Madagascar reports 4 confirmed cases of plague

October 14, 2026

According to a statement released by the Madagascar Ministry of Public Health on the 13th, the country reported 4 confirmed cases of plague and 5 suspected cases.

The statement indicated that four confirmed cases were found in the towns of Akazoube, Anduzourube, and Ambohimanarizua, north of the capital Antananarivo, while five suspected cases were found in the South Midongi district in the southeastern part of the country. Currently, 200 close contacts are under medical monitoring and have received preventative treatment.

The statement said that Madagascar has entered its peak plague season since the end of July this year. The health authorities have implemented several control measures, including proactively screening suspected cases, tracing close contacts, and spraying insecticides in and around affected areas to eliminate fleas that transmit the plague.

HEALTH – Cases of plague in Analamanga

Miangaly Ralitera - Published on October 13, 2026

The plague, a recurring phenomenon, began at the end of July. Attention is particularly focused on Analamanga, which is among the regions where cases have been formally confirmed.

According to the epidemiological report compiled by the Ministry of Public Health, four cases of plague have been confirmed nationwide to date. These cases are spread across three districts: Ankazobe and Anjozorobe in the Analamanga region, and Ambohimahasoa in the Haute Matsiatra region. No deaths have been recorded in these areas, according to a source at the Ministry. These cases highlight the ongoing vulnerability of the Analamanga region to rodents and flea vectors during the cool, wet season.

Alongside these confirmed cases, the vigilance of the epidemiological surveillance network has enabled the detection of other potential outbreaks. inOn October 10, 2026, the Directorate of Communicable Disease Surveillance received an alert from the Midongy Atsimo district. Investigations conducted in the field by rapid response teams revealed five (5) additional suspected cases, samples of which are currently being analyzed at the reference laboratory.

To contain any risk of spread around the identified clusters in Analamanga and other regions, the Ministry of Public Health implemented an emergency protocol. Those infected were immediately taken into care and placed under medical treatment. To break the chain of transmission, preventive antibiotic treatment was administered to nearly 200 people who had been in direct contact with the infected individuals, all of whom are under strict medical supervision.

On the ground, the vector control response is organized around the systematic disinfestation of homes and their surroundings to eliminate fleas, followed by the active search for new cases. In parallel, community health workers are intensifying sanitation efforts and vector control against rat proliferation.

HEALTH - Probable deaths from pneumonic plague in Midongy

Miangaly Ralitera - Published on October 12, 2026

Three deaths likely linked to pneumonic plague have been reported in Andranolalina. Other suspected cases are being monitored in Midongy Atsimo and Iakora.

Three deaths associated with probable cases of pneumonic plague have been reported in the rural commune of Andranolalina, in the Midongy Atsimo district. Tests are still needed to confirm the cause of death, while health investigations are ongoing.

The Ministry of Public Health reported yesterday suspected cases of plague in the South. Three people have died in the rural commune of Andranolalina, and two other patients are currently receiving treatment at a basic health center in the Midongy Atsimo district, according to the Director General of Preventive Medicine at the Ministry, Dr. Désiré Rakotoarimino.

The first death recorded in a hospital setting dates back to October 10. "This death prompted us to conduct investigations, which led to the discovery of two other cases of community death," a local source indicated. Rapid diagnostic tests reportedly came back positive. However, the results of the sample analyses are still pending to confirm or rule out the suspicion of plague. According to the ministry, the three deceased individuals belonged to the same family: a couple and their grandchild.

The response faces access difficulties. In Begogo, in the Iakora area, three more suspected cases of bubonic plague were discovered by health teams during the vaccination campaign carried out last week, according to a local source.

"These are plague hotspots," health sources say of the affected areas. In 2016, dozens of deaths were recorded there, according to statements from local authorities at the time.

The remoteness of health centers complicates treatment. Residents of some villages have to travel long distances to consult a doctor, which can delay access to care from the onset of the first symptoms.

Active case finding continues, alongside response measures. According to information gathered, residents of the affected villages are being isolated to limit the risk of further spread.

In Iakora, health teams are also facing shortages of certain supplies, including rapid diagnostic tests. The Ministry of Public Health has announced the deployment of teams to reinforce local staff. The shipment of medications, rapid diagnostic tests, and a mobile laboratory has also been announced.

HEALTH – Three members of the same family have died from pneumonic plague in Andranolalina Midongy Atsimo; nearly 200 people are under treatment

The grandfather and grandmother died at their home in the commune of Andranolalina, and the grandson died at the hospital in Midongy Atsimo. Three members of the same family who, according to initial reports, died from pneumonic plague. As soon as the cases were reported, health authorities mobilized teams in the commune to carry out response activities, including disinfection and pest control. Nearly 200 people who had been in contact with suspected or confirmed cases are also receiving prophylactic treatment.

According to information provided by a local health official, three patients arrived at the Midongy Atsimo hospital on October 9. One of them, a man in his twenties, died, while the other two remain hospitalized and are continuing their treatment. The other two deaths occurred within the same family in the community toward the end of September. In total, five cases of pneumonic plague have been recorded in this commune.

Community mobilization, however, still faces a challenge. Despite awareness campaigns, some people delay seeking medical attention and only go to the hospital when symptoms become severe, our source laments. To limit the spread of the disease, a team from the local Joint Planning Body visited the area to raise awareness among residents.

Around fifteen homes were also sprayed with disinfectant. Contact tracing and outreach efforts are continuing. Health authorities also indicate that surveillance has been stepped up, especially since Midongy Atsimo is one of the country's plague hotspots, particularly between September and December.

Vigilance is also being maintained in Iakora, where a suspected case of bubonic plague has been reported. As in Midongy Atsimo, rapid diagnostic tests performed on patients in Iakora have come back positive, in addition to samples sent to the Pasteur Institute of Madagascar. Laboratory results are awaited to confirm the diagnosis. In the meantime, response activities are already underway on the ground to limit the risk of further spread.
