Hat-tip Pathfinder. This looks really bad. The translator is a bit rough, but I believe this article says that a baby from Tebessa was brought to the hospital in Annaba, and died of a flu-like illness, and now in the past 48 hours, seven babies have died of this virus, and a couple more seem infected.

My only question here is: Is anyone else (besides the babies) such as HCW or the mothers ill? A contagious deadly virus (like H5N1) would affect everyone, not only the babies. Hospital acquired bacteremia outbreaks can result in lots of dead babies quickly, but those don't typically invovle respiratory or flu-like symptoms like this one does.

The article explicitly (in French) calls this a flu virus. This obviously isn't H1N1, as H1N1 doesn't have this high of a fatality rate. It is not clear if they are calling this flu because of the symptoms, or if someone has tested positive for flu.

This is VERY scary :(

It is possible that some respiratory symptoms could be shown in such an outbreak (the one in Mongolia was due to Klebsiella pneumoniae, which does infect the lungs), and such an outbreak would develop almost simultaneously as this has, but I wonder whether doctors could mistake such an illness for flu. In such a case, the imported baby from Tebessa would be a red herring. OTOH, I doubt if flu or some other contagious virus would only affect the babies. Also, keep in mind the timing - this all happened within two days, according to the article. Puzzling.

H1N1 could be possible, especially if algerian hospitals are limited in their facilities and availability of IV antivirals - this is an area where H1N1 has not hit before, and newborn's could be especially susceptible - especially if there are secondary bacterial infections with some nasty bacterial pathogens in the hospital.

It is in places like Africa that we are more likely to see how severe H1N1 might be in the basence of hi tech interventions - but then again, these hosptials could be better equipped that I give them credit for. anyone have any data on this?

Nasty bacteria in the hospital. That is exactly what would cause a hospital acquired bacteremia outbreak (and you wouldn't even need any H1N1 at all). I posted the links to several such outbreaks around the world in post #3 of this thread. While it is not clear exactly what causes such outbreaks (an FT member in Mexico commented on the Mexico thread that he believes it is due to contamination of IV fluids in the hospital), they do happen worldwide. To me, that might be the better diagnosis here, in which case the description of "contagious flu" would just be wrong. I hope...

If anyone at FT is good at sending articles to ProMED (I am not), this would be a good one to send.

Going through the ministry of health H1N1 updates (which stopped on Jan 7), there have been cases of H1N1 in the district with a fatality in Tebessa on Dec 11.

Oh, that's very different. I didn't expect that. Seven deaths in eight months is a whole different story. Now H1N1 is possible, even likely (although it could just as easily be RSV or another seasonal pathogen).

There's really no reason to think it is all the same pathogen at that point.

This obviously didn't happen in 48 hours. This article now puts it in a 15-day period, but notably includes hundreds of "milder" cases. Could be seasonal flu or H1N1. What is very odd is that the babies are all under 6 months old. I still wonder about a nosocomial bacteremia outbreak (maybe on top of an H1N1 outbreak) that would explain why only the extremely young are dying - H1N1 kills across all ages.

One other possibility worth mentioning, in light of the mention of seasonal virus, also in light of the large number of "milder cases": RSV. There are many threads here on FT (http://www.flutrackers.com/forum/showthread.php?p=342642, for example) indicating that RSV can be much more severe in infants, which would explain the pattern in this outbreak,

infant deaths was A/H5N1, A/H1N1, or some other seasonal influenza
virus. [In fact the term "flu" may have been used loosely and the
illness may not be related to influenza virus at all. - Mod.DK]. The

20100119.0221) stated that, "In North Africa, limited data suggest
that transmission of pandemic influenza virus [A/H1N1] remains active
throughout the region, particularly in Morocco, Algeria, and Egypt."
The number of infant fatalities seems high, but denominator numbers
are not provided. Nor is there any information about contact with
sick or dead poultry that would suggest infection by the more serious
A/H5N1 virus.

the specific virus involved in these cases.

Does anyone know if the infected babies were already in hospital for another reason? Or, did they became infected outside the hospital? I am not sure if I am reading this right. I think there are both infants with prior health problems already admitted and babies that got sick outside the hospital. Anyone else able to figure this out?

I don't think this is spreading within the hospital; otherwise mothers and HCW would be taking ill too (unless this really is something like RSV that would be much milder in adults). I think these babies are being admitted with respiratory illness, despite what the first article seemd to indicate. That said, I think bacterial contamination within the hospital itself might be a contributing factor.

The ProMED post does explain that an "EHS" is apparently a hospital for maternal and infant health, which might explain the age distribution - only infants are treated in such a hospital.

There do seem to be three different time frames for this event: 48 hours, 15 days, eight months. The first one of the three is almost certainly wrong.
