Many people are born with a primary immunodeficiency and many of these are undiagnosed. Perhaps these conditions account for some of the unexpected H1N1 deaths.
One commonly undiagnosed condition, selective immunoglobulin A deficiency, is present in about 1 of every 333 people and it is an inability to produce IgA, primarily on surface of the respiratory and digestive system.

The Australian hospital testing that uncovered the high frequency of IgG2 in severely ill patients shows one aspect of this association.
Novel influenzas may exploit some of the more common immunodeficiencies which may be present primarily in the young (like low IgG2 levels).
