28 confirmed measles cases across Quebec: health ministry

The number of confirmed measles cases in Quebec was up to 28 Tuesday afternoon, according to the latest data from the provincial government.
It represented an increase of seven cases from Friday.
The new cases are believed to be in the Mauricie et Centre-du-Québec, Laurentides, Montérégie, or Lanaudière regions. There are also now five confirmed measles cases in Laval.
The number of cases in Montreal (15) remained unchanged.
Several of the recent locations of exposure include hospitals, clinics, pharmacies and grocery stores.

No cases of measles were reported in Montérégie at the beginning of March, but in April there were now nine cases of this very contagious disease in the region.

As of April 9, 43 confirmed cases of measles have been reported since the start of the year across the province. The Montérégie Public Health Department (DSPM) recalled that this disease is very contagious. A person can catch it if they have been in the same room as another person who has had measles.

The DSPM clarified that measles has already been widespread in North America. It has become rarer because of the effectiveness of vaccination campaigns. Its first symptoms are fever; nasal congestion with runny nose; cough; conjunctivitis (red, runny eyes); eyes sensitive to light; generalized discomfort. Subsequently, redness appears on the face, then on the body.

We add that it can take 10 to 14 days (exceptionally up to 21 days) between exposure to the virus and the start of symptoms, i.e. the incubation period. A person begins to be contagious 4 days before the rash appears and can remain contagious for up to 4 days after their appearance. Measles lasts 1 to 2 weeks.

Possible complications of measles are: otitis (5 to 9% of cases); persistent diarrhea, especially in infants; pneumonia (1 to 5% of cases); convulsions (the body stiffens and the muscles contract jerkily and involuntarily); loss of vision or hearing; permanent brain damage (1 in 1,000 cases); death (1 in 3,000 cases).

Case of measles confirmed on a plane that landed in Montreal

A person with measles was on board a flight from Turkey that landed in Montreal on April 3, the Quebec Health Ministry and public health authorities have confirmed.

The person was on board Turkish Airlines flight TK35, that landed at 6 p.m. at Pierre-Elliot-Trudeau Airport.

Authorities have indicated that people on this flight may have been exposed to measles. Travellers or staff who were at the airport in the arrivals and baggage area on April 3, between 6 p.m. and 12.30 a.m., may also have been exposed.

They are considered measles contacts cases by Quebec public health.​..

​Quebec appears to be winning its battle against the rising tide of measles after 45 cases were confirmed province-wide this year.

"We've had no locally transmitted measles cases since March 25, so that's good news," said Dr. Paul Le Guerrier, responsible for immunization for Montreal Public Health.

There are 17 patients with measles in Quebec currently, and the most recent case is somebody who was infected while abroad, he said.

But it was no small task to get to this point.

Le Guerrier said once local transmission was detected, news was spread fast among health centres to ensure proper protocols were followed — such as not letting potentially infected people sit in waiting rooms for hours on end.

Then about 90 staffers were put to work, tracking down those who were in contact with positive cases and are not properly vaccinated. They were given post-exposure prophylaxis, which prevents disease, said Le Guerrier.​..

Montreal Public Health is warning of a measles case which has exposed travellers at the Pierre-Eliott-Trudeau International Airport.

The infected person was on board an Air Transat flight, TS403, from Rome that landed in Montreal on July 8 at 3:30 p.m.

This is the second measles case caught abroad that Montreal public health authorities confirmed since declaring the end of an outbreak in the city last month.

Last Friday, officials said a traveller who caught the measles during a trip to Africa was on board the Delta Airlines flight DL9288 from Amsterdam that landed in Montreal on July 3.

Measles is a highly contagious airborne disease, and its spread has become rarer because of immunization campaigns. The best way to protect yourself is to get vaccinated against it.

Authorities say travellers and staff who were at the arrivals and baggage section of the Trudeau airport between 3:30 p.m. and 6:30 p.m. on July 8 may have come in contact with the infected person from the Air Transat flight.

The next day, the person carrying measles reportedly visited two stores in the Montreal area.

Public health officials say you have been exposed to measles if you were at the Prima Goccia store in Montréal-Nord between 10:25 a.m. and 1 p.m. and the Nour FX currency exchange office in Saint-Michel between 11 a.m. and 1:30 p.m.

Two flights from Europe

The people with measles were on board two separate planes that landed at Pierre-Elliot-Trudeau Airport, one from Amsterdam and the other from Rome .

Amsterdam

The first, Delta Airlines flight DL9288/KL67, arrived from Amsterdam on July 3 at 5:08 p.m. People present at the airport from 5:15 p.m. to 8:00 p.m. that day may have been exposed to measles.

Rome

The second, Air Transat flight TS403, arrived from Rome on July 8 at 3:30 p.m. People present at the airport from 3:30 p.m. to 6:30 p.m. on that date may have been exposed to the virus.

