Hello,
Were these deaths just reported today due to lack of prior reporting from previous weeks or are we getting one death per day.
What do you think is happening in Michigan?
It's hard to tell. Michigan seems to be updating their data almost daily, but I doubt these new deaths all occurred over the past few days. Some might be deaths from months ago that have just recently been confirmed. I've seen that in some other states recently.

Week 5: Influenza continues at low, sporadic levels; RSV activity is increasing

During January 31-February 6, 2010, 9842 cases of flu-like illness and confirmed and probable cases of seasonal and novel influenza were reported in Michigan. 9 hospitalizations and 9 deaths associated with influenza were reported during this time; all 9 deaths were from October to December 2009 and were
identified by additional surveillance efforts.

Although respiratory complaints have increased steadily over the past few weeks, they are comparable to what was seen at this time last year. In the past week, there were six constitutional alerts in the C(4), N(1), and SW (1) Influenza Surveillance Regions, and three respiratory alerts in the C(2) and N(1) Regions.

During the week ending February 13, 2010, the proportion of visits due to influenza-like illness (ILI) remained low levels but slightly increased to 1.3% overall.

Emergency department visits from constitutional complaints remained steady near the previous week’s levels, while respiratory complaints again increased slightly. Both constitutional and respiratory complaints are comparable to what was seen at this time last year. In the past week, there were four constitutional alerts in the SW (2) and N(2) Influenza Surveillance Regions and four respiratory alerts in the SE(1), C(1) and N(2) Influenza Surveillance Regions.

14 sentinel labs reported for the week ending February 20, 2010. 2 labs reported sporadic influenza A activity (C). 1 lab reported sporadic influenza B activity (SW). 10 labs reported low or increasing RSV positives (SE, SW, C, N), 2 labs had moderately elevated RSV positives (SE, SW), and 1 lab reported highly elevated numbers of RSV positives (C).

Michigan Wild Bird Surveillance (USDA, as of March 4): For the 2009 testing season (April 1, 2009- March 31, 2010), HPAI subtype H5N1 has not been recovered from any of the 111 Michigan samples tested to date, including 58 live wild birds, 39 hunter-killed birds and 14 morbidity/mortality specimens. International, Disease Severity (Eurosurveillance edition 2010; 15(9) [abstract], March 4): Infection with the recently emerged pandemic influenza A (H1N1) 2009 virus causes mild disease in the vast majority of cases, but sporadically also very severe disease. A specific mutation in the viral haemagglutinin (D222G) was found with considerable frequency in fatal and severe cases in Norway, but was virtually absent among clinically mild cases. This difference was statistically significant and our data are consistent with a possible causal relationship between this mutation and the clinical outcome. The 2009 influenza A (H1N1) pandemic has been characterised by mild and self-limiting disease in the overwhelming majority of cases. However, severe and fatal cases, many of them with primary viral pneumonia, have been occurring in age groups where such clinical outcomes are very rarely seen in seasonal influenza. It is important to better understand what viral and host-related factors determine this dichotomy. The unusual death of chickens were proved to have been caused by highly pathogenic avian Influenza (HPAI) after laboratory tests conducted in Yangon and Mandalay, said the New Light of Myanmar. The findings have been further reported to Food and Agriculture Organization (FAO) and World Animal Health Organization (OIE), the report said. According to the report, the authorities have culled chickens and destroyed eggs bred in the farm as its control measures. It was the third bird flu H5N1 case since the disease re-struck Myanmar in February, nearly two years after the country was free from the disease. According to the Livestock Breeding and Veterinary Department (LBVD), in the first case taking place in Mayangong township, unusual deaths of some chickens were found in a farm where 2,500 chickens were bred and the suspected deaths were proved to have been caused by the HPAI after experiment.

