Nipah virus claims one more life in Kerala; 17 deaths till now

KOZHIKODE: One more person undergoing treatment for Nipah infection died at Kozhikode Medical College Hospital, on Thursday. The deadly virus has so far claimed 17 lives.

The victim Rasin, 25, hailed from Naduvannur in the district. Rasin, who was tested negative for Nipah a few days ago was tested positive for Nipah after second test.
The health department officials are yet to confirm whether Rasin was in the contact list. Rasin was shifted to Medical College from Balussery taluk hospital following severe fever and symptoms of Nipah.

R.L. Saritha, Director, Health Services, told the media that [Raisin] was admitted to the hospital on May 26. He was on the contact list of persons who were suspected to have contracted infection from those who died earlier. Rasin is believed to have got the infection from Ismail when both of them were undergoing treatment at the Balussery taluk hospital.

Ismail was later referred to the MCH, where he died due to Nipah infection on May 20. However, the authorities are yet to figure out the source of infection of Akhil, who died on Wednesday.

Dr. Saritha said that Rasin did not have any contact with the family members of V. Moosa of Chengaroth, near Perambra, who are considered to be the first source of infection. As he contracted it from a second source, the department has now begun activities considering this as a second wave of infection, she pointed out.

More people were getting added to the contact list, which now has 1,407 people in it.

Doctors, nurses of taluk hospital in Kerala asked to go on leave after Nipah virus scare

Several staffers, including nurses and four doctors, of a hospital in nearby Balussery have been asked to go on leave from today as a precautionary measure after the death of two people being treated for Nipah virus, officials said.

The virus has so far claimed 16 lives in Kerala?s northern districts.

Two people, who succumbed to the virus in the past two days, had undergone treatment at the taluk hospital before being shifted to the Kozhikode Medical College Hospital.

A senior health official said the health staff, including four doctors and nurses, of Balussery hospital who were in contact with the patients were asked to go on leave for a week as a precautionary measure.

At around 2 a.m. on May 17 morning, a grievously sick Mohammed Salih, a 28-year-old architect from Kerala?s Perambra town, was rushed by his family to Kozhikode?s Baby Memorial Hospital. Salih was vomiting, had a high fever, and was in a mentally agitated state. The doctor on call, critical care physician A.S. Anoop Kumar, knew these symptoms meant encephalitis, an inflammation of brain tissue that kills hundreds in India every year. Kumar tried to stabilise Salih, but by around 9 a.m., when the hospital?s neurologists came to examine him, it was obvious that something was very wrong.

Even though Salih was receiving top-end care, his condition was worsening rapidly. He had some very peculiar symptoms, recalls Chellenton Jayakrishnan, one of the neurologists who treated him. His heart was racing at over 180 beats per minute and his blood pressure had shot up. His limbs were limp, displaying no reflexes. These symptoms were unlike any encephalitis cases that the team had ever seen. Jayakrishnan and his colleagues ruled out, one by one, dozens of common causes of encephalitis. Salih couldn?t have Japanese encephalitis. The mosquito-borne infection typically doesn?t affect more than one person in a household, and his younger brother, Sabith, had died about 12 days ago after showing similar symptoms. His father and aunt, too, had contracted the infection.

Woman under observation for Nipah symptoms dies

Kozhikode: A 39-year-old woman, who was under observation for Nipah virus symptoms, died today. However, the samples of the woman were found to be negative, Kerala Health Minister K K Shailaja told reporters. The cause of her death has to be ascertained, she said.

A health department official said the woman had accompanied a patient to Kozhikode Medical College Hospital where a Nipah-affected person was being treated. After she fell ill, she was shifted from the Pariyaram Medical College to the Kozhikode Medical College hospital as she showed symptoms similar to that of the Nipah virus.

The health authorities had drawn up a list of 1,949 people who had come in contact with the Nipah-affected persons to monitor their health condition. The woman also figured on the list and had tested negative for Nipah virus earlier as well.

KOZHIKODE: After a fortnight filled with reports on the death of people infected with Nipah virus in the district, two patients who tested positive and undergoing treatment at the Medical College Hospital here are now known to have recovered.

Health Department officials confirmed that they are out of danger and under observation at the isolation room.Health Services director Dr R L Saritha said it is a positive news that the two patients are showing signs of recovery. ?But it is too soon to declare that they have completely recovered. We need to keep monitoring them,? she said.

Two patients who have shown signs of recovery were administered with ribavirin brought from Malaysia, said Dr Saritha. ?We have been administering them the medicine from the start. It is now understood that they are recovering. Symptoms with regard to Nipah virus infection has reduced. Even their test results are now negative,? she said.

