In a statement, Green Technology, Energy and Water Minister Datuk Seri Peter Chin Fah Kui said the Pahang Water Supply Department, which has two water treatment plants downstream of the waterfall, had been taking precautionary measures.

He added the locals should have nothing to worry and that the water supplied was safe for consumption.

In Kuantan, three more villagers who volunteered in the same search and rescue operation were admitted in Jengka Pusat Hospital on Monday after they were suspected to be affected by eptospirosis and melioidosis.

Their condition was reported as stable.

Cleaning Operation At Lubuk Yu Next Thursday


KUANTAN, July 15 (Bernama) -- Next Thursday, an integrated cleaning operation will be carried out at the Lubuk Yu recreational area in Maran to tackle outbreaks of the Melioidiosis and Leptospirosis diseases detected in the area recently.

The operation will involve six government agencies. They are the health, veterinary, forestry, water supply and wildlife departments, and the Maran District Council.

Pahang State Secretary Datuk Muhammad Safian Ismail told a press conference here today, that the cleaning operation involved disposal of wastes, grass-cutting and disinfectant-spraying to get rid of germs.

He said the move was an immediate action taken by the Minor Disaster Committee which was set up after the state executive council meeting Wednesday.

The aim of the committee is to discuss matters and coordinate action pertaining to the diseases.

The committee which Muhammad Safian chairs, also comprises Kuala Sentul assemblyman Datuk Shahaniza Shamsuddin and representatives of several state government agencies.

Muhammad Safian said that following the operation, the recreational area would undergo strict monitoring before it was re-opened to the public.

He said the committee would also monitor water flow in the area, including that at the Ulu Jempol water treatment plant located nearby.

"Currently, water at the Ulu Jempul water treatment plant meets the standards set by the health department, but we would like to advise the public to boil the water before consumption," he added.

So far, 15 people were reported to have contracted the diseases after participating in a search and rescue operation to locate a drowning victim in the area between June 26 and 30.

Six of them died, five have been warded at a hospital while another three were discharged, and one is currently being monitored at home.

Other than the Bernama "boil water" advisory I cannot find any other news entry warning people in the area to boil their water. Given the potential gravity of the situation I am very surprised, especially as this notice was issued 7 hours ago. Perhaps someone else could shed some light on this.

The kind of exposure that results from hours or days of repeatedly diving into the raw, untreated water is much more likely to produce infection than drinking or bathing in the treated water that comes out of the treatment plant.

If the treated water was still as contaminated as the river, the resulting outbreak would be obvious - it would look like plague hit the villages.

Nevertheless, considering the severity of the infections, better safe than sorry.

After cleaning work undertaken, monitoring will be done within a week and if still not effective, a second cleaning will be done before it re-opened to the public, "he said.

Meanwhile, Dr Rosnah said, as yesterday, five are still being treated at the hospital each one in Selayang Hospital, Medical Center Hospital, Kuantan and Maran, while two treated in Hospital Sultan Haji Ahmad Shah, Temerloh.

He said the nine identified were involved in rescue operations during Lubuk Yu could not be detected because no detailed information.

It is possible that these later cases are the longer incubating Meliodosis cases (we really won't be in the clear until July 21), but also possible that these are additional exposures independent from the rescue attempt. Presumably, the additional suspects are NOT among the 110 rescuers.

The 9am operation, led by Pahang Health director Datuk Dr Rosnah Ismail, saw the team divided into four groups to carry out a number of jobs, including disinfection, garbage-collection, washing the restrooms and also collecting the soil, and water samples.

As a precaution, the members from the district office, state Health, Forestry, Veterinary, Wildlife and National Parks, and Waterworks department, underwent a health screening before the operation.


They also wore a one-piece overall protective gear known as "Personal Protective Equipment", which included gloves, rubber boots, safety goggles and face mask.


After a short briefing, the groups were driven to the waterfall in four 4WD vehicles, accompanied by an excavator and two tankers from the water works department.

Lubuk Yu, one of the more popular waterfalls in Pahang, became the talk of the country following a string of mysterious deaths that hit the search party for a 28-year-old drowning victim, Saiful Azhar Azizam, at the site on June 26.


Six men, including Saiful Azhar's father, Azizam Abdullah, 58, along with Timothy Hoo Seng Heng, 29, Lim Sin Lai, 55, Mohd Shah, 60, Zakaria Ayub, 52, and Zainal Abidin Ismail, 56, died within a week.

More than 83 people, comprising rescuers and villagers, were involved in the search operation from June 26-30.

Health Minister Datuk Seri Liow Tiong Lai later said the victims had contracted the deadly melioidosis and leptospirosis bacteria from contaminated water and soil.