In the second case in Mingaladon township, two domestically- bred chickens died of virulent avian influenza in a poultry farm and were later confirmed with carrying the virus after laboratory test. The authorities took control measures by culling hundreds of similar chickens, some ducks and eggs, banning sale of chickens and eggs in six bazaars, 7 kilometers around the poultry farm where bird flu recurred, to prevent the spread of the disease.
People have also been being urged to step up bio-security measures, change of livestock breeding system, avoidance of illegal import, transport and trading of chickens and its products, and prompt report of suspected bird flu case. International, Vaccination (New York Times [edited], March 10): An unusual study done in 49 remote Hutterite farming colonies in western Canada has provided the surest proof yet that giving flu shots to schoolchildren protects a whole community from the disease.
Although previous studies have demonstrated what scientists call ?herd immunity,? none have been so incontrovertible, because they were done in less isolated places with more sources of flu passing through. Also, only one other study, done 42 years ago, immunized over 80 percent of a community?s children, as this one did. Success repeated in many separate communities with very high vaccination rates implies that the shots themselves ? rather than luck, viral mutations, hand-washing or any other factor ? were the crucial protective element.

The study, done by scientists from several Canadian universities and St. Jude?s Children?s Research Hospital in Tennessee, was paid for by the governments of Canada and the United States. It was published online Tuesday by The Journal of the American Medical Association. ?This is quite a definitive study, and it took a Herculean effort,? said Dr. Carolyn B. Bridges, an expert in influenza epidemiology at the Centers for Disease Control and Prevention. ?My hat?s off to them.? Dr. Anthony S. Fauci, director of the National Institute for Allergy and Infectious Diseases, which supported the project, called it ?a really nice study? and added that, even though it was done with seasonal flu shots in the 2008-9 winter, its results validated the American government?s decision to vaccinate children first during the recent swine flu pandemic...

International, Human (WHO, March 12): The Ministry of Health of Egypt has announced two new cases of human H5N1 avian influenza infection. The first case is a 20 year-old pregnant female from El Khanka district, Qaliobia Governorate. She was hospitalized on 5 March where she received oseltamivir treatment and died on 9 March. The second case is a 1 and a half year-old male from Elhamool district, Kfr Elsheikh Governorate. He was hospitalized on 2 March, where he received oseltamivir treatment. He is in a stable condition. Investigations into the source of infection indicated that the two cases had exposure to sick and dead poultry. The cases were confirmed by the Egyptian Central Public Health Laboratories, a National Influenza Center of the WHO Global Influenza Surveillance Network (GISN). Egyptian authorities have also reported the death of a previously announced case; the 53 year-old male from Shobra Elkhima district, Qaliobia Governorate who developed symptoms on 27 February. Of the 106 laboratory confirmed cases of Avian influenza A(H5N1) reported in Egypt, 32 have been fatal.

International, Human (WHO, March 16): The Ministry of Health has reported a new confirmed case of human infection with the H5N1 avian influenza virus. This case was confirmed at the National Institute of Hygiene and Epidemiology (NIHE). The case is a 25 year old woman residing in Soc Son District, Hanoi. She developed symptoms on 5 March 2010 and was hospitalized on 7 March 2010 at North Thang Long Hospital. On 10 March 2010, her condition had worsened so she was transferred to Bach Mai Hospital where she is being provided with ventilation. An epidemiological investigation showed that the patient had exposure to sick and dead poultry. Of the 116 cases confirmed to date in Viet Nam, 58 have been fatal.

International, Poultry (Voice of Vietnam website, March 15): The Bac Ninh provincial People's Committee on March 14 announced the occurrence of bird flu in Nam Son commune, Bac Ninh city [Vietnam]. Previously, the poultry of a farmer family in Da Cau hamlet of this commune died in mass numbers. The provincial animal health department said farmer Dam Thi Chinh raised 400 heads of fowl, which had been vaccinated for four months but died massively on the night of March 11. They first showed signs sickness on March 10. The veterinary agency inspected the family's farm on March 12 and concluded that Ms Chinh's poultry had signs of being infected with avian flu. It later destroyed all the birds and sent two samples of the disease to a central veterinary diagnosis centre for further testing. The agency also had the entire Da Cau hamlet disinfected with lime and cloramin B. The Bac Ninh Animal Health Department has set up two temporary checkpoints at the hamlet, suspended all activities involving poultry in markets within Nam Son commune, vaccinated all the fowl in the commune and is closely monitoring the slaughter of poultry in an effort to prevent the epidemic from spreading.

