If infections were coming directly from camels, more disease would be expected in workers

...Now scientists have grown the virus from nasal swabs of two dromedaries in Saudi Arabia, genetically sequenced it and found it was an "indistinguishable" match to MERS coronavirus found in humans ? an indication that the virus in the animals is capable of infecting humans.

"Together with data indicating widespread dromedary infection in the Kingdom of Saudi Arabia, these findings support the plausibility of a role for dromedaries in human infection," Thomas Briese, associate director of the Center for Infection and Immunity at Columbia University in New York and his co-authors concluded in Tuesday?s online issue of the journal mBio, published by the American Society for Microbiology.

The finding strengthens the argument that the camels are a reservoir for the virus, Briese said.

"We have evidence going back as far as the early 1990s that this virus has been in camels. Human cases didn't start till 2012. So one very important question is what happened that caused this virus to cause severe disease in humans, noticeable disease in humans for the first time in 2012?" said Dr. Allison McGeer, an infectious disease specialist at Toronto?s Mount Sinai Hospital who has helped investigate the new virus in Saudi Arabia as part of a World Health Organization-led team.

Veterinarian Dr. Ehab Al-Shabori in Riyadh said everything is normal among camels.

"If it were risky or there were any problems, we would have received an alert or brochures," Al-Shabori said in Arabic to Reuters. "We would have witnessed cases infected with coronavirus. But there is nothing."

If the infections were coming directly from the camels, you would expect disease in the people who work more closely with camels, but that?s not what scientists are seeing, McGeer said.

"So you can speculate about a number of different explanations for why it's true, but we clearly need to understand what's happening and how it's happening."...

I don't understand the logic in that expectation. People who who work with camels also work with their diseases and likely have developed immunity. They are also likely healthy if they are doing that kind of work, while the initial patients seemed to all have chronic illness, some necessitating treatment with immune suppressing medicine.

What happened could be a change in human behavior. In 2009 an article was in the Saudi press about modern research being done on cancer and autoimmune disease treatment with camel urine and milk. This research was based on traditional medicine and while it may have great merit and potential, when desperate patients read things like that, they might be trying these treatments themselves prior to research being completed.

These patients could be urban dwellers with no specific immunity to zoonotic camel diseases unlike traditional practitioners. They have poor general immunity so they get sick and take the virus to a modern invasive medicine venue where the virus gets into equipment and HCW's who also have no immunity to zoonotic camel diseases.

I suspect camel urine is the culprit more so than camel milk but I haven't read about any investigation along those lines. Urine is NOT sterile, contrary to popular belief. It is suspected in the spread of another zoonotic:

Cross referencing to this article, which shows why breaking the camel/human interface in the region will be nigh on impossible. The most effective course of action will be to develop a camel vaccine as quickly as possible.

Vibrant, I'm wondering about unintended consequences to attempting to eradicate a virus in the species to which it is harmless. Could the attempt influence the evolution of characteristics more harmful to camels or worse - to humans? Even if the attempt to eradicate this virus is successful, will the ecological niche it occupies be filled with more dangerous replacement?

That is true, but there is perhaps just a small niche of interaction to destroy - the new behavior that put people at risk in very recent years. Maybe all that needs to be done is testing and pasteurization.
