UPDATE ON THE EBOLA VIRUS DISEASE IN DRC No. 3
28 August 2014

1. Update

Between 28 July and 27 August 2014, a total of 42 cases have been identified in the Area, including 13 deaths (mortality rate: 31%).

13 people died from the disease (31% mortality rate), including 5 health professionals. 11 people have been put in isolation.

147 contacts identified for follow-up.

6 positive samples out of 8 after a second analysis carried out at the INRB in Kinshasa and at the CIRMF of France Ville in Gabon.

Other samples taken in Boende on 27 August being sent to Kinshasa.

Index case identified in Ikanamongo (100 km from Boende): a woman died on 11 August after handling and eating bush meat hunted by her husband that contaminated both her husband and her doctor.

Rumours on suspected cases in other provinces have not been confirmed.

2. Coordination

 Establishment and holding of regular meetings of the National Committee on Coordination (CNC).

 Existence of seven thematic commissions at national and local levels to ensure the coordination of humanitarian action; Six of these commissions meet on a daily basis: Epidemiologic Surveillance; Communication; Laboratory and Research; Support for the Sick; Water, Hygiene and Sanitation; and Logistics.

 Establishment of a UN Ebola Task Force and holding of its first meeting on 26 August 2014.

 Recommendation: participation of UN entities and clusters to technical commissions, in line with
their expertise, notably with regard to Logistics, Food Security and WASH.

3. Security and logistics

 Availability of UNHAS and ASF-F aircraft: UNHAS: One 37-seats, cargo 2,7 MT aircraft aircraft; one helicopter available in Goma and
possibility to have an additional helicopter from South Africa, in case of need. ASF-F: one/two 9-seats 1,2 MT cargo aircraft.

 Drafting of protocols for the regulation of travel in the quarantined area, on the basis of protocols currently used in West Africa by UNHAS. These protocols will be shared with other aviation companies for their consideration.

 Announcement by the Government of the provision of a daily cargo to send material.

4. Response

4.1. Ongoing
 A second team of experts has travelled to the field on 27 August 2014, in view of the establishment of the International Committee on Technical Coordination (CICTS) in Lokolia, epicentre of the disease. This Committee will comprise experts from the Ministry of Health and WHO.

4.2. Planned
 Strengthening of field teams with technical staff and additional inputs.

5. Financing

 Process to request CERF ($2 million) and CHF ($1.5 million) funds under way.

6. Gaps

 Need to strengthen the technical field team with an anthropologist

Update

* The total number of cases identified in the Boende health zone as of 29 August 2014 is 53, including 13 confirmed cases.

* Among the remaining 40 cases, 21 are probable and 12 are suspected cases and are being followed up by the field teams.

* The number of deaths remains 13, no additional death recorded.

* Analysis conducted on all samples from Boende which had been sent to Germany reveals the ZAIRE strain, 99% homologous to KIKWIT.

* The cities of Boende, Monkoto (Equateur) and Lomela (northern Eastern Kasa?) are currently considered to be at risk. One of the patients hospitalised at the HGR Boende had managed to escape before being recovered by the services deployed to the field.

Coordination


* Publication of guidelines for travellers to foreign countries from DRC border posts by the National Programme on Hygiene at the Borders (NPHB).

* Holding of daily meetings of the National Committee on Coordination (CNC) at the office of the Minister of Health.

Security and logistics

* Announcement by the Government of the provision of a cargo flight today, 30 August 2014.

A one percent divergence from Kikwit places it around the 2007 DRC Zaire ebolavirus, according to this chart: http://currents.plos.org/outbreaks/files/2014/04/EBOV_cds_mb_path.png

Both had high CFRs.

I guess that rules out the initial Sudan results. It's odd; I know that Bundibongo cross-reacts with Zaire in some tests, but never knew that Sudan did.

 Communication of guidelines on appropriate conduct carried out by clinicians in the areas affected by the epidemics:
o Surgeries and injections to be reduced to the minimum for three weeks
o Vaccination activities to be postponed
o Malaria rapid diagnostics to be suspended

*The number of suspected cases has fallen, as some have been laboratory confirmed.

 **12 alerts including one with death in Kokolo military camp in Kinshasa, a second reported in Kambove in Katanga and 10 cases of which 8 deaths reported in Bongandanga, Equateur Provincer. Five samples from Bongandanga and from Nyakunde in Ituri (Orientale Province) are being analyzed.

 Between 2 and 10 September, 35 cases of EVD have been reported in the area of Djera in Equateur Province in DRC, increasing the cumulative number of cases to 66 (see the above table for details).

 Two additional treatment centers are operational since 9 September with the capacity of 50 beds

 Some local/provincial committees of the CNC (Ebola response coordination) are not operating optimally like Kinshasa; some coordination tools for the smooth functioning of these structures at local / provincial level are United Nations Office for the Coordination of Humanitarian Affairs

 Epidemic Control Commission (ECC) has submitted a health roadmap on the management and prevention of the Ebola Virus Disease to the DRC Minister of Health as per recommendations of the latest meeting of SADEC Ministers of Public Health held in Zimbabwe.

 Two hotline numbers have been set up to allow the population to get information on the disease and ongoing efforts: 081 08 000 20 and 099 99 710 05. An additional hotline for information on the prevention of the EVD is available using Tigo: 119.

 The contingency plan for Kinshasa has been finalized, but actions for its full activation are yet to be taken.

*Four suspected cases were negative after laboratory tests on two different samples and therefore discharged and declared "non-case", bringing down to 68 the total number of cases on 18 September (see table above for details ) of which 41 deaths; an overall lethality rate of slightly over 60%.

 **8 health personnel have died of Ebola Virus Disease (EVD) since the outbreak of the epidemic.

 The centrifuge of the mobile laboratory in Lokolia has broken-down, making it impossible to carry out on-site laboratory tests in Lokolia. A centrifuge parts is expected from the National Institute of Biomedical Research (INRB) in Kinshasa

 INRB is facing the shortage of reagents and inputs for both its laboratory in Kinshasa and the mobile laboratory in Lokolia. A request for reagents and laboratory inputs has been made to the DRC Ministry of Health for an urgent solution.

The death rate of animals and poultry continues to be very high in the area affected by the EVD, requiring thorough veterinary investigations

 According to the National Coordination Committee (NCC), the overall analysis of the epidemiological situation suggests that Ebola outbreak declared on August 24 in the area of Djera is under control.

 The Food Security Cluster and the Ministry of Social Affairs, Humanitarian Action and National Solidarity will distribute over 1,000 tons of food, including 4.9 tons to members of the response team, the indoors patients and to the family members of people affected by the EVD. This distribution is facing important logistical constraints related to poor road conditions in the area of Djera.