Yesterday's clean-up covered a 500-metre radius from the waterfall site. The operation was divided into two zones -- Zone A and Zone B.

Rosnah said the group were driven up to Zone A, which was about 100 metres from the waterfall, before they changed into their protective gear.

The clean-up site was identified as Zone B, and the area was strictly for those wearing protective gear.

The garbage collected from the waterfall site was put into garbage bags before it was buried into a hole.

Rosnah also said those who had completed the clean-up task were told to shower at special shower compartments set up by the water works department at Zone A.

Personnel from the Health Department were the first to make their way into the site to spray disinfectant before another team moved in to collect all the garbage.

This was followed by the group which was sent to wash the restrooms and tidy the stalls.

Finally, the disinfectants were sprayed again.

Besides the six government agencies, Rosnah said personnel from the Pahang Mineral and Geoscience Department were also present at the site to gather samples from the soil and water.

She said if there were no new cases reported by July 28, the State Natural Disaster Relief Committee, headed by State secretary Datuk Muhammad Safian Ismail, would declare the outbreak at Lubuk Yu over.

"So far, all those hospitalised and put under observation were cases recorded between June 26 and June 30. Since the clean-up has been completed, we will now monitor the waterfall site and discuss with the experts the outcome of the water and soil tests before deciding to reopen the waterfall," she said.

She said those who joined the clean-up operation yesterday, including four members of the local media, would be placed under observation for a month.

Rosnah said 20 people, including the victims, were believed to have contracted melioidosis and leptospirosis at the Lubuk Yu recreational area near here.

She said six people were still warded at the hospitals in Jengka, Selayang and Temerloh, while the others were allowed to go home.

KUANTAN, July 23 (Bernama) -- The Pahang Health Department has confirmed that 10 blood samples taken from 19 people involved in the search and rescue operation to locate a drowned victim in Lubuk Yu in Maran last month were infected with melioidiosis and leptospirosis.

Pahang Local Government, Environment and Health Committee chairman Datuk Hoh Khai Mun, who received the blood test results yesterday, said six of the blood samples were infected with melioidiosis and four others with both diseases.

He said the blood samples taken from nine other rescuers, however, were not infected.

"Six people are still warded, comprising three at the Sultan Haji Ahmad Shah Hospital in Temerloh, two at Jengka Hospital and one at Selayang Hospital in Selangor," he told reporters after launching the state-level "No Plastic Bag" campaign on Friday.

Hoh said 148 peole were believed to have contracted the diseases compared with 83 previously.

"However, these people only showed symptoms of fever, coughing and dizziness," he said.


On July 10, the Lubuk Yu recreational area was closed to the public after six people died while 83 others contracted leptospirosis after participating in the operation to locate a drowned man in the area between June 26 and 30.

Yesterday, 52 officers and personnel from various government agencies began an integrated cleaning-up operation in the area.

10 Blood Samples From Lubuk Yu Infected By Melioidosis, Leptospirosis

KUANTAN, July 23 (Bernama) -- The Pahang Health Department has confirmed that 10 blood samples taken from 19 people involved in the search and rescue operation to locate a drowned victim in Lubuk Yu in Maran last month were infected with melioidiosis and leptospirosis.

Pahang Local Government, Environment and Health Committee chairman Datuk Hoh Khai Mun, who received the blood test results yesterday, said six of the blood samples were infected with melioidiosis and four others with both diseases.

He said the blood samples taken from nine other rescuers, however, were not infected.

"Six people are still warded, comprising three at the Sultan Haji Ahmad Shah Hospital in Temerloh, two at Jengka Hospital and one at Selayang Hospital in Selangor," he told reporters after launching the state-level "No Plastic Bag" campaign on Friday.

Hoh said 148 peole were believed to have contracted the diseases compared with 83 previously.

"However, these people only showed symptoms of fever, coughing and dizziness," he said.


On July 10, the Lubuk Yu recreational area was closed to the public after six people died while 83 others contracted leptospirosis after participating in the operation to locate a drowned man in the area between June 26 and 30.

Yesterday, 52 officers and personnel from various government agencies began an integrated cleaning-up operation in the area.

Leptospirosis has a quite low fatality rate. It is conceivable that most if not all of the 83-148 people could have contracted that illness. But my guess is that only the 19-20 severe cases contracted the meliodosis.

And the negative results could easily be due to antibiotic treatment.

Lubuk Yu rescuer seeks help of bomoh

MARAN, MALAYSIA - The health authorities may have confirmed that 19 people involved in the rescue operation in Lubuk Yu here last month were infected with meliodosis and leptospirosis, but this has not stopped some in the search party from seeking treatment from traditional healers.

