"Local tests showed he was infected with the H5N1 virus," he said, adding that the man was from eastern outskirts of the capital Jakarta and had reportedly had contact with birds.

A 44-year-old man has died as a result of bird flu infection, say authorities in Indonesia. The man was from eastern Jakarta. He was hospitalized on 10 July with a temperature, breathing problems and a bad cough.

Authorities said the man had been in close contact with live poultry.

Local tests indicated he died of H5N1 avian flu infection. As with all local confirmed tests, samples have been sent on to a WHO recognized laboratory in Hong Kong for final confirmation.

This last case will bring Indonesia equal to Vietnam regarding total human bird flu deaths. Thirty people have died so far in Indonesia this year alone, while not one confirmed death has been reported in Vietnam (this year).

As Indonesia's efforts to combat the bird flu spread have not been organized in a uniform, systematic way, experts say it is most likely the virus will thrive in the country for many years to come. Authorities in Indonesia have been reluctant to organize mass culls of poultry in areas where infections have been found because of the huge costs involved in compensating farmers and smallholders.

It is now a foregone conclusion among the scientific community that the virulent H5N1 bird flu virus strain will eventually mutate into a more human transmissible form. For the moment, H5N1 cannot infect humans easily. For it to spread easily from human-to-human it will need to mutate. In order to mutate, it will most likely infect a human who has the normal flu. The bird flu virus could exchange genetic information with the normal human flu virus and gain its ability to transmit easily from human-to-human. If, or when this happens, we could be facing a serious pandemic. How serious the pandemic is depends on how virulent the new, mutated virus is.

The reason why humans cannot catch bird flu easily is that it needs to get deep down in the human lung to make a person ill. In order to do that a person must have a large cluster of H5N1 around him/her for some of them to get deep down. H5N1 does not infect the human upper-respiratory tract. Precisely for this reason humans cannot infect other humans easily. An infected person who coughs and/or sneezes does not expel many bird flu viruses because they are too deep down. For H5N1 to spread easily among humans it will need to mutate so that it infects the upper-respiratory tract. On the positive side, upper-respiratory tract infections are easier to treat than lower-respiratory tract infections. Hopefully, if and when the virus mutates, even though there may be many more human infections, each one will be easier to treat and survival rates will be much higher than they currently are.

Indonesia: Suspected (42nd) Death from H5N1 Avian Influenza

A 44-year-old Indonesian who died 4 days ago has tested positive for bird
flu, a senior Health Ministry official said on Sunday [16 Jul 2006], citing
a local test. If the diagnosis is confirmed by a laboratory sanctioned by
the World Health Organization (WHO), the East Jakarta man would be the 42nd
bird flu death in Indonesia, a sprawling archipelago where the virus has
killed millions of fowl and caused more human deaths this year [2006] than
in any other country. Confirmation would take Indonesia level with Viet Nam
as the country recording the most human fatalities from the H5N1 strain of
the avian flu virus. Viet Nam's 42nd death occurred in 2005. "Like so many
other bird flu cases, the man had contact with a dead chicken in his
neighborhood. He died on 12 Jul 2006, and we are now waiting for the
confirmation," I Nyoman Kandun, director general of communicable disease
control at the Health Ministry, told Reuters.

According to the WHO, the H5N1 strain has killed 132 people across the
globe since 2003. The global health arm has so far confirmed 41 deaths in
Indonesia, where the virus is endemic in poultry in nearly all of the
country's 33 provinces. Earlier this month [July 2006], a senior government
official said Indonesia's poultry death rate from bird flu was worsening,
possibly due to poor vaccination coverage. In 2005, [poultry] deaths for
the year as a whole were 1.2 million.

Indonesia has been criticized for doing too little to stamp out the H5N1
virus, which remains essentially an animal disease but which experts fear
could spark a pandemic if it mutates into a form easily transmissible
between humans. The government has so far shied away from mass poultry
culling, citing lack of funds and the impracticality of the move in a
country with millions of backyard fowl. Vaccination is the preferred method
to prevent the spread of bird flu among poultry.

