Nosocomial infections and anti-infective treatments in healthcare establishments: results of the 2022 national prevalence survey
The national survey on the prevalence of nosocomial infections and anti-infective treatments in health establishments (ENP), carried out in France every five years since 1990, makes it possible to produce reference indicators contributing to the evaluation of policies for the prevention of infectious risks in these establishments.

Since 2011, it has been part of a program of similar surveys conducted in all the countries of the European Union, under the aegis of the ECDC. In 2022, Public Health France coordinated, within the framework of the Healthcare-Associated Infection Prevention Network (Repias) and in collaboration with the Support Centers for the Prevention of Healthcare-Associated Infections (CPias), a new edition of this investigation and today publishes the first results.
Nosocomial infections: one in 18 hospitalized patients concerned
The ENP 2022 was carried out on a given day, between May 15 and June 30, 2022. It focused on nosocomial infections, i.e. those contracted in health establishments which occur during or after of the care of a patient and who were neither present nor incubating at the start of this care.
A total of 1,155 health facilities participated including 151,676 patients. Nearly one in two establishments took part in this survey, a very satisfactory proportion in view of the severe constraints linked to the COVID-19 pandemic which then weighed on these establishments.
The results show a prevalence of infected patients of 5.71%, i.e. one hospitalized patient out of 18 presenting at least one nosocomial infection. This prevalence of infected patients increased between 2017 and 2022 (+14.7%). Nosocomial SARS-CoV-2 (COVID-19) infections accounted for half of this increase. Excluding the latter, the prevalence of infected patients was 5.35%, relatively stable compared to that estimated in 2017 (4.98%).
The prevalence of infected patients varied according to the category of establishment and the department caring for the patient. It was higher in cancer centers (15.81%) and CHR/CHU (8.58%), intensive care units (23.17%) and medical units (6.84%). ), category of institution and types of services in which patients are at increased risk of infectious complications: age over 65 years, medical condition (life-threatening condition, immune status and malignancy), surgical intervention or exposure to invasive devices.
In addition, there was great stability in the distribution of the main locations of nosocomial infections and main microorganisms isolated from infection between 2017 and 2022. Thus, the four main locations of nosocomial infections (urinary tract infections, pneumonia, infections of the surgical site and bacteraemia) represented 70.7% of the infectious sites documented, i.e. a stable proportion compared to 2017 (71.5%). Similarly, the four main microorganisms ( Escherichia coli , Staphylococcus aureus , Enterococcus faecalis , Pseudomonas aeruginosa ) accounted for 48.3% of microorganisms isolated from infection, a relatively stable proportion compared to 2017 (50. 2%).
An increasing prevalence of patients treated with antibiotics
The results of the survey show that one out of 6 patients hospitalized in a health establishment received at least one antibiotic treatment for systemic use in 2022, i.e. a prevalence of 16.24%. The prevalence of patients treated with antibiotics increased by 7.5% between 2017 and 2022.
One in two patients was treated with antibiotics in intensive care units (49.19%) and one in four in medicine (25.72%) or surgery (26.97%) departments.
The 5 most prescribed molecules or combinations of molecules in 2022 were not identical to those observed in 2017 and were: amoxicillin-clavulanic acid combination, ceftriaxone, piperacillin-tazobactam combination, amoxicillin and cotrimoxazole . They accounted for half of the molecules prescribed in healthcare establishments (49.9%).
These results encourage us to pursue actions to prevent healthcare-associated infections by targeting them on the most frequent infections and to strengthen actions in favor of the proper use of antibiotics.
The data from the French survey will be transmitted to the ECDC; the first results produced at European level by the ECDC should be available in early 2024 and will allow the situation in France to be compared with that of other countries.
