak update: As of today, 27 August 2014, we have a total of 244 patients who have survived Ebola Virus Disease and subsequently discharged. The total number of new confirmed cases for today is 6: Kenema 2 and Western Area Urban 4.

Western Area confirmed figures are cumulative for 25th and 26th August 2014.

The total number of cumulative confirmed deaths is 363 and cumulative number of confirmed cases is 961 with Kailahun 440, Kenema 328; Kono 1; Kambia 1; Bombali 20; Tonkolili 13; Port Loko 57; Pujehun 7; Bo 35; Moyamba 9; Bonthe 1; Western Area Urban 34; Western Area Rural 15. Koinadugu district still remains the only district that has not registered confirmed cases of Ebola in Sierra Leone.

The cumulative number of probable cases is 37 and probable deaths 37 while the total cumulative number of suspected cases is 59 and suspected deaths is 11.

As of 31 August 2014, 3685 (probable, confirmed and suspected) cases and 1841 deaths have been reported in the current outbreak of Ebola virus disease by the Ministries of Health of Guinea, Liberia and Sierra Leone.

In Nigeria, there have been 21 cases and 7 deaths.

In Senegal, one case has been confirmed and there have been no Ebola deaths or further suspected cases.
