Measles is gaining ground and is causing concern for public health

Measles is on the rise, and not just in the United States, where a child has died from the infection. Public Health Canada is also concerned about the "worrying trend" in the spread of the highly contagious virus.

What's happening with measles in Canada?

With more cases reported in the first two months of 2025 than in all of 2024, Canada is on a “concerning trend” for measles, Dr. Theresa Tam, Canada’s chief public health officer, said in a statement Thursday. In total, Canada has recorded 227 cases of measles this year, many requiring hospitalization. The majority of cases are children who had not received their two doses of vaccine and acquired the infection at social events, daycares, schools or health care settings. The notable increase is due to outbreaks in Quebec, New Brunswick, Ontario and Manitoba.

What about the United States?

So far this year, there have been nearly 150 cases of measles in Texas. Among them, an unvaccinated child died from the infection. It was the first death from the virus in the United States in nearly a decade. On Thursday, an unvaccinated adult who contracted measles died in New Mexico, but the exact cause of death has not yet been confirmed by health officials.

"Measles travels and crosses borders. We can therefore expect to have other cases," says Dr. Jasmin Villeneuve, medical advisor at the Institut national de santé publique du Québec (INSPQ).

Is Quebec spared?

No. A measles outbreak broke out in early December in the Laurentides region after an infected person from outside Quebec infected attendees at a conference. To date, 30 cases have been confirmed in Quebec, the majority in the Laurentides region. A few cases have been confirmed in the Montreal, Laval and Montérégie regions. “This is not a trivial disease. Infected people can be very sick,” warns Dr. Villeneuve .

Why is this a concern?

Measles is a highly contagious virus. “It is considered one of the most contagious diseases. In a population where no one is protected against measles, one case could generate 15 to 17 secondary cases. The flu generally causes three to five,” explains Dr. Villeneuve . It is not necessary to have direct contact with an infected person to contract the virus, since it can remain suspended in the air for several minutes or up to two hours. Symptoms of measles can appear one to two weeks after exposure. This is why it is essential that people in contact with a confirmed case isolate themselves to wait to see if symptoms appear.

How to protect yourself?

Vaccination is the most effective way to protect yourself. Two doses of vaccine provide 95% protection against the disease, and this for life. Quebecers can get the vaccine for free by making an appointment on the Clic Santé website or by calling 1-877-644-4545. Adults born before 1970 do not need to be vaccinated, since they are considered immune due to the widespread circulation of the virus when they were younger. Since the outbreaks this year are due to travellers having been exposed to measles in another country, Dr. Tam encourages Canadians to get vaccinated against measles and to check that their vaccinations are up to date before travelling. “The vaccine should be administered at least two weeks before departure, but even last-minute vaccinations offer protection,” she explains.

How do you know if you are infected?

The main symptoms of measles are usually a high fever, cough, runny nose, red eyes, general malaise, and redness—first on the face and then on the body. However, there is a risk of serious complications. These complications include blindness, deafness, and permanent neurological damage. People at greatest risk of complications from the disease include babies under 1 year old, people with weakened immune systems, and pregnant women who are not adequately vaccinated against measles.

Vermont Department of Health Confirms Case of Measles
For Immediate Release: March 11, 2025

A school-aged child contracted the disease while traveling internationally

WATERBURY, VT — The Vermont Department of Health has confirmed a case of measles in a school-aged child in Lamoille County. The child became sick after returning with their family from traveling internationally in recent days. The risk to the public is believed to be low, as the child has been isolated from most community settings while they have been contagious. Investigation is ongoing.

This is the first case of measles in Vermont in 2025, following two cases in 2024 and two cases in the decade prior, one in 2011 and one in 2018. This case is not related to the three ongoing domestic outbreaks of measles in the United States or to the current measles outbreak in Québec.

The positive test result for measles was confirmed the evening of Monday, March 10. One known site of exposure is at Copley Hospital, where the child was evaluated on Sunday, March 9. Copley Hospital has reached out to known visitors to the Emergency Department during that time. The Health Department is asking anyone who was inside the Emergency Department at Copley Hospital in Morrisville, Vermont between 3:15 p.m. and 6 p.m. on Sunday, March 9 to take the following actions:

Three questions about the measles outbreak

Professor Eve Dubé analyzes the causes of the current measles outbreak and the solutions that should be put forward to curb it.

Typically, Quebec records between 0 and 4 cases of measles per year. Between February and June 2024, 51 cases were reported. Since December 2024, a new outbreak has been underway, and as of March 26, 40 cases had been reported. Eve Dubé , a professor in the Department of Anthropology at Laval University and a specialist in health anthropology, analyzes the phenomenon.

Are recent measles outbreaks due to decreased vaccine effectiveness or under-vaccination against the disease?

The increase in cases is not linked to a decrease in vaccine effectiveness. It is estimated that one dose protects between 85% and 95% of children, and two doses, 100%. The measles vaccine is therefore very effective, and the increase in cases is linked to a decrease in vaccination rates. Measles is an extremely contagious disease. At least 95% of the population must be vaccinated to develop herd immunity. In Quebec, in 2021, it was estimated that 92% of one-year-olds and 89% of two-year-olds were vaccinated against measles. Local variations in vaccination rates can lead to outbreaks, for example, if several unvaccinated people live in the same community. Finally, elsewhere in the world, measles vaccination rates can be much lower, and viruses can also travel!

What factors are responsible for the decline in measles vaccination coverage?

There are generally two main causes: problems accessing vaccination services and acceptability issues. When it comes to access, we think of factors such as difficulty accessing health care. This is the case, for example, for people without a family doctor or those who do not have regular health checkups. Not knowing that a vaccine is necessary can also be a barrier. In Quebec, the first dose of the measles vaccine is given when the child is 12 months old. Many parents have then returned to work and may forget the appointment or not know it is necessary. Finally, lockdowns during the pandemic may have had a negative impact on vaccination. Despite catch-up efforts, it is possible that some people could not be reached. When it comes to acceptability, we are talking about fears about the safety or effectiveness of the vaccine, negative attitudes about vaccination in general, and the negative influence of misinformation. The measles vaccine has been associated with a persistent myth since the late 1990s, when a fraudulent study alleged it could cause autism. Although this study has been debunked, fears persist among some parents.

How can we encourage measles vaccination?

It is essential to provide simple and easy access to vaccination, including walk-in vaccination clinics, evening or weekend clinics, or mass clinics. Parents must also be reminded of the importance of measles vaccination by informing them through schools, pharmacies, etc. For parents who may have more fears, a proven strategy is to be able to speak with a vaccination counselor or a health professional trained in motivational interviewing. There is no one-size-fits-all solution. Interventions that aim to inform people, easily offer them the vaccine, and address their fears are useful. With the number of measles cases increasing worldwide, it is even more essential to ensure that all measures are taken to increase vaccination rates.

Measles is a highly contagious viral infection that is transmitted through the air. It can be prevented by receiving two doses of the measles vaccine.
Possible symptoms include fever, cough, runny nose, conjunctivitis (red eyes), and a widespread rash.

If you are considered immune to measles, no action is required. You are considered immune if:

  • You have received two doses of the measles vaccine (if you were born in 1970 or later)
OR
  • You have not received any doses of the vaccine, but:
    • You were born before 1970
    • A serological test demonstrates the presence of antibodies against measles.
    • A medical certificate confirms the acquisition of measles before January 1, 1996.
    • Diagnostic evidence confirms a previous infection (e.g., serology).
If you are not immune:
  • Please contact your healthcare provider or call Info-Santé at 811.
  • You could develop measles during the incubation period, which is between January 6 and January 22.
