PUNE: A seven-year-old boy and 45-year-old man succumbed to the H1N1 influenza on Friday, taking the death toll in the city to 121.

"The victims' throat swab reports came on Saturday, confirming them positive for the infection," said Pradip Awate, medical officer of the state health services.

Second wave? Toll reaches 121, show-cause to 8 docs

Even as the swine flu toll in the district rose to 121 last week, the district health officials have served show-cause notices to eight doctors for failing to administer tamiflu tablets at an early stage of the disease.

The doctors have been asked to give an explanation for ignoring the directives.
The health department had instructed all government and private doctors to give tamiflu tablets to patients once they are suspected to have contracted the virus as a precautionary measure.

According to a senior health officer, many private practitioners were found to have ignored the instruction, leading to a deterioration of the patients? health and even deaths. ?We need to be strict about this and ensure that private practitioners heed to the guidelines in letter and spirit.?

The Health Secretary had also issued guidelines that those not adhering to the instruction should be served notices as they are ignoring the government health advisory.

H1N1 action plan depends heavily on pvt doctors

PUNE: The district administration's action plan to tackle the anticipated second wave of H1N1 infection seems to be depend more on private medical practitioners than government doctors. The action plan has estimated availability of 411 doctors from the district hospital and Pune Municipal Corporation (PMC) hospitals, while 3,748 doctors would be available from the private health sector.

The action plan, prepared by district collector Chandrakant Dalvi with the assistance of the health department and experts from BJ Medical College, has anticipated that around 20 per cent population of the district would be infected with H1N1. Out of this, around five per cent (16,000 people) may be required to be admitted to intensive care units (ICUs) and 64,000 patients may have to be quarantined. Assuming that a patient infected with H1N1 virus takes 8 to 10 days to recover, the provision of beds has been made in the action plan.

A total of 411 doctors from 173 hospitals including health department's Aundh Hospital, Sassoon Hospital and Pune and Pimpri-Chinchwad municipal corporations' hospitals would be on duty to treat swine flu patients. Moreover, assistance from 3,748 doctors from 1,010 private hospitals may be needed.

The entire action plan depends on assistance of private health providers. In all, 295 ventilators at private hospitals and 22 ventilators at government-run hospitals have been arranged. From the doctors' safety point of view, 400 N95 masks and 600 gloves would be made available as per the action plan.

Three swine flu deaths in a day

Posted: Tuesday , Dec 15, 2009 at 0339 hrs

Three more people died of H1N1 in the city on Monday taking the toll to 131 in Pune district. The fresh spurt has led to concern among officials at the district swine flu coordination committee, which is trying to figure out whether the second wave of the flu has surfaced. They have not been able to come to a conclusion.

Health officials said that 48-year-old Anil Agarwal from Mangalwar Peth, 45-year-old Sulabai Lohakare from Ambegaon taluka and 50-year-old Razia Khan from Gandhinagar slum in Bopodi died of swine flu on Monday.
Agarwal had showed the symptoms on December 7 and was admitted to Ruby Hall Clinic on December 10. He had an underlying illness, myelodysplasia syndrome, and was on steroids. Lohakare, who died at YCM Hospital, Pimpri, had the symptoms since November 29 while Khan started showing them on December 11.

It is being widely felt that with winter setting in, there seems to be a revival of H1N1. Dr Arun Jamkar, Dean, B J Medical College and Sassoon Hospital, said a month ago there were no swine flu patients on ventilator. Now, over 10 patients are in ICUs of various hospitals.

Pune The Sassoon General Hospital authorities have urged the government to import drug Peramivir, recommended by the American Food and Drug Administration (FDA) for emergency use in combating swine flu.
Dr Arun Jamkar, dean, Sassoon General Hospital and B J Medical College who recently participated in a meeting on swine flu organised by the Johns Hopkins University, USA, said that Peramivir was used as an ‘emergency drug’ for patients who were sick and not responding to treatment. “We have written to the government to allow the import of such a drug that would help towards the better management of the patients in the Intensive Care units,” said Jamkar.

While Jamkar also called for creating awareness on the use of Tamiflu within 36 hours of swine flu-like symptoms, according to the Chemists Association of Pune there has been a very poor response to the sale of the drug at drug stores. Two-fold rise in throat swabs reaching NIV
Umesh Isalkar, TNN, 20 January 2010, 07:09am ISTText Size:|Topics:H1N1
NIV

PUNE: The number of throat swabs of critical patients being sent to the National Institute of Virology (NIV) for testing H1N1 infection has witnessed a two-fold increase in the last one month (from December).
Similarly, the infection rate of H1N1 influenza has also rose to 25 per cent during the same period, said Mandeep Chadha, deputy director of NIV, on Tuesday. ?We now receive 85 to 100 throat swabs every day. They are mainly from Pune and rest of Maharashtra. Mumbai has stopped sending us the swabs after testing started at the Haffkine Institute,? said Chadha.

?Before winter, the number of swabs reaching us was in the range of 40 to 45 per day. Now, there is marked increase. Besides winter, which provides conducive conditions for its spread, the virus?s stride in virgin interiors of the state is also making the matter worse,? said Chadha.

The infection rate of the swine flu virus has also increased. ?Earlier, the infection rate was between 15 to 20 per cent, whch has now touched the 25 per cent mark,? said Chadha.

Commenting on the point genetic mutation seen in throat swabs of three patients who succumbed to the virus, Chadha said, ?We didn?t see any change in the genetic make-up of the virus after that.? Of the three patients, two were from Pune and one from Nashik, said Chadha. The mutation was detected in the haemagglutinin (HA) region of the virus? gene, she added.

Elaborating it further, state epidemiologist Pradip Awate said, ?The weekly infection cases of H1N1 virus in Maharashtra has increased from 100 to over 135 from December onwards. Winter provides ideal setting for increased activity of any influenza virus. The same principal applies to the swine flu virus as well.?

The state, so far, has registered 5,604 positive cases of the H1N1 influenza, of which more than 2,000 cases are from Pune alone, added Awate. The district also tops in death chart with 166 casualties of the 300 registered in the state.