Epidemiology

Quebec is facing its 2nd [SUP]measles[/SUP] outbreak in 2024. This outbreak began in December 2024 and is still ongoing. Fewer than 5 cases have been reported as of December 30, 2024. The first outbreak of the year occurred in early 2024, included 51 cases and ended on June 5, 2024. A few sporadic cases were reported between the two outbreaks.

Measles is present worldwide, particularly in Europe, Asia, the Middle East and Africa. Large outbreaks have occurred in several European countries and in some states in the United States in recent years. In 2023 and 2024, cases of measles were reported in several Canadian provinces, including Quebec.

People at risk

Some people are at greater risk of complications if they contract measles. If exposed to the virus, these people should contact Info-Santé 811 quickly to be evaluated and receive preventive treatment if necessary. The latter consists of an injection of antibodies that must be administered within seven days of the date of exposure.
People at risk of complications are:

  • babies under one year old;
  • people with weakened immune systems;

The Quebec Ministry of Health has also shared a list of places in Montreal, Laval, Montérégie and the Laurentians where people may have been exposed to this disease.

According to retired virologist Jacques Lapierre, although the vaccination rate is very high and the vaccines are very effective, this highly contagious disease must be taken seriously.

"It's a fairly serious disease," he said in an interview on the show Le Québec matin . "It's dangerous. There are side effects to measles that can be quite serious, such as encephalitis. People can go blind. So, it has to be taken seriously."

“Measles had almost disappeared in Quebec,” he adds. “We hardly saw it anymore because vaccination controls this virus very well. People who are vaccinated, normally, will not catch measles. And vaccination [for this virus] is for life.”

There is no reason to worry at the moment, he said, but we must remain vigilant.

"It takes a few cases among unvaccinated people to trigger a small epidemic," he says. "There is not a huge number of cases at the moment, it is still very reasonable, but it raises a surveillance bell. We absolutely have to monitor so that it does not spread in a population."

This disease has been more prevalent in the province for two years.

"What seems to be the cause is that during COVID, the vaccination rate has decreased, so there are a lot fewer people getting vaccinated for measles," says the retired virologist.

“Obviously, COVID required 4 or 5 vaccinations, so there was also a kind of vaccine fatigue. People said they were tired of getting vaccinated,” he continues. “It has to be done again. We have to re-explain why it is important to get vaccinated.”

All it takes is one unvaccinated person contracting the disease while traveling and returning home for an outbreak to occur.

"Currently, in some countries, there is still measles, so when people travel, they risk bringing this virus back with them," says Mr. Lapierre. "That's why when there is an outbreak, we make sure to identify these people fairly quickly because it is extremely contagious."

"Unfortunately, among them, there is a person who went to Carrefour Laval and stayed there for several hours," said Quebec's national director of public health, Dr. Luc Boileau, in an interview with La Presse . People who went to the establishment on January 7 from 10:30 a.m. to 2:45 p.m., particularly to the Renaud-Bray store, could have been infected.

Dr. Boileau would not be surprised if people who frequented the mall are now infected. This is not the first time that such a situation has occurred. About six years ago, a person infected with measles who shopped at Carrefour Laval contaminated other people, the doctor recalls.

So far, 11 cases have been reported in Quebec in recent weeks. "We anticipate that there will be other cases," says Dr. Boileau .

Why is this worrying?

Measles is a highly contagious virus. It is not necessary to have direct contact with an infected person to contract it, since the virus can remain suspended in the air for several minutes to up to two hours.

"So every case of measles is a cause for concern, since this virus is transmitted very easily," says Dr. Boileau .

Studies have shown that a person infected with COVID-19 transmits the virus to about three to five unvaccinated people. By comparison, a person with measles can infect between 16 and 18, says Dr. Boileau .

When exposed, it takes time for the virus to develop and cause illness. Symptoms of measles can appear one to two weeks after exposure.

This is why it is essential that people in contact with a confirmed case isolate themselves in order to check if symptoms appear. "It is very important for these people to follow the instructions that public health gives them," says Dr. Boileau .

How to protect yourself?

Vaccination is the most effective way to protect yourself. Two doses of vaccine provide 95% protection against the disease, and this for life. “The vast majority of people who are protected will not get measles if they are exposed,” says Dr. Boileau .

Adults born before 1970 do not need to be vaccinated. They are considered immune because the virus was widely circulating when they were younger.

The measles vaccine is well known in Quebec, and in many regions, more than 90% of children have received their doses. “But sometimes, people who come from outside have not had the privilege of being properly vaccinated. They need to know that it is available [for free] and encourage them to do so,” says Dr. Boileau .

How do you know if you are infected?

The main symptoms of measles are usually a high fever, cough, runny nose, red eyes, general malaise and redness first on the face and then on the body.

"The famous measles patches do not appear on the first day. They often appear four or five days after the onset of symptoms," says Dr. Boileau .

There is, however, a risk of serious complications. These complications include blindness, deafness and permanent neurological damage. About 1 in 2,000 people will end up with neurological problems for the rest of their lives as a result of the infection.

Those most at risk of complications from the disease include babies under 1 year old, people with weakened immune systems, and pregnant women who are not adequately vaccinated against measles.