International, Research (Reuters Health, March 19): In a study of Japanese schoolchildren, vitamin D supplements taken during the winter and early spring helped prevent seasonal flu and asthma attacks. The idea for the study, study chief Dr. Mitsuyoshi Urashima, told Reuters Health, came from an earlier study looking at whether vitamin D could help prevent the bone-thinning disease osteoporosis. The researchers in that study noticed that people taking vitamin D were three times less likely to report cold and flu symptoms...The vitamin D group was 58 percent less likely to catch influenza A, the researchers report in the American Journal of Clinical Nutrition. Vitamin D also appeared to suppress asthma attacks in children with a history of asthma. Two children taking vitamin D had asthma attacks during the study, compared to 12 children taking placebo. Urashima admitted to being a bit surprised by this finding and hopes to confirm it in a randomized trial targeting children with asthma...

Based on the current study, giving kids vitamin D supplements during the winter may help reduce cases of influenza A, the researchers conclude. Urashima suggests that children could take 1,200 IU per day starting in September to prevent flu and asthma attacks during the flu season, but best for parents to check with their pediatrician first..."

During March 21-27, 2010, 6406 cases of flu-like illness and confirmed and probable cases of seasonal and novel influenza were reported in Michigan. 2128 hospitalizations and 78 deaths associated with influenza have been reported since September 1, 2009.

National, Surveillance (Atlanta Journal Constitution [edited], March 29): Georgia is seeing a spike in swine flu [influenza pandemic (H1N1) virus infection] hospitalizations, having the most in the country for 3 weeks in a row, federal health officials said Monday [29 Mar 2010]. It is too early to say whether Georgia or the country are seeing another wave of the illness, which had diminished across the nation for several months, the officials said. But the number of people being hospitalized in Georgia equals the number of pandemic (H1N1) hospitalizations when the disease peaked here last September [2009]. Officials stressed that no other state is seeing such an increase. In the 1st 2 weeks of March [2010], Georgia had 80 and 72 hospitalizations respectively, according to the state Department of Community Health. In contrast, the 1st week of February 2010 saw 17 hospitalizations. The Atlanta-based Centers for Disease Control and Prevention [CDC] was so concerned about the Georgia situation that it sent a team of researchers to investigate. CDC officials held a news conference
Monday [29 Mar 2010] that focused on the Georgia problem. Georgia's increase is reflected to some degree in other southeastern states but not in other parts of the country, she said. She described the illness as "circulating intensely" here...

National, Antiviral Resistance (NIH press release [edited], March 26): Two people with compromised immune systems who became ill with 2009 H1N1 influenza developed drug-resistant strains of virus after less than two weeks on therapy, report doctors from the National Institute of Allergy and Infectious Diseases (NIAID), part of the National Institutes of Health. Doctors who treat prolonged influenza infection should be aware that even a short course of antiviral treatment may lead to drug-resistant virus, say the authors, and clinicians should consider this possibility as they develop initial treatment strategies for their patients who have impaired immune function...

The people in the current case report had immune limitations due to blood stem cell transplants that occurred several years previously. Both recovered from their influenza infections...

Both people in the current report received oseltamivir for extended periods but they continued to shed virus in their nasal secretions throughout treatment. When one patient's condition worsened despite 24 days of oseltamivir treatment, doctors administered peramivir for 10 days. The drug did not reduce viral shedding and the patient remained ill, demonstrating what the authors described as clinically significant resistance to peramivir. Next, doctors administered the only other available flu drug, zanamivir, for 10 days. The person then fully recovered...

During March 28 - April 3, 2010, 5686 cases of flu-like illness and confirmed and probable cases of seasonal and novel influenza were reported in Michigan. 2129 hospitalizations and 78 deaths associated with influenza have been reported since September 1, 2009.

(ILI) slightly increased to 1.0% overall. However, activity continued to remain below baseline levels (1.3%); 59 patient visits due to ILI were reported out of 6,107 office visits. Twenty-nine sentinel sites provided data for this report. Activity increased in one surveillance region: Central (1.9%); remained the same in two regions: Southwest (0.2%) and Southeast (0.5%) and decreased in the remaining surveillance region: North (0.0%).

Week 15:
MDCH BOL and sentinel laboratories reported no influenza positive tests for the past week.

