Q fever restrictions in the Netherlands lifted

The Dutch Agriculture Ministry has lifted restrictions on breeding and transporting milk goats and milk sheep, which had been in force since the outbreak of Q fever earlier this year.

The disease is spread by bacteria that use pregnant goats and sheep as hosts; it affects humans and can be lethal for weakened patients. Since January 420 people have been infected with Q fever, and 5 people who were already suffering from other afflictions died.

The lifting of restrictions enables goat farmers to repopulate their stables. In an attempt to minimise the spread of Q fever, 89 goat and sheep farms were cleared of all livestock. This has led to a shortage of some 35,000 goats, according to farmers' umbrella organisation LTO. Many farmers have been financially crippled by the loss of income, and LTO warns that many of them may not be able to restart their businesses.

Recent research in the Netherlands says vaccination of goats works. The shedding of Q-fever bacteria stops (almost..) after vaccination.
New policy of Min of Agr. says: no more culling of pregnant goats, since all goats are vaccinated.

The human epidemic seems to be over.

The 91st farm to be found Q-fever infected in the Netherlands is a
dairy goat farm in Oirschot, North Brabant, about 12 km (7.5 miles)
north-west of Eindhoven (map at

testing of milk, has occurred in spite of vaccination, which had been
duly and timely carried on. Vaccination does not rule out infection
altogether, but is expected to significantly reduce bacteria
proliferation and spread by infected animals, and will prevent abortions.

The case was announced by the Ministry of Agriculture on 13 Jul 2010.
The vaccinated flock will not be culled nor will a breeding ban
implemented. To be on the safe side, prescribed biosecurity measures
have been applied.

The new infected location has been added to the Dutch Q-fever map,

According to the data of the public health authorities, out of the
420 human Q-fever cases notified during 2010, 310 patients have

6th and 7th Q-fever fatality reported in 2010 - total to 18


All fatalities in 2010 are chronic Q-fever patients, with underlying medical problems, who were infected in previous years, says RIVM.

RIVM institute also has the number of people with their 1st day of infection in a certain year.

Abstract

SUMMARYThe 2007-2009 human Q fever epidemic in The Netherlands attracted attention due to its magnitude and duration. The current epidemic and the historical background of Q fever in The Netherlands are reviewed according to national and international publications.

Seroprevalence studies suggest that Q fever was endemic in The Netherlands several decades before the disease was diagnosed in dairy goats and dairy sheep. This was in 2005 and the increase in humans started in 2007. Q fever abortions were registered on 30 dairy goat and dairy sheep farms between 2005 and 2009. A total of 3523 human cases were notified between 2007 and 2009.

Proximity to aborting small ruminants and high numbers of susceptible humans are probably the main causes of the human Q fever outbreak in The Netherlands.

In general good monitoring and surveillance systems are necessary to assess the real magnitude of Q fever.

The Netherlands National Institute for Public Health and the Environment continues to report an outbreak of Q fever in The Netherlands. From January 1 through October 6, 2010, 482 human cases have been reported, including 7 deaths.

Of these reported cases, the exact number of confirmed cases is not known. Most of these cases have been in Noord (North) Brabant, Gelderland, Limburg, and Utrecht Provinces in the southern part of the country, although cases have been reported throughout The Netherlands.

These cases represent an outbreak of Q fever that has been ongoing in The Netherlands since 2007: 168 cases were reported in 2007, 1,000 cases in 2008, and 2,357 cases, including 6 deaths, in 2009.

Report says: Min of Agriculture slowed down the process, measures where not taken until to late.

Min of Health and Dutch CDC (RIVM) stood by and watched as it happened. They were overruled by Min of Agriculture and let it happen.

Only after some doctors of a regional Health Service (GGD) published alarming reports on Dutch television, a breakthrough was achieved.



Q fever report criticises government

The previous government made serious mistakes in its approach to the goat and sheep disease Q fever, according to an official report into the crisis.

The health and farm ministries did not make it clear enough what risks people ran of developing Q fever and what they could do to prevent the spread, the report says.

It was not until 2009 that ‘decisions were taken at high speed’ even though the number of cases of Q fever had been going up since 2007, the report says.

Last year 2,354 people were diagnosed with the disease and six people died.

