NEPALGUNJ: With the death of two persons in Nepalgunj and Dang today, diarrhoea toll in the mid-western region in the past week has topped 14.

Superintendent, Bheri Zonal Hospital, Dr Bimal Dhakal claimed that symptoms seen in the patients point to cholera. Dr M Kidwai, director, Nepalgunj Medical College, claimed that the cholera causing bacterium had been found in four patients.

However, a member of team from Epidemiology and Disease Control Department in Kathmandu that reached Nepalgunj today, Resham Lamichhane, said it was yet to be confirmed whether the disease was cholera or not. He added, ?Blood samples of three patients were sent to national laboratory on Friday and the report will be out on Monday.? Blood samples of 16 patients were sent to the laboratory today. Water from five different places was also sent to the laboratory on Friday.

According to Ram Bahadur Chand, focal person, DPHO Banke, four-year- old Muskan Gajjar succumbed to diarrhoea at his home in Nepaljung

today. With his death, six persons have succumbed to the disease in Nepalgunj.

Four-and-a-half-year-old diarrhoea patient Anita Chaudhari of Rampur VDC in Motipur of Dang was brought to the Rampur health post yesterday. According to Bhuwan Rijal, chief at

the health post, she died on the way to district hospital after

primary treatment in the health post today. With her death,

diarrhoea toll in the district has reached eight.

As many as 96 diarrhoea patients visited health facilities, including government hospitals, today. According to Dr Kidwai, 23 patients visited Nepalgunj Medical College on July 27 and the disease has spread since then.

Medical Superintendent Dr Bimal Dhakal said 90 per cent patients were from Nepalgunj municipality. Patients are being treated on the floor, as few beds are available. A senior officer of EDCD team, Resham Lamichhane, claimed that the disease was spreading fast.

Speaking at a press meet organised by Press Chautari Banke, Lamichhane, Dhakal and Dr Kidwai alleged that the disease spread due to the contaminated water distributed by drinking water office. DPHO Banke Chief Thirjung Shah said medicines to purify water have been distributed among the locals.

Cholera bacteria surfaces in Nepalgunj
RUDRA KHADKA AND GAJENDRA BOHARA

NEPALGUNJ, Aug 2: The District Public Health Office (DPHO) of Banke has confirmed that cholera bacteria has been found in stool samples taken in Nepalgunj.

Tests at Nepalgunj Medical College had shown the presence of vibrio cholerae at the start of an outbreak of diarrhea but the DPHO had refused to accept the report for lack of verification from the National Public Health Laboratory in Kathmandu.

The DPHO, however, said Sunday that one of three samples sent to Kathmandu on July 30 has shown cholera bacteria. It said that reports on 18 other samples sent on July 31 have yet to come in. But doctors allayed fears, saying there was no need to panic as the treatment procedures for diarrhea and cholera are similar.

Meanwhile, the number of diarrhea patients in Nepalgunj has started to decline one week after the outbreak. The DPHO claimed that instant mobilization of health workers and easy availability of medicines have helped in controlling the disease.

Diarrhea outbreak in Dang

DANG: The DPHO has established camps in the hill villages of Dang following the spread of diarrhea.

The diarrhea toll in the district has reached eight with the death of Anita Chaudhary, 6, of Rampur-6, Motipur on Sunday. The DPHO, however, said there have been just four diarrhea deaths in the district and other deaths that media attributed to diarrhea were actually due to other reasons.

?The girl died after eating wild mushroom. The other deaths in Panchakule and other VDCs were also due to toxic mushroom,? claimed DPHO official RD Shah, who has returned after visiting the villages.

The number of patients has been increasing rapidly although teams of health workers from district headquarters have been distributing medicines in the villages. Locals say the number of patients in the hill VDCs of Rampur, Hansipur, Kavre, Saigha, Loharpani and Laxmipur in the east has been increasing.

But the DPHO said there is no exact record of patients as villagers take only the serious patients to the health centers.

Meanwhile, a meeting of district disaster management committee today decided to stay on high alert to curb further spread of the disease.

The meeting held in the presence of regional administrator Dr Dhanraj Ratala concluded that the outbreak of the disease was due to contaminated water.

Chief District Officer Bashudev Dahal informed that special measures would be taken to stop the further spread of the

disease and that the administration had directed

the drinking water corporation to managing drinking water.

??A lab test proved that the water distributed by the corporation is contaminated,?? revealed Dahal.

Meanwhile, the Nepalgunj folks picketed the drinking water corporation today protesting the impure water supply. They also gheraoed the corporation staffers.

Dipakjyoti Shakya, chief of the corporation, ate humble pie, saying the lack of low electricity voltage and generator and leakage in the water distributing system were to blame. ??Water in the tank otherwise is pure,?? explained Shakya.

Nepal: Potential diarrhoea outbreak (MDRNP004) Operations update n?3 - 10 August 2010
Source: International Federation of Red Cross And Red Crescent Societies (IFRC)

Date: 10 Aug 2010

Full_Report (pdf* format - 234.3 Kbytes)
http://www.reliefweb.int/rw/rwb.nsf...gent&shortid=LSGZ-887HXM&file=Full_Report.pdf

Period covered by this Operation Update: 11 June 2010 to 28 July 2010.

Appeal target (current): CHF 738,960 (USD 664,175 or EUR 527,232).

Appeal coverage: To date, the appeal has received cash contributions of 21 per cent, while those in the pipeline brings the coverage to approximately 30 percent.

Funds are urgently needed to support the Nepal Red Cross Society operation in reducing the risk of a diarrhoea outbreak in the vulnerable districts of Nepal.