During April 11-17, 2010, 2446 cases of flu-like illness and confirmed and probable cases of seasonal and novel influenza were reported in Michigan. 2136 hospitalizations and 78 deaths associated with influenza have been reported since September 1, 2009.

Emergency department visits from constitutional and respiratory complaints decreased slightly from the levels seen during the previous week.

Sentinel provider data for the week of April 11-17 was unavailable at the time of publication; it will be included in the next edition.

Week 16: Sporadic
During April 18-24, MDCH Bureau of Laboratories identified no influenza isolates.

During April 18-24, 2010, 3867 cases of flu-like illness and confirmed and probable cases of seasonal and novel influenza were reported in Michigan. 2137 hospitalizations and 78 deaths associated with influenza have been reported since September 1, 2009.

Emergency department visits from constitutional and respiratory complaints decreased slightly from the levels seen the previous week. In the past week, there was one constitutional alert in the SW Influenza Surveillance Region, and two respiratory alerts in the C(1) and N(1) Influenza Surveillance Regions.

During the week ending April 24, 2010, the proportion of visits due to influenza-like illness (ILI) slightly increased to 0.5% overall; activity continued to remain below baseline levels (1.3%). 33 patient visits due to ILI were reported out of 7,135 office visits. 24 sentinel sites provided data for this report. Activity slightly increased in one surveillance region: Southwest (0.4%); remained the same in two regions: Central (0.7%) and North (0.0%); and decreased in the Southeast (0.0%) region.

ILI: 0.1% (MDSS data for the week ending May 8th showed that aggregate influenza, individual influenza, and 2009 novel influenza reports remained steady near previous week?s levels.)

Activity increased in one surveillance region: Southwest (0.2%); decreased in two surveillance regions: Central (0.2%) and North (0.0%); and remained the same in the Southeast surveillance region (0.1%). Please note that these rates may change as additional reports are received. As part of pandemic influenza surveillance, CDC and MDCH highly encourage year-round participation from all sentinel providers.

MDSS data for the week ending May 29th showed that aggregate influenza case reports were half that of the previous week; this decrease is most likely related to delays in reporting due to the Memorial Day holiday.

Emergency department visits from constitutional complaints slightly increased, and respiratory complaints slightly decreased, from the previous week’s levels.

During the week ending May 29, 2010, the proportion of visits due to influenza-like illness (ILI) slightly decreased to 0.4% overall.

Six sentinel laboratories reported for the week ending June 12, 2010. All laboratories (SE, SW, C, N) reported no influenza A or B positive test results, with very few specimens being tested.

Week 24:

MDSS data for the week ending June 19th showed that aggregate influenza case reports continued decreasing to levels normally seen after schools dismiss for summer vacation. Individual reports, including influenza and 2009 novel influenza cases, remained near the previous week’s reported levels of little to no activity. Aggregate influenza cases are similar to levels seen during the same reporting period in 2009, while individual influenza reports are drastically lower.The decrease in individual reports is attributable to the rapid increase of cases in 2009 due to the H1N1 pandemic; current levels are consistent with expected levels based on surveillance data from previous non-pandemic influenza seasons.

Emergency Department Surveillance: Emergency department visits from constitutional complaints were comparable to the previous week’s levels, while respiratory complaints decreased slightly. Both constitutional and respiratory complaints are at similar levels compared to the same reporting period last year. In the past week, there were four constitutional alerts in the SE(1), SW(1), C(1), and N(1) Influenza Surveillance Regions. There were eight respiratory alerts in the C(6) and N(2) Influenza Surveillance Regions.

Sentinel Provider Surveillance (as of June 24): During the week ending June 19, 2010, the proportion of visits due to influenza-like illness (ILI) slightly increased to 0.6% overall. Thirty-nine patient visits due to ILI were reported out of 6,817 office visits. Twenty-three sentinel sites provided data for this report, limiting the validity of the slight increase. Activity increased in one surveillance region: Central (1.2%) and no ILI activity was reported in the remaining three regions: Southeast, Southwest and North. Please note that these rates may change as additional reports are received.

During the week ending July 10, 2010, the proportion of visits due to influenza-like illness (ILI) slightly increased to 0.5% overall.
