[Skinner] With us today is Sylvia Mathews-Burwell (HHS) and Tom Frieden.

[Burwell] Want to recognize the service of both patients in Dallas and all HCWs. Announced a number of steps yesterday: call with 5600 clinicians to discuss preparedness and review Emory/Nebraska lessons learned. Also announced the CDC would provide a "go" team. As you're aware, this morning, the second HCW reported with a low-grade fever and was isolated. She traveled by air the evening of the 13th - we're notifying all passengers. Individuals determined to be at risk will be monitored.

One of the patients at Texas Pres will be transported to Emory later today. We are also talking to other specialty hospitals about whether they are prepared to accept patients.

We do know how to stop this - we do know the protocols work. However, we know there are opportunities to learn more. We need to stop this epidemic at this source. Domestic preparedness is a top priority, but we need to stop it in Africa.

[Frieden] Ebola can create another generation of spread in 8-10 days. We're open about what we know, when we know it. This is the second HCW from exposure to the index patient, Mr. Duncan, admitted 9/28. Stopping the spread will require many partners.

Second patient is ill, but clinically stable. She was rapidly isolated, tested, and about 12:00 or 1:00 last night, the presumptive test was positive and will be confirmed by the CDC lab shortly. She will be tranferred to Emory. We are working closely with DPHS to create a safe environment. We've continued with our investigation and interventions to increase safety. Investigations increasingly suggest the first several days before the patient was diagnosed are the highest-risk period: 28th, 29th, and 30th. These HCWs both worked on those days and had extensive contact during vomiting/diarrhea. We're continuing to identify that level of intensive contact. I note that those days are prior to index patient diagnosis and prior to the CDC being on the ground.

There were 48 contacts prior to Duncan's isolation - none have developed symptoms. 1 contact of Phan has not developed symptoms. This most recent contact had 3 contacts. There are approximately 50 other workers [lost feed]

Q. Can you clarify what these two women were doing? What roles did they have in the hospital? Any indication of path of infection?

A. It has been stated publicly that the first patient is a nurse. Second patient had extensive contact when patient was having substantial amounts of both vomiting and diarrhea. In those first several days, a variety of forms of PPE were used. There are several ways to use it safely - it's critical that it be done consistently and correctly. That's one area of active investigation.

Q. How did this nurse get on an aircraft? Why was she not being monitored or quarantined?

A. The patient traveled to Ohio before it was known that the first HCW was ill. At that point, that patient and the rest of the HC team were undergoing self monitoring. The second HCW reported no symptoms or fever. However, because she was in a group of individuals known to have exposure, she should not have traveled on a commercial airline. The CDC guidance outlines the need for controlled movement. That can include a charter plane or car, but not public transport. We'll ensure that no other individual being monitored will not undertake commercial travel.

Q. National Nurses United talked to nurses at the hospital. They don't think any protocols are in place there, or really anywhere. Response?

A. We are working closely with the hospital. We have staff there around the clock. There are intensive efforts to train, retrain staff. The single most important way to get consistency is a site manager. 24/7, there will be a site manager who will monitor PPE donning.

[Skinner] We have two nurses from Emory who have gone to Dallas. They are doing peer-to-peer training. We hear the concerns and want to make sure we are doing more communications. It's so important that front-line workers doing intake for everyone to know at the point at which they see a fever, ask a travel history. We need to use every means to communicate some of the education issues that have been raised.

Q. Sounds like the PPE worn by these two HCWs was not the right type. There have been reports their necks were exposed. What, specifically, were they wearing?

A. When we reviewed the records before he was diagnosed, we see a lot of variability. When our team arrived, we noted some HCWs were putting on three or four layers in the belief it would be more protective. Other things were done, such as taping protective gear. We understand the fear and anxiety - these are good, dedicated people, but by putting on more layers, it becomes much harder to put them on and take them off. The risk of contamination is much higher during removal. That's true for several different areas of the body.

I know the issue of PPE has gotten a lot of focus. There are several right ways to do it. Familiarity results in better control. We're looking at what are the ways to provide the maximum possible support and minimizing risk of contaminatinon?

Q. Which patient is going to be transferred? Just one? Are there any other people who are currently being tested/isolated?

A. The second HCW is being transferred to Emory. We continue to assess anyone who has symptoms. We encourage them to come forward at the slightest concern. We have not identified anyone else who merits blood tests. This can change from minute to minute. [lost feed]

Q. What powers are in place for the CDC to ensure no other HCWs will travel? What's your legal authority?

A. We work this out with state and local health authorities.

Q. Was there skin exposure with the PPE being used for the two infected HCWs? Did the hospital not tell any of the HCWs to limit their travel before today?

A. Some of the forms of PPE did allow exposure of the skin. We have discussed in detail - there are right ways to do it, and some do allow for exposure of the skin. The problems we've identified involved using PPE in unfamiliar ways.

Regarding travel: when the second HCW left, the first HCW had not been diagnosed. All were under self monitoring - not being actively monitored. It was now known there had been exposures during the care of the patient. Patient #2 didn't have a fever, nausea or vomiting on the plane. That suggests the risk to any on the plane was extremely low. We are putting in extra margins of safety and reaching out to passengers and crew.

Q.In a video message to West Africa, Pres. Obama told people they could not get Ebola from bus travel. But the CDC has told people to avoid public transportation. Did the CDC vet the message before it was posted on embassy web sites? Is it true there's no risk from travel on a bus?

A. Yes, we vetted it, and yes, we think it's accurate. No worry about sitting next to someone if you are healthy. If you're sick, you should not get on a bus. We are contacting people on the plane for extra margin of safety.

Q. What sort of treatments will Vinson be receiving? How will it be possible to stop people who treated Mr. Duncan from getting on public transportation.

A. The treatment is per their treating physicians - need to ask hospital. Controlled movement - we worked it out with public health authorities. We require anyone who may have been exposed to travel by controlled movement only. The HCW #2 should not have been allowed to travel by plane by virtue of the fact that she was in an exposed group. She didn't meet the temperature threshold, but it was 99.5. She should not have been on that plane. However, that doesn't change the level of risk to those around her. Level of risk was extremely low.

Ebola is hard to fight. We know how to fight it, we know how to beat it. The situation changes every day. We're always going to share what we know. Extra margin of safety. Second case is very concerning - thoughts with family. Investigation is identifying additional HCWs who will be monitored. We are planning for the possibility of additional cases in the coming days.

HHS: drugs or therapeutics are being reviewed by the FDA. Assistant secretary of preparedness is doing this. Fighting this in West Africa - efforts across government. Great work that DoD is doing as well.