Indonesia drew international attention in May 2006 when the virus killed 7
members of a single family in North Sumatra. Experts said there could have
been limited human-to-human transmission in the cluster case. On Thu [13
Jul 2006], the leading science journal Nature reported that multiple
mutations had been found in the H5N1 virus that killed the Sumatra family,
although scientists are unsure of their significance [see: Avian influenza,

The sample of blood will from people who carried out direct contact with casualties also be checked to anticipate the possibility of the spread of the virus against people in and around the patient.

Beside the case that struck K, a child was three years old 10 months [A.A.] were in Cisauk, Tangerang, that died on July 6 2006 also stated positive was infected by the bird flu virus by Balitbangkes and it was confirmed also positive by the WHO collaboration laboratory in Hong Kong on Saturday (15/7).

Therefore till at this time the number of cases of bird flu in Indonesia that was it was confirmed positive the virus infection H5N1 was recorded by as many as 55 cases and 42 including resulting in the death.

URI easier to treat? It doesn't increase the insufficient med supplies.

Many more human infections means exceeding hospital surge capacity, so I'm less than confident the survival rates will be as low as we'd like.

Grim mark for RI with 42nd bird flu death

Indonesia has set an unenviable mark, recording 42 human deaths from bird flu, the same as Vietnam, and is likely to pass that country with more suspected samples being tested at WHO-approved laboratories.

A Health Ministry official overseeing bird flu monitoring, Arman, told The Jakarta Post his office received confirmation from WHO on Sunday that a 3-year-old girl in Tangerang, Banten, died of bird flu.

The girl died July 6. Indonesia has 55 confirmed human cases of bird flu, giving it a mortality rate of over 75 percent.

"We received the laboratory test results from a WHO reference lab in Hong Kong on Sunday at 10 a.m. It confirmed the girl died of H5N1," he said.

The Health Ministry's director general for communicable diseases and environmental health, I Nyoman Kandun, said local tests also showed that a 44-year-old man died of H5N1 last week.

The man, a fried chicken vendor, lived in East Jakarta and died July 12 after being hospitalized for two days with a high fever, coughing and breathing difficulties.

"The man had a history of physical contact with dead or sick chickens," Kandun told the Post.

If the local tests are confirmed, Indonesia will have recorded 43 bird flu deaths from 56 confirmed cases, giving it the grim record of the country with the most human deaths from the virus.

"We still expect more human casualties as long as avian influenza continues circulating among poultry across the nation," Kandun said, adding that the deadly disease was endemic in 27 of the 32 provinces across the archipelago.

He said the government was doing all it could to eradicate the virus, but needed help from other parties to prevent further human casualties.
Saying culling millions of chickens throughout the country would be too costly, he suggested people who keep chickens in their backyards should start improving biosecurity measures in their homes.

He also urged the public to take such basic safety measures as washing their hands after having contact with sick birds.

"I also call on help from the international community. Unless we want to see Indonesia become the center of a global bird flu pandemic, they should go all out in supporting us," he said.

The government has said it needs Rp 9 trillion (about US$980 million) to finance its bird flu programs from 2006 to 2008. This money is needed to provide compensation for culled chickens, purchase vaccines and perform research, as well as preparing for a possible global flu pandemic that experts fear could be triggered by a mutated form of the virus that is easily transmitted between humans. A senior official with WHO Indonesia, Dr. Steven Bjorge, told the Post the rest of the world continued to be concerned with avian influenza in the country, and hoped that cooperation between health and agriculture ministries would be sped up to combat bird flu. "But keep in mind that Indonesia is a much larger country compared to Vietnam," he said, asking the public not to make a simple comparison of the bird flu situation in the two countries.

