KATHMANDU, Jan. 25 (Xinhua) -- Nepal has found a patient from Damak, eastern Nepal, to be suspected case of human bird flu infection, Nepali national news agency RSS reported on Sunday.
Damak of Jhapa district is some 320 km east of Nepali capital Kathmandu. In mid-January, the first case of bird flu in poultry was detected in Kakarbhitta of the same district.
According to the report, bird flu has been suspected to have infected Santosh Dahal from Damak who was taken to nearby BP Koirala Institute for Health Sciences (BPKIHS) in Dharan. Communication spokesperson for BPKIHS Bijay Rimal said bird flu virus has been suspected to have been found in Dahal's blood sample during test in institute's laboratory. He said Dahal's blood sample has been sent for further tests in a sophisticated laboratory. He said Dahal was showing symptoms of bird flu infection. The BPKIHS however did not reveal where Dahal's blood sample was sent. Rimal, however, said it cannot be officially confirmed whether Dahal is infected with bird flu yet as the institute lacked a specific lab to test bird flu. Dahal was referred to Dharan by Lifeline Hospital in Damak following three-day treatment there.

Background information on Damak

Damak is situated in the western part of Jhapa district, which lies in the eastern part of Nepal.Damak lies between the border of Ratuwa and Mawa rivers,beside it itself acts as a border of Jhapa and Morang District,and Mechi and Koshi Zone too.

Seperated into 19 wards, the total area of this city is 7531 hector ,which spreads from north to south. Nine of the wards touches the highway, i.e. the Mahendra Highway. Ward no.10,11,12,13 and 14 lies in the bazar area. With132 meters above the sea level, it has Lakhanpur in its east, Urlabari, Rajghat and Madhumalla in the west and Chulachuli Illam and Kohabari V.D.C in the north and the south, respectively. The total population of Damak municipality is about 58590 in which 76.93% are literate.​

Damak दमक is a town and municipality in Jhapa District in the Mechi Zone of south-eastern Nepal. It is situated between the Ratua Khola (River) in the east and the Mawa Khola (River) in the west. At the time of the 1991 Nepal census it had a population of 41,321 people residing in 7,644 individual households.<SUP class=reference id=cite_ref-0>[1]</SUP>
Damak was changed to a municipality from a VDC (Village Development Committee) in the year 1982 AD. The Damak municipality consists of 19 wards which also includes the large Himalayan Tea State, where the Royal Family has also invested. The municipality covers an area of 7,513 hectares and is at an average of 100 meters above the sea level. Lakhanpur is to the east, Urlabari, Rajghat and Madhumalla in the west and Chulachuli Ilam and Kohabara vdc in the north and the south, respectively.
According to the census of 2001, Damak had 7,178 households containing about 35,000 people. The total area is 7,513 hectares out of which 1406.7 hectares is covered by residential area whereas 5,586.30 hectares is used for agriculture. The market area consists of 400.00 hectares, 215 hectares as forest plant area and 265.00 hectares is used in other purposes. In 056/057 there was 1 Health post, 1 Hospital (AMDA) and 1 Nursing Home (Life Line) which contains 50 beds in total. The Population Growth Rate is 5.14 per annum as per 2047/48 census. As per 056/057 census, there were about 88 industries, 1011 shops, 244 hotels and 9 financial institutions. Black topped road were 29 km, graveled roads were 150 km and earthen road were 500 km. In addition to it, the drainage runs 3.5 km through the main highway. There are 512 street lights and 1,114 telephone lines to public.

A meeting held here today made a decision not to allow the sale of the checked poultry, Hamal said. According to district veterinary office, 350 poultry farms and 1,35,000 chickens were in their watch. Police said the watch had been extended to Damak and bordering Gaurigunj as well.

Dahal was referred to Dharan by Lifeline Hospital in Damak following three-day treatment there.

The above comments describe the first lab confirmed H5N1 case in Nepal. The sample has been sent for third party confirmation. Since Weybridge confirmed H5N1 in Nepal poultry, it seems likely that the sample was sent to Mill Hill in England.

