The Ministry of Health confirmed the first case of monkeypox in Mexico —mpox subclade Ib— in a 50-year-old man, originally from Germany, who traveled through Mexico City, Puebla and Guerrero.
Subclade Ib is a genetic classification of the mpox virus obtained through sequencing studies, which allows the virus to be identified and differentiated within the same lineage for epidemiological surveillance purposes, without this designation in itself indicating the severity of the clinical picture.
German patient traveled through Mexico City, Puebla and Guerrero
According to the Undersecretariat of Health Policies and Population Welfare and the General Directorate of Epidemiology, the patient is a resident of Berlin, Germany, and works as a social worker.
Epidemiological investigation indicates that the onset of symptoms occurred on December 12, 2025, when the patient presented with a fever quantified at 38.6 degrees Celsius, headache, rhinorrhea, and diaphoresis.
On December 16, an exanthem characterized by papules and vesicles was added, with a total of 35 lesions, with a cephalocaudal distribution.
The injuries affected the head, face, neck, upper and lower extremities , genitals, abdomen, back, and palms. In addition, the patient presented with asthenia, myalgia, tenesmus, and anorectal pain.
By tracing the critical path of the case, the health authority identified that the patient traveled through Mexico City, Puebla and Guerrero , and that at the time of the report he was in the state of Guanajuato.
The agency specified that the case was asymptomatic at the time of the report's closing.
The patient stated that he traveled from Germany to the United States on December 8, 2025, for a stay of two days, and then arrived in Mexico with the following flight itinerary:
Berlin-Frankfurt (flight UA9214).
Frankfurt-Denver (flight UA181).
Denver-Mexico City (flight UA783).

Mexico confirms first case of lethal variant of Mpox subclade Ib in foreign patient
The Ministry of Health detected the first infection of the clade Ib variant of Mpox in a German citizen who entered the national territory.
Demis Salinas January 22, 2026
The Ministry of Health (SSA) , through the Institute of Epidemiological Diagnosis and Reference (InDRE) , confirmed on January 21, 2026 the detection of the first case of Mpox (simian pox) belonging to subclade Ib in Mexico .
This is a German national who presented symptoms after arriving in the country. This variant has generated international concern due to its greater transmissibility and severity compared to clade II, which previously circulated in the region.
The patient was identified through active epidemiological surveillance protocols after presenting with fever, fatigue, and characteristic skin lesions. Health authorities immediately activated isolation protocols and contact tracing to contain any potential local outbreak.
The National Committee for Epidemiological Surveillance (CONAVE) issued an urgent notice to all medical units in the 32 federal entities to strengthen the timely detection of suspected cases.
Subclade Ib is the variant that prompted the WHO to declare a Public Health Emergency of International Concern in late 2024, due to its rapid spread in Central Africa.
Until now, Mexico had only registered cases of clade II, so the arrival of this new strain represents an additional challenge for the public health system, especially at points of entry such as international airports and tourist areas of Baja California Sur , Quintana Roo and Mexico City .
Epidemiological surveillance and response to the new clade
Within the context of prevention, COFEPRIS and PAHO have collaborated to ensure that diagnostic reagents are specific to differentiate subclades.
For context, subclade Ib is associated with more frequent person-to-person transmission, including close non-sexual contact.
WHO statistics indicate that this variant may have a slightly higher fatality rate in vulnerable populations, so institutions such as UNAM and IPN urge the population to maintain basic hygiene measures and avoid direct contact with people who have unexplained skin rashes.
The Ministry of Health emphasized that there is no reason to panic, but there is a need for close monitoring. State public health laboratories have been instructed to send any suspected cases to the InDRE ( National Institute of Epidemiological Diagnosis and Reference) for genomic sequencing.
For now, the German patient is under strict medical observation and is reported to be stable, while the sanitary cordon is being completed in the places where he was present since his arrival in the country.