Appeal history:

- This Emergency Appeal was launched on 13 May 2010 seeking CHF 738,960 (USD 664,175 or EUR 527,232) in cash, kind, or services to support the Nepal Red Cross Society to assist 189,000 beneficiaries for nine months in seven districts of Jajarkot, Rukum, Achham, Dadeldhura, Bajura, Baitadi and Bajhang.

- CHF 200,000 (USD 179,855 or EUR 142,718) was allocated from the International Federation of Red Cross and Red Crescent's Disaster Relief Emergency Fund (DREF) to support this operation.

Summary:

The urgency to reach people affected by diarrhoea in the mid- and far-western remote districts of Nepal has intensified, following the onset of monsoon. Over the past weeks, the number of deaths has risen from 28 to 43, with many of these occurring within the past week. Over 1,700 cases have been officially reported nationwide across 18 districts, and among those worst affected are children and the elderly. Many people in remote areas are not seeking treatment at health posts but they stay at home. Additionally, some communities remain isolated by the recent floods and landslides, many with reduced-access to safe drinking water. Thus the actual number of cases across the country is feared to be higher.

Nine people have died in Dang district over the past two weeks. Likewise, an outbreak of diarrhoea in Banke district claimed six lives, with several hundred people requiring medical treatment in all 17 wards of the district capital Nepalganj. Of three samples tested so far, one has been confirmed with Vibrio Cholera.

The government's Epidemiology and Disease Control Division (EDCD) of the Department of Health Services has issued instructions to all district health offices to be well prepared for further outbreaks and has been leading the response activities in coordination with district committees as well as the water, sanitation and hygiene (WASH) cluster, which includes the Nepal Red Cross Society (NRCS).

In the seven districts covered by this appeal (Achham, Baitadi, Bajhang, Bajura, Dadeldhura, Jajarkot and Rukum),

NRCS has mobilised a WASH coordinator for each district and deployed more than 240 trained volunteers and WASH mobilizers to disseminate knowledge of safe hygiene practices in the community through door to door visits, distribution of information, education, communication (IEC) materials and health education. These activities are being carried in 70 of the most vulnerable village development committees (VDC) in the selected districts.

During the reporting period, the operation has significantly scaled up, with the following key activities undertaken:

- A three-day "WASH in Emergency" training was conducted for 21 participants, including WASH coordinators, district health focal persons from government and governance volunteers from the NRCS district chapters of the seven operation districts. The training was held from 8-10 June in Nepalgunj, Banke.

- Diarrohea prevention kits (containing oral rehydration solution, chlorine solution/water guard and soap), IEC materials (brochure and flex) and volunteer visibility material (T-shirts and caps) were procured and dispatched to all the operational districts.

- A baseline survey has been completed in all the operational districts. For this purpose, NRCS volunteers were oriented together with other local volunteers and mobilized for door to door visits.

- NRCS district chapters in each of the operational districts have provided orientations on the operation's response and prevention strategy to concerned stakeholders (including representatives from district health offices, district disaster relief committees, district drinking water and sanitation offices).

- A WASH coordination committee has been formed in each of the selected VDCs comprising 11-15 members, including representatives of different political parties, district health office, VDC secretary, school teachers, female community health volunteers and VDC WASH coordinators. Each committee is responsible for coordinating other stakeholders and providing guidance to staff and volunteers for the planning and implementation of the proposed activities in the selected VDCs.

- A detailed plan of action for the next five weeks of the operation has been developed (up to end of August), which takes into account the current level of funds available. Similarly, plans have been finalized for monitoring and evaluation and contingency plan for increasing rains.

- The district chapters in the operational districts have been conducting a two-day WASH training for the volunteers to empower them with knowledge of environment, health and hygiene promotion.

In the areas where NRCS has been operating, the numbers diarrhoea cases have reduced and there have been no further deaths reported. However, considering the high possibility of further outbreaks due to the late appearance of the monsoon this year, there is an urgent need to scale up these efforts to reduce the risk of potential outbreaks.

KATHMANDU: The parliamentary team formed to study reasons behind the diarrhoeal outbreak in Nepalgunj and Bardiya said the disease had spread due to contaminated water and poor sanitation. Among 69,580 diarrhoeal patients, seven had died till August 8.

The report mentions that the water supplied to these areas was contaminated due to seepage from drains. The water contained 180 per cent of coliform. Poor waste management and drainage are behind the outbreak, mentioned the report, accusing Banke District Development Committee of apathy.

The team said there was no reason to worry as there was adequate medicine and health personnel in the outbreak zone and the concerned people were involved in assisting patients and fighting the disease. The team has urged that the Health Ministry be directed to fill all vacant positions in the affected area.

In Brief: Cholera outbreak in Nepal under control, says WHO

Photo: WHO

NEPALGUNJ, 20 August 2010 (IRIN) - The World Health Organization (WHO) says a cholera outbreak that struck 1,400 people in mid-western Nepal and resulted in eight deaths - including at least five children - is now under control.

The outbreak in Nepalgunj, a city on the border with India, started around 28 July, said Nirmal Kandel, epidemiologist and national professional officer with WHO. "After about five days [from 28 July], the cases started to decline. It was cholera. It was controlled by 13-14 August. Diarrhoea is endemic, and it's now at the endemic level. I cannot say there are no cases, but there's no outbreak. It's at the normal routine level."

Nepal is nearing the end of its monsoon season, during which time diarrhoea-related illnesses normally spike because of contaminated water sources.

"The problem you'll find now is gastrointestinal - diarrhoea, dysentery - because of the rainy season," said Ravi Kafle, surveillance medical officer for WHO in Nepalgunj. "The number of cases [from the cholera outbreak] has decreased by a great extent and there have been no more casualties."