Shahrin Shukor, 31, came down with fever on July 10 after diving into the water to search for the victim. He was subsequently warded at the Sultan Haji Ahmad Shah Hospital in Temerloh from June [July] 12 to 14.

The odd-job worker from Bandar Jengka, after finding out that six members of the search party had died, sought alternative treatment as soon as he was discharged from hospital.


"I believe in both modern and traditional treatment, so I sought the help of a bomoh who said that the site was keras (haunted) and my sickness was due to evil spirits.

"He performed some rituals and I later had a special cleansing bath. I was still feeling weak at the time but now I feel much better.

"I used to visit the waterfall and bathe there but this is the first time I got sick. I don't plan on going there again."

Shahrin claimed that the supernatural beings, who safeguarded the waterfall, were furious with picnickers, especially youngsters who arrived in big groups during the weekends and also at night.

"Besides the heaps of rubbish, some of the youths consume alcohol and indulge in immoral activities. Visitors can find empty alcohol bottles and packets of used condoms in the vicinity of the waterfall."

Felda settler, Mohd Asim Manan, 58, paid a heavy price for his curiosity when he had to be warded at the Jengka 3 Hospital on June [July] 11 after showing flu-like symptoms and severe headaches.

Asim, who visited the waterfall with three other friends during the second day of the search and rescue operation, said he was not part of the operation and was just visiting.

"Since the weather was not promising, we decided to return home after spending some two hours at the waterfall. It was crowded with people at that time.

"Upon returning home, I began to feel unwell and my condition worsened on June 11 forcing my family members to rush me to the hospital's emergency ward."

Asim said he felt very weak and had no appetite to eat. He took some herbal medicine but it did not help.

"My family members were worried especially when I could not sleep at night. Now I feel much better. Perhaps there is a mystery surrounding the waterfall."

Tan Eng Seng was in the hospital for 10 days but this has not dampened the spirit of the volunteer fireman.

The 46-year-old vegetable farmer from Kampung Sungai Jerik in Maran, Pahang, was among the 20 people who came down with the severe fever after taking part in the five-day search. He developed chills and severe body aches a day after the end of the operation.

Tan sought treatment at Bandar Jengka Hospital but when his condition worsened, his family decided to send him to a private hospital in Kuantan where he is still being warded.

"This is the most challenging experience I've encountered since I registered to join as a volunteer fireman 20 years ago.

"I knew some of the victims who died and when I heard about their deaths, I actually thought I was going to suffer the same fate as our symptoms were similar."

He said the fire and rescue volunteer team was called in on July [June] 27 and was given fibreglass boats to search for the victim.

"During the five-day search operation, we would arrive early in the morning and only leave the site near midnight.

"The Fire and Rescue Department had arranged for a caterer to deliver meals to the site and usually after a tiring day at work, we would just eat whatever was available there and return home to rest."

Share | KUANTAN: One of the six patients treated at the Bandar Jengka hospital in Maran for suspected melioidiosis and leptospirosis bacteria infection was allowed home on Saturday.
Pahang Local Government, Environment and Health Committee chairman Datuk Hoh Khai Mun said the patient, who was warded at the hospital since early this month, was discharged after his blood sample confirmed that he was not infected by the deadly bacteria.

Hoh declined to provide further details on the patient.


He said the others were stable and were in the midst of recovering.


One of the patients from the Sultan Haji Ahmad Shah Hospital in Temerloh was moved to the Bandar Jengka hospital on Saturday for antibiotics drips and would be allowed to return home soon.

The six patients were hospitalised after they were involved in a search and rescue operation at the Lubuk Yu waterfall in Maran last month.

KUALA LUMPUR - Forest Recreational Lubuk Yu in Maran, Pahang which closed after the location and spread of bacteria Meliodisis Leptospirosis on July 10 and will reopen in a week or two weeks.

And finally a ProMED post. I'm a little surprised we didn't get one earlier. In response to the moderator's comments, the presentation of multi-organ failure and sepiticemia within days indicated in the first few posts of this thread indicates that acute melioidosis is likely present here, not just previous exposures.

[Both melioidosis and leptospirosis can be acquired from contact with
water. Melioidosis is caused by _Burkholderia pseudomallei_ and is
not a zoonosis, since transmission of the infection is basically
related to water/soil exposure in the endemic areas (primarily
southeast Asia and northern Australia during wet seasons). The
infection can be acquired, however, by humans and animals.

_Leptospira_ species infection, however, is a world-wide zoonosis
acquired directly or indirectly from exposure with infected animal
urine, usually either rodents or livestock.

It is not stated whether the test for melioidosis was culture or
serology. The latter is not useful in endemic areas. As a reminder,
melioidosis is a category B bioterrorism disease
