EVENT DESCRIPTION

On 15 January 2018, WHO was notified of a new cluster of five Lassa
fever cases reported from Ebonyi State. The index case was a 15
year-old boy from Abakaliki local government area who was admitted
to hospital on 28 December 2017 with fever, unconsciousness, and
bleeding from the mouth and nose. He died a few days later. Four
healthcare workers (HCWs) who treated the index case fell ill. Three
of the infected HCWs died (on 14 January and 15 January 2018) and
the fourth remains hospitalized. Samples taken from the HCWs tested
positive for Lassa fever by PCR. A total of 139 contacts have been
identified and are being monitored.

This Lassa fever cluster is part of an ongoing outbreak. Since the
resurgence of the current wave in December 2016 (week 49), a total of
1 022 suspected cases including 127 deaths (case fatality rate 12%)
were reported as of 24 December 2017. During this outbreak, 19 out
of 36 states have reported at least one confirmed Lassa fever case
(Ogun, Bauchi, Plateau, Ebonyi, Ondo, Edo, Taraba, Nasarawa, Rivers,
Kaduna, Gombre, Cross-River, Borno, Kano, Kogi, Enugu, Anambra,
Lagos, and Kwara). In week 1 of 2018 (ending 7 January 2018), one
confirmed case was reported from Bauchi State.

PUBLIC HEALTH ACTIONS

The Nigeria Centre for Disease Control Lassa fever response working group is leading coordination of weekly Lassa fever review meetings with
participation by WHO, MSF, US CDC, and other partners.

The Ebonyi State Emergency Outbreak Committee has been activated by the state governor and meets daily to coordinate response activities.
Funds have been released by the Ebonyi State government to initiate the response, with particular focus on ensuring case management.
Active case finding and monitoring of contacts is ongoing in Ebonyi State.

A standard Lassa fever case definition has been shared for posting in health facilities.

Community sensitization activities are ongoing and radio jingles are planned.

Clinician sensitization is planned for Ebonyi State.

SITUATION INTERPRETATION

Lassa fever is endemic in Nigeria. However, the recent cluster of cases in Ebonyi State suggests that current preparedness and response activities
are insufficient to prevent significant morbidity and mortality. Implementation of measures to prevent infection among healthcare workers, including
infection prevention and control and clinician sensitization to Lassa fever, are urgently needed to prevent future transmission of Lassa fever in
healthcare facilities. Continuation of enhanced surveillance in Ebonyi State and other states affected by the outbreak is also advisable to facilitate early
detection and treatment of new cases.

Lassa fever infection soars in Nigeria

At least 21 people died from Lassa fever in Nigeria last month. DW talks to the head of the Nigeria Centre for Disease Control (NCDC) about what is being done to prevent more fatalities.

DW: Why is it so hard to fight Lassa fever, given that it was first identified in Nigeria in 1969?

Chikwe Ihekweazu: Lassa fever has been endemic in our country for many years. What has changed in the last two years is our ability to detect it. At NCDC (Nigeria Centre for Disease Control) and our collaborating labs across the country, over the last two years, we have strengthened our surveillance infrastructure, we have strengthened our laboratories. We have increased people's awareness of the need to use primary health care services. In a way, what has changed is our ability to detect an illness that has been there all along...

EVENT DESCRIPTION

The Lassa fever outbreak in Nigeria continues to be closely monitored
by local authorities and WHO and partners. In the reporting week
ending 28 January 2018, 15 new confirmed cases and two deaths
were recorded from five states; Edo (6), Ondo (4), Delta (1), Imo (1)
and Taraba (3). From 1-28 January 2018 a total of 297 suspected
cases and 22 deaths (case fatality rate 7.4%) have been reported from
13 active states (Anambra, Bauchi, Benue, Delta, Ebonyi, Edo, Imo,
Kogi, Lagos, Nasarawa, Ondo, Plateau, and Taraba).
Since the start of 2018, among the 80 cases for which laboratory
results are available, 77 are confirmed as positive for Lassa fever, with
three probable cases and 21 deaths (18 in confirmed cases and three
in probable cases). The case fatality rate in confirmed and probable
cases is 26.3%.

Ten healthcare workers have been affected in four states ? Ebonyi
(7), Nasarawa (1), Kogi (1) and Benue (1), with four deaths in Ebonyi
(3) and Kogi (1). Confirmed cases are being treated at identified
treatment/isolation centres across the states. A total of 415 contacts
have been identified and are being followed up.
...
SITUATION INTERPRETATION

The upsurge in Lassa fever cases and deaths since the start of 2018 is of concern.
Available treatment facilities are overstretched and lack of funding
is preventing contact tracing in Edo State. These challenges, coupled with problems with logistics for deployment of supplies between states, delays
in samples reaching diagnostic centres and gaps in surveillance and contact tracing in some states need to be addressed urgently. National authorities
and international health cluster partners must move rapidly to prevent further spread of the disease across the country.
