So very strange- none of the symptoms of EVD are so clear cut, so why do they think it is? That said, very sudden death and illness in two related persons is highly concerning....

Yes, I was thinking along these lines- esophageal varicise caused by chronic alcoholism could lead to acute bloody vomiting, diarrhea, rapidly followed by exsanguination and death. The patient might not have been brought to the attention of the local health care system before he suddenly collapsed.

However, I also read reports that he had a fever, and with the companion having symptoms it's just not clear. But that's more likely than some type of exposure to Ebola when the poor man had not even been to Africa.

Kostovska said on Friday initial tests suggested only a "small probability" the man died of Ebola, but that blood and tissue samples had been sent to Frankfurt for tests. The results are expected on Saturday.

So what we know of this case is:

*Has history of travel to Africa 6 years ago (we can only infer that this means he has a potentially unknown epidemiological link to West Africa)

*Arrived from London Thursday October 2nd

*Died Thursday October 9th

*Was very sick in his hotel room for 3 days before he died, meaning onset of symptoms was Tuesday October 7th at the latest

*Authorities are taking this case seriously by continuing to hold 35 people in quarantine

In discussion of my post above, what initial test would indicate "a small probability of Ebola?" Maybe there is something lost in translation in that statement. There are conflicting reports that he did and didn't have a fever and that he was and wasn't drinking heavily. If he had spent the last 3 days in the hotel room drinking heavily as an end stage alcoholic, I would think that information would be out there. Spending 3 dry days in bed unable to eat or dress yourself isn't entirely incompatible with end stage alcoholism but if he had a fever as well then alcohol becomes less likely. Without a complete travel history it is difficult to know what other hemorrhagic illnesses he could have been exposed to.

this region is on the illigal migration route into Europe, and it is feasable there could have been a contact somewhere along the way.

Officials said the man?s colleague said the deceased had not travelled to any country where Ebola was a risk, although the Macedonian health ministry suspected he had visited Nigeria. It gave no reason why this was suspected.

I don't like what that adds up to.

We only have the friend's word that he and the dead man had not travelled to any country where Ebola was a risk. When in doubt, test first and ask questions later.

The hospital source also mentioned that the symptoms of a relatively low body temperature of 37.4 was one indication it was not Ebola, but other symptoms were similar to Ebola, like 'vomiting, internal bleeding and a fast death'.

Temperature rises in end stage alcoholism can be lots of origins, spontaneous infection of the ascitic fluid, bacterial peritonitis, spontaneous rupture of abdominal hernias, kidney infections etc etc.

Not saying this poor man had anything like this, just saying it is a possibility.

Perhaps travel history got a bit mixed in the chaos of the emergency situation. Man with him giving patient history was 72 and reported to be very upset,. Perhaps when asked about travel to Africa he had replied Nigeria, but didn't quite get across the " six years ago" bit.

True but only a subset of these (even in combination) cause internal bleeding and take 3 days to kill. It is certainly possible that this is an alcohol related death but the deceased had some kind of link to Africa, traveled to a place that is on an illegal migration route and authorities are concerned enough to hold 35 people in quarantine pending test results.

Man has been named as Colin Jeffrey 58. He was the head of the Union of Foreign Investors in Macedonia, and he has a wife and three kids in Cambridge.

The head of Disease Prevention at the Department of the Ministry of Health in Macedonia, Jovanka Kostovska, revealed on Friday there was no other case reported with symptoms similar to Ebola.

Reference is the Daily Mirror which is normally seen as a rag tabloid in UK. Suddenly it finds itself being praised as the only paper to have stuck with the Ebola situation right through since March, while others were accused of ignoring the situation for too long.. I will fold it nice in the future instead of throwing it in bin.

I didn't see anywhere that it said what kind of internal bleeding. Alcoholics can get esophageal varices, if one ruptures they can get in trouble fast. That would match the vomiting, fast death, internal bleeding description.

That's what I thought, though where the idea came up that he was a chronic alcoholic, I'm not sure. I believe that it's just a rumor. He was a pretty high-functioning person to have that kind of end-stage alcoholism.

Just a question: Since so many suspected cases are now being found negative...is it because people are panicking or real cases are being hidden?

IMHO these days the authorities want to stay on the safe side.
