I think that you raised an excellent point that I haven't seen extensively discussed in this forum (although I am a newcomer).

The use of serum containing antibodies from recovered patients has been used in the past as a rescue treatment for others newly infected. I have seen it used primarily among those infected with viral illness when there is no other effective treatment.

As I recall, it was used in Kikwit, Zaire during the 1995 Ebola outbreak, and a read a recent report that it was used on the latest case in China (the father of the latest fatality) who is now recovering.

Folks, given the limitations on vaccine development and the availability and effectiveness of antivirals during a pandemic, antibody treatment may be our best hope for saving the lives of those already infected.

Sources told The Post on Mon [12 Dec 2007] that 3 suspected cases of
human-to-human transmission [but see JW comment below] of bird flu
virus have been detected in North West Frontier Province (NWFP). Of
these 3 cases, 2 people died and the Ministry of Health has collected
samples to determine the
cause of their deaths.

Sources said about 3 weeks ago the Ministry of Health obtained blood
samples of people suspected of being infected with bird flu virus,
but their results have yet to be announced. Ministry officials said
on 21 Oct [2007], bird flu virus H5N1 was confirmed at Ijaz Shah
Poultry Farm in Abbotabad and a veterinarian, Dr Ishtiaq Durrani,
from the NWFP Livestock Department was tasked with culling as many as
3000 hens in Abbotabad from 21 to 23 Oct 2007.

As many as 200 [208 as of 13 Dec 2007 according to WHO - Mod.CP]
people have died of bird flu virus all over the world but no case of
human-to-human transmission of bird flu virus has been confirmed so
far. Pakistan Veterinary Medical Council secretary and registrar Dr
Alamdar Hussain Malik said the Ministry of Health and the World
Health Organization (WHO) were the final authority to confirm the
presence of bird flu virus in human beings. The final report about
the presence of bird flu virus in suspected cases detected in NWFP
would be released by the ministry, which has obtained their samples,
he added. He said [there are] about 25 000 poultry farms in the
country, out of which 90 percent were working without boundary walls
and precautionary measures. He said the government should register
all these poultry farms and start an awareness campaign about
bio-safety.

[This report provides the additional information that as well as the 2
people who have died, a 3rd person has been suspected to be an H5N1
case and remains alive. An unstated number of other people are
suspected cases and are under surveillance awaiting the outcome of
diagnostic tests. Confirmation of these H5N1 diagnoses from an
independent laboratory is awaited. - Mod.CP].

[An additional detail is that onset was "about 3 weeks ago" (end of
November 2007). Just because 2 brothers were involved does not imply
human-to-human transmission; both were in contact with the same flock

the authors of the study used blood samples from Vietnamese patients that had recovered from H5N1 infection, to produce specific monoclonal antibodies (mAbs) against H5N1 influenza virus.

These monoclonal antibodies were then tested for their ability to neutralize H5N1 and other flu viruses in animal experiments in the laboratory.

Four of the selected antibodies neutralized homologous H5N1 viruses in mice, although with different efficacies.

The antibodies provided protection from infection with the original virus when given a day before or one to three days after infection.

Three antibodies also partly protected the mice against heterologous H5N1 (clade 2).

Finally, the researchers showed that the antibodies protected mice by limiting viral replication, especially in the lungs, and by stopping viral spread out of the lungs.

According to author’s conclusions the strengths of their approach for the development of human mAbs are: (1) it uses the human immune response rather than that of animal surrogates—the antibodies selected will be those that have been generated in response to the natural infectious pathogen and have protected the individual, (2) it is fast, (3) screening can be performed using functional assays, i.e., neutralization, (4) it allows screening of a large repertoire of antibodies to select those with the most favorable profile (potency and breadth of reactivity), and (5) since the antibodies are of human origin the risks of self-reactivity against self-antigens is minimized when compared with antibodies generated in mice or through phage display.

ECDC Comment (13/06/2007):
This study provides the conceptual basis for the development of an influenza (H5N1) treatment based on passive immunotherapy.

Such approach in not new in the field of prophylaxis and treatment of infectious disease and it proved successful for prophylaxis and/or treatment of diseases such as Hepatitis A and B, varicella, rabies, etc.

There are also some anecdotal reports of successful use of convalescent blood products during the 1918/19 Spanish influenza(1).

Having an additional tool for prophylaxis and treatment of patients affected with H5N1 influenza virus and/or a pandemic virus would undoubtedly be an important public health advance.

However issues such as effectiveness and safety of the product in humans, ability to test the first products in trials in what at present are isolated cases, the capacity to produce it in large quantities at affordable costs, cross protection against drifted variants of H5N1 or other possible pandemic strains, should all be carefully assessed before a clinical use of the product can be reasonably considered.