This report provides more specifics to reports of human cases in Nepal. An earlier report described 5 patients in critical condition, but specifics were lacking. They were said to have developed symptoms about a week ago, which is consistent with the above case, who was hospitalized locally for 3 three days prior to transfer to Dharan (see satellite map).

More detail on the symptoms and condition of this patient would be useful.

If confirmed, the case would cast serious doubt on India?s claim that there are no human H5N1 cases there.

People under medical supervision in bird flu area discharged

Three people held under medical supervision after their symptoms matched those of bird flu are now considered to be out of the infection, a concerned official confirmed.

"All three, that include two members of a culling team and a woman of Dhulabari, near to Mechinagar ? 10, government-declared bird flu crisis zone, have been discharged from hospital," said Manas Kumar Banarjee, coordinator of the Avian Influenza Control Project, under the Ministry of Health and Population. "They were not infected with the bird flu."

Two members of the chicken culling team, an animal health worker suffering diarrhea and a veterinary doctor suffering cough, throat pain and fever, were admitted to the Mechi Zonal Hospital.

Similarly, a woman of Dhulabari suffering fever, cough and throat pain was also admitted to the hospital.

The hospital discharged the animal health worker after confirmation his health problem was not due to avian flu.

Similarly, the veterinary doctor was diagnosed with kidney problems, while the woman?s symptoms were found to be attributed to a medical abortion. She was discharged yesterday, Dr Banarjee said.

On January 22, the Ministry began active surveillance on those living around the crisis zone.

Symptoms of bird flu are siimilar to the symptoms of general flu among human, such as cough, fever and throat infection.

The RSS had reported that a Nepali from Damak town in eastern Nepal’s Jhapa district - where the first bird flu infection in poultry was reported officially earlier this month - was admitted to BPKIHS with suspected avian influenza.
“The speculation was triggered probably because bird flu was reported in Jhapa and the patient, Santosh Dahal, is from Damak, also in Jhapa district,” Das said.
“Dahal is actually suffering from bronchial pneumonia, some symptoms of which resemble bird flu. He is still under our care but not in any danger.”
Nepal’s epidemiology division also ruled out reports of any suspected bird flu infection among humans.
“We have opened a hotline to monitor the situation and get hourly updates,” said Manash Kumar Banerjee, a doctor. “No human infection has been reported.”
Over 18,000 chickens and ducks were slaughtered in Nepal in the past week after detection of the avian influenza virus in Mechinagar in Jhapa district bordering India’s West Bengal state.
The government has banned the sale of poultry products from Jhapa, announced a high alert in 26 districts adjoining India and stepped up an awareness campaign to keep the disease confined to Jhapa.

Absent an etiological agent for the pneumonia, a diagnosis of pneumonia does NOT rule out H5N1, since H5N1 causes pneumoinia (and most H5N1 fatalities follow the development of atypical pneumonia).

Kathmandu , Jan 25 Panic gripped Nepal today when the official media reported the first case of bird flu infection in human in the Himalayan nation, prompting the government to launch a probe which found that it was a case of pneumonia.
Hours after the reports went on wire, B P Das, Director of BP Koirala Institute of Health Sciences (BPKIHS) in Dharan rebuffed the news terming them as rumour.
Das said that Santosh Dahal (18) was admitted in the hospital after being infected with pneumonia, not bird flu.
However, he added that hospital has made preparation for a separate&apos Tropical Ward&aposfor treatment of avian flu keeping in view the possible outbreak of the disease and have asked the government to provide additional manpower, medicine and tools.
Earlier, Nepalese National News Agency (RSS) quoting a spokesperson from BPKIHS in Dharan, reported that bird flu virus has been suspected to be found in blood sample of Santosh Dahal-- a poultry farmer from Damak area of Jhapa district-- during test in institutes laboratory.

The title on this thread is in error. The government says there is no CONFIRMED cases of H5N1 in Nepal.

There has been no statement that the positive local lab result is in error.

I have added the word "Confirmed" to the title of the thread, however, the government IS denying any human case of bird flu and is blaming the media for causing false rumours....