The state announced that schools in Kozhikode, Wayanad, Kannur & Malappuram districts will re-open on June 12. They were supposed to re-open on June 1st after summer vacation, which was then extended to June 5th.

Shri J P Nadda takes stock of the public health measures for Nipah Virus disease

The Union Minister for Health and Family Welfare, Shri J P Nadda today reviewed the ground situation with the Health Minister of Kerala and took stock of the public health measures in the affected areas. On directions of the Union Health Minister, a multi-disciplinary Central Team led by Director, National Centre for Disease Control (NCDC) including experts from NCDC, AIIMS, Safdarjung hospital, EMR division, NIV Pune was immediately deployed in Kozikode, Kerala and are constantly monitoring the situation of the Nipah Virus Disease and supporting the State Health Authorities in managing the outbreak.

After reviewing the situation, the Central Team suggested triage of Nipah cases and establishment of dedicated isolation facilities in district and strict infection control practices. Majority of the cases were found to be having contact with cases of Nipah virus disease. The Central teams recommended enlisting and daily follow up of all contacts.

Nipah virus disease has affected two districts of Kerala viz. Kozhikode and Malappuram and Government of India has issued the following guidelines.

In addition, to above specific case definition, contact tracing and treatment and clinical management protocol has been provided to State authorities.

National Centre for Disease Control (NCDC) is monitoring the situation in various States trough (Integrated Disease Control Surveillance Programme (IDSP) network. Advisories have been sent to all the states, video conferencing have been conducted with States for training and clarifying their doubts. Further samples were sent from various States viz. Goa, Telangana, Karnataka, Himachal Pradesh and all samples were negative. Details of cases and deaths, as on date there are 19 reported cases (18 lab confirmed). Out of 19 reported cases 17 died, from the two affected districts in Kerala State.

Dr Arunkumar, who was among the first to reach the spot in Perambra after the outbreak, said that a timely and effective intervention had helped rein in the virus. "All the 18 tested positive, of which 16 have died, got it from the index case, Sabith, who died on May 5," he said. They contracted the virus from the index case at two points, Perambra Taluk Hospital and Kozhikode Medical College Hospital (KMCH). Several bystanders of others got infected from the narrow corridor of the radiology section at KMCH where the index case underwent scanning on May 5.

"As the virus-infected Ismayil and Rasin were at Balussery Taluk hospital, a couple of cases may emerge from there. Also, the deceased Akhil had gone to EMS Hospital Mukkam and another private hospital before coming to KMCH.? "So a few cases may appear from there also. All have developed symptoms in 11/12 days except one, which went to 16 days." NiV will spread only from a patient in an aggravated stage. Respiratory problems, including a high cough, are the symptoms.

"If the mammal-bitten mango caused the virus in Sabith, the same should have happened to many others in the locality,? he said. ?Sabith is a pet lover. I suspect he came across a bat or a little one and petted it out of affection."

Fruit bats, rabbit test negative for Nipah

Focus to be on containing deadly virus infection now.

KOZHIKODE: The fight against Nipah virus (NiV) has narrowed down to controlling its spread. Since the outbreak, an effort to trace the virus from bats had gone parallel with various measures put in to contain the deadly virus. But, as finding the virus from lakhs of fruit bats is a tedious process, it got shelved. ?Tracing the virus from bats cannot be done overnight. It has been stopped temporarily,? said Dr A.C. Mohandas, the district animal husbandry officer.

The genesis of India's third outbreak of the rare but deadly Nipah virus is likely to remain a mystery forever. But disease detectives suspect it involved a 26-year-old plumber in Kerala's Sooppikkada village - a two-hour drive north of Kozhikode - who liked birds and animals and kept ducks and rabbits for pets.

Scientists, probing the outbreak that has claimed 15 lives over the past one month, believe the plumber was likely the "index patient" - the first person to have picked up the Nipah virus in Kerala from a fruitbat. This species of fruitbat, called Pteropus giganteus, is found across the Indian subcontinent and elsewhere.

"It was a chance event. Adult bats avoid humans. He possibly had a close encounter with a baby bat with a high viral load," Govindakaranavar Arunkumar, professor and head of the Manipal Centre for Virus Research told KnowHow. Arunkumar is one of the investigators who visited the plumber's home and quizzed his family members about his activities during the days before he fell sick on May 2 - with high fever, headache, and disorientation - and died on May 5.

Health authorities believe each one of the 17 subsequent patients in Kozhikode and Mallapuram confirmed positive for Nipah, acquired the infection from the plumber - most likely through exposure to virus-contaminated droplets shed by the patient during the last stage of his disease.