Caution would also be needed to extension of the concept back to the approach undertaken in 1918-19 using convalescent human blood products.

Transfusions with human blood products always carry a degree of hazard.

Blood products are frequently given to patients in resource poor settings, probably excessively so and sometimes with untoward consequences (unintended infections) for recipients and donors. 2)

HUMAN ANTIBODIES PROTECT MICE FROM AVIAN FLU

An international team of scientists, including researchers from the National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health, report using antibodies derived from immune cells from recent human survivors of H5N1 avian influenza to successfully treat H5N1-infected mice as well as protect them from an otherwise lethal dose of the virus.

"The possibility of an influenza pandemic, whether sparked by H5N1 or another influenza virus to which humans have no natural immunity, is of serious concern to the global health community," says NIAID Director Anthony S. Fauci, M.D. "If the success of this initial study is confirmed through further laboratory and clinical trials, human monoclonal antibodies could prove to be valuable therapeutic and prophylactic public health interventions for pandemic influenza."

The research, to be published May 29 in "PLoS Medicine", represents a three-way collaboration among Kanta Subbarao, M.D., and her coworkers at NIAID; Antonio Lanzavecchia, M.D., and colleagues from the Institute for Research in Biomedicine, Bellinzona, Switzerland; and Cameron Simmons, Ph.D., from the Oxford University Clinical Research Unit, Hospital for Tropical Diseases, Ho Chi Minh City, Vietnam.

Four Vietnamese adults diagnosed with H5N1 influenza infection between January 2004 and February 2005 agreed to donate blood soon after they had recovered from their illness.

In Switzerland, Dr. Lanzavecchia extracted antibody-producing white blood cells, called memory B cells, from the Vietnamese samples and treated them with a process he developed so that they rapidly and continuously produced large amounts of antibody.

Next, researchers in Dr. Subbarao's lab screened 11,000 antibody-containing samples provided by the Swiss team and found a handful able to neutralize H5N1 influenza virus.

Based on these results, Dr. Lanzavecchia purified the B cells and ultimately created four monoclonal antibodies (mAbs) that secrete H5N1-specific neutralizing antibodies.

Dr. Subbarao and her coworkers first tested whether the human H5N1 mAbs could protect mice from severe H5N1 infection.

Groups of five mice received either of two human H5N1 mAbs at one of three dosages or human mAbs against diphtheria or anthrax.

One day later, the mice were exposed through their noses to lethal doses of H5N1 influenza virus.

All the control mice -- those receiving non-H5N1 mAbs -- rapidly developed severe illness and died within a week.

In contrast, all the mice that received the first H5N1 mAb tested -- regardless of dose -- survived, while 80 percent of mice receiving the highest dose of the second H5N1 mAb survived.

Additional tests showed that mice receiving either of the two protective H5N1 mAbs had levels of virus in the lungs that were 10 to 100 times lower than those in control mice, and little or no virus moved beyond the lungs.

The investigators also tested the therapeutic potential of the human H5N1 mAbs.

Using blood products from influenza survivors is an old idea, the researchers note.

During the flu pandemic of 1918-19, for example, physicians took serum from recovered flu patients and gave it to new victims; recent historical research indicates that those blood transfusions, when given early in the illness, sometimes saved recipients' lives.

In their study, Dr. Subbarao and her colleagues infected groups of mice with a lethal dose of an H5N1 virus that had circulated in Vietnam in 2004.

A total of 60 mice were given one of the four H5N1 mAbs at 24, 48 or 72 hours after infection while a control group received non-influenza mAbs.

All the mice in the control group died within 10 days of infection, while 58 of the 60 treated mice survived.

All four H5N1 mAbs conferred robust protection.

Most surprisingly, says Dr. Subbarao, the survival rate was excellent even when treatment was delayed for three days.

Spurred by these results, the NIAID investigators next tested whether the H5N1 mAbs might be used to treat mice infected with a related but distinct H5N1 virus.

Although the four mAbs used in the experiment originated after infection with the 2004 H5N1 virus, three of them nevertheless prevented the mice from dying when given 24 hours after they were infected with a 2005 H5N1 virus.

This suggests, the researchers say, that human mAbs may provide broad protection against variant H5N1 viruses -- a desirable quality in any therapeutic aimed at the constantly evolving flu virus.

Taken together, the findings from this international collaboration are encouraging, says Dr. Subbarao.

They show that fully human mAbs with potent H5N1 influenza virus neutralizing ability can be rapidly generated from the blood of convalescent patients and that these mAbs work well to both treat H5N1 infection and prevent death from such infection in a mouse model. The authors plan to take the research forward by scaling up the production of H5N1 mAbs and, if the technique proves safe and effective in additional animal tests, to evaluate these human mAbs in clinical trials in humans.

