Since the beginning of the Pandemic, various clinical manifestations of SARS COv2 (Severe acute respiratory syndrome associated with coronavirus type 2) have been described [1]. Cardiogenic shock is a rare complication of SARS Cov2 infection [2] with two distinct patterns: Myocarditis induced by SARS Cov2 and a novel entity named Multisystemic Inflammatory syndrome (MIS) [3], identified in both Pediatric and Adult populations. MIS is reported to occur later in the course of SARS coV2 infection than myocarditis [[3], [4], [5]]. Clinical descriptions are scarce and based mostly on epidemiological studies [6]. In this retrospective case serie, we report clinical characteristics and outcome of cardiogenic shock related to SARScov2 infection hospitalized in University Hospital of Guadeloupe ICU from February 2020 to March 2022.

Since February 2020, Guadeloupe has been plagued by 4 large epidemic surges of SARS COV2 (due to Wuhan Wild Type, Beta, delta and omicron variants). Of note, only Wuhan and omicron related cardiogenic shock were documented, despite large Beta and delta waves [7], probably due to viral organ tropism differences. These results need to be confirmed on larger studies. Main limit of our study is its retrospective, monocentric design and the small population size. Based on the good prognosis of severe cardiogenic shock related to COVID study and full cardiac recovery in survivors in our study, we recommend a maximal ICU support level for these patients, including mechanical circulatory support.