JAKARTA, July 20 (Reuters) - An international test has confirmed a 44-year-old Indonesian man who died this month had bird flu, a senior health ministry official said on Thursday.

The man, a fried chicken vendor in east Jakarta, died on July 12.
His death brought to 42 the number of Indonesians killed by the bird flu virus, which has claimed at least 132 lives worldwide since it re-emerged in east Asia in 2003.

"The man was confirmed to have bird flu by the Hong Kong laboratory. There were dead chickens around him, but chickens around his neighbourhood tested negative for bird flu," I Nyoman Kandun, director-general of communicable disease control at the Health Ministry, told Reuters.

Contact with sick or dead poultry is the usual mode of transmission of the H5N1 bird flu virus, which is endemic in nearly all of the country's 33 provinces.

While Indonesia has sophisticated laboratories, bird flu tests still need to be confirmed by World Health Organisation-affiliated testing centres, usually in Hong Kong or the United States.

Human cases of bird flu have been rising steadily in Indonesia since its first known outbreak in poultry in late 2003. The populous Southeast Asian country has recorded more human deaths from the disease this year than any other country.

Earlier this month, a three-year-old girl died of the virus.

Vietnam has also recorded 42 deaths but has managed to bring the disease under control. Its last human death occurred in 2005.

Indonesia has been criticised for not doing enough to stamp out H5N1, which still remains essentially an animal disease but experts fear could spark a pandemic if it mutates into a form that can pass easily among people.

The agriculture ministry said earlier this month Indonesia's death rate from bird flu was worsening, possibly due to limited vaccination of poultry.

Indonesia is poised to surpass Vietnam as the country hardest hit by avian flu. And while Vietnam has not had a single human case or poultry outbreak this year, public health officials and experts say the situation in Indonesia is likely to get worse.

Indonesia received word from a Hong Kong laboratory that a 44-year- old man who died last week near Jakarta had tested positive for the H5N1 virus, the Indonesian Health Ministry said Thursday. That brought number of confirmed bird flu deaths in Indonesia to 42 since the first human case was confirmed a year ago, equal to the toll in Vietnam.

The flu is ubiquitous in thousands of Indonesian backyard flocks, and appears to be killing more birds every month, increasing the likelihood of human cases.

"It's like trying to fix the roof while there's a storm going on," said Dick Thompson, a spokesman for the World Health Organization. "Until the animal situation gets under control, there's going to be this steady drip, drip, drip of human cases, and that's a problem."

Although the H5N1 flu came relatively late to Indonesia, it soon spiraled out of control, and deaths have mounted quickly.

Unlike Thailand, which quenched outbreaks by killing millions of chickens, or Vietnam, which used mandatory vaccination, Indonesia has tried a mix of limited culling and vaccinating in rings around the cull - so far, with little success.

Mathur Riady, chief of livestock for the Indonesian Health Ministry, said recently that more than a million birds had died of the flu between January and March, about the same number as died all of last year.

The biggest obstacle to beating the disease, international flu experts say, is the decentralized Indonesian government. Health officials in the capital, Jakarta, have been described as having powers extending no further than their office walls, while real power resides in the governors of the 33 provinces and the elected bupatis, or regents, of 480 districts.

"It's a real mish- mash," said Dr. Jeffrey Mariner, a veterinary medicine professor from Tufts University, in Boston, who is helping the UN Food and Agricultural Organization train new veterinary workers. "You have to sit down with each decision-making unit and get them all on board. It's hard to mount a coordinated response."

As a result, the country is not only slow to report human cases, but it no longer even reports poultry outbreaks to the World Organization for Animal Health in Paris.

However, decentralization is not a principle that Indonesians are likely to abandon. Like the former Yugoslavia under Marshal Tito, the 245 million Indonesians, living on about 6,000 populated islands, are a contentious mix of ethnic and religious rivalries, formerly held together by harsh central control from Jakarta.

