A paper published in the Medical Journal of Australia today provides a possible reason for CSL?s 2010 flu vaccine causing febrile convulsions in children. The authors hypothesise that suboptimal use of the detergent called deoxycholate ? used in the manufacturing process by CSL (one of the few vaccine manufacturers that use it) ? to split the flu virus from its membrane may be at fault.

Professor Anne Kelso, Director of the WHO Collaborating Centre for Reference and Research in Influenza, explains the manufacturing process of flu vaccines and provides the context for understanding the authors' hypothesis.

AN unusual ingredient used by Australia's biggest pharmaceutical company, CSL, may have triggered fits in children given the flu shot last year, the Medical Journal of Australia reports today.



It says clinical trials of CSL's previous flu vaccines were also linked to high rates of fever in young children - years before the latest version of Fluvax had to be suspended for the under-5s.

The article by Heath Kelly - who heads the epidemiology unit at the Victorian government's infectious diseases laboratory in Melbourne - hypothesises that "suboptimal virus splitting" by CSL may have played a role.

To make a vaccine, pharmaceutical companies usually kill the virus and then split it with a detergent or solvent to minimise any side effects.

But Professor Kelly says CSL is "one of only a few manufacturers globally" to use an acid known as deoxycholate to split the virus. "While there may be other causes, the use of deoxycholate should be further explored," his paper says. "Comprehensive ... investigations of (side effects), especially those involving children, are necessary to prevent their recurrence and to maintain public confidence in vaccination programs."