At the end of last year, the government announced a cull of some 40,000 goats in an effort to eradicate the disease.

Dutch Government Faulted in Massive Disease Outbreak

AMSTERDAM—The Dutch government failed to mount a robust response in the face of the world's worst outbreak of Q fever, a disease transmitted from farm animals to humans, according to an expert panel that presented its report on Monday. More than 4000 people have fallen ill since the outbreak began in 2007, and 14 have died.

Q fever, caused by the bacterium Coxiella burnetii, is primarily an infection of cattle, goats, and sheep. It occasionally sickens humans, usually after they breathe in air contaminated with the bacteria, which are present by the billions in amniotic fluid and placenta from infected females and in aborted fetuses. While most people have few or no symptoms, others develop fever, pneumonia, severe headaches, coughing, painful muscles and joints, or fatigue.

During the first years of the outbreak—which earned the Netherlands a travel warning from the U.S. Centers for Disease Control and Prevention—the government appeared to have a wait-and-see attitude, says the panel, chaired by agricultural economist Gert van Dijk of Nyenrode Business University. It took too long, for instance, to establish that goat farms were the main source of the outbreak and to adopt measures to contain the risk. Alarmed regional public health officials in North Brabant, the hardest-hit province, had trouble getting their voices heard by the national government in The Hague, says the report, which is in Dutch.

As a result, 2008, in which more than 900 people fell ill, was a "lost" year; only in 2009, when the crisis had escalated further, was a robust control program launched, including compulsory vaccination of goats and sheep and culling of 62,500 pregnant animals on infected farms.

One cause of the delays, says the report, was that the issue was handled by two departments that didn't always communicate well. The agriculture ministry was worried that aggressive control measures, if not backed by solid science, would lead to lawsuits from farmers; the health ministry didn't push back hard enough, nor did it have the formal powers to ensure that public health prevailed. In future outbreaks of zoonoses, diseases that are transmitted from animals to humans, the health ministry should have the lead, says the panel.

Numbers released earlier this month by the National Institute for Public Health and the Environment show that the control strategy eventually paid off: So far, only 382 people have fallen ill with Q-fever in 2010, compared with more than 2300 in 2009.

Acute Q fever related in-hospital mortality in the Netherlands.

Abstract

Introduction: A large outbreak of acute Q fever has been reported in the Netherlands with over 3500 cases from 2007 to 2009, during which 749 patients were hospitalised. In foreign cohorts, reported mortality rates in patients hospitalised with acute Q fever, ranged from 0.9 to 2.4%. We analysed mortality among hospitalised patients with acute Q fever in the Netherlands.

Methods: Physicians from hospitals in the afflicted region were asked to provide details about patients who died with a diagnosis of acute Q fever between 2007 and 2009.

Results: Nine patients (seven males, median age 72 years) from six hospitals were reported, who died within approximately one month following hospitalisation for acute Q fever. Six definite acute Q fever cases and three probable cases were identified. Six patients presented with infiltrates on the chest X-ray and a median CURB-65 score of 3. Median time of hospitalisation was 13 days (range 1-33).
All patients had serious, often coinciding, underlying conditions including chronic cardiovascular disease, chronic lung disease, diabetes mellitus and malignancy.

Conclusion: The mortality rate of patients hospitalised because of acute Q fever was estimated at approximately 1%. Patients who died with acute Q fever were often male, of older age, and had chronic coinciding underlying conditions, which gives an a priori higher risk of death.

"Outbreak continues " , some thoughts:

- The "Q-fever" bacteria (Coxiella Burnettii) is a very persistent bacteria, once in the environment it can survive for years. You can find it almost everywhere, including in cities, schools, postoffices etc., according to research in the USA.

- The shedding of the CB bacteria in the Netherlands has (almost) stopped because of vaccination of all dairy goats and sheep.

-In 2011 untill now 10 new cases were reported; in 2010 and 2009 at this time of year it was some 135.

- We don't know what the baselevel of Q-fever infection is. Q-fever is a neglected disease, all over the world. After 4000+ cases and 20+ deaths Dutch doctors know now they have to check for Q-fever in some cases. So if you look for it, you will find it.

- Serosurveillance showed the % of Dutch with Q-fever antibodies went down from ca 50% 30-40 years ago to ca 2% or 3% today. People seem to have become more vulnerable?