With the collapse of the Suharto dictatorship in 1998, independence movements sprouted from East Timor to Aceh, and Jakarta responded with different mixes of military power and grants of semi-autonomy.

The regions prefer having more self- government, and "decentralized units get very wary when the center takes on emergency powers," said Dr. David Nabarro, a UN chief pandemic flu coordinator.

Shortages of trained veterinarians and slow compensation of farmers have also been major obstacles to crushing the outbreak, Mariner said.

He and nine colleagues from Tufts and the UN agency are training local people to find and do rapid tests on sick birds, cull flu-infected flocks and vaccinate others in a ring around them. But each trainee needs two to three months of class and field work to become proficient, he said, and then many of them must, in turn, become trainers.

"By February, we should have enough for 157 districts on three islands," he said. Asked how long it would take to train enough disease-trackers to cover all 480 districts, he said "I don't have an answer. Maybe two or three years."

Another problem, he said, is the sheer profusion of backyard chickens. The outbreak is not a big problem in commercial flocks, but "in the country, every household has poultry," he said. "Retired people here keep chickens like other retirees take up woodworking. It's household food, and income, and something to do. Asking Indonesians to give up their chickens is like asking Americans to give up their dogs and cats."

Until recently, he said, many farmers refused to let their birds be killed because they received only vouchers that could take six months to be paid.

"Now we're seeing the districts willing to advance money, so people are paid in a few days," he said. "That's begun real cooperation."

The government pays about $1 per bird - just a bit below market value, which veterinary experts suggest is the best way to get compliance while not creating the temptation to breed just for the culling payments.

Indonesians also raise fighting cocks, songbirds and trained doves worth much more than $1, he said, but they are paid nothing extra, giving the owners little incentive to cooperate.

Making matters more difficult, some outbreaks begin in remote villages which may themselves be estranged from local government. For example, Kubu Sembilang, the village in the Karo District in northern Sumatra where the flu killed seven members of one family, was a Christian and animist village, while northern Sumatra is a center of fundamentalist Islam.

Some wary villagers there blamed the deaths on witchcraft and refused to take the antiviral drug Tamiflu that the authorities offered.

Given the huge Indonesian population - Vietnam has 84 million people - avian flu remains a relatively rare disease there, said Dr. Tim Uyeki, an epidemiologist from the U.S. Centers for Disease Control and Prevention who was part of a World Health Organization investigative team.

But the way Indonesian cases have clustered - often infecting just the blood relatives in one family in a village - has boosted a theory that some people are genetically more likely than others to get infected, he noted.

"We can't prove that; it remains a hypothesis," Uyeki said. "But when a village has a lot of people and a large outbreak and they all have lots of contact - burying or slaughtering or whatever - and only one family gets sick, you have to ask: is there something unique in that family that creates susceptibility?"

Studies to test that theory are needed, he said.

Under the right circumstances, Indonesia can rouse itself to beat a threatened epidemic. Just last year, it defeated an unexpected polio outbreak. After a polio-free decade, it had a lone imported case in May, 2005, that quickly became 303 cases, the third-biggest outbreak in the world after Nigeria and Yemen. (Each detectable case of paralysis means there are 200 people with "silent" cases, who can still spread the virus.)

A huge immunization campaign drove new cases down to a mere two this year so far.

That was possible partly because polio involved only the health bureaucracy, not the battered agriculture sector too, said Dr. David Heymann, the World Health Organization director general's representative for polio eradication.

Also, polio prevention is cheaper because farmers do not need to be compensated, and longtime polio donors like Rotary International buy the vaccine and pay stipends to vaccination teams, even though they are nominally "volunteers."

Also, Heymann said, "We pushed them and we pushed them hard."

Luckily for the effort, the rumors that polio vaccine is a plot to sterilize Muslim girls - rumors that still hinder vaccinations in northern Nigeria helping to spread polio across Africa - never took hold, even though Indonesia is a largely Muslim country.