2 die of bird flu in Pakistan


Islamabad: Authorities in Pakistan?s North West Frontier Province have said that two men have died of bird flu in a state-run hospital and were the first victims of the virus in the country.
The Khyber Teaching Hospital confirmed that two brothers, Mohammad Ilyas and Mohammad Tariq, who were brought to the facility from Mansehra on suspicion of being infected with the H5N1 virus, died of bird flu on December 10. (PTI)

I have not received the press release.

First, I would like to clarify my source. He is a current employee of the CDC. I have his email, phone, and cell phone numbers. I had the opportunity to meet him when I was in Atlanta. He is a very nice person and working hard to be one of the "good guys" at the CDC. He told me what he knows. He does not work in the area that handled this event.

As far as today goes, this what I was told:

1) A 3rd brother of 2 brothers who died of H5N1 returned to the U.S.

2) He returned to the Long Island area.

3) Nassau County Public Health officials were the officials in charge of this event.

4) This 3rd brother was placed in medical isolation for a period of time.

5) He tests negative for H5N1.

6) The flurry of activity surrounding this event occurred last weekend.

He may have been there only for the funeral of his 2 brothers. This is not clear.

I'm not sure that Farooq and Tariq are that different. All reports have the brother as Ilyas, so I think the two names are just variations.

It's just odd that they both died on the same day. If in fact they died on the 10th, this would be after the "flurry of activity" regarding the brother who flew back to the USA.

</TD></TR><TR><TD>ISLAMABAD: The confirmation of two human deaths by the administration of Khyber Teaching Hospital (KTH), Peshawar put Pakistan on Serial 15, a list of the countries where H5N1 deadly virus had killed 207 people altogether.

Dr Siddiqur Rahman, Acting Chief Executive of KTH and the Spokesman of the health ministry, bird flu surfaced in 1918 (Spanish Flu pandemic, 1957 South East Asia Flu pandemic and 1968 Hong Kong Flu pandemic) and killed 50 million. The killing viruses at that time were H5N1 influenza virus H2N2 influenza virus and H3N2 influenza virus respectively.

The Post had already splashed news of the death of the two people on December 11, 2007. So far 207 people including Pakistanis died through H5N1 virus in 11 countries namely Azerbaijan (5), Cambodia (7), China (17), Egypt (15), Indonesia (91), Iraq (2), Lao People's Democratic Republic (2), Nigeria (1), Thailand (17), Turkey (4), and Vietnam (46).

Talking to The Post, Pakistan Veterinary Medical Council Secretary/Registrar Dr Alamdar Hussain Malik, said that H5N1 is highly infectious and extremely virulent.

Dr Malik said that Avian influenza viruses were subdivided into subtypes based on hemagglutination (H) and neuraminidase (N) protein spikes from the central virus core. There are 16 H types, each with up to 9 N subtypes, yielding a potential for 144 different H and N combinations. In addition, all A1 viruses fall into one of 2 pathotypes: low (LPAI) and high (HPAI) pathogenicity, based on their virulence in poultry populations.

The influenza virus is a masterful quick-change artist. It changes the outer proteins on its shell, known as antigens, easily, he added.

The changes occur because the influenza virus lacks the genetic "proofreading" ability seen in many more stable viruses. When it makes an error in reproducing its own genetic material, the influenza virus passes along these errors to progeny viruses. In these ways, even a tiny error becomes permanent-or as permanent as a mutable agent like the flu virus.

He said antigenic drift occurs every year or so, rendering any prior immunity to influenza, whether acquired naturally or through a vaccine, ineffective after a very short time.

"Antigenic shift has happened three times in last century. What's going on now may be the fourth. On the 16H types known, only subtypes H5, H7 and H9 are known to be capable of crossing the species barrier from birds to humans.

It is feared that if the avian influenza virus undergoes antigenic shift with a human influenza virus, the new subtype created could be both highly contagious and highly lethal in humans.

Responding to a question, Dr Malik said that direct contact with infected poultry, or surfaces and objects contaminated by their faeces, is presently considered the main route of human infection.

He said the stakeholders of the poultry industry claimed that the poultry is the second largest industry after textile but the Ministry of Food, Agriculture and Livestock and the provincial governments have failed to do any thing in this regard.

He said a high-level probe committee must be constituted to find out where the billions of rupees in funds/grants provided for the control and epidemiological studies of the H5N1 virus have gone?

The responsible must be punished because they are the main culprits, which brought the poultry industry on the verge of collapse.

Finally, he said that an inquiry should be held into why without having personal protection equipment kit the department concerned officers approached the H5N1 infected poultry farms and carried out the culling of birds.

They have to stop referring to these two guys as Pakistan's first suspected fatalities. The brothers of the vet are the first suspected fatalities -- not these poultry farm workers.