- It could very well be the outbreak has stopped and the cases we see now are "base level".

- You can't draw any conclusions on the basis of a number of 10 cases.

- Most infections are during May and June; last year - 2010 - 506 cases were reported; in 2009 it was more than 2350 cases. Some graphs here.

- You can't say in february: "the outbreak continues" ; we will know more in June.

Abstract

SUMMARY - The Netherlands has experienced large community outbreaks of Q fever since 2007. Sera and questionnaires containing epidemiological data from 5654 individuals were obtained in a nationwide seroprevalence survey used to evaluate the National Immunization Programme in 2006-2007.

We tested these sera for IgG phase-2 antibodies against Coxiella burnetii with an ELISA to estimate the seroprevalence and to identify determinants for seropositivity before the Q fever outbreaks occurred. Overall seroprevalence was 1?5% [95% confidence interval (CI) 1?3-1?7].

Corrected for confirmation with immunofluorescence results in a subset, the estimated seroprevalence was 2?4%. Seropositivity ranged from 0?48% (95% CI 0?00-0?96) in the 0-4 years age group to 2?30% (95% CI 1?46-3?15) in the 60-79 years age group.

Keeping ruminants, increasing age and being born in Turkey were independent risk factors for seropositivity.

The low seroprevalence before the start of the outbreaks supports the hypothesis that The Netherlands has been confronted with a newly emerging Q fever problem since spring 2007.

Q-Fever update

On may 12 the number of Q-Fever patients reported in 2011 in the Netherlands was 41, including 1 fatality. The patient who died had chronic Q-fever, was infected in a previous year and underlying medical problems according to the RIVM (= Dutch CDC).

This number of patients seems to be close to the "base level" .

Last year on may 12 307 patients were reported, compared to 367 patients in 2009 during the same period.

The number of people killed by Q-fever, a flu-like illness which mainly infects sheep and goats, was at least 24, the head of the public health institute RIVM told television current affairs show Nieuwsuur on Tuesday.

The previous official death toll stood at 19.

Q-fever victims to sue government for compensation

Some 400 people who became infected with the sheep and goat disease Q-fever during the recent epidemic are planning to sue the government for damages, lawyer Luc Rohof told television show Nieuwsuur.

Nieuwsuur also accuses ministers of misinforming parliament about the epidemic, which killed at least 25 people.

Brabant health chief Jos van de Sande told the programme: 'The cabinet is trying to keep its street clean but the problems keep on piling up.'

The cabinet has set aside ?10m to support people who became ill, but is not willing to set up a compensation fund, despite serious criticism of its position from the national ombudsman about the way it handled the epidemic.

Chronic conditions

Hundreds of people are said to still have chronic health problems since the outbreak which ended two years ago. Q-fever was a serious problem on Dutch goat farms between 2007 and 2010. Over 40,000 goats were killed in an effort to eradicate the disease.

The national ombudsman Alex Brenninkmeijer said last month the government had failed to take the outbreak seriously and was too slow to react. He also said ministers should apologise to victims and pay them reasonable compensation.

In February, it emerged a number of victims are also in the process of suing the goat farms where they became infected.

Screening for Coxiella burnetii seroprevalence in chronic Q fever high-risk groups reveals the magnitude of the Dutch Q fever outbreak


SUMMARY
The Netherlands experienced an unprecedented outbreak of Q fever between 2007 and 2010. The Jeroen Bosch Hospital (JBH) in 's-Hertogenbosch is located in the centre of the epidemic area.

Based on Q fever screening programmes, seroprevalence of IgG phase II antibodies to Coxiella burnetii in the JBH catchment area was 10?7% [785 tested, 84 seropositive, 95% confidence interval (CI) 8?5?12?9]. Seroprevalence appeared not to be influenced by age, gender or area of residence. Extrapolating these data, an estimated 40 600 persons (95% CI 32 200?48 900) in the JBH catchment area have been infected by C. burnetii and are, therefore, potentially at risk for chronic Q fever.

This figure by far exceeds the nationwide number of notified symptomatic acute Q fever patients and illustrates the magnitude of the Dutch Q fever outbreak. Clinicians in epidemic Q fever areas should be alert for chronic Q fever, even if no acute Q fever is reported.
